1.Animal experiment of nerve regeneration promoted by predegenerated nerve graft

BACKGROUND: Many studies thought that predegenerated nerve graft (PNG) is superior to fresh nerve graft (FNG).OBJECTIVE: To observe the effect of predegeneration on nerve functional recovery from histological,immunocytochemical,electrophysiological and microstructural aspects based on predegenerated nerve graft to common peroneal nerve at different time periods.DESIGN: Grouping and controlled experiment SETTING: General Center of Department of Orthopaedics, Tangdu Hospital, Fourth Military Medical University of Chinese PLA MATERIALS: Totally 42 rabbits, with body mass ranged from 2.3 to 2.6 kg,of either gender, were used in this experiment METHODS: This experiment was conducted at the General Center of Department of Orthopaedics, Xijing Hospital, Fourth Military Medical University of Chinese PLA in 1996. Totally 42 rabbits were divided into 7experiment groups A, B, C, D, E, F, G for nerve grafts of different predegenerated periods. In groups A, B, C, D, E, F, both side of the rabbits peroneal nerves were allowed to undergo predegeneration for 0, 4, 7, 14,21, 28 days; common peroneal nerve of the rabbits in the Groups B, C, D,E and F were cut off; The skin of the animals in Group A was cut open,exposed but the nerve was not cut off. Only the right common peroneal nerve in Group G was cut off. The cut nerve in Groups B, C, D, E and F were re-exposed respectively at 4,7,14,21 and 28 days after operation. 1.5 cm-length degenerated distal nerve segment was chosen for transplantation and reparation contrafateral nerve. Two sides were transplanted mutually. Common peroneal nerve in Group A was re-exposed 2 weeks later, 1.5 cm-length nerve segment was cut in the same way for mutual transplantation. In Group G, the predegenerated nerve segment of the right side was transplanted to the fresh receptor of the left side 2weeks later. The fresh nerve segment of left segment was transplanted to degenerated receptor of the right side.Histological observation, passing rate of axon counting shortened significantly the initial delay period of axonal regeneration (from 3.04 to 0.04 day), accelerated the axonal growing rate (1.73-2.59 mm/d)as compared with FNG; Electrophysiological study showed that N-MCV in the Groups D, C and B was the fastest, followed by Group A and Group E,Group F was the slowest; there was no significant difference in passing rate appeared in the animals of Group D after operation, middle-quantity of myelin sheaths appeared in the Group C and few myelin sheaths in the Groups A, B, and F.CONCLUSION: PNG excels FNG at decreasing initial delay period of axonal regeneration, promoting regeneration velocity of axon, boosting up passing rate of axon and enhancing N-MCV. Nerve grafting for predegenerated for 14 days can enhance significantly nerve regeneration than fresh nerve grafts, then grafts degenerated for 7 days. Grafts degenerated for 4 and 21 days can also enhance axonal regeneration lightly.
2.Determination of Oleic Acid and Linoleic Acid in Brucea javanica Oil by RP-HPLC

OBJECTIVE:To establish an RP-HPLC method for determination of the contents of oleinic acid and linolic acid in Brucea javanica oil. METHODS:The derivative fatty acid was obtained from extracting and separating process using ?-bromo-acetophenone as derivative reagent,K2CO3 as base and N,N-dimethylformamide (DMF) as solvent. The sample was separated on Shim-pack VP-ODS column(150 mm?4.6 mm,5 ?m); the detection wavelength was set at 242 nm. The mobile phase consisted of methanol-acetonitrile-water(30∶20∶50). RESULTS:The linear ranges of oleinic acid and linolic acid were 0.025~1.0 mg?mL-1(r=0.999 6) and 0.1 ~ 0.8 mg?mL-1(r=0.999 0),respectively. The average recoveries were 98.54% and 98.50%,respectively,and their RSD were 1.81% and 1.90%,respectively (n=6 each). CONCLUSION:The method is accurate and specific,and it is applicable for the quality control of B.javanica oil.
3.New progression in diagnosing the recurrence and metastasis of colorectal cancer by combining detection of tumor markers

Extensive researches suggest that the metastasis of colorectal cancer involves multiple factors and genes.Recurrence and metastasis after surgery are the important reasons for poor prognosis of thesc patients with colorectal cancer.Tumorigenesis is a dynamic process.The levels of tumor markers are changed in different stages.It has not yet been found any kind of tumor markers specific for a certain type of colorectal cancer.The sensitivity and specificity of diagnoising colorectal cancer by detecting an individual tumor marker are not ideal.The better combination of tumor markers has its advantages,can improve the early diagnostic rate.Combining detection of tumor markers will improve the diagnostic yield.
4.Clinical research of hyperthermia combined with radiotherapy and chemotherapy in colorectal cancer

Colorectal cancer is a common malignancy in China.The incidence of it showed an increasing trend with the economic development,living standards improvement and lifestyle changes.The traditional treatment methods include surgery,radiotherapy and chemotherapy.In recent years,hyperthermia has become a method for treating tumor.Hyperthermia kills tumor cells by thermal effect and no-thermal effect.Not only does it have a direct therapeutic effect on tumor cells,hyperthermia also has some synergy combined with radiotherapy and chemotherapy.Hyperthermia combined with conventional therapy can improve the local control rate and extend survival for patients.In this paper,the efficacy of hyperthermia has been reviewed,combined with a variety of treatment methods for colorectal cancer in China.
5.Effect of early gait training on the functional rehabilitation after total knee arthroplasty

BACKGROUND: Knee arthroplasty can correct the joint deformity, alleviate the pain and improve the joint function,thereby improving the patients' quality of life. Early rehabilitative training is the key to the functional recovery of the joint.OBJECTIVE: To observe the clinical effectiveness of early gait training on the rehabilitation of patients after knee arthroplasty.METHODS: Eighty-six cases undergoing unilateral knee arthroplasty in the Department of Sports Medicine were selected and randomly divided into experimental and control groups (n=43 per group). The experimental group wasgiven rehabilitation instruction before operation, and the gait training was carried out just after the operation. The patients in the control group only received the basic rehabilitation training. The Visual Analogue Scale and the Hospital for Special Surgery scores were evaluated at baseline and 2, 4, and 8 weeks, and 6 months postoperatively, and the clinical effectiveness was compared between two groups.RESULTS AND CONCLUSION: (1) The range of motion of the knee in the experimental group was significantly higher than that in the control group at 4 and 8 weeks, and 6 months postoperatively (P < 0.05). (2) At 8 weeks and 6 months postoperatively, the Visual Analogue Scale scores in the two groups both were significantly decreased (P < 0.05). (3) At 2 weeks postoperatively, the Hospital for Special Surgery scores in both groups were significantly increased (P < 0.05);the scores at 8 weeks postoperatively in the experimental group were higher than those in the control group (P > 0.05);the scores at 6 months postoperatively in the experimental group were significantly higher than those in the control group (P < 0.05). (4) These findings suggest that compared with conventional training, early gaiting training can significantly improve the range of motion and pain of the knee, as well as enhance the functional recovery of the knee joint, which exhibits remarkable curative efficacy in the rehabilitation following knee arthroplasty.

7.Chinese Medicine and Western Medicine Prevention the Anaphylaxis of Ionic-type Radiography Agent in Brain Enhancement CT

Objective To study Chinese medicine and Western medicine preventing the anaphylaxis of ionic-type radiography agent,to solve the anaphylaxis in brain enhancment CT.Methods Eight hundred and forty cases have been diagnosed in brain enhancement CT at our hospital,from Fed.1995 to May 2000.Results Chinese medicine,ginger,pinellia and hormone medicine were all useful in preventing the anaphylaxis of ionic-type radiography agent in brain enhancement CT.Conclusion Chinese medicine:ginger and pinellia could prevent the anaphylaxis of ionic-type radiography agent.
8.Effect of the general practitioners training in northern Anhui province

Objective To understand the effect of the general practitioners training in northern An-hui province, so as to provide a scientific basis for improving training strategies and measures. Methods In accordance with the requirements of Implementation of General Practitioners Training Program in Anhui Province, the training courses was designed, which includes the differences between general medicine and special medicine, general medicine service psychology, ethics related skills, general teaching skills, general clinical skills, and general practice policies, with a total of 32 credit hours. Participants were examined through the self-designed electronic questionnaire before and after the training. A total of 118 participants were involved in the training, and 109 of them were examined before the training, with a recovery rate of 92.4%. After the training, they were examined again, and the recovery rate was 100%. All data were analyzed by SPSS 17.0 software, and P<0.05 was considered statistically significant. Results Scores of the general medicine knowledge and skills after the training were higher than that before the training, and the difference was statistically significant (P<0.05). As for the course evaluation, the average score was over 4 in organiza-tion and management, training facilities, teacher knowledge, training materials, training methods and skills, while the average score was no more than 4 in training content and helping teaching. Conclusion The training achieves good results, and in the future, we should strengthen the training of teaching ability, thus further improving the effect of general practitioners training. Also, we should design the training content reasonably and pay attention to improve the quality and effectiveness of individual courses accordingly. What's more, we can use diversified teaching methods to further improve the teaching ability of general practitioners.
9.Influence of different chemotherapies on the immunological function and part of the quality of life of patients with breast cancer

Objective To explore the influence of TAC protocol (pirarubicin,cyclophosphamide and docetaxel) and AC-T protocol (pirarubicin,cyclophosphamide and sequential docetaxel) on T lymphocytes cell subsets (included CD3+ T cells,CD3+ CD4+ T cells,CD3+ CD8+ T cells,CD3+ CD4+/CD3 + CD8+),natural killer (NK) cells and part of the quality of life in the peripheral blood of patients.Methods A total of 66 patients with breast cancer included 31 cases for TAC protocol (6 cycles of chemotherapy) and 35 cases for AC-T protocol(8 cycles of chemotherapy).Collected respectively before and after the last chemotherapy of peripheral blood,used flow cytometry technique to detect the proportion of T cell subsets and NK cells in peripheral blood,and compared the change of the part of the quality of life before and after chemotherapy.Results After chemotherapy of total of 66 patients CD3+ T cells,CD3+ CD4+T cells and NK cells of peripheral blood ((68.58± 11.03) %,(33.53 ± 8.84) %,(18.27 ± 10.65) %) significantly lower than before chemotherapy ((74.03±13.04)%,(38.42±9.79)%,(19.83± 10.19)%),the differences were statistically significant(t =4.296,3.387,4.092,P< 0.05).Grouping was TAC protocol with AC-T,T cell subsets and NK cells before chemotherapy differences had no statistical significance.But after chemotherapy,CD3+ T cells,CD3+ CD4+T cells,CD3+CD8+ T cells and Karnofky score((64.63±10.01)%,(31.19±7.41)%,(24.66±7.40)%,(68.63± 5.100)%) of peripheral blood on TAC protocol were more decreased than AC-T protocol((72.29± 10.78)%,(35.74± 9.60) %,(30.15 ± 11.12) %,(77.29 ± 4.58) %),the differences were statistically significant (t =2.99,2.15,2.35,2.237,P<0.05).CD3+ CD4+/CD3+ CD8+ and NK cells were also decreased,but without statistical significance.The body quality of 66 patients was slightly increased after chemotherapy,but without statistical significance.Conclusion The body's immune function and the part of quality of the breast cancer patient's life have declined by chemotherapy drugs in varying degrees.Compared TAC,AC-T protocol has less influence on the immunological function and the part of living quality of patients with breast cancer.
10.Comparative study on health risk behaviors between male and female freshmen among medical universities, scientific and technologic universities

ObjectiveTo explore the distribution and characteristics of health risk behaviors between the male and female college freshmen in the medicine and science,and to provide evidence for appropriate health intervention measures.MethodsA questionnaire investigation was performed on college students'health risk behaviors among 1019 freshmen from medical and science college in urban area of Harbin.Results (1)There were statistically significant among the different gender in difference of risky behaviors except playing video games,net surfing,considered suicide,drug abuse and a few activities outside the 60 minutes every day (P < 0.01 ).(2)Medical male risk behavior of the frequency(56.4%,19.2%,12.7% and 47.1% )higher than that of science and engineering male(35.6%,5.2%,1.6% and 17.3% )in trying to smoking,current smoking,sexual behavior and breakfast skipping.Recent medical males drinking rate was 85.3% higher than male of science and engineering 72.2%.The incidence of female science and engineering students,respectively( 89.3%,50.8% and 38.5% ) than female medical students (72.4%,36.9% and 8.0% ) in trying to drinking and recent drinking and breakfast skipping.ConclusionThe risk behaviors of college freshmen were prevalent.The frequency of risk behaviors are different among the College Freshmen in different genders and with different major,respectively.Therefore,it is necessary to take measures for intervention.
