1.Infection status of principal human parasites in Nanchang City in 2014

Objective To understand and analyze the infection status of human parasites in Nanchang City, so as to offer a scientific basis for control of parasitic disease. Methods In 2014, a survey was performed according to the scheme of The 3rd National Survey of Principal Human Parasites. Intestinal parasites were surveyed among the residents in Jinxian County and Anyi County, Qingyunpu District and Qingshanhu District in Nanchang City, including the soil-transmitted nematodes (Ascaris lumbricoides, Trichuris trichiura and hookworm), Enterobius vermicularis, tapeworm and intestinal protozoa. Clonorchis sinensis was surveyed among the residents in Qingyunpu District and Qingshanhu District. Toxoplasma gondii was surveyed among the residents in two rural areas (Nanchang County and Xinjian County) and two urban areas (Donghu District and Xihu District). The ovums of the soil-transmitted nematodes, Clonorchis sinensis , tapeworm and other helminths were examined by the Kato-Katz, while Enterobius vermicularis among children was examined by cellophane anal swab, trophozoites and cysts of intestinal protozoa by saline smearing and iodine smearing , and IgG antibodies of Toxoplasma gondii by enzyme-linked immunosorbent assay (ELISA). Results Intestinal parasites were surveyed among 2424 residents in the whole city, in which 2414 residents were tested for the infection of soil-transmitted nematodes and tapeworm;1875 residents were tested for intestinal protozoa infection;74 children were tested for eggs of Enterobius vermicularis; 539 residents were tested for Clonorchis sinensis; and 2400 residents were tested for Toxoplasma gondii. Six kinds of intestinal parasites were found citywide, with a total infection rate of 9.49% (230/2424). The infection rate of soil-transmitted nematodes was 8 . 70% ( 210/2414 ) , in which the infection rate of Ascaris lumbricoides was 0.04% (1/2414), the rate of Trichuris trichiura was 0.91% (22/2414), and of hookworm was 7.83%(189/2414). The infection rate among 3 - 6 years old children of Enterobius vermicularis was 22.97% ( 17/74 ) . Tapeworm and Clonorchis sinensis were not found . The infection rate of intestinal protozoa was 0.21%(4/1875). The positive rate of Toxoplasma gondii's IgG antibody was 5.17% (124/2400). Conclusions In Nanchang City, the infection rate of soil-transmitted nematodes (Ascaris lumbricoides, Trichuris trichiura) and intestinal protozoa was lower. However, the infection rates of hookworm, Toxoplasma gondii and Enterobius vermicularis among 3 - 6 years old children are increasing in local areas. That should be the focus in prevention and control of parasitic disease in the future.
2.Correlation of HIF-1α level with hypoxia in keloid fibroblasts

Objective To investigate the correlation of HIF-1α and hypoxia in keloids fibroblasts, and to investigate the mechanism that hypoxia promotes abnormal scarring by HIF-1α pathway. Methods Keloid fibroblasts cultured in vitro were placed in an incubator with different O2 concentrations. After 24 h, the keloid fibroblasts were collected for further study. Western blotting was performed to detect the expression of HIF-1α in the keloid fibroblasts. Results Relative amounts of HIF-1α in keloid fibroblasts cultured under O2 concentrations at 20 %, 10 %, 5 % and 1 % were 0. 007 ±0. 006, 0. 133 ±0. 006, 0. 537±0. 015 and 0. 903±0. 021, respectively. It indicated that hypoxia could increase the expression of HIF-lα in keloid fibroblasts. Conclusions Hypoxia can induce the expression of HIF-1α in fibroblasts of keloids. Moreover, there still is a positive relation between hypoxia and the expression of HIF-1α. Therefore, a close relationship exists between abnormal scarring and HIF-1α pathway by hypoxia.
3.Role of Curved-Cutter-Stapler in Anus-Preserving for Low Rectal Cancer

Objective To evaluate the role of curved-cutter-stapler in anus-preserving for low rectal cancer.Methods The clinical data of 32 patients with low rectal cancer from June 2007 to December 2008 who received low anterior resection and ultra low anterior resection by using curved-cutter-stapler were reviewed retrospectively.Results No operation death case,complete cutting and safe closure in all cases,one case was complicated with anastomotic leakage,and one case of rectovaginal fistula.Thirty patients were followed up 4 to 22 months after the operation,with an average time of 12.6 months,no hemorrhea of pelvic cavity and anastomotic stoma or anastomotic stenosis cases.Conclusion Curved-cutter-stapler has the advantages of complete cutting,safe closure and low complications,and easy being used in anus-preserving operation for low rectal cancer,which can increase the rate of anus-preserving.
4.Bacteriological analysis of pulmonary infection in hemorrhagic brain injury patients from neurosurgical intensive care unit

Objective To investigate the characteristics of pathogen distribution and drug resistance of pulmonary infection in hemorrhagic brain injury patients from neurosurgical intensive care unit (NICU).Methods Clinical data of 234 patients with hemorrhagic brain injury hospitalized in NICU from March 2013 to September 2014 were retrospectively analyzed.According to the incidence of pulmonary infection,the patients were divided into pulmonary infection group and non-pulmonary infection group.Parameters estimated were admission GCS,sex,age,history of smoking,time of coma,duration of mechanical ventilation,NICU length of stay.Patients in pulmonary infection group were analyzed on the distribution of pathogens and incidence of drug resistance.Results A total of 158 patients (67.5%) had pulmonary infection.Among them 60 cases (38.6%) were found to be co-infected including infection with two pathogens in 26 cases (16.5%),three pathogens in 19 cases (12.0%),and four and more pathogens in 16 cases (10.1%).Age and smoking increased the incidence of pulmonary infection (P < 0.05).Time of coma,duration of mechanical ventilation,and NICU length of stay were prolonged in pulmonary infection group than in non-pulmonary infection group (P < 0.05).A total of 219 strains of pathogens were isolated from the patients in pulmonary infection group.Specifically,there were 193 strains of gram negative bacteria (88.1%),13 strains of gram positive bacteria (5.9%),and 13 strains of fungi (5.9%).Gram negative were sensitive to amikacin,imipenem,cefoperazone/ sulbactam and ciprofloxacin.Staphylococcus aureus isolated were 100% sensitive to vancomycin,linezolid and teicoplanin,and were completely penicillin resistant.Fungi were not resistant to voriconazole,itraconazole,ketoconazole,fluconazol,and amphotericin B.Conclusions High incidence of pulmonary infection is noted among the hemorrhagic brain injury patients in NICU,and the pathogens are diverse dominated by Gram negative bacteria.Incidence of multi-drug resistant pulmonary infection is high,indicating that the key point is to choose antibiotics rationally based on drug sensitivity test.
5.Guiding Significance of "Mistreatment by Warming Therapy" in Treatment of Non-small Cell Lung Cancer.

"Mistreatment by warming therapy" refers to various symptoms/syndromes caused by improper use of fire methods mentioned in Treatise on Febrile Diseases, which are similar to damaged yin induced fire-heat syndrome (injury of yin, consumption of qi, loss of yin, consumption of body fluid) manifested after radiotherapy and/or chemotherapy in modern clinics. Better efficacy was obtained in treating various symptoms/syndromes by using nourishing yin purging fire method. In this paper authors focuses on exploring guiding significance of using "Mistreatment by warming therapy" in treating non-small cell lung cancer (NSCLC).

6.Value of strain ratio in differentiating benign and malignant breast lesions of BI-RADS-US Ⅳ

ObjectiveTo assessment the value of strain ratio(SR) in differentiating benign and malignant breast lesions of BI-RADS Ⅳ diagnosed by conventional ultrasound.MethodsElastosonography was performed on 64 patients with breast lesions of BI-RADS Ⅳ diagnosed by conventional ultrasound.SR (strainnormal neighboring tissues /strainbreast lesion) was calculated by the software equipped with the ultrasonic machine.Pathology of the lesions after biopsy or operation was considered as standard,SRs of benign and malignant breast lesions were compared with independent-samples t test.The diagnostic value of SR was evaluated by receiver operating characteristic (ROC) curve analysis.Results SRs of 34 benign breast lesions and 30 malignant breast lesions were 2.85 ± 1.30 and 5.03 ± 2.50 respectively,there was significant difference between them( t =- 4.29,P =0.000).Area under the curve of diagnosing benign and malignant of breast lesions was 0.875 for SR( P <0.0001).If the cut-off point of SR was 3.26,the sensitivity in differentiating breast lesions was 83.33%,while the specificity was 82.35%.Conclusions SR is a helpful elasticity parameter for differentiating benign and malignant breast lesions of BI-RADS Ⅳ diagnosed by conventional ultrasound.
7.Analysis of related factors affecting in condylar buttress plate surgical treatment of distal femoral fractures

Objective To analyze of related factors in affecting condylar buttress plate (CBP) surgical treatment of distal femoral fractures.Methods One hundred patients with distal femoral fractures were grouped according to eight related factors (gender,age,fracture type,fracture classification,surgical time,reset quality,whether bone graft and plaster braking),respectively.Operative efficacy was scored by the Neer knee functional score table,and score ≥ 72 was excellent,while < 72 was good.Eight related factors were carried out single factor analysis with x2 test to compare the operative efficacy of each group,then the related factors with single factor analysis result of P<0.05 were put in Logistic multifactor regression analysis,in order to clarify the independent risk factors for CBP treatment of distal femoral fractures.Results There was no significant difference (P>0.05) in the operative efficacy between male and female group,open fracture and closed fracture group.The difference between youth,middle-aged and elderly group,type A and type C fracture group was statistically significant (P<0.05).The operative efficacy of patients got surgery within 7 d was obviously better than those who got surgery 7 d later (P<0.05).The operative efficacy of patients who had no intraoperative bone graft was better than those who had bone graft (P<0.05).The operative efficacy of satisfactory reset group and none plaster braking group was better than that of unsatisfied reset group and plaster braking group (P<0.05),respectively.Conclusions Age,fracture type and reset quality are CBP therapy independent risk factors for the efficacy of operation for patients with distal femoral fractures,which will provide some reference for improving the operative efficacy of CBP operation in treatment of distal femoral fractures.
8.THE EFFECT OF IMMOBILIZATION PROCESS TO THE ACTIVITY OF MTHASE AND MTSASE

Amylum can be transformed into trehalose on the action of the MTSase and MTHase, however, the percentage of transformation is low. The research on the enzyme immobilization through four kinds of immobilization carrier is described in this paper. The result shows that the carriers, which are put into with enzyme liquid and crossbonded with chi-tosan and glutaraldehyde at first, can adsorb many enzymes independent from this process of trehalose synthesizing, consequently, the enzyme activity is increased sharply. By comparing with the influence of the immobilizing process and some factors in the reaction conditions, such optimize condition can be reached: the time for carrier crossbonding with glutaraldehyde is 18hours , the time with enzyme is another 18hours , and the time of reaction with amylum (10%) is 9hours.The result of the content of trehalose can reach 27.22g/L and the percentage of transformation can reach 54.43% , which previously is 2.67g/L and 5.33% respectively before dealt with by carriers.
9.The diagnostic value of contrast-enhanced ultrasound in detecting small liver metastasis from breast cancer

Objective To explore the diagnostic value of contrast-enhanced ultrasound (CEUS) in detecting small liver metastasis (SLM) from breast cancer. Methods A total of 92 cases diagnosed as breast cancer by surgical pathology and suspected SLM by conventional ultrasonography were included in this study. CEUS was used to check liver lesion in patients. The biopsy operation was used as the gold standard. The coincidence rate of benign and malignant pathology was used to compare the consistency between CEUS and biopsy pathology. Results In 92 cases suspected liver metastases of breast cancer by routine ultrasound, there were 54 cases were diagnosed as benign by pathology, and all 92 cases were diagnosed as benign by CEUS. Thirty-eight cases were diagnosed as metastasis by pathology, in which 36 cases were correctly diagnosed by CEUS. The sensitivity of CEUS for the diagnosis of liver metastases of breast cancer was 94.74%(36/38), specificity was 100%(54/54), positive predictive value (PPV) was 100%(36/36) and negative predictive value (NPV) was 96.43%(54/56). Conclusion CEUS has a higher diagnostic value in SLM of breast cancer, and has clinical practical value.
10.Clinical characteristics and correlative factors of constipation in patients with Parkinson's disease

Objective To investigate the characteristics and the correlative factors of constipation in Parkinson's disease (PD) patients.Methods The demographic information,clinical features and history of medications of 193 patients with idiopathic PD consulting in the outpatient department of the First Affiliated Hospital,Anhui Medical University were collected.Patients were evaluated using following scales:Unified Parkinson's Disease Rating Scale Ⅲ (UPDRS Ⅲ),Hoehn-Yahr stage,Bristol Scoring Scale,Cleveland Constipation Scoring Scale (CCS),Scale for Outcomes in PD-autonomic for Autonomic Symptoms,Simple Food Frequency Questionnaire,Hamilton Depression Scale (HAMD),Mini Mental State Examination.The patients were divided into constipation group and non-constipation group based on Rome Ⅲ Criteria for Diagnosis of Functional Constipation,and the correlative factors of constipation were compared and analyzed between the two groups.The severity of constipation and influencing factors were also compared between patients with early onset constipation (occurred before present of motor symptoms) and patients with late onset constipation (occurred after present of motor symptoms).The impacts of anti-PD medication adjustments on constipation were assessed by observing the alteration of constipation severity in 41 PD patients.Results The incidence of constipation was 56.4% (109/193) in our cohort of PD patients,and 21.1% (23/109) of constipation was severe according to the assessing by CCS.The spectrum of constipation symptoms included defecation straining (89.9%,98/109),poor stool output (67.9%,74/ 109),reduced stool frequency (63.3%,69/109) and dryness of stool (60.5%,66/109).The age,disease duration,scores of Hoehn-Yahr stage,UPDRS Ⅲ and HAMD,levodopa equivalent dose (LED),frequency of urination disturbance in constipation group were significantly higher than those in nonconstipation group,while the daily quantities of vegetable and water intake in constipation group were significantly lower than those in non-constipation group.Age and HAMD scores were the independent risk factors of constipation (OR=1.049,95% CI 1.014-1.086,P=0.006;OR=1.316,95% CI 1.185-1.461,P =0.000).Among the 109 constipation patients,the course of PD,scores of Hoehn-Yahr stage,UPDRSⅢ and LED were positively correlated with the severity of constipation (r =0.269,0.338,0.315,0.341,0.371,all P < 0.05),with HAMD score being the independent risk factor of constipation severity (OR =1.175,95% CI 1.044-1.322,P < 0.05).The severity of constipation and risk factors of constipation in patients with early onset constipation were not distinct from those with late onset constipation.Conclusions The incidence of constipation increases with the increment of age,disease duration,Hoehn-Yahr stage,UPDRS Ⅲ scores,LED,HAMD scores,urination disorder severity and the decrement of daily water and vegetable intakes.The severity of constipation is positively related to the severity of motor symptoms,the daily dose of anti-PD drugs and depression levels.PD patients with early onset constipation are not distinct from the patients with late onset constipation in terms of severity and risk factors.The influences of anti-PD drugs on constipation are variable depending on the specific drug used and individual constitution.Individualized treatment regimes are proposed with respect to the management of constipation according to the specific risk factors in PD patients.
