Objective To investigate the effects of meglumine adenosine cyclophosphate administrated before percutaneous coronary intervention (PCI) on curative efficacy, cardiac function and oxidative stress in patients with acute myocardial infarction (AMI).Methods118 patients of AMI meeting the inclusion criteria were selected as research objects.According to therapeutic schemes, those patients were divided into the control group (n=52) and the observation group (n=66).Before PCI, the control group was given conventional treatments.Besides that, the observation group was given meglumine adenosine cyclophosphate.Then, the curative efficacy, indices of cardiac function and oxidative stress were compared.ResultsAfter the treatments, the observation group had higher rates of ST decrease (>50%), thrombolysis in myocardial infarction(TIMI)3 grade (90.9% vs76.9%,84.8% vs 69.2%, P<0.05) and lower peak concentration of creatine kinase isoenzyme(CK-MB), cardiac troponin(cTn)I[(140.6±26.5) U/L比(193.7±30.8) U/L,(1.92±0.24)μg/L 比(2.71±0.32) μg/L,P<0.05].After the treatment, indices of stroke volume (SV), cardiac output (CO) and left ventricular ejection function (LVEF) which reflect cardiac function in the observation group were statistically higher than those in the control group [(57.26±9.37)mL vs.(52.16±9.15)mL,(5.23±1.17) L/min vs(4.49±1.12)L/min,(0.62±0.12) % vs.(0.51±0.15)%,P<0.05].24 hours after PCI, as to oxidative stress, the observation group had a statistically lower level of malondialdehyde(MDA) and higher level of superoxide dismutase[(4.39±0.43)nmol/L vs.(6.45±0.46) nmol/L, [(87.12±10.34)U/mL vs.(68.29±9.48)U/mL), P<0.05].ConclusionMeglumine adenosine cyclophosphate administrated before PCI is effective for patients with AMI.And it can significantly improve cardiac function and alleviate oxidative stress.

Objective To investigate the effect of rosuvastatin on vascular endothelial function and inflammatory factors in patients with acute myocardial infarction ( AMI) after PCI.Methods A total of 217 patients from May 2014 to May 2016 in The Second Hospital of Baoding were randomly divided into the observation group 109 cases and the control group 108 cases.Two groups were treated with conventional basic therapy,and the control group was treated with atorvastatin,while the observation group was treated with rosuvastatin.The clinical efficacy,the levels of FMD and NMD,vascular endothelial function and inflammatory factors were compared between the two groups,and adverse reactions were recorded.Results The effective rate of the observation group was 95.41%,significantly higher than that of 86.11% in the control group (P<0.05);the levels of FMD and NMD in the two groups were significantly higher than before treatment,and the observation group was higher than the control group(P <0.05);the level of ET-1 was significantly decreased,and the levels of NO and NOS were significantly higher in the two groups,and the changes in the observation group were more significant(P<0.05);the levels of hs-CRP,TNF-α,IL-8,IL-6 was significantly decreased,the observation group was significantly lower than the control group( P <0.05 );and no serious adverse reactions were observed.Conclusion Rosuvastatin can improve the vascular endothelial function and inflammatory factor level of AMI patients after PCI,improve the curative effect and improve the quality of life of patients.

Objective To investigate the possible association between vascular endothelial growth factor (VEGF), endostatin (ES) and type 2 diabetic nephropathy. Methods The level of serum VEGF and ES were measured by ELISA in eighty type 2 diabetic patients including thirty subjects without diabetic nephropathy , twenty with microalbuminuria and thirty with macroalbuminuria, and the correlation was analyzed. Results The level of serum VEGF and ES in patients with macroalbuminuria were both significantly higher than that of patients with and without microalbuminuria (P < 0.05). The level of serum VEGF and ES in patients with microalbuminuria were significantly higher than that of patients without diabetic nephropathy (P<0.05). Both VEGF and ES level were positively correlated with the level of microalbuminuria (mAlb) (r=0.226, P<0.01; r=0.491, P<0.01), and negatively correlated with the level of estimated glomerular filtration rate (eGFR) (r=-0.237, P<0.05;r=-0.620, P<0.01). There was a significantly positive correlation between VEGF and ES in patients with microalbuminuria and macroalbuminuria (r1=0.633, P1<0.01; r2=0.532, P2<0.05). However, there was no correlation between VEGF and ES in patients without diabetic nephropathy (r3=0.175, P3>0.05). Conclusion The levels of VEGF and ES in type 2 diabetic nephropathy had varying degree of increasing in different diabetic nephropathy stage , and closely related to mAlb and eGFR. Disequilibrium of VEGF and ES may speed up the progression of diabetic nephropathy.

Objective To explore the differences of presonal backgrounds of patients on their perceived value and service strategy for such patients.Methods Based on a literature review,the survey data of patient perceived value of ten hospitals from 2011 to 2013 were selected for an analysis of typical influences on their perceived value incurred by their background characteristics.Results The patient perceived values of the highest attention were functional value(3.91 in average)and emotional value(3.92 in average);regular changes toward these two values were found among patients of different backgrounds.Comparatively patients tend to demand higher emotional value featuring outpatient visit,minor symptoms,females,youths,higher education background and high income.Conclusions Medical institutions are expected to clarify their value positioning,and provide based on such differentiated services to their patients in view of the characteristic backgrounds and medical visits,thus improving the perceived value of their patient population in general.

Objective To evaluate the role of MR imaging (MRI) and MR cholangiography (MRC) for the diagnosis of hepatic hilar cholangiocarcinoma (HHC) and assessment of tumor resectability. [WT5”HZ]Methods [WT5”BZ] MRI and MRC were performed in 280 patients with obstructive jaundice from Mar.1997 to May.1999. The diagnosis of HHC was proved in 38 cases (13 6%, 38/280) by laparotomy and pathology. Imaging features and classification as related to surgical therapy of the tumor were analyzed. [WT5”HZ]Results [WT5”BZ] Tumor was identified in 24 out of 38 HHC cases. The contrast enhancement scanning further found tumors in 8 out of 14 HHC cases,with a total rate of tumor finding of 84% (32/38). MRI also suggested hepatic metastasis in 4 cases, portal vein invasion in 7 cases. MRC found hilar obstruction and dilatation of the intrahepatic biliary tree in all HHC cases. The surgical resection rate was 24%(9/38) including 4 cases of typeⅠ,3 cases of typeⅡ,1 of type Ⅲa and 1 of typeⅢb. Resection was impossible or not worthwhile in cases with hepatic metastasis, invasion of portal vein and/or hepatic artery. Conclusion [WT5”BZ]MRI and MRC plays an important role in the diagnosis of hepatic hilar cholangiocarcinoma and assessment of tumor resectability.

OBJECTIVE: To probe into the application status of the microbial ecosystem agents in our hospital.METHODS: By a retrospective method,the application of the microbial ecosystem agents in our hospital from 2007 to 2008 was analyzed statistically.RESULTS: 14.32% of the outpatients used microbial ecosystem agents,and the consumption sum of which accounted for 4.16% of total.The rational use rate was 81.26%.CONCLUSION: The use of microbial ecosystem agents in our hospital was rational on the whole;however,measures should be taken to tighten control on the rational use of which to prevent beyond-range use so as to reduce medical expenses for children patients.

Objective To introduce the Bernese periacetabular osteotomy through ilioinguinal approach for the treatment of adult acetabular dysplasia. Methods From October 1997 to April 2001, 51 patients(53 hips) with acetabular dysplasia were operated upon with Bernese periacetabular osteotomies through ilioinguinal approach. The mean age of the patients at the time of surgery was 30.6 years(range, 13-48 years). Male∶female= 1∶10. Preoperatively, the average duration of the symptoms was 3.4 years(3 months to 15 years), the range of motion(ROM) of the hip was normal in most patients. Average CE angle was 1.1?(?15?-15?); the Tonnis osteoarthritis grade was Ⅰto Ⅱ. Fifty three percent of the hips had Shentons line broken. Results Twenty four hips had 12 to 38 months followed up. Most patients were pain free; the hip ROM remained normal. Harris hip score increased from 79.8(64-83) pre operatively to 93.8(75-100) post operatively. CE angle increased to 40?(20?to 60?). Shentons line broken rate decreased to 33%. The complications include: 1 femoral nerve damage, 1 bladder malfunction and 2 skin incision hernia. Conclusion The Bernese periacetabular osteotomy through ilioinguinal approach is a safe and efficient procedure for the treatment of adult acetabular dysplasia. The deformities could be easily corrected, the anatomic relations could be restored nicely, the pelvic ring and the abductor muscles are reserved. The post operative recovery is fast, and it is efficient to prevent the development of osteoarthritis of the hip joint.

Objective: To investigate the difference in behavioral characteristics among different phenotypes of attention deficit hyperactivity disorder (ADHD) using amplitude of low-frequency fluctuation (ALFF), so as to provide insights into clinical differentiation of behavioral characteristics among different phenotypes of ADHD. Methods: The children with ADHD admitted to The Affiliated Hospital of Hangzhou Normal University were enrolled and classified into the inattentive type (ADHD-I), hyperactive/impulsive type (ADHD-HI) and combined type (ADHD-C). The reaction time (RT) was measured using integrated visual and auditory continuous performance test, and the mean (RT-mean) and standard deviation of RT (RT-SD) were estimated. In addition, the ALFF was calculated at 0.010 to 0.027, 0.027 to 0.073, 0.073 to 0.167 Hz, and the difference of ALFF was compared among children with different types of ADHD. Results: A total of 107 children with ADHD were enrolled, including 95 boys and 12 girls, with a mean age of (8.89±1.93) years. There were 69 children with ADHD-I, 8 children with ADHD-HI and 30 children with ADHD-C. The RT-SD was significantly higher among children with ADHD-C than among children with ADHD-I [(126.003±51.619) ms vs. (97.720±45.302) ms; P=0.007]; however, there was no significant difference in RT-mean among children with various ADHD phenotypes (F=1.386, P=0.255). There was an interaction between frequency and ADHD phenotypes (F=2.754, P=0.032), and the ALFF was significantly higher among children with ADHD-C than among children with ADHD-I at 0.010 to 0.027 [(5 590.567±231.595) ms vs. (4 694.001±154.397) ms; P=0.002] and 0.073 to 0.167 Hz [(4 312.609±174.709) ms vs. (3 690.805±116.473) ms; P=0.005]. Conclusions The ALFF varies in ADHD phenotypes, and there is a frequency-specific difference.

OBJECTIVE:To provide reference for rational use of carbapenem antibiotics in the clinic. METHODS:Enterobac-teriaceae bacteria were collected from our hospital during Jan. 2014-Dec. 2015;semiautomatic microbiological assay instrument was used for strain culture,identification and drug sensitive tests. Modified Hodge test and K-B test were adopted to confirm Klebsiella pneumoniae carbapenemases (KPC)-producing and ESBLs-producing drug resistant strains. RESULTS:During 2014-2015,1 035 strains of Enterobacteriaceae bacteria were detected in our hospital,among which there were 732 strains of Escherichia coli,157 strains of K. pneumonia,136 strains of Enterobacter cloacae and 10 strains of Serratia marcescens. Citrobacter freundii was not found. E. coli and K. pneumonia were highly sensitive to amikacin and carbapenems,but slightly sensitive to most cephalosporin. A total of 64 strains of carbapenems-resistant Enterobacteriaceae bacteria(6.18%)were detected,among which there were 31 strains of carbapenems-resistant E. coli(4.23%),30 strains of carbapenems-resistant K. pneumonia(19.11%),1 strain of carbapenems-re-sistant E. cloacae(0.74%)and 2 strains of carbapenems-resistant S. marcescens(20.00%). The samples were mainly from sputum and urine specimens,which were mainly from neonatal department and ICU. Of 64 drug resistant strains,there were 59 KPC-pro-ducing strains (92.19%) and 3 ESBLs-producing strains (4.69%). CONCLUSIONS:E. coli occupies high proportion among En-terobacteriaceae bacteria,and the number of carbapenems-resistant E. coli and carbapenems-resistant K. pneumoniae is in high lev-el. Drug resistance of Enterobacteriaceae bacteria to carbopenems may be associated with KPC and ESBLs producing. Carbapenem antibiotics should be selected rationally in accordance with medication indications and the results of drug sensitivity test.

OBJECTlVE To investigate the myocardiaI toxicity of doxorubicin on the myocardium of rabbits and mechanism. METHODS Doxorubicin 2 mg·kg-1 was injected once a week for eight weeks. After discontinuation of doxorubicin,observation was performed for another 8 weeks. Every weekend, uItrasound examination,cardiac catheterization,angiotensinⅡ(AngⅡ)Western bIotting and pathoIogi-caI examination were performed to anaIyze eject fraction( EF),maximaI rate of rise of Ieft ventricuIar pressure(+dp/ dtmax ),AngⅡexpression IeveI,apoptosis index(AI)and the structure of the myocardium. RESULTS At the 7th injection,EF decreased( P ﹤0.05),but reached the bottom vaIue at the 8th injection. At the 3rd injection,Ieft ventricuIar +dp/ dtmax decreased( P ﹤0.05)and reached the bottom vaIue one week after withdrawaI. After that,it increased and reached a high vaIue six weeks after withd-rowaI. But it was stiII Iower than before administration. At the 2nd injection,AngⅡ expression increased (P﹤0.05). At 1 week after withdrawaI,it reached the top vaIue,but than decreased and reached a Iow vaIue six weeks after withdrowaI,but was stiII higher than before administration. At the 1st injection,AI increased( P ﹤ 0.05). At 1 week after withdrawaI,it reached the top vaIue,but then decreased and reached a Iow vaIue 5 weeks after withdrawaI. But it was stiII higher than before administration. CONCLUSlON Doxorubicin cardiac toxicity can induce an eIevated IeveI of myocardiaI AngⅡ,possibIy associated with increased aIdosterone and myocardiaI tension. Increased Ang Ⅱ may induce further myocardiaI structuraI damage and ventricuIar remodeIing through the ROS and caIcium imbaIance.
