Objective To investigate the clinical value of combined detection of the serum levels of CA125,CA72-4 and tumor specific growth factor(TSGF)in the diagnosis of ovary cancer and the curative effect.Methods Serum levels of CA125,CA72-4 and TSGF in 68 patients with ovary cancer,53 patients with benign ovary rumor and 50 normal controls were measured by chemiluminescent immunoassay(CLIA)and chemical colorimetry.Results and clinical data were statisticaly analyzed.Results The levels of CA125,CA72-4 and TSGF in ovary cancer group were higher than that in the benign ovarian tumor group and healthy controls(all P ＜0.01).When use three tumor markers united detection,sensitivity,specificity and accuracy were 91.2％,83.0％ and 87.4％ respectively.The sensitivity and accuracy was higher than any single detection.The specificity with single CA125 testing was consistently.The levels of CA125,CA72-4 and TSGF in patients with ovary cancer were significantly different after surgical treatment for five days(all P ＜0.05).Conclusion The combined detection of the three markers may increase the positive rate in the early diagnosis of ovarian cancer,and help to differentiate the benign and malignant ovarian tumor,but also is valuable to observe the curative rate and postoperative monitor of the ovary cancer.

Objective To summarize perioperative care and effect of surgical treatment of total anomalous pulmonary venous connection (TAPVC) in newborns and babies within 6 months.Methods From September 2001 to May 2013,ninetytwo consecutive patients with TAPVC received surgical correction under cardiopulmonary bypass (CPB) with moderate or deep hypothermia.The anatomic subtype included supracardiac type 48,intracardiac type 36,infracardiac type 6 and mixed type 2.There were 36 newboms.Pathological diagnosis was made by echocardiogram,computerized tomography and operative findings during operation.The option of procedure was determined by findings.Respirator assisted breathing after operation and cardiac function was maintained by a variety of positive inotropic drugs,to prevent the happening of the pulmonary hypertension crisis.Results Severely low cardiac output syndrome occurred in 8 patients and they died within 24 hours after operation.The remaining cases postoperative recover smoothly and receive postoperative follow-up about 1 month to 12 years,3 cases of postoperative pulmonary vein stenosis (PPVS) occurred.Conclusion Total anomalous pulmonary venous connection should be surgical treatment as soon as possible with satisfied effect.The key of perioperative care is maintenance of left heart function,negative balance of liquid,prevention and treatment of pulmonary hypertension.

Objective To study the chemical constituents from the bark of Juglans mandshurica. Methods The compounds were isolated and purified by column chromatography on silica gel,HPLC,and recrystallization.Their structures were elucidated by the physicochemical and spectroscopic evidences. Results Fifteen compounds were identified as:4,8-dihydroxynaphthalenyl-O-?D-(6′-acetoxyl)gluco- pyranoside(Ⅰ),dihydrokaempferol(Ⅱ),juglone(Ⅲ),daucosterol(Ⅳ),kaempferol(Ⅴ),4,8-dihy- droxynaphthalenyl-1-O-?-D-[6′-O-(3″,5″-dimethoxy-4″-hydroxybenzoyl)] glucopyranoside(Ⅵ), kaempferol-3-O-?-L-rhamnoside(Ⅶ),3,3′-dimethoxylellagic acid(Ⅷ),naringenin(Ⅸ),quercetin (Ⅹ),reginolone(Ⅺ),quercetin-3-O-?-L-rhamnoside(Ⅻ),naringenin-7-O-?-D-glucoside(ⅩⅢ),4,8- dihydroxynaphthalenyl-1-O-?-glucoside(ⅩⅣ),4,5,8-trihydroxy-?-tetralone-5-O-?-D-[6′-O-(4″-hy- droxy-3″,5″-dimethoxy-benzoyl)] glucoside(ⅩⅤ).Conclusion CompoundⅠ(juglamanol)is a new compound.CompoundsⅡ,Ⅶ—Ⅸ,Ⅻ,andⅩⅢare isolated from plants of Carya Nutt.for the first time.

Objective To investigate whether blocking NR2B receptor can reverse the process of cytotoxicity to spiral ganglion neurons induced by sodium salicylate in guinea pig by applying ifenprodil (a NR2B antagonist) at the round window niche .Methods Sixty healthy guinea pigs provided by the experimental animal center of Guangxi medical university were randomly and evenly divided into a control group (Group I ,no treatment) ,an APL group (Group II ,60μl APL directly applied to the round window ) ,a sodium salicylate group (Group III ,60 μl APL di-rectly applied to the round window and then be given intraperitoneal sodium salicylate injection ) ,and an ifenprodil group (Group IV ,60μl of 10μmol/l ifenprodil in APL directly applied to the round window and then be given intra-peritoneal sodium salicylate injection ) .Sodium salicylate was given at 400 mg · kg -1 · d-1 for 7 days .Auditory brainstem responses (ABRs) were recorded before animal sacrifice by decapitation .The left cochlea was removed and prepared for detection of caspase -3 expression in spiral ganglion neurons via immunohistochemistry .From each group ,6 cochleae were used to test apoptosis index in spiral ganglion neurons using the TUNEL technique .Results Before salicylate administration ,the ABR threshold was less than 40 dB SPL in all animals .After salicylate ad-ministration ,the ABR threshold was 33 .33 ± 5 .17 dB SPL in Group II ,64 .17 ± 7 .36 dB SPL in Group III and 49 .17 ± 5 .85 dB SPL in Group IV ,in contrast to 31 .67 ± 5 .16 dB SPL in Group I (controls) .The caspase -3 ex-pression was not changed obviously in Group I and Group II ,but was significantly changed in Group III and Group IV (P<0 .01) .The caspase-3 expression appeared to be decreased in Group IV compared to those in Groups III (P<0 .05) ,but still increased compared to those of in Group I and II (P<0 .05) .The apoptosis index among spiral ganglion neurons in Groups III and IV increased significantly compared to those of in Group I and II (P<0 .001) .It was however ,lower in Group IV than in Group III (P<0 .01) .Conclusion Blocking NR2B receptor with specificity can reverse the process of cytotoxicity to spiral ganglion neurons induced by sodium salicylate in guinea pig .

Metastatic epidural compression of the spinal cord is a significant source of morbidity in patients with systemic cancer. With improvment of oncotheray, survival period in the patients is improving and metastatic cord compression is en- countered increasingly often. Surgical management performed for early circumferential decompression for the spinal cord com- pression with spine instability, and spine reconstruction performed. Patients with radiosensitive tumours without spine instabili- ty, radiotherapy is an effective therapy. Spinal stereotactic radiosurgery and minimally invasive techniques, such as vertebro- plasty and kyphoplasty, percutaneous pedicle screw fixation, radiofrequency ablation are promising options for treatment of cer- tain selected patients with spinal metastases.

Objective To explore an anatomical basis for the medial plantar skin flaps and its clinical value of repairing heel defects. Methods The origin,branches,course and distribution of the medial plantar artery and the nerve of flaps were studied in 10 adult cadavers(20 legs).8 cases of homonymy and opposite side heel soft tissue defects with medial plantar skin flaps,aged 10-42 years.Free skin transplantation on the donors. Results The length of media plantar artery deep branch was (8.9±0.2)cm,gives off 3-5 cutaneous branches.Medial plantar nerve have 5-8 branches.it inchdes 3～5cutaneous branches distribute plantar skin and 2-4 plantar digital nerves distribute toes skin.The area of flaps was 4.0 cm×3.0 cm-6.0 cm×5.0 cm in the 8 flaps. 6 pedicle skin flaps were transfenrred to repair homonymy side heel soft tissue defects.2 opposite side heel soft tissue defects were repaired by 2 free flaps.All the flaps were survived. All patients were followed up one months to one year,the function and appearance of the flaps were good.Conclusion The flaps have less anatomic variation or sacrificing major vessels but have reliable blood supply and can restore good sensation after operation.They are ideal flaps for repairing heel soft tissue defects.

Objective To evaluate the effects of reserved jejunal feeding tube during postoperative chemotherapy of gastric cancer. Methods Forty-two patients with gastric cancer underwent radical gastrectomy and going to adjuvant chemotherapy,conventional placed jejunal feeding tube. All of the patients weredivided into group A and group B randomly by pathological staging and tumor site, group A reserved jejunal feeding tube and received enteral nutrition through the tube during chemotherapy, and group B non-reservedjejunal feeding tube and been given daily diet,compared nutrition and immune indicators of two groups beforeand after chemotherapy ,compared the rate of vomiting,and observed complications long-term reserved jejunal feeding tube. Results In post-chemotherapy,nutrition and immune indicators of group A were betterthan those of group B, the difference was statistically significant (P＜0.05) ,the rate of vomiting in group Awas significantly lower than that of in group B (X2= 9.75, P＜0.01 ), no serious complieations occurred forlong-term reserved jejunal feeding tube. Conclusions Reserved jejunal feeding tube and received enteralnutrition through the tube during postoperative chemotherapy of gastric cancer can significantly improve the nutritional and immune status. It is safe and reliable, worth promoting.

Objective To investigate the influence of motor impersistence on functional recovery and prog- nosis of patients with left hemiplegia. Methods A total of 64 in-patients with left side hemiplegia who were able to understand the oral instructions were assigned into a motor impersistence group (n = 31 ) and a control group (n = 34) , respectively, according to the assessment with Scale of Motor Impersistence developed by Joynt. Both groups were comparable in terms of the basic clinical characteristic and motor function of patients except motor impersis- tence. The conventional rehabilitation training programs were adopted for all patients in this study. Brunnstrom func- tional category, Ueda Satoshi hemiplegic finger function assessment scale, upper limb function assessment scale, Berg balance scale, Functional ambulatory category, Modified Barthel Index were employed to evaluate the subjects before and after 1 and 2 months of treatment, with regard to the motor function of their upper limbs, walking ability, activities of daily living performance and balance ability. Results After 1 month of treatment, the patients in the M1 group were significantly improved with regard to the motor function of upper limbs, hand function, standing ba- lance and activity of daily living performance( P0.05 ) , while those in the control group were improved with all the parameters except the lower limb function. Objective To investigate the imaging features of sarcomatoid carcinoma(SC)of lung on chest radiography and CT and to improve knowledge of SC.Methods The chest plain films,CT images and clinical data of all 7 cases of pathologically proved lung SC were retrospectively analyzed.Results All cases presented with a spheroid solid lung mass ranged from 3 to 11 cm in diameter(average 7 cm).All the lesions were located in middle and lower fields of the lung:3 lesions in right lower lobe,2 in right middle lobe,and the other 2 in left lower lobe.Among the 7 cases:6 cases were of peripheral type and 1 case was of central type.The peripheral-typed lesions of 5 cases had clear margin and 3 of them were lobulated.Three massess were homogenous in density,and the other 4 were inhomogeneous in density with formation of central cavity or calcification.After intravenous contrast administration.Three lesions showed homogeneous enhancement,and the other 4 lesions had inhomogeneous enhancement.The lesions with diameter larger than 6 cm showed marginal or patch-like enhancement.Five cases had pleural or chest walls invasion.Two cases had mediastinal lymph nodes metastases.One case had multiple remote metastasis.Conclusion There are some relatively specific features of lung SC on chest radiography and CT,which may be helpful for diagnosis.

Objective To discuss the frequent etiology of spontaneous sub-cortical hemorrhage and its diag- nosis.Methods The clinical materials of 79 cases of spontaneous sub-cortical hemorrhage were analyzed.Results 56% of the hemorrhage was caused by arterial-venous malformation.48% of the hemorrhage was caused by occult AVM.Conclusion AVM is the most frequent etiology of spontaneous sub-cortical hemorrhage.CTA plays a special role in its diagnosis.
