As antigen-presenting cells, DCs are the most potent and unique,they can stimulate the primitive T cells. They play a major role in initiating the immune response. Tumor infiltrating dendritic cells (TIDCs) can reflect the antitumor immune capability of the host. The density of TIDC perhaps is associated with the prognosis in some cancers. The disablement of TIDCS is partly due to the phenotype they have. Perhaps it is the immunosuppressive factors secreted by tumor cells that have changed their phenotype.The studies about TIDCs provide the basis for DC-mediated antitumor immune therapy,and suggest the use of DC to treat tumors through different approaches.
2.Immunological impact of autologous peripheral blood stem cell infusion on lung adenocarcinoma

BACKGROUND: Although clinical methods for autologous peripheral blood stem cell separation and identification arerelatively more, there is no unified standard. Moreover, little is reported on immune effects on lung adenocarcinomaduring autologous peripheral blood stem cell transplantation.OBJECTIVE: To study the effect of autologous peripheral blood stem cell transplantation on immunological function oflung adenocarcinoma rats by exploring the separation and identification methods of autologous peripheral blood stemcells.METHODS: The mononuclear cells were isolated from the peripheral blood of Wistar rats by density gradientcentrifugation method and then identified. Eighteen Wistar rats were randomly divided into routine treatment group andcell transplantation group, with nine rats in each group. Animal models of lung adenocarcinoma were established in therats, and underwent no treatment in the routine treatment group but injection of autologous peripheral blood stem cellsuspension (0.2 mL, 2.5×106/L) via the tail vein in the cell transplantation group. Immune indicators were detected in twogroups after treatment. Three rats from each group were anesthetized and scarified at 4, 10, 14 days after treatment,respectively. Afterwards, we detected the effects of autologous peripheral blood stem cell transplantation onimmunological function of lung adenocarcinoma rats.RESULTS AND CONCLUSION: Newly isolated mononuclear cells from rat peripheral blood showed a round or ovalshape and were relatively small in size. After 5 days of culture, autologous peripheral blood stem cells grew rapidly, andthe round cells began to spread to both ends and became spindle-shaped, thereby forming small colonies. Autologousperipheral blood stem cells induced in vitro experienced three stages successively: cell arrest, logarithmic and plateauphases. Flow cytometry showed that sorted cells were identified as autologous peripheral blood stem cells by expressingCD90, CD133 and Flk-1. After 4, 10 and 14 days culture, the expression of CD133 increased first and then descended,and Flk-1 showed an increasing trend. Compared with the routine treatment group, the expression of tumor necrosisfactor-α induced protein 6 in the cell transplantation group was significantly higher (P < 0.05), while the expression ofnuclear factor κB was significantly lower (P < 0.05). In conclusion, the isolation and identification methods of autologousperipheral blood stem cells are simple and can be used to secrete a large number of nutritional factors in the treatment oflung adenocarcinoma, which can enhance body immunity in the treatment.

The etiology and pathogenesis of intrahepatic cholestasis are complex,and those are not still very clear in current.Studies suggest that genetic factors play an important role in the pathogenesis of the disease.Some familial cholestasis have been confirmed by gene mutation causing.Bile secretion process regulated by a number of bile relation gene at the molecular level.Farnesoid X receptor (FXR) gene is related to intrahepatic bile secretion process.Bile secretion is indirect control by FXR which formats a complex network,becoming more attention to researcher in recent years.
4.Fish bone in retropharyngeal treatment with transoral laser CO2 microsurgery: a case report.

Fish bones are the most common pharyngeal foreign bodies encountered in the ENT emergency. Many fish bones may be buried in the surrounding soft tissue of the pharynx and larynx. Any delayed treatment for these foreign bodies may cause catastrophic complications. We present a case of 51-year-old woman who had an ingested fish bone migrating to the retropharyngeal space. We used spiral Computed tomography scanning (CT, taken at intervals of 2mm) with dimensional reconstruction for the accurate location diagnosis and surgical planning. We treated successfully before any complication by transoral Laser CO2 microsurgery.

5.Surgical treatment of postoperative rebleeding in portal hypertension.

ObjectiveTo evaluate the effect of different operative methods for treating rebleeding in patients with portal hypertension(PHT). MethodsThe clinical data of 66 patients with postoperative ebleeding out of 373 PHT cases during the last 30 years was retrospectively analyzed. Their first operations were splenectomy, portoazygous devascularization, various non-selective shunt and combined operation(devascularization combined with shunt), of which, the rebleeding rate were 26.67 %, 17.86 %, 14.58 % and 4.35 %, respectively. Among 55 cases receiving re-operation, there were 42 with distal esophago-fundusectomy, 11 mesocaval shunt(MCS)and 2 re-devascularization. The other 11 cases received nonoperative therapy. ResultsThe mortality, mean follow-up time, rebleeding rate and encephalopathy rate were 9.52% (4/42), 11 years,9.52% (4/42)and 14.29% (6/42)in distal esophago-fundusectomy group;0,7.5 years,0 and 9.09% (1/11)in MCS group. All 2 cases in re-devascularization group died within one postoperative month. All 11 cases in non-operative group were with portal hypertension gastropathy(PHG). ConelusionCombined operation may effectively reduce postoperative rebleeding rate in PHT cases and MCS is a relatively ideal method for treating rebleeding.

7.Effects of comprehensive geriatric assessment and health intervention on outcome in discharged patients

Objective To investigate the effects of comprehensive geriatric assessment (CGA) and health intervention on outcome in discharged patients.Methods Discharged patients in geriatric ward from May to July 2012 were randomly assigned to control (n=36) and intervention (n=36) groups.Patients in the two groups were given comprehensive geriatric assessment (CGA) before discharging from hospital.Both groups were followed up for 3 months.Patients in intervention group received health intervention when leaving hospital,at 1 week,3 weeks and 3 months after discharge from hospital,while patients in control group were not given any intervention.The comprehensive illness condition (MCIRS-G),ability of daily life (ADL and IADL),rehospitalization and survival rate were compared between the two groups 3 months after the intervention.Results After 3 months,the scores of MCIRS-G,ADL and IADL were significantly better in intervention group than in control group[(29.06±4.51) vs.(33.09±4.34),(75.69±16.57)vs.(63.86±19.37),(11.72±3.20) vs.(9.77±2.52),respectively,t=3.84,2.77,2.85,all P=0.01].The total times ofreadmission and readmission length were less in intervention group than in control group [3 vs.17,x2 =8.79,P=0.02; (1.94±6.68) days vs.(8.81±14.09) days,t=2.64,P=0.01].There was no difference in survival rate between the two groups [100% (36/36) vs.97.2% (35/36),x2=1.01,P=0.30].Conclusions Comprehensive geriatric assessment and health intervention can alleviate symptoms and improve the ability of daily life.
9.Social support of elderly in-patients and its influential factors

Objective To investigate the social support of the elderly hospitalized individuals and its relationship with outcomes and to study its influential factors.Methods The elderly in-patients aged>65 years old who were treated from April to June 2012 in Geriatric Department were recruited.The comprehensive geriatric assessment (CGA) that included social support scale (SSRS) was used.According to SSRS,the patients were assigned to the lower or higher score group.The participants were followed up for 1 year,and 12-month mortality and re-hospitalization were recorded.Student's t test,Chi-square test and multiple logistic regression were used for data analysis.Results The total score of social support was 26.58±8.99,which was significantly lower than the national norm (34.56±3.73,t =-7.52,P<0.05).Those with advanced age,less household,depression,insufficient ability of daily life and instrument daily life showed relatively lower SSRS (t values were-2.54,8.22,-6.61,6.72 and 6.95,respectively; all P<0.05).The patients with no spouse also got less social support (x2 =8.58,P<0.05).At 1 year,no significant difference of re-hospitalization and mortality was found between the two groups (x2 values were 1.94 and 3.28,respectively; both P>0.05).In multiple logistic regression analysis,fanily member,characteristics and living ability were the major influential factors of social support.Conclusions Medical staff should pay more attention to the elderly hospitalized patients and offer them sufficient social support to improve their quality of life.
