China Journal of Orthopaedics and Traumatology 2016;29(3):242-247
OBJECTIVETo analyze the factors in the non-operative treatment of cervical spinal cord injury without fracture or dislocation.
METHODSThe clinical data of 122 patients with cervical spinal cord injury without fracture or dislocation from January 2009 to December 2012 treated by non-operative treatment were retrospectively reviewed. There were 84 males and 38 females, aged from 18 to 83 years with an average of (52.37 +/- 13.27) years. The clinical features, such as age, gender, cause of injury, time from injury to treatment, ASIA grade of spine cord injury, MRI type of spine cord injury, range of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, segment of intervertebral disc herniation, intervertebral disc ligament complex injury, treatment of high-dose methyiprednisolone, were observed. Single factor and multiple factor Logistic regression analysis were used in the clinical data in order to analyze the influencing factors of above items to prognosis.
RESULTSFor univariate analysis, the factors such as MRI type of spine cord injury, extent of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, segment of intervertebral disc herniation, ASIA grade of spine cord injury, associated with prognosis (P < 0.05). Multiple linear regression analysis showed that the main prognostic factors including MRI type of spine cord injury, range of spine cord injury, effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, ASIA grade of spine cord injury according to its effective intension (P < 0.05).
CONCLUSIONThe main prognostic factors on non-operative treatment of cervical spinal cord injury without fracture or dislocation were MRI type and extent of spine cord injury, meanwhile, correlate with effective cervical spinal canal ratio, Pfirrmann grade of intervertebral disc herniation, ASIA grade of spine cord injury. It is deliberative to choose no-operative treatment, for it only refers to the patients with the mild localized edema type or no signal change of spinal cord in MRII. Operative treatment shoud be recommended for other patients.

Objective To investigate the expression of c-erbB-2 in breast cancer and its relationship with prognosis. Methods The expression of c-erbB-2 in breast cancer was detected by immunohistochemistry assays in 60 cases of breast cancer. Results The positive expression level of c-erbB-2 in invasive breast cancer was signifi-cantly higher than that in other kinds of breast cancer(P<0.01).The positive expression level of c-erbB-2 in the metastatic breast cancer with four lymph nodes was higher than in the early cancer with no lymph node involvement (P<0.01).The 10 years survival rate in negative expression of c-erbB-2 was higher than in positive expression(P<0.05).Conclusion c-erbB-2 is important for physicians to instruct the treatment of the breast cancer and judge the prognosis.

Objective The aim of this article is to study the clinical features, image, electroencephalogram and gene mutation of mitochondrial encephalomyopathy with MELAS and to improve understanding of the disease. Methods Clinical data was collected from 12 MELAS cases. Their clinical symptoms, head imageological result,electroencephalo-gram and gene mutation were analyzed. Results The average onset age was 28 years old. The main manifestations of 12 patients were epilepsy(66.7%), stroke-like episodes(50.0%), mental behavious disorder and decrease of perceive (41.7%),diabetes or abnormal glucose tolerance(41.7%), hearing impairment(41.7%). The imaging results showed long T1, long T2 and high signals on DWI. The nidus involved occipital lobe, temporal lobe, parietal lobe most. Five of six cases undergoing MRS imaging had large lactate peak at 1.33 ppm. There were 9 cases carrying mtDNA A3243G muta-tion. Conclusion The clinical presentation of MELAS is highly variable without any obvious specificity on MRI and EEG. Large lactate peak on MRS may indicate the disease. Most patients carry A3243G mutation.

Objective To observe the effect of active components(glycyrrhizic acid monoaminate, total ginsenoside and to-tal ophiopogonoside)of Zhi Gancao Decoction(ZGD)and their combination on the triggered activity and myocardial dam-age in the isolated ischemia -reperfusion rat heart. Methods Isolated rat heart was treated with L angendroff perfusion and monophase action potential technology was applied. Results &Conclusion Total extract of ZGD,single active c omponents and their combination can markedly l ower the incidence of cardiac triggered activity, prevent the myocardiu m from injury and decrease the incidence of arrhythmia induced by ischemia -reperfusion in rats and the effects of their com bination are positively related to the doses.

Objective: To explore the correlation among plasma level of brain natriuretic peptide (BNP), serum D-dimer level and patient's status in patients with acute pulmonary embolism (APE).Methods: A total of 122 APE patients who were hospitalized from Mar 2012 to Dec 2015 were selected.According to right heart function and clinical symptoms, patients were divided into low risk group (n=61), medium risk group (n=36) and high risk group (n=25);after discharge, all patients were followed up for six months, then according to survival or not, they were divided into survival group (n=105) and death group (n=17).Plasma BNP and serum D-dimer levels were measured and compared among all groups, then correlation among BNP, D-dimer levels and severity of patient's status was analyzed. Results: Compared with low risk group, there were significant rise in levels of plasma BNP [(133.28±74.92)ng/L vs.(273.89±179.29)ng/L vs.(834.80±509.09)ng/L] and serum D-dimer [(761.80±333.65)ng/ml vs.(966.67±370.33)ng/ml vs.(1228.40±569.11)ng/ml] in medium risk group and high risk group, and those of high risk group were significantly higher than that of medium risk group, P<0.01 all.Compared with survival group, there were significant rise in levels of plasma BNP [(257.33±27.11)ng/L vs.(696.47±64.92)ng/L] and serum D-dimer [(818.57±64.92)ng/ml vs.(1513.25±48.14)ng/ml] in death group, P<0.01 all.Pearson correlation analysis indicated that plasma BNP, serum D-dimer levels were significant positively correlated with severity of patient's status (r=0.69, 0.41, P<0.01 both), and plasma BNP level was significant positively correlated with serum D-dimer level (r=0.79, P=0.002).Conclusion: Measurements of plasma BNP and serum D-dimer in APE patients are help to assess severity of patient's status and short-term prognosis, and provide basis for clinical individualized therapy.
