3.Study and Application of Outpatient Infusion Procedure

Objective To design a new procedure for outpatient safe infusion.Methods Aiming at the errors possibly occur in current management of hospital outpatient infusion process,modern wireless and intelligent recognition technology are applied to research and design a new infusion procedure.Results Based on its program difference from the LAN technology of fixed-point check transfusion management style,this solution truly met the needs of actual medical infusion process and achieve the dynamic information check,exchange and matching between the hospital staffs and patients.Conclusion It ensures the safety of transfusion patients,and creates a harmonious environment for treatment.
4.Clinical analysis of high intraocular pressure after trabeculectomy

AIM: To analyze the common causes and treatments methods of high intraocular pressure (>21mmHg) after trabeculectomy.
METHODS: Twenty-four eyes of glaucoma patients with high intraocular pressure after trabeculectomy in our hospital from July 2010 to September 2014 were retrospectively studied. The common cause and processing methods were analyzed.
RESULTS:The causes which lead to early postoperative high intraocular pressure included high filtering bleb scar in 11 eyes ( 46%) , filtration obstruction in 4 eyes ( 17%) , malignant glaucoma 3 eyes ( 12%) , iris resection of malignant glaucoma unreasonable 2 eyes ( 8%) , hyphema 2 eyes ( 8%) , wrap cystic filtering bleb 2 eyes (8%), etc. After proper treatments, intraocular pressures of all patients were bellowed 21mmHg.
CONCLUSION: High intraocular pressure with surgery for glaucoma is caused by multiple factors, preoperative and intraoperative avoid as far as possible, postoperative early detection for symptomatic treatment is the key to successful operation.
5.Study on Pregnancy Complicated Syphilis in Shanghai

Objectives To learn the incidence and the related factors of pregnancy complicated syphilis and provide basis for policy making. Methods A cross sectional study was carried out for all pregnant women who were checked up in 29 hospitals of Shanghai and delivered between 20th, Sep. to 20 th, Dec. in 1999. Basic condition, marital and obstetric history, lab test, treatment, pregnancy outcomes and lab test on neonate were investigated. Results There were 39 cases of pregnancy complicated syphilis among 12 324 pregnant women, with an incidence of 3.16‰. All cases were in early stage. Among them, 35 cases were latent syphilis, which accounted for 89.7%; 3 cases (7.7%) were primary syphilis; 1 case (2.6%) was secondary syphilis. The average age of the patients was (27?5) years old. The related factors included: couple's education, occupation, the lab index of syphilis at premarital medical examination, the number of sexual partners of pregnant women and residential area. Only 64.1% of partuients received treatment and 48.7% of infants received serological test. Conclusions the incidence of pregnancy complicated syphilis was relatively high and most cases were latent syphilis which were widely dispersal and had adverse sequelae on fetus and neonates; The treatment and followup were not standardized. So it is in urgent need to advocate the standardized treatment and followup for pregnancy complicated syphilis. It's also important to educate among the doctors, nurses in gynecologic & obstetric department and women at their reproductive age on the knowledge about prevention and treatment on syphilis.
6.Plasma visfatin levels in patients with newly diagnosed and untreated type 2 diabetes mellitus and impaired glucose tolerance

Objective To investigate the plasma visfatin levels and explore the relationship between visfatin and anthropometric parameters,glucose,lipid profile,insulin sensitivity index and insulin secretion index in the subjects with different glucose tolerances.Methods Fifty-three patients with newly diagonosed type 2 diabetes(T2DM),7 subjects with impaired glucose tolerance(IGT)and 35 healthy controls were enrolled.Visfatin levels were measured along with the BMI,blood pressure,lipids,glucose,insulin and HOMA-IR indexes.Results Plasma visfatin levels were elevated in patients with newly diagnosed T2DM compared with IGT group or controls.There was no significant difference in the visfatin levels between IGT group and healthy controls.Plasma visfatin level was positively correlated with glucose levels and HOMA-IR,and negatively correlated with insulin secretion index(HOMA-B).Multiple regression analysis showed that postprandial plasma glucose was independent related factor influencing plasma visfatin levels.In T2DM group,plasma visfatin concentration was positively correlated with HbA1c level.Conclusion The increased plasma visfatin concentration in newly diagnosed and untreated type 2 diabetes mellitus represents a compensatory mechanism aimed at ameliorating the functional consequences of insulin deficiency.

8.Outcome after ventriculoperitoneal shunt for post-traumatic hydrocephalus

Objective To investigate the outcome after ventriculoperitoneal (V-P)shunt for post-traumatic hydrocephalus (PTH). Methods A retrospective study investigated the outcome of 51 patients after head injury, who had undergone V-P shunt due to PTH.The Glasgow outcome scale (GOS),age and intracerebral pressure(ICP) were analyzed. Results Twenty-two of 33 patients with COS 3 or less than 3 scores had clear-cut benefit from V-P shunt,while only 6 of 18 patients with GOS 4 scores did.Tbe GOS before V-P shunt had correlated with the GOS 6 months after operation. The patients with GOS 3 or less than 3 scores had good outcome than patients with COS 4 scores (P<0.05). Patients' age and ICP before the V-P shunt did not influence the outcome (P>0.05 ). Conclusion V-P shunt has much benefit for the patients with PTH, GOS before operation is a best predictive parameter for outcome after V-P shunt.
9.The effect observation on tension-free herniorrhaphy between anterior and posterior approach in inguinal hernia patients

Objective To choose the best surgical procedure,the surgical results and clinical outcomes of inguinal hernia repair with either an open anterior or posterior approach,were studied.Methods The 366 cases of inguinal hernia patients according to surgical side approach in different ways,is divided into A,B groups,Among group A of 186 cases repaired with application of prolene hernia system (PHS) via anterior approach,while group B of 180 cases surgically treated with Kugel technique with a posterior approach,and surgical procedure,postoperative recovery and complications were compared between these two groups,respectively.Results No significant differences were found in operation time,hospital stay,incisional infection rate and the recurrence rate (P > 0.05).However,compared with group,the group B patients had less pain of the wound,slight postoperative swelling response,less foreign body aesthesis and hydrocele(P < 0.05).Conclusion Compared with the anterior approach surgery,the posterior approach (Kugel technique) has less traumatic,low complication rate and rapid recovery after operation.
10.The influence of carnosine in expression levels of bcl-2 and bax after focal cerebral ischemia in rats

Objective To explore the effect of carnosine in the expression of B cell lymphomal/leukemia-2 (bcl-2) and bcl-2-associated X protein (bax) after focal cerebral ischemia in rats. Methods Thirty male SD rats (SPF scale) were ran?domly divided into 3 groups:sham-operated group, model group and carnosine treated group (n=10 for each group). The mid?dle cerebral artery occlusion model (MCAO) was induced in model group and carnosine treated group. Rats were received carnosine [1 000 mg/(kg·d), orally] in carnosine treated group, and the other rats were received the same volume of normal sa?line (NS) in shame-operated group and model group. The neurological deficit score was used to evaluate the neurological function at 24 h and 72 h after MCAO. Morphological changes were observed by HE staining. TCC staining was used to label infarct volume, and immunohistochemistry was used to detect the expression of bcl-2 and bax. Results Compared with model group, the score of neurological function and infarct volume were significantly declined in carnosine treated group at 72 h after injury (P<0.05 or P<0.01). The changes of ischemic impairment were lighter in carnosine treated group than that of model group. Compared with sham-operated group, the expression levels of bcl-2 and the ratio of bcl-2/bax were de?creased while the expression of bax was increased in model group (P<0.05). Compared with model group, carnosine could sig?nificantly increase the expression of bcl-2 and the ratio of bcl-2/bax, and reduce the expression of bax (P<0.01 or P<0.05). Conclusion Carnosine can enhance bcl-2 expression, decrease bax expression and increase the ratio of bcl-2/bax, which is likely to be one of the mechanisms of neuroprotection.
