Objective To evaluate the clinical significance of detecting platelet parameters in platelet transfusion patients by Investigating the relationship between platelet parameters and efficacy of platelet transfusion and prognosis.Methods A total of 50 patients who underwent platelet transfusion in our hospital in 2014 to 2016 were reviewed.Patients divided into two groups (with normal platelet counts group or subnormal platelet counts group) according to outcomes of blood biochemistry and routine after five days of platelets transfusion.Platelet parameters (platelet count,platelet distribution width,mean platelet volume,large platelet ratio) and CCI were evaluated by statistical analysis.Results The numbers of patients with CCI＞4.5 in normal platelet counts group is 21 (77.78％),that in subnormal platelet counts group is 11 (47.83％).Platelet parameters before transfusion showed no significant difference between two groups (P＞ 0.05).After transfusion,in normal platelet counts group Plt,PDW,MPV and P-LCR were significantly higher than another group (P＜0.01).MPV and P-LCR were increased in normal platelet counts group,and decreased in normal platelet counts group after transfusion.Condusion Detection of platelet parameters,especially MPV and P-LCR,is important for evaluating the efficacy and prognosis of platelet transfusion.

Objective To analyze the effect of perioperative blood transfusion in orthopedic surgery.Methods A total of 201 patients who underwent an orthopedic surgery in our hospital in October 2015 to September 2016 were reviewed.Hemoglobin concentrations,hematocrit,postoperative hospital stay,infection and other related data were analyzed.Results 90 patients were transfused with blood and in 111 patients were not during orthopedic surgery (massive blood transfusion group including 17 cases;non massive blood transfusion group including 94 cases).Hemoglobin concentrations of transfusion group (103.09±16.37)g/L was lower than the non transfusion group (115.79± 13.68)g/L before liying hospital,postoperative hospitalization time of transfusion group (12.012±8.36) was longer than that of non transfusion group (16.14 ± 10.47)days.Postoperative infection rate in non transfusion group was 1.11％,and in transfusion group was 15.32％,in massive transfusion group is 52.94％.Conclusion Perioperative blood transfusion is not conducive to the recovery of hemoglobin level patients in orthopedic surgery,prolong hospital stay and increase the rate of infection after surgery.Perioperative blood transfusion makes it difficult to support the outcome of the disease

Objective Investigate the relationship between thromboelastography and coagulation tests,and evaluate of two kinds of methods in guiding significance in cardiac surgery patients perioperative blood transfusion.Methods Analysis of 108 patients with cardiac surgery in September 2014-August 2016 treated in our hospitalwere randomLy divided into TEG detection guide blood transfusion group (experimental group) and coagulation test guide blood transfusion group (control group),Respectively on two groups of blood transfusion rate,different components of blood transfusion and transfusion effect comparison.Results Through the comparison of the heart surgery perioperative blood use rate guided by TEG and coagulation test,found the transfusion rate of experimental group guide cryoprecipitate 25.9％ and fresh frozen plasma(FFP) 33.3％ was significantly lower than the control group 68.5％,however,platelet transfusion rate was significantly higher than the control group of 57.4％ to 20.4％.There was no statistically significant difference of red cell suspension (RCS) infusion rate between two groups,but the RCS infusion quantity of experimental group(3.6+ 1.6) was clearly lower than the control group (5.8+ 1.9),and the FFP and cryoprecipitate infusion quantity of experimental group was obviously lower than control group(P＜0.05),too.Experimental groups of intraoperative blood loss,postoperative blood loss and rebleeding rate was lower than control group (P＜ 0.05).Concltsion That TEG guide cardiac surgery patients perioperative blood transfusion can reduce the use amount of blood products,and the prognosis of disease outcome is better than that of routine coagulation tests guidance.TEG can reduce the surgical complications and the bleeding amount,and it has great sense to guide clinical composition blood transfusion,so it is worth promoting.

As an important neurotransmitter, adenosine displays its functions by acting on the adenosine receptors. Recent studies have shown that the distribution, expression and balance among subtypes of adenosine receptors are closely related with cognitive activities, and changes of adenosine receptors play key roles in neurodegenerative disorders including Alzheimer's disease. It has been pointed out that prolonged activation of adenosine receptors by high level adenosine may lead to the disturbance of balance among adenosine receptor subtypes. This imbalance mainly performed as increased expression of A2a receptor and decreased expression of A1 receptor, and enhancement of the excitatory signals mediated by A2a receptor and weakened inhibitory signals mediated by A1 receptor. Changes of these two subtypes of adenosine receptors may lead to a lot of disorders of neurological activities which developed into dysfunction of cognition to the end. These findings imply that the potential of maintaining the balance among adenosine receptors on the treatment of AD would facilitate both the revealing of the mechanism and the cure of AD.

Objective To evaluate the prognostic factors in locally advanced non-small-cell lung cancer (LA-NSCLC) for selectively carrying out prophylactic cranial irradiation (PCI).Methods 114 patients with LA-NSCLC between Jun 2006 and Oct 2010 were retrospectively analyzed. Related risk factors and features about brain metastases were analyzed.Results The 2-year incidence rate of brain metastases was 31.58 ％ (36/114),the first brain metastases was 20.18 ％ (23/114),and sole brain metastases was 9.65 ％(11/114),respectively.Variables involved in the equation of binary logistic regression analysis were pathology (OR =5.892) and treatment mode(OR =2.888).The incidence rate of brain metastases in patients of non-squamous carcinoma and single treatment mode was higher than others (P ＜ 0.01) Model fitting is better (P ＞ 0.05).Overall accuracy rate of predicting brain metastases is 67.7 ％.The increased rate of lactate dehydrogenase in the patients with brain metastases or death was 17.54 ％, which was higher than that in the survival patients without brain metastases (P ＜ 0.01).At the same time,the station number and the number of mediastinal lymph node metastases were positively correlated (r =0.716, P ＜ 0.01).The incidence rate of brain metastases or mortality rate was higher in the adenocarcinoma cases than that in the squamous carcinoma cases (P ＜ 0.01,P ＜ 0.05),with more frequent occurrence of mediastinal metastases.The mean diameter of squamous carcinoma and adenocarcinoma were 5.8 cm and 3.9 cm, respectively (P ＜ 0.01).Conclusions The incidence rate of brain metastases was higher in patients with single treatment.Large primary tumors, high lactate dehydrogenase, non-squamous carcinoma, multiple stations, and multiple mediastinal lymph nodes metastases can be regarded as risk factors of brain metastases to perform PCI.

Drug resistance and relapse are the major challenge for current treatment of acute leukemia. It is critical for ultimately curing leukemia to overcome chemoresistance of leukemic stem cells (LSC) and to eradicate LSC. Recent studies have found that abnormal activated Hedgehog (HH) signaling pathway plays an important role in a wide variety of tumors and regulates multi-drug resistance of LSC. This review briefly summarizes the molecular mechanism of HH signal pathway inducing drug resistance of LSC and leading to novel strategies for eradicating LSC.

This study aimed to explore the outcomes of progestin-primed ovarian stimulation protocol (PPOS) in aged infertile women who failed to get pregnant in the first IVF/ICSI-ET cycles with GnRH-a long protocol.A self-controlled study was conducted to retrospectively investigate the clinical outcomes of 104 aged infertile patients who didn't get pregnant in the first IVF/ICSI-ET treatment by stimulating with GnRH-a long protocol (non-PPOS group),and underwent PPOS protocol (PPOS group) in the second cycle between January 2016 and December 2016 in the Center for Reproductive Medicine,Renmin Hospital of Wuhan University.The primary outcomes included clinical pregnancy rate of frozen-thawed embryos transfer (FET) in PPOS group,and good-quality embryo rate in both groups.The secondary outcomes were fertilization rate,egg utilization rate and cycle cancellation rate.The results showed that there were no significant differences in basal follicle stimulating hormone (bFSH),antral follicle count (AFC),duration and total dosage of gonadotropin (Gn),number of oocytes retrieved,intracytoplasmic sperm injection (ICSI) rate,fertilization rate,and cycle cancellation rate between the two groups (P＞0.05).However,the oocyte utilization rate and good-quality embryo rate in PPOS group were significantly higher than those in non-PPOS group (P＜0.05).By the end of April 2017,62 FET cycles were conducted in PPOS group.The clinical pregnancy rate and embryo implantation rate were 22.58％ and 12.70％,respectively.In conclusion,PPOS protocol may provide better clinical outcomes by improving the oocyte utilization rate and good-quality embryo rate for aged infertile patients who failed to get pregnant in the first IVF/ICSI-ET cycles.
