1.Research of the face effect on its interference to the survey for patient perceived value

Objective To analyze the characteristic influence and deviations caused by the face effect to the survey outcomes of the patient perceived value (PPV) of medical treatment.Methods Based on the theoretical research,a PPV test & evaluation plan is designed for two groups of patients.Comparative study of outcomes of the two groups aims at identifying patterns in this regard.Results Findings of the patients evaluation showed that the face effect poses strong interference for the survey outcomes,as it significantly raises the evaluation outcomes.Among the factors causing face effect,characteristics of the investigator in the survey are found to be the most important stimulating,which include the power distance and the strength of relationship between patients and the hospital.Such factor will push higher the subjective PPV of the patient,while the general satisfaction of the medical workers in the survey can be 0.24 higher than that of third-party survey.In addition,friends and patients in the same wards are also an important factor to cause the face effect of the patient,while such factor works at a wide range to push higher both objective and subjective estimation value of the patient.For joint actions of the investigator and the reference group,the general satisfaction between the two-high scenario and the two-low one can be up to 39.43%.Conclusion The PPV surveys should minimize interference of the face effect by means of evaluating environment redesign,avoidance of the friends and relatives,and selection of optimal investigators,for the purpose of eliminating interference of the face effect.
2.The relationship between vascular endothelial growth factor, endostatin and type 2 diabetic nephropathy

Objective To investigate the possible association between vascular endothelial growth factor (VEGF), endostatin (ES) and type 2 diabetic nephropathy. Methods The level of serum VEGF and ES were measured by ELISA in eighty type 2 diabetic patients including thirty subjects without diabetic nephropathy , twenty with microalbuminuria and thirty with macroalbuminuria, and the correlation was analyzed. Results The level of serum VEGF and ES in patients with macroalbuminuria were both significantly higher than that of patients with and without microalbuminuria (P < 0.05). The level of serum VEGF and ES in patients with microalbuminuria were significantly higher than that of patients without diabetic nephropathy (P<0.05). Both VEGF and ES level were positively correlated with the level of microalbuminuria (mAlb) (r=0.226, P<0.01; r=0.491, P<0.01), and negatively correlated with the level of estimated glomerular filtration rate (eGFR) (r=-0.237, P<0.05;r=-0.620, P<0.01). There was a significantly positive correlation between VEGF and ES in patients with microalbuminuria and macroalbuminuria (r1=0.633, P1<0.01; r2=0.532, P2<0.05). However, there was no correlation between VEGF and ES in patients without diabetic nephropathy (r3=0.175, P3>0.05). Conclusion The levels of VEGF and ES in type 2 diabetic nephropathy had varying degree of increasing in different diabetic nephropathy stage , and closely related to mAlb and eGFR. Disequilibrium of VEGF and ES may speed up the progression of diabetic nephropathy.

4.A Meta-analysis of the accuracy of interferon-γrelease assays in differentiating intestinal tuberculosis from Crohn's disease in Asia

Objective This meta-analysis assessed the value of interferon-γ release assays (IGRAs) in the differential diagnosis of intestinal tuberculosis (ITB) from Crohn's disease (CD).Methods Systematic search without language restriction was conducted in the main computerized databases until June 2015.Studies that have evaluated the performance of IGRAs (QuantiFERON-TB Gold or T-SPOT.TB) in distinguishing ITB from CD were eligible.Main outcome measures included sensitivity and specificity.Area under the curve (AUC) of the summary receiver operating characteristic (sROC) curve was used to evaluate the accuracy of IGRAs.Results Twelve studies (all from Asia) were finally included.The pooled sensitivity and specificity of IGRAs for the differential diagnosis of ITB from CD were 82.8% (95%CI 78.4%-86.6%) and 86.7% (95% CI 83.2%-89.6%) respectively.The positive likelihood ratio (PLR) and negative likelihood ratio (NLR) were 6.870 (95% CI 5.345-8.830) and 0.171 (95% CI 0.105-0.279).The diagnostic odds ratio was 44.030 (95% CI 27.964-69.325).And the AUC of sROC was 0.939.Conclusions IGRAs have a high sensitivity and specificity for the diagnosis of ITB,and specificity is consistent from study to study.IGRAs may be considered as a supplementary method in the differential diagnosis of ITB from CD.
5.Clinical Application of Microbial Ecosystem Agents in Pediatrics

OBJECTIVE: To probe into the application status of the microbial ecosystem agents in our hospital.METHODS: By a retrospective method,the application of the microbial ecosystem agents in our hospital from 2007 to 2008 was analyzed statistically.RESULTS: 14.32% of the outpatients used microbial ecosystem agents,and the consumption sum of which accounted for 4.16% of total.The rational use rate was 81.26%.CONCLUSION: The use of microbial ecosystem agents in our hospital was rational on the whole;however,measures should be taken to tighten control on the rational use of which to prevent beyond-range use so as to reduce medical expenses for children patients.
6.Identification of human papillomavirus in vulvar intraepithelial neoplasia

Objective To evaluate the rate and types of human papillomavirus (HPV) in vulvar intraepithelial neoplasia (VIN).Methods We detected HPV DNA in 67 lesion tissues collected from 40 VIN patients with PCR based reverse line blot hybridization and DNA sequencing. The PCR using GP5/GP6 primers in HPV L1 region resulted in a 150-bp fragment. When the PCR did not amplify the target DNA,another PCR using SPF1/SPF2 primers was performed to amplify the 65-bp fragment.Results HPV DNA amplified with primers GP5/GP6 was positive in 52.2% (35/67) of the lesions. Of the 32 negative lesions,26 (81.2%) were positive for HPV DNA amplified with SPF1/SPF2 primers. The total positive rate was 91.0% (61/67). Ninety percent of the HPV belonged to high risk types. Sequencing data showed that the genotypes of 31 mono-infection lesions were the same as those of the reverse line blot results,yet the sequences in 4 multi-infection samples could not be determined. Of the 26 SPF PCR products,24 could be sequenced. 80.6% (25/31) patients with multiple lesions displayed the same genotype,suggesting HPV in the different lesions from the same patient was monoclonal.Conclusion The high risk type of HPV is associated with vulvar intraepithelial neoplasia and may play an important role in the development of invasive vulvar carcinoma.
7.Protein kinase C-α expression in kidney of rat with chronic arsenic poisoning

ObjectiveTo investigate the expression and relevant function of protein kinase C (PKC)-α in kidney of rat with chronic arsenic poisoning.MethodsTotally 60 healthy SD rats of clean grade were randomly divided by body weight into 3 groups:high-dose arsenic exposure group (10.0 mg/kg),low-dose arsenic exposure group (0.4 mg/kg),and control group.The rats were exposed by drinking arsenic solution which was mixed with distilled water.Rats were weighed every 10 days and dose volume of arsenic solution was adjusted.After continuous exposure for 4 months,blood and urinary arsenic were determined.Rat kidneys were taken and stained by Immunohistochemistry SABC.PKC-o positive cells in the kidney were observed and counted,and its average gray value was analyzed with image analysis software (Biomias).ResultsProximal tubules PKC-α-positive cell count [(3.62 ± 1.90),(10.07 ± 3.22)/field],glomerular PKC-α-positive cell count [(3.62 ± 1.90),(10.07 ± 3.22)/field]in high and low arsenic group of SD rat kidney were lower than those of the control group [(60.00 ± 9.63),(18.57 ± 2.71/field,all P < 0.05]; both urinary arsenic level[(7366.62 ± 1086.50),(1744.31 ± 300.12)μg,/L]and blood arsenic level [(31.59 ± 9.24),(16.58 ± 2.08)μg/L] in high-dose and low-dose groups were higher than those of the control group [(18.97 ± 3.58),(18.97 ± 3.58)μg/L,all P < 0.05] ; the average gray values of SD rat kidney proximal tubule,glomerular PKC-o positive cells in high-dose and low-dose groups( 142.79 ± 11.16,122.15 ±5.91 ) were higher than that of the control group (114.33 ± 6.70,all P < 0.05).ConclusionsArsenic can decrease SD rat kidney PKC-α -positive cells.The regulatory function of PKC-o in inhibiting cell apoptosis of kidney of rats with arsenic poisoning is weakened.
8.Primary discussion of qualitative and quantitative recognition on deqi after acupuncture: a study report of West China School of Medicine.

OBJECTIVETo determine the recognition of healthy medical students on deqi after acupuncture, reveal the qualitative and quantitative rules of deqi and understand whether these rules are the factors of the clinical application of acupuncture therapy.
METHODSThe class questionnaires were used for the investigation study on the understanding of deqi after acupuncture at Hegu (LI 4) or Zusanli (ST 36) in 86 healthy students in the clinical medicine class.
RESULTS(1) Deqi was a kind of complicated compound feelings, with many sensation qualities such as distending pain, distension and pain. (2) Deqi was a kind of mild and moderate sensations. In 10-score credit sys tem of Massachusetts General Hospital acupuncture sensation scale (MASS), the scores of distending pain (4.69 +/- 2.83), distension (4.39 +/- 2.91) and soreness and distension (3.93 +/- 2.93) were around 5 (moderate degree), the scores of stabbing pain (1.89 +/- 2.02) were around 2 (mild degree). (3) The differences in the quantitative scores were significant for stabbing pain, distending pain, distention, soreness and distention and the others before and after treatment (all P<0.05).
CONCLUSIONDeqi of acupuncture is the mild and moderate complicated sensations manifested as distension, soreness, pain and numbness. As the invasive therapy, the filiform needle puncture will bring a certain psychological impacts on the receptors. The subjective sensation is possibly the factor for the patients' selection of acupuncture treatment.
