2.Efficacy of different surgical methods in the treatment of papillary thyroid carcinoma of the isthmus and their effects on parathyroid function and thyroglobulin

Objective:To investigate the therapeutic effects of different surgical methods on papillary thyroid carcinoma of the isthmus (PTCI) and their effects on parathyroid function and thyroglobulin.Methods:Eighty patients with PTCI who underwent treatment in Zhejiang Xin'an International Hospital from January 2016 to January 2021 were included in this study. They were randomly allocated to undergo ipsilateral lobectomy with removal of the isthmus combined with ipsilateral central neck lymph node dissection (group A, n = 40) or total thyroidectomy combined with ipsilateral central neck lymph node dissection (group B, n = 40). We compared intraoperative and postoperative conditions, complications, and hypoparathyroidism between the two groups. We also compared serum thyroglobulin level measured before and 3 days after surgery between the two groups. Results:Operative time was significantly shorter in group A than in group B [(78.95 ± 13.52) minutes vs. (104.23 ± 27.38) minutes, t = -5.23, P < 0.05]. Intraoperative blood loss was significantly less in group A than in group B [(52.32 ± 6.59) mL vs. (75.41 ± 9.98) mL, t = -12.21, P < 0.05]. There were no significant differences in voice handicap index and reflux symptom index scores between the two groups (both P > 0.05). The incidence of complications was significantly lower in group A than in group B (10.00% vs. 30.00%, χ2 = 5.00, P < 0.05). The incidence of hypoparathyroidism was significantly lower in group A than in group B (7.50% vs. 27.50%, χ2 = 5.54, P < 0.05). At 3 days after surgery, serum thyroglobulin level was significantly lower in group A than in group B [(0.82 ± 0.17) μg/L vs. (1.26 ± 0.23) μg/L, t = -9.73, P < 0.05]. Conclusion:Ipsilateral lobectomy with removal of the isthmus combined with ipsilateral central neck lymph node dissection is more effective on PTCI than total thyroidectomy combined with ipsilateral central neck lymph node dissection. The former has little effect on parathyroid function and can reduce serum thyroglobulin level. The study is highly innovative and scientific.

Methods:
Twenty-eight healthy men in their 20s were classified into 13 with flat foot and 15 with normal foot through evaluation of navicular drop test. Using a motion analysis system, loading rate (N/sec), peak vertical ground reaction force (N/body weight [BW]), sagittal plane hip, knee, and ankle joint range of motion (°), peak moment (Nm/BW), peak joint power (W/BW) and peak joint work (J/BW) were calculated and analyzed during single leg drop landing.
Results:
During single leg drop landing, the flat foot and normal foot groups showed no significant differences in loading rate, peak vertical ground reaction force, hip and knee joint range of motion, peak knee and ankle joint moment, peak joint power, and peak joint work (p> 0.05). However, the flat foot group showed greater ankle range of motion and peak hip joint flexion moment compared to the normal foot group (p=0.040 and p=0.018, respectively).
Conclusion
The flat foot group shows sagittal plane landing mechanics that are different from the normal foot group during single leg drop landing and appears to try to distribute shock by relying on the distal joint compared to the normal foot group.

9.Influenza-associated-excess-hospitalization in children,Wuxi city,Jiangsu province,2005-2010

Objective To estimate the rates due to influenza-associated-excess-hospitalization in children aged 0-14 years in Wuxi city,Jiangsu province in 2005-2010. Methods We collected data on hospitalization due to influenza,pneumonia and other respiratory diseases from fourteen 2nd level or above hospitals in Wuxi,as well as data on influenza virological surveillance in southern China to fit the negative binomial regression models,to estimate the rate on influenza-associated-excess hospitalization. Results During 2005-2010,an average annual hospitalization rate appeared as 91.6‰(79.2‰-99.3‰). Among the total hospitalization eases,respiratory diseases accounted for 54.2%,while both influenza and pneumonia accounted for 38.1%. The average annual influenza-associated-excess-hospitalization rates due to influenza and pneumonia appeared as 1.28‰(95%CI:0.29‰-4.84‰),while 2.18‰(95%CI:0.61‰-6.79‰) due to respiratory diseases. In 2009,A(H1N1)pdm induced influenza pandemic caused 993 excess hospitalizations due to influenza/pneumonia and 1 042 excess hospitalizations due to respiratory diseases,with rates as 1.14‰ and 1.20‰ respectively. Conclusion Both seasonal and pandemic A(H1N1)pdm influenza caused considerable burden on hospitalization in children aged 0-14 years in Wuxi.
10.Metagenomic next-generation sequencing-based retrospective investigation of the drug resistance sites of Mycoplasma pneumoniae in children

Objective:To analyze the drug-resistant gene loci of Mycoplasma pneumoniae (MP) using metagenomic next-generation sequencing (mNGS). Methods:From November 2022 to October 2023, 697 clinical samples (including sputum, alveolar lavage fluid and blood) of 686 children with Mycoplasma pneumoniae positive detected by mNGS were retrospectively analyzed. Samples were divided into intensive care unit (ICU) group and non-ICU group, Chi-square test was used to compare groups, and Mann-Kendall trend test was used to analyze the change trend of the detection rate of drug resistance gene loci over time. Results:Of the 697 samples, 164 were from the ICU group and 533 were from the non-ICU group. The detection rate of Mycoplasma pneumoniae resistance gene was 44.3% (309/697), and all detected drug-resistant gene loci of MP were A2063G. The detection rate of Mycoplasma pneumoniae in ICU group was 50.0% (82/164), and the detection rates of Mycoplasma pneumoniae resistance gene loci in sputum, alveolus lavage fluid and blood samples were 75.0% (18/24) and 48.4% (62/128), respectively. The detection rate in sputum was higher than alveolus lavage fluid samples ( χ2=5.72, P=0.017). The detection rate of Mycoplasma pneumoniae in non-ICU group was 42.6% (227/533), the detection rate of Mycoplasma pneumoniae resistance gene loci in sputum and alveolar lavage fluid was 40.0% (16/40), 44.3% (201/454), and no detection rate in blood samples (0/12). There was no significant difference in the detection rate of alveolar lavage fluid and sputum ( χ2=0.27, P=0.602). From November 2022 to October 2023, the detection rate of submitted samples showed an increasing trend month by month (overall: Z=3.99, ICU inspection group: Z=2.93, non-ICU group: Z=3.01, all P<0.01). Among the bacteria commonly detected with Mycoplasma pneumoniae, Streptococcus pneumoniae accounted for the highest proportion, the detection rate was 15.5% (108/697), and Epstein-Barr virus accounted for the highest proportion of 17.6% (123/697). Conclusions:From November 2022 to October 2023, the detection rate of Mycoplasma pneumoniae drug resistance gene loci showed an increasing trend. The detection rate of drug resistance gene loci in sputum samples of ICU group was higher than alveolus lavage fluid. No new drug resistance site were detected.
