1.Clinical therapeutic effect of transradial emergency PCI in 50 patients with acute myocardial infarc-tion

Objective:To analyze the salvage effect of transradial emergency percutaneous coronary intervention (PCI)on patients with acute myocardial infarction (AMI).Methods:A total of 100 AMI patients,who hospitalized in our hospital from Feb 2009 to Jun 2013 and received PCI treatment,were randomly selected as subjects.They were divided into transfemoral artery intervention group (TFA group,n=50)and transradial artery intervention group (TRA group,n=50)according to random number method.Door-to-balloon time,operation duration,expo-sure time of X-ray,pressing time of puncture point,length of hospital stay,success rate of operation and incidence rate of complications were compared between two groups.Results:There were no significant difference in operation duration and success rate of operation between two groups (P>0.05);compared with TFA group,there were sig-nificant reductions in pressing time of puncture point [(14.8±5.7)min vs.(3.9±1.6)min]and incidence rate of complications (16.0% vs.4.0%),and significant rise in door-to-balloon time [(18.7±5.6)min vs.(20.1±6.7) min],exposure time of X-ray [(16.7±5.6)min vs.(22.1±6.7)min]in TRA group,P<0.05 or<0.01.Con-clusion:Compared with transfemoral coronary intervention,the pressing time of puncture point of transradial inter-vention therapy and complication are less,so it can be used as preferred therapeutic method for acute myocardial in-farction,and especially more suitable for primary hospitals.
2.Advances in the development and influence factors of osteophytes in osteoarthritis

Osteoarthritis (OA) is a common chronic arthritis, mainly suffering the middle and elderly patients, and it is the main cause of arthralgia and articular functional disturbance. Studies have suggested that osteophyte formation was closely con?nected with the development of OA due to the stabilization of suffered joints and arthralgia induced by osteophytes. In tissue con?stitution and pathology, osteophyte is made up of bone, cartilage and connective tissue, stemming from MSCs. With the develop?ment of osteophytes, the proportion of bone increases while the proportion of cartilage and connective tissue decreases. What's more, in promotion factors of osteophytes, osteophyte formation was induced by the biomechanical and biochemical factors and their both effect. The biomechanical factors result from imbalance of articular biomechanics induced by cartilaginous degeneration and ligament trauma. And the biochemical factors result from alteration of articular microenvironment induced by metabolic disor?der of chondrocytes and synovitis. Furthermore, the both effects that the biomechanical factors induces biochemical alterations al?so co?promote osteophyte formation. And studies on the mechanism indicated that osteophyte formation was regulated by several signaling pathways and cytokines. TGF?β superfamily and Hedgehog signaling pathway were involved in modulating osteophyte formation. TGF?βand BMPs modulated the initiation and late stage of osteophyte formation respectively. And Hedgehog signaling pathway regulated the osteophyte formation with modulating the hypertrophy of chondral osteophytes. This paper emphatically re?views the development and mechanism of osteophytes in OA for deeply understanding the OA and bringing new ideas and methods for the precaution and treatment of OA.
3.Relationship between lymphocyte-monocyte ratio and prognosis of malignant tumors

Related studies have been confirmed that tumor-associated inflammation is associated with tumor progression and prognosis in a variety of tumors.As an inflammatory biomarker,lymphocyte-monocyte ratio shows significant value in predicting the prognosis of malignant tumors.It can not only be appropriate for patients with cancer risk stratification,but also provide the basis for the subsequent development of individua-lized treatment programs.
4.Pathogenesis and interventions of cancer-related sarcopenia

The pathogenesis of cancer-related sarcopenia is complicated,involving all aspects of tumor and the body's metabolism,mainly related to accelerated proteolysis and insufficient protein synthesis.Cancerrelated sarcopenia can predict antineoplastic-induced dose-limiting toxicities,increase complications,and reduce survival rate.The interventions of cancer-related sarcopenia includes nutrition,exercise,hormone therapy and other interventions.A single treatment can not reverse clinical outcome.Various interventions are still in the investigation stage,in which more evidence-based studies are needed.
6.Identification of pulmonary fungal colonization and invasive fungal infection

The incidence of invasive fungal infection in critically ill patients is increasing year by year. Its clinical manifestation lacks specificity which is difficult for early diagnosis. It′s hard to identify inva-sive fungal infection with fungal colonization in clinical practice. We need to find out the way to distinguish them.
7.Advances in Physical Activity among Patients post Operation for Breast Cancer (review)

Physical activity is closely related to the quality of life of postoperative patients with breast cancer, which is expressed in fre-quency, duration, intensity, and others factors. There is no uniform measurement of physical activity, the common measurements may be classified as subjective and objective, such as exercise log, physical activity questionnaire and self-reported physical activities of the former;doubly labeled water, indirect calorimetry, motion sensors and heart rate monitors of the latter. The age, educational level, social and eco-nomic status, and body mass index are related to the level of physical activity in patients post operation for breast cancer, while illness and treatment, psychological and social support, and health-related quality of life also affect the level of physical activity, which suggest that such patients need to improve their physical activity before operation.
8.Right hemihepatectomy through the right retrohepatic tunnel

Objective To study the clinical value of anatomizing and ligating the short hepatic veins (SHVs) and right hepatic veins through the right Retrohepatic Tunnel (RT) in right hemibepatectomy of giant hepatic carcinoma.Methods We performed the right hemibepatectomy on 23 patients with huge tumors which diameters were lager than 8 cm.There were four main procedures during the operation.Firstly,sperated the interspace between right hepatic veins and middle hepatic veins through secundum porta hepatis.Secondly,seperated and ligated the right hepatic portal vein and hepatic artery in the Glisson's system.Thirdly,anatomized and ligated SHVs and right hepatic veins through the right RT conversly.Finally,set a blocking-belt at the left of the central liver and then cut the right hemi-hepar.Results The anatomy of right hepatic artery,Portal Vein,retrohepatic inferior vena cava (RHIVC),right hepatic veins was well done.The intraoperative blood loss of 7 patients was less than 400 mL,and 11 was 500-700 mL,2 was 800-1 000 mL,3 was 1 100-1 400 mL,and average was 640 mL.The hepatic function was changed in the third day afteroperation.The numerus of TBIL of 16 patients was 20-40 μmol/L,and 6 was 45-50 μmol/L,and 1 was 60 μmol/L.The numerus of ALT of 14 patients was 150-200 U/L,and 9 was 250-400 U/L.The numerus of AST of of 13 patients was 160-200 U/L,and 9 was 230-400 U/L,and 1 was 430 U/L.The numerus of GGT of 14 patients was 160-200 U/L,and 8 was 220-310 U/L,and 1 was 420 U/L.The hepaticfailure happened to 1 patient because of deep venous infection.Conclusions The right hemibepatectomy through the right retrohepatic tunnel is feasible in the tumor without invasion the RHIVC and diaphragm,or the volume was not too huge.The operation could reduce the blood loss and make for the recovery of hepatic funtions.

Estrogen has the protective effect on cerebral ischemia in v ar ious ways. It not only can improve the regional cerebral blood flow during ische mia and rescue the remaining neurons alive, but also can reduce the injury of is chemia-reperfusion. In this article, we enumerate at the molecular level that e strogen has the properties of inhibiting the expression of VCAM and cytokines li ke TNF-? in the ischemia region, educing the overload of calcium and cytotoxic ity of EAA by modulating the calcium channels, and regulating related genes expr ession and anti-apoptosis. It is crucial for estrogen replacement therapy as a neutoprotectant clinically.
