1.Study of Expression of Pneumococcal Surface Protein and Immunogenicity

The specific fragment of Pneumococcal surface protein A(PspA)and Pneumococcal Surface Adhesin A(PsaA)gene was amplified by PCR from Streptococcus pneumonia 5 and Streptococcus pneumonia 19.The amplified fragnent of PspA and PsaA gene was ligated into pET-27b(+)vector and transformed into BL 21 E.coli for expression and obtain the expressive production of PspA and PsaA.Induced by IPTG,the expression level was as high as 75 % of the total disolube protein.The result showed that the recombinant plasmid could express a specific 75 kDa and 37 kDa fusion protein in E.coli BL 21,which showed the good immunogenicity and a broadly cross reactivity with the other serotypes.
2.Clinical observation of ocular alkali burn by Breviscapinun

AlM:To find better ways of treating ocular alkali burn, and to reduce the suffering of patients and social burden.METHODS:Totally 100 patients were graded according to the degree of chemical burns to four major groups, each half were randomly divided into the control group and the treatment group. Control group underwent conventional treatment. ln addition to conventional therapy, patients in each treatment group were also added a Breviscapine intravenous injection of 40mg daily. Corneal recovery time, changes in vision, degree of corneal opacity, number of corneal neovascularization and other complications were observed. Curative effects were analyzed statistically.
RESULTS:There was no significant difference in levelⅠgroup between control group and treatment group ( P>0. 05); There were significantly different in level Ⅱ, Ⅲand Ⅳ group ( P<0. 05 ). Compared to the degree of corneal opacity and the number of corneal neovascularization, the treatment group was obviously better than the control group(P<0. 05).
CONCLUSlON: Breviscapine in the treatment of ocular alkali burns can shorten the course of treatment, reduce corneal scarring, and improve vision.
3.Based on infrared spectrum to study the changes in the intrinsic chemicalcomponents in Trichosanthes before and after steaming

Objective To analyze the change in infrared(IR)spectral information and to screen out the classification of major compounds affecting information difference in IR spectra of Trichosanthes before and after steaming. Methods The similarity between the original IR fingerprint and the first derivative IR fingerprint of Trichosanthes and their steamed products was calculated by using the Computer Aided Similarity Evaluation System. The principal component analysis(PCA)model and the partial least squares dis-criminant analysis(PLS-DA)model of IR spectral data of Trichosanthes before and after steaming were established by using SIMCA-P 11 statistical software for PCA and PLS-DA,and the classification of major compounds affecting information difference in IR spectra of Trichosanthes before and after steaming was selected by 3D scatter plot,load 3D scatter plot and variable important in project(VIP) value. Results The similarity between the original IR fingerprint and first derivative IR fingerprint of Trichosanthes and their steamed products were 0.9165 and 0.2832. Seven VIP>1 spectral peaks were screened out by using SIMCA-P 11 statistical software,of which,the absorption peak of 1456 cm-1 was νc=c,the absorption peak of 1726 cm-1 was νc=o and the VIP values were 1.6290 and 1.4256 respectively. Conclusion The categories of compounds of Trichosanthes before and after steaming did not change,but the chemical components changed. Compounds of Trichosanthes before and after steaming affect the difference in IR spectral information may mainly contain C=C-C=C or C=O or both of them.
4.Analysis of the changes in the chemical components in Fructus Trichosanthes, before and after the processing of steaming,based on the PCA and PLS-DA algorithms

Objective To analyze the changes in the chemical components in Fructus Trichosanthes before and after the pro?cessing of steaming,so as to explore the material basis of the pharmacodynamic changes between Trichosanthes and steamed Tricho-santhes.Methods The peaks matching data of Fructus Trichosanthes and steamed Fructus Trichosanthes were obtained by using the similarity evaluation system for chromatographic fingerprints of traditional Chinese materia medica.The principal component analysis (PCA)model and the partial least squares discriminant analysis(PLS-DA)model for the analysis of the Fructus Trichosanthes and steamed Fructus Trichosanthes data were established using the SIMCA-P 11 statistical software for PCA and PLS-DA,from which the score chart,load chart and Variable Importance(VIP)value were obtained,so as to identify the main different components in Fructus Trichosanthes and steamed Fructus Trichosanthes.Results The PCA(R2X=0.96,Q2=0.552)model and PLS-DA(R2Y=0.917,Q2=0.579)model were established,and 8 chromatographic peaks with significant difference in peak area were selected.Among them,two of the chromatographic peaks were assigned to be 5-hydroxy methyl furfural and vanilla acid,and 5-hydroxy methyl furfural had the largest VIP value.In addition,an unknown component was also found in the steamed Fructus Trichosanthes,which was generated in the process of steaming and needed to be identified in future studies.Conclusion The content of some chemical components in Fruc?tus Trichosanthes were changed after the process of steaming,and the processing of steaming also caused the formation of an unknown chemical component.5-Hydroxy methyl furfural and vanillic acid seem to be a likely choice for exploring the material basis of the phar?macodynamic changes in Fructus Trichosanthes after the processing of steaming.
5.Characteristics analysis on death among elderly people aged 60 years and above in Shanghai in 2014

[ Objective] To discuss the characteristics of death among elderly people aged 60 years and above in Shanghai, who were classified into different age groups, and to provide a basis for making public health policy. [ Methods] On the basis of the data covering whole population death registry system in Shanghai, data on the elders aged 60-plus was collected and classified into 3 age groups according to WHO standards for descriptive analysis. [ Results] The crude death rate among the elders aged 60-plus in shanghai in 2014 was 3 001.76/105 ,accounting for 88.37%of the total mortality.The crude death rate of male was higher than that of female in each age group.The main causes of death in 60-74 age-group were cancer and coronary heart disease ( CHD) which had shorter course of disease with worse prognosis. The main causes of death in 75-89 age-group were cardiovascular and cerebrovascular diseases and chronic obstructive pulmonary disease ( COPD) which had longer course.The main causes of death in 90-pluse group included functional degradation and accidental fall, apart from cardiovascular and cerebrovascular diseases and COPD.And 54.35%of the elders aged 60-plus died in hospital, while 34.12%at home,and 6.63%at nursing home.The proportion of death at home was higher in non-central urban area than in central urban area.And the proportion of death in hospital decreased with increasing age. [ Conclusion] The proportion of the elderly death was large in total mortality.As the characteristics of death varied in different age groups, government should adopt different prevention and control measures.Rational allocation of medical and rehabilitation resources, as well as terminal care, need more attention and exploration by all institutions concerned.
6.Treatment for posterior root tear of lateral meniscus through bone tunnel suture under arthroscopy.

OBJECTIVE:
To evaluate clinical effects of posterior root tear of lateral meniscus through bone tunnel suture under arthroscopy.
METHODS:
From January 2012 to December 2014, 23 patients with posterior root tear of lateral meniscus repaired through bone tunnel suture under arthroscopy, including 15 males and 8 females, aged from 19 to 48 years old with an average age of (25.0±4.7) years old; 10 knees on the left side and 13 knees on the right side. Complications were observed, Lysholm score before and after operation at 12 months were used to evaluate clinical results, and VAS score was applied to assess pain relief. MRI was used to check recovery outcomes of lateral meniscus injury.
RESULTS:
All patients were followed up from 13 to 24 months with an average of (17.0±4.3) months. No injury of vessels, nerve and incision infection occurred. Motion of knee joint of 19 patients reached normal, 4 patients manifested limited activity of knee joint at12 months after operation. Postoperative Lysholm score 88.52±6.48 at 12 months was higher than that of before operation 46.12±7.35; Postoperative VAS score 0.8±0.7 at 12 months was lower than that of before operation 4.3±1.6.
CONCLUSIONS
Bone tunnel suture under arthroscopy for the treatment of posterior root tear of lateral meniscus could relieve pain, decrease postoperative complications and obtain good clinical efficacy.

7.Effects of puerarin on proliferation and differentiation of umbilical cord mesenchymal stem cells into osteoblasts in vitro.

This study is to investigate the effects of puerarin on the proliferation and differentiation of umbilical cord mesenchymal stem cells (MSCs) into osteoblasts. Umbilical cord MSCs were cultured by tissue adherence and the third passage of cells was used in the experiment. The effect of puerarin on proliferation of umbilical cord MSCs was measured with MTT. The effects of puerarin on umbilical cord MSCs were evaluated by ALP immunohistochemisty and von kossa staining. The OD value decreased with the increase of puerarin concentration. On 7th day, ALP expression of puerarin group was higher than that of control group. On 14th day, ALP staining showed that the positive rate of puerarin group was higher than that of control group. Von kossa staining showed the quantity of calcium nodules was higher in puerarin group than that of control group. Puerarin can promote the umbilical cord MSCs to differentiate into osteoblasts and has an effect on the proliferation of umbilical cord MSCs.

8.The penis prolongation and augmentation combined with autologous granular fat injection and silicone implantation.

OBJECTIVETo prevent the retraction of the penis after prolongation and augmentation.
METHODSAfter all the superficial and part of the deep suspensory ligament amputation, we implanted the silicon sheet (the length 2.3-3.6 cm, the width 1.5-2.5 cm, the thickness 2-3 mm) and injected autologous granular fat (30-48 ml) into penis.
RESULTS16 patients (age 22-63 years, averagely 38 years) underwent this kind operation, the prolongation length is 1.8-5.1 cm, the average was 2.91 cm, the increased diameter of penis was 0.6-1 cm, the average is 0.85 cm, the following period is 3 months to 2 years. The results are satisfactory with the penis retraction less than 8%, and less than 10% decrease in diameter.
CONCLUSIONSThis method is an ideal way of the penis prolongation and augmentation, the implantation of the silicon sheet is effective way to prevent the retraction of the penis.

9.Diagnostic value of MRI for posterior root tear of medial and lateral meniscus.

OBJECTIVETo explore diagnostic value of MRI on posterior root tear of medial and lateral meniscus.
METHODSFrom January 2012 to January 2016, clinical data of 43 patients with meniscal posterior root tear confirmed by arthroscopy were retrospective analyzed, including 25 males and 18 females, aged from 27 to 69 years old with an average age of(42.5±8.3)years old;27 cases on the right side and 16 cases on the left side. MRI examinations of 43 patients with tear of posterior meniscus root confirmed by knee arthroscopies were retrospectively reviewed. MRI images were double-blinded, independently, retrospectively scored by two imaging physicians. Sensitivity, specificity and accuracy of MRI diagnosis of lateral and medial meniscus posterior root tear were calculated, and knee ligament injury and meniscal dislocation were calculated.
RESULTSForty-three of 143 patients were diagnosed with meniscus posterior root tears by arthroscopy, including 19 patients with lateral tears and 24 patients with medial tears. The sensitivity, specificity and accuracy in diagnosis of posterior medial meniscus root tears for doctor A were 91.67%, 86.6% and 83.9% respectively, and for doctor B were 87.5%, 87.4% and 87.4%, 19 patients with medial meniscal protrusion and 2 patients with anterior cruciate ligament tear. The sensitivity, specificity and accuracy in diagnosis of posterior lateral meniscus root tears for doctor A were 73.7%, 79.9% and 79% respectively, and for doctor B were 78.9%, 82.3% and 82.5%, 4 patients with lateral meniscus herniation and 16 patients with cruciate ligament tear. Kappa statistics for posterior medial meniscus root tears and posterior lateral meniscus root tears were 0.84 and 0.72.
CONCLUSIONSMRI could effectively demonstrate imaging features of medial and lateral meniscal root tear and its accompanying signs. It could provide the basis for preoperative diagnosis of clinicians, and be worthy to be popularized.
