1.Expression of Lgr5 in cervical squamous cell carcinoma and its significance

Purpose To investigate the expression of Lgr5 in cervical squamous cell carcinoma (CSCC) and its significance.Methods Lgr5 protein expression was evaluated by immunohistochemistry of EnVision two-step in 83 paraffin-embedded CSCC specimens,56 normal cervical tissues specimens and 32 cervical intraepithelial neoplasia (CIN) specimens.The expression of Lgr5 mRNA was evaluated by RT-PCR in 13 pairs of surgically removed CSCC and adjacent normal cervical tissues.The correlation between Lgr5 expression and clinicopathological features were statistically analyzed.Results The positive proportion of Lgr5 protein in CSCC tissues was significantly higher than those in adjacent normal cervix (P < 0.01) and CIN (P < 0.05).The expression of Lgr5 in CSCC tissues was associated with grade of tumors (P < 0.01) and depth of invasion (P < 0.05),but not associated with age,tumor size,clinical stage and lymph node metastasis (P > 0.05).Kaplan-Meier analysis showed that the 5-year survival rate of patients with Lgr5 overexpression was significantly lower than that of patients with lower expression (P < 0.01).Conclusion The expression of Lgr5 in CSCC tissues may associated with grade of tumors and depth of invasion and may be involved in initiation and development of CSCC,and it may be a new assistant marker for prognosis of CSCC.

Objective To investigate the diagnostic value of SPECT,TCD and BAEP to vertebral basilar artery ischemia vertigo Methods 100 cases of vertebral basilar artery ischemia vertigo were inspected by SPECT,BAEP and TCD All the results were analyzed and compared Results Among 100 cases, it shows that the diagnostic rate of SPECT is 80 percent,BAEP is 58 3 percent and TCD is 78 percent Conclusion SPECT is found having the highest diagnostic rate,it sensitively and accurately reflects the position of cerebral ischemia Although BAEP having the lowest diagnostic rate, it can still effectively shows Brain Stem Ischemia Comparing with SPECT and BAEP, TCD is simple and convenient to carry out It sensitively reflects the conditions of cerebral vessel and cerebral blood stream,but it is insensitive to make sure the position Therefore, three of them offer the valuable,external and diagnostic proof to vertebral basilar artery ischemia vertigo,and they can have a role of inter reinforce
3.Effect of oxymatrine treatment on the invasion of human cervical cancer cells

Objective To investigate the anticancer effect of oxymatrine on cervical cancer cell line (HeLa). Methods MTT assay was used to detect the anti-proliferative effect of oxymatrine.Transwell chamber was used to detect the anti-metastatic effect of oxymatrine.Real-time PCR was used to detect the mRNA levels of MMP-2 and MMP-9.Western blot was used to detect the protein levels of MMP-2,MMP-9,AKT,p-AKT and GADPH. Results We found that application of oxymatrine significantly inhibited the growth of HeLa cells at the concentration above 0.8 mg/mL.We also found that oxymatrine (0.1,0.2 and 0.4 mg/mL)inhibited the invasion of HeLa cells under cytotoxic dose,which was (77.07±20.43)%,(53.95±18.17)% and (20.35±11.20)% of cells that migrated through the matrigel when compared with those of non-oxymatrine treatment group (P<0 .0 5 ). Further research found that oxymatrine (0.1,0.2 and 0.4 mg/mL)could reduce the expression of MMP-2 at the mRNA level,i.e.(82.76±8.71)%,(39.51±12.79)% and (21.53±5.38)% of the expression level when compared with that of non-oxymatrine treatment group (P<0 .0 5 ).The protein expression level of MMP-2 in 0 .4 mg/mL group was (64.69 ±16.52)% of non-oxymatrine treatment group (P<0.05).The phosphorylation level of AKT in 0.4 mg/mL group was (41.27±7.13)% of non-oxymatrine treatment group (P<0.05).Conclusion Oxymatrine can inhibit the invasion of HeLa cells by reducing the expression of MMP-2 via inhibiting the activity of AKT signal pathway.All together,our findings bring new insights into the mechanism of the anticancer effects induced by oxymatrine treatment.
4.Effect of rAAV2-TGFβ3 on collagen degradation of liver fibrosis in rats

Objective To investigate effect of transforming growth factor β3(TGFβ3)on expression of MMP-9,MMP-2,TIMP-1 and collagen I in rats with liver fibrosis through transduction by recombinant adeno-associated virus 2(rAAV2).Methods Rats were randomly divided into 4 groups:normal control group,model group,negative control group and TGFβ3 group.Liver fibrosis model was induced by hypodermic injection of 40% CCl4.rAAV2-TGFβ3,was injected via vena caudalis of the rats one week before CCl4 Was given.All rats were sacrificed and the liver tissues were taken 8 weeks afler injection of CCl4.The histopathological changes were observed on HE sections;the expressions of MMP-9,MMP-2,TIMP-1 and collagen I were examined by histochemistry and the positive area rates were semi-quantitatively analyzed.Results Compared with the model group and negative control group,the decreases of inflammatory infiltration and collagen fibers hyperplasia were observed in the TGFβ3 group,and the expression of MMP-9 increased(q=23.664,27.746,P<0.01),the expression of collagen I(q=5.503,5.251,P<0.01)and TIMP-1(q=5.800,8.608,P<0.01)decreased,but that of MMP-2 was not changed(q=2.1 08,0.996,P>0.05).Conclusion rAAV2-TGFβ3 can reduce the histological damage and degree of liver fibrosis in rats by inhibiting expression of TIMP-1 and collagen I,and upregulating expression of MMP-9.
5.Investigation into Causes and Remedial Measures for Inoculation Failure of Hepatitis B Virus-Vaccine in Children

Objective To analyze the failure ratio and the causes of the inoculation failure of hepatitis B virus(HBV)-vaccine in children and relevant the remedial measures. Methods One thousand three hundred and sixty cases treated in Suzhou Wuzhong people′s hospital during Jan.2007 to Jul.2008 were chosen,of whom 286 children from 1-5 years old to be anti-HBs negative or anti-HBs titre to be 0-10 IU/L were screened,and specific failure reasons for the vaccination were analyzed,also the timely treatment measures were taken.Then 286 children were divided into 5 groups randomly.Apart from one group was set up as blank control,the other 4 groups were arranged to accept different immunization methods with 0,1,2 month schedule,group A simply got revaccinated with HB vaccine(10 ?g) 3 times;group B revaccinated with double dosage of HB vaccine(20 ?g) 3 times;group C besides being revaccinated 3 times,the immune regulatory agent was jointly used;group D revaccinated 3 times with genetically engineered CHO hepatitis B vaccine. Results The ratio of failure of HBV-vaccine was 21.03%,what caused failure of hepatitis B vaccine included immunologic inadequacy 218(76.22%),repeated respiratory infection 192 cases(67.13%),abuse hormone 140 cases(48.95%),zinc deficiency 129 cases(45.10%),anaemia 108 cases(37.76%),passive smoking 80 cases(27.97%),the mother being chronic parenchymatous nephritis or HBV carrier 63 cases(22.03%),premature 54 cases(18.88%),adiposity 38 cases(13.29%),dystrophy 29 cases(10.14%).There were 4 methods of revaccination,the positive rate for group A,B,C,D were 90.00%,96.47%,99.08%,95.83%,respectively.Group C had the highest positive rate,compared with the other 3 groups,which were statistically significant(P a
6.Comparison of TGF-beta1 expression of lung cancer in Gejiu miners with that in non-miners in other regions.

OBJECTIVETo investigate the different expression of transforming growth factor beta(1) (TGF-beta(1)) in lung cancer specimens of Gejiu miners, and non-miners in other regions.
METHODSThirty specimens of Gejiu miners' lung cancer and 30 specimens of non-miners' were observed in this experiment. The expression of TGF-beta(1) protein and TGF-beta(1) mRNA were detected by the methods of immunohistochemistry and in situ hybridization. The results were quantitatively analyzed using image analysis system.
RESULTSThe positive rate of TGF-beta(1) protein expression in Gejiu miners and non-miners was 75.39%and 44.78% respectively, and the positive rate of TGF-beta(1) mRNA was 63.96% and 34.07% respectively. There were significant differences between the two groups (P < 0.01).
CONCLUSIONThe expression of TGF-beta(1) in lung cancer of Gejiu miners was significantly higher than that of non-miners. The pathogenesis of lung cancer may be different between Gejiu miner and non-miners. High expression of TGF-beta(1) may be one of the reasons of high incidence of lung cancer in Gejiu miners.

7.Comparision of non-fasting with fasting blood lipid testing in in-hospital patients

Objective To explore the feasibility of the application of non fasting blood lipid in the hospitalized population.Methods Self-control study was used.608 patients(aged 20~86 years old) were enrolled from April 2015 to October 2016 in lipid center of FuWai hospital.Fasting sample and non-fasting sample(1~4 h after breakfast) were collected from every patient and lipid profile including TG (triglyceride), TC (total cholesterol), HDL-C (high density lipoprotein cholesterol) and LDL-C (low density lipoprotein cholesterol) were measured in clinical laboratory.The results of two tests were compared using the Wilcoxon signed-rank test.Results The differences between non-fasting and fasting lipid test were +0.47 mmol/l (+30%) for TG,-0.03 mmol/l (-2.8%) for HDL-C,-0.09 mmol/l (-3%) for LDL-C and-0.24 mmol/l (-8.7%) for calculated LDL-C (P<0.001 respectively).The differenceswere +0.01 mmol/l for TC and +0.02 mmol/l for non-HDL-C,therefore no statistical difference was observed.When the TG level was stratified,the level of non-fasting LDL-C using directing test method was not significantly different between TG> 4.5 mmol/L and the whole (0.07 vs.0.09),but the level of non-fasting LDL-C using formula method wassignificantly different between TG> 4.5 mmol/L and the whole (0.66 Vs.0.24),andthe drops were 34.9% vs.8.7%.Conclusion Non-fasting lipid test could be an effective routine method for lipid evaluation in the hospitalized population.
8.Modified laparoscopic hepatic hemangioma resection

Objective To evaluate modified laparoscopic hepatic hemangioma extraction methods during laparoscopic hepatectomy.Methods From May 2009 to June 2013, 25 hepatic hemangioma patients were admitted for laparoscopic hepatectomy.Patients were divided into modified laparoscopic group with modified laparoscopic hepatic hemangioma extraction methods (MLH, n =11) and traditional laparoscopic group (TLH, n =14).Perioperative clinical parameters were compared between the two groups.Results Operative time (148.6 ± 18.0) min, time to removal of the HCH (15.7 ±4.6) min, visual analog scale pain score on the first postoperative day (2.3 ± 0.9), time to off-bed activity (2.5 ± 0.8) d, and postoperative hospital stay (8.5 ± 2.6)d in the MLH group were all less than those in the TLH group [(163.9±13.2)min, (33.9±3.1)min, (3.4±0.9), (3.1±0.8)d, (10.6±1.9)d] (t=-2.451,t =-11.800, t =-2.931, t =-2.156, t =-2.338,P < 0.05).There were one in the MLH group and two cases in the TLH group developing postoperative complications (P =1.000).Conclusions Modified laparoscopic hepatic hemangioma extraction methods during laparoscopic hepatectomy for hepatic hemangioma is safe, feasible and of good therapeutic effect.
9.Damage control surgery for treatment of flail chest combined with multiple trauma

Objective To investigate clinical effect of damage control surgery (DCS) in treatment of patients with flail chest combined with severe multiple injuries.Methods A total of 187 cases of flail chest combined with severe multiple injuries treated by fixation of floating chest wall were enrolled and divided into three groups on the basis of different treatments:DCS group (66 cases) underwent early suspension traction of ribs and delayed internal fixation of the ribs ; Group A (70 cases) underwent rib suspension traction alone; Group B (51 cases) underwent initial internal fixation of rib.Complications,mortality,and main parameters before and after operation in each group were analyzed and compared.Results Complications including pulmonary infection (32 cases),atelectasis (38 cases),and acute lung injury (ALI)/ARDS (39 cases) were found.Twenty-two cases died,including 13 deaths from ARDS,two from tension pneumothorax,one from massive hemoptysis,three from cardiac shock,two from craniocerebral injury,and one from liver trauma and thus the overall death rate occupied 11.8%.Oxygenation index (OI) had significant rise postoperatively both in the DCS group and Group A (P < 0.01),but the change of OI was inappreciable in Group B.Mortality,complication rate,cases treated with mechanical ventilation,tracheotomy or fiberoptic bronchoscopy,and average length of ICU and hospital stay were the lowest in the DCS Group,followed by a relatively higher result in Group B and a much higher result in Group A (P < 0.01).Conclusion DCS decreases mortality and complications dramatically when appolied to treat flail chest combined with severe multiple trauma.
10.Spleen conserving laparoscopic azygoportal disconnection for cirrhotic portal hypertension

Objective To investigate the feasibility and safety,and short-term therapeutic effect of laparoscopic azygoportal disconnection without splenectomy for cirrhotic portal hypertension (PLT count > 50 × 109/L).Methods Clinical data of 48 patients with bleeding portal hypertension and secondary hypersplenism (PLT count > 50 × 109/L) undergoing laparoscopic splenectomy and azygoportal disconnection (LSD,n =26) vs.laparoscopic azygoportal disconnection (LD,n =22) between January 2014 and August 2015 were analyzed.Results Operative time (82 ± 29) min,intraoperative blood loss 20(10-50) ml,days of postoperative fever 0(0-3) d,rate of postoperative fever 10/22,postoperative hospital stay (7.0 ± 1.3) d,and WBC counts (3.8 ± 1.6) × 109/L,PLT counts 64 (49-88) × 109/L,and the incidence of portal vein thrombosis on POD 7 (14%),were significantly less in LD group than in LSD group [(180±41) min,80(20-500) ml,2(0-4) d,(22/26),(10.8 ±3.0) d,(9.1 ±3.1) × 109/L,156 (78-630) × 109/L,(42%)],(t =9.637,Z =-4.746,Z =-2.314,x2 =8.224,t =5.794,t =7.785,Z=-5.508,x2 =4.742,all P < 0.05).Immune function was better in LD group than in splenectomy group at postoperative month 3.The serum proportion of CD4 + (58 ± 11) and the CD4 +/CD8 + ratio (1.9 ±0.7) at postoperative month 3 were significantly higher after LD than after LSD [(43 ± 14),(1.2 ± 0.9)],(t =-3.755,t =-2.509,all P < 0.05).Conclusion Laparoscopic azygoportal disconnection without splenectomy is safe and effective for esophagogastric variceal hemorrhage and moderate hypersplenism (PLT > 50 × 109/L) secondary to portal hypertension.
