1.Analysis on HIV/AIDS epidemic characteristics in Henan Province

Objective To analyze HIV/AIDS epidemic characteristics in Henan Province.Methods The database of HIV/AIDS cases information and follow-up information from the national online case reporting were downloaded.Comprehensive analysis was performed.Results 42 879 cases were confirmed and reported HIV positive,69.05% were former paid plasma donors at the end of 2008.Among these cases reported in 2008,26.64% were heterosexual transmission,and 49.59% (1 802) were found via voluntary counseling and testing.Conclusions The epidemic characteristics of HIV/AIDS in Henan Province was mainly among former paid plasma donors.But the transmission through plasma donor had been interdicted.While the proportion of sexual transmission was increasing,this suggested that education and behavior intervention should be strengthened in order to interrupt further transmission.Counseling and testing become main source of detecting HIV infection.
2.Effects of up-regulation of NF-E2-related factor 2 on mitochondrial biosynthesis and function in an immortalized human vitiligo melanocyte cell line PIG3V

Objective To explore the effects of NF-E2-related factor 2 (Nrf2) overexpression on mitochondrial biosynthesis and function in melanocytes.Methods An immortalized human vitiligo melanocyte cell line PIG3V was used in this study.An overexpression plasmid Nrf2-pEX-1 containing the full-length Nrf2 gene was constructed.PIG3V cells were divided into 3 groups:blank group receiving no treatment,control group transfected with the pEX-1 plasmid,overexpression group transfected with the Nrf2-pEX-1 plasmid.After transfection,real-time quantitative reverse transcription-PCR (RT-PCR) and Western blot were performed to determine the mRNA and protein levels of mitochondrial biosynthesis-related factors (including Nrf2,nuclear respiratory factor 1 (NRF1) and mitochondrial transcription factor A (TFAM)) respectively;RT-PCR was also conducted to measure the copy number of mitochondrial DNA (mtDNA),and flow cytometry to estimate mitochondial membrane potential (MMP);luciferase reporter system was used to estimate the intracellular adenosine triphosphate (ATP) level.Statistical analysis was carried out by using a two-sample t-test.Results After transfection,a significant increase was observed in the mRNA expression levels of Nrf2 and NRF1 at 24 hours (both P < 0.001) and in those of Nrf2 and TFAM at 48 hours (both P < 0.05),but no significant change was noted in the mRNA expression level of TFAM at 24 hours (P > 0.05) or in that of NRF1 at 48 hours (P >0.05) in the overexpression group compared with the control group.In the case of Nrf2,NRF1 and TFAM protein levels,the overexpression group showed significant increases compared with the control group at 48 hours after transfection (all P < 0.05),while no significant difference was noted between the two groups at 24 hours.Compared with the control group,MMP in the overexpression group increased by 2.313% at 24 hours (t =5.546,P =0.005) and by 14.872% at 48 hours (t =8.537,P =0.001) after transfection.Both the relative copy number of mtDNA and ATP level were similar between the overexpression group and control group at 24 hours after transfection (both P > 0.05),but significantly higher in the overexpression group than in the control group at 48 hours (t =5.760,P =0.005;t =22.040,P =0.008).Conclusion Up-regulation of Nrf2 pathway can improve mitochondrial function and biosynthesis in PIG3V cells likely by promoting the expressions of mitochondrial biosynthesis-related genes and proteins.
3.The newly reported HIV/AIDS cases and death of HIV cases from 2008 to 2009 in Henan province.

OBJECTIVETo explore the situation and cause of new reported HIV/AIDS cases and death of HIV cases in Henan province from 2008 to 2009.
METHODSThe data of HIV/AIDS cases in Henan province before the end of 2009 was downloaded from "the history card downloading site" of the national online case reporting system on Jan. 5(th), 2010. There were 6990 HIV/AIDS new cases reported from 2008 to 2009, and 1214 cases died. Descriptive study was implemented including population characteristics of new reported HIV/AIDS cases and dead cases from 2008 to 2009. Survival time and cause of death were analyzed.
RESULTSAmong all the dead cases, the Han nationality accounted for 99.0% (1202 cases), and males were more than females (62.0% (753 cases) vs 38.0% (461 cases)), 64.7% (786 cases) were married, those under junior high school education level accounted for 93.7% (1137 cases), AIDS patients accounted for 93.2% (1131 cases) and 6.8% (83 cases) were HIV carriers; 65.5% (795 cases) were former plasma donors and transfusion blood/blood produces. The main cause of death were AIDS-related diseases, the proportion was 71.9% (873 cases). The median survival time after confirmed HIV positive was 62 days (Q(L) = 14 d, Q(U) = 151 d), 35.9% (436 cases) cases died in one month and 79.0% (959 cases) cases died in six months after confirmed HIV positive.
CONCLUSIONThe newly reported dead cases of HIV/AIDS were mainly infected through former plasma donation and transfusion blood/blood produces in 2008 and 2009. The main cause of death was AIDS-related diseases.

4.Studies on HIV-1 strains in Henan province

Objective To study the epidemic status of human immunodeficiency virus type 1 (HIV-1) subtypes and sequence variation in Henan thus to explore the characteristics and sources of transmission. Methods HIV-1 env and gag gent were amplified by nested PCR from uncultured peripheral blood mononuclear cells (PBMCs) obtained from 1287 HIV-1 carriers confirmed in Henan from 2006 to 2007. 1287 env and gag genes were obtained from the patients. Results Among 1287 samples, there were 4 HIV-1 strains including subtype B', C and recombinant subtype BC and AE, accounting for 95.882%( 1234/ 1287), 0.466% (6/1287), 2.875% (37/1287), 0.777% (10/1287) respectively. In comparison with the sequence of the international strains of RLA2, C.95in21068, 07-BC.CN.97.C54A, 01AE.TH.90.CM240, the genetic divergence was 9.327%±0.245%, 5.214%±0.183%, 6.278%±0.194% and 5.332%±0.158%, respectively. Conclusion There were 4 HIV-1 strains including subtype B' , C and recombinant subtype BC and AE in Henan with main dominant subtype as subtype B' which was closely related to HIV-1 strains of Thailand B'. The major transmit route in subtype B' was through blood donation in the past years while with BC, it was through sexual transmission. The major transmit routes in subtype AE were sexual transmission and blood donation. The major route of transmission in subtype C was through sexual contact.
5.Effect of cetirizine hydrochloride on the expression of substance P receptor and cytokines production in human epidermal keratinocytes and dermal fibroblasts.

To investigate the effect of cetirizine hydrochloride on the expression of neurokinin 1 receptor (NK-1R) and cytokines production induced by substance P (SP) in HaCaT cells (a human epidermal keratinocyte cell line) and dermal fibroblasts. The effect of cetirizine on the expression of NK-1R protein was detected by flow cytometry and Western blotting analysis. The modulation of cetirizine on the production of interferon (IFN)-gamma, interleukin (IL)-1beta, IL-6 and IL-8 in HaCaT cells and fibroblasts was measured by ELISA. The results showed that cetirizine significantly inhibited the expression of NK-1R in HaCaT cells and fibroblasts. SP induced the production of IFN-gamma, IL-1beta and IL-8 in both cell types. Cetirizine 1-100 micromol x L(-1) inhibited SP-induced IL-1beta and IL-8 production in HaCaT cells and fibroblasts, while had no effect on the production of IFN-gamma in both cells. Both SP and cetirizine had no effect on the secretion of IL-6 in HaCaT cells and fibroblasts. These findings suggest that cetirizine may be involved in the treatment of SP-induced skin inflammation by inhibiting the expression of substance P receptor and regulation the production of IL-1beta and IL-8 in epidermal keratinocyte and dermal fibroblasts.

6.Detection and analysis of 26 cases of long-term non-progressors who infected HIV in Henan province.

OBJECTIVETo investigate the progression and drug resistance of long-term non-progressors during three follow-up in Henan province.
METHODSIn May 2009, 26 cases of long-term non-progressors were recruited who infected HIV more than 10 years with blood collection and supply routes, did not receive anti-retroviral therapy, CD4(+)T lymphocyte count ≥350/μl and did not show typical symptoms of AIDS from Weishi, Shangcai, and Linying of Henan Province. Continuous follow-up were conducted three times since 2009 every two years with cohort analysis, the epidemiological information of infection routes, infection time and blood were collected, and 78 parts of 10 ml EDTA anticoagulated whole blood were collected. The changes of CD4 (+) T lymphocytes, viral load, and virus gene variety were characterized from 2009 to 2014. In-house methods were used to explore primary drug resistance of long-term non-progressors. Nonparametric Kruskal-Wallis test were used to compare CD4(+) T lymphocyte count and viral load changes during different follow-up times.
RESULTSThe average age and infection time of 26 cases were (48.51 ± 6.75) years, (13.42 ± 4.26) years, respectively. Three follow-up times, CD4(+) T lymphocyte count P50 (P25-P75) was 573.5 (487.4-789.8), 499.8 (403.5-635.7), and 418.8 (297.6-537.8)/μl (H=63.99,P<0.001), respectively. And natural logarithm of viral load P50 (P25-P75) were 3.93 (3.43-4.55), 4.29 (3.78-4.75), 4.50 (4.01-4.81) (H=3.19,P=0.355), respectively. Subtype and phylogenetic analysis of HIV showed that prevalent cases were B subtype, accounting for 88.5% (23/26), and three cases showed restructuring changes. Two cases appeared highly resistant of 18 infected patients whose viral load >1 000 copies/ml.
CONCLUSIONThe CD4(+)T lymphocyte had a declining trend, virus subtype recombinant changes in a few cases, and primary drug resistance was found of long-term non-progressors in Henan province.

7.Expression of matrix metalloproteinase-9 and its complex in the urine of breast cancer patients.

OBJECTIVETo investigate the expression and clinical significance of matrix metalloproteinase-9 and its complex in the urine of the patient with breast cancer.
METHODSUsing substract gel electrophoresis and western-blot analysis, expressions of MMP-9 and MMP-9/NGAL complex in breast cancer (n = 97), breast benign (n = 41) and normal (n = 60) were observed.
RESULTSThere MMP-9 and MMP-9/NGAL complex expressions were 76.29% and 64.95% in breast cancer, 46.34% and 43.90% in breast benign, and 23.33% in normal respectively. The MMP-9 and MMP-9/NGAL complex expressions were higher in breast cancer than those in breast benign and in normal (chi(2) = 7.456, P < 0.01). MMP-9 and MMP-9/NGAL complex expressions in urine of breast cancer had not any relationship with tumor size, TNM stage, patient age, menopause status as well as ER status, but was correlated to lymphatic node status (chi(2) = 5.206, P < 0.05).
CONCLUSIONSMMP-9 and MMP-9/NGAL complex expressions in urine are significant in estimating lymphatic node metastasis in breast cancer and a valuable early prognostic factors and screening in breast cancer.

8.Analysis on sero-conversion status of HIV-discordant spouse from 2006-2010 in Henan province

Objective To study the management status of HIV-discordant family in Henan province from 2006 to 2010.Methods Data on AIDS infected family from “AIDS Infected Family Follow-up Management and Analysis System” was collected and analized to uderstand the management status of HIV-discordant situation.Results The sero-conversion rates of HIV-discordant spouse were 1.94/100 person-years in 2006,1.79/100 person-years in 2007,0.59/100 person- years in 2008 and 0.41/100 person-years in 2009,showing the yearly decrease.Regarding the number of sero-conversion in the same year,male was significantly higher than female in 2007,but not in the other years.The ratio on the course of sero-conversion which was >2.5 years was increasing.Frequency of most HIV-discordant couples' sexual activities was 1-4 times/month with consistent use of condoms.Conclusion The population of HIV-discordant spouse in Henan province was stable.Regarding management of those HIV-discordant spouse,the acceptance on the rate of test rate was increasing.The course of sero-conversion was also increasing,with most HIV-discordant couples insist on condom use while the sero-conversion rate of HIV-discordant spouse appeared to be low.
9.Survival analysis on AIDS antiretroviral therapy in Henan province during 2003-2009

Objective To study the effect of a government-provided-free highly activeantiretroviral treatment (HAART) program,on the reduction of mortality and relevant risk factorsamong adult (AIDS) patients in Henan province.Methods Data on the survival and deaths of AIDSpatients were collected from the National HAART reporting system between 2003 and 2009.Coxproportional hazards model was applied to analyze those factors that affecting the survival time of thepatients.Results 24 669 cases were enrolled to this study in Henan province,from 2003 to 2009.The overall mortality declined from 20.1/100 person-years in 2003 to 5.1/100 person-years in 2009.There was significant difference between the survival curves of different CD4 + T counts and differentnumbers of opportunistic infection syndromes.Results from the Multivariate Cox proportional hazardsregression analysis indicated that CD4+T cell counts ( >200 cells/μl,50-199 cells/μ l) was a riskfactor to death,with hazard ratio as 4.4 and 2.0 respectively.Hb of the patients that lower than 90,was a risk factor to death with the HR=1.8.Number of opportunistic infection (OIs) episodes was arisk factor to the mortality (HR=1.7).In addition,other risk factors would include age ( ≥60 years,old),being male,unmarried or divorced,ALT>100 μl,and other routes of infection,other thanFormer Plasma Donors (FPDs),with HR as 2.2,1.6,1.5,1.3 and 1.2.However,the protectivefactors would include:(1)the earlier the HAART began,the longer the survival time would last(HR =0.8 ) ; (2) when one spouse had already had the infection of HIV,it seemed helpful for the otherspouse to live longer (HR=0.8).Conclusion The National Free Treatment Program hadsignificantly reduced the AIDS mortality rate.Some effective measures should be further taken tomonitor the CD4 + T and the opportunistic infection of the AIDS patients.Patients who were in need totake the HARRT should be adopted into the ART timely,At the same time,the occurrence ofopportunistic infections should be actively prevented.
10.Factors associated with high-risk behaviors among people newly diagnosed to be infected with HIV through heterosexual contact before and after diagnosis in some areas in Henan province

Objective To understand the factors associated with high risk behaviors among people newly diagnosed to be infected with HIV through heterosexual contact before and after diagnoses in some areas in Henan province,and evaluate the risk of secondary transmission.Methods A face to face interview was conducted among people infected with HIV through heterosexual contact by using self-designed questionnaires during January-May in 2015.Results Among 361 HIV infected persons,the proportions of those with commercial heterosexual behaviors or sex with irregular sex partners decreased from 77.3%(279/361) and 28.5%(103/361) before diagnosis to 13.6% (49/361) and 2.5%(9/361) after diagnosis,the difference was significant (x2=16.66,P<0.001;x2=4.80,P=0.03).The subjects surveyed always had more commercial heterosexual behaviors in Henan,Guangdong and Zhejiang provinces before and after diagnosis.After HIV infection confirmation,the condom use rates were 51.0%(25/49) for commercial heterosexual behaviors,88.5% (184/208) for sex with regular partners and 88.9%(8/9) for sex with irregular partners,respectively.Multivariate logistic regression analysis indicated that risk behaviors associated with HIV transmission included commercial sexual behaviors,previous HIV detection and age of 35 years or older.Conclusions Extra-marital heterosexual behavior has posed serious challenge to the prevention and control of HIV spread.It is necessary to inform the HIV test results,improve the intervention and promote condom use in people with history of commercial sex and people aged ≥35 years.
