1.Therapeutic effect of urapidil and nitroglycerin in elderly female non-valvular heart failure patients

Objective To assess whether urapidil (anα1 receptor blocker)is more effective than ni-troglycerin in treatment of non-valvular heart failure accompanying hypertension and diabetes mellitus (DM ) in elderly female patients .Methods Fourty-six elderly female patients with non-valvular heart failure accompanying hypertension and DM were randomly divided into urapidil treatment group (n= 24) and nitroglycerin treatment group (n= 22) .Patients in urapidil treat-ment group were treated with urapidil (50 -300 μg/min) and those in nitroglycerin treatment group were treated with nitroglycerin (5 -20 μg/min) for 48 -140 h according to their blood pressure (BP) and cardiac function .Their BP ,HR ,and metabolic activity were observed before and on days 1 ,2 ,3 and 7 after treatment .Results The effect of urapidil was significantly lower than that of nitroglycerin on SBP on day 7 after treatment (P<0 .05) .The serum NT-proBNP level was significantly lower in nitroglycerin treatment group than in control group (P<0 .01) . Conclusion The effect of urapidil is better than that of nitroglycerin on reducing and stabilizing SBP ,attenuating cardiac afterload ,and improving cardiac function in elderly female patients .Ura-pidil can thus be used as one of the first choiced drugs in treatment of elderly female patients with non-valvular heart failure accompanying hypertension and DM .

The metabolic syndrome,as a group of concurrent cardiovascular risk factors,is attributed to both genetic and environmental causes and correlated with many gene polymorphisms and their mutations,such as adiponectin gene,peroxisome proliferator activated receptor gamma gene,lamin protein A/C gene,uncoupling protein 2 gene,?-adrenic receptor gene and some other inflammation marker genes.This paper reviews the advances in the studies of the gene polymorphisms associated with the metabolic syndrome.
3.Clinical effect of linezolid in pulmonary infection with methicillin resistant Staphylococcus aureus

Objective To investigate the clinical effect of linezolid in the treatment of pulmonary infection with methicillin resistant Staphylococcus aureus (MRSA).Methods 74 cases of pulmonary infection with MRSA were selected and randomly divided into control group and study group, 37 cases in each group.The control group received norvancomycin(1.0 g per time,twice daily) and the study group received linezolid (600 mg per time, twice daily) , with a consecutive treatment of 2 weeks.The inflammatory factors and related laboratory indicators were detected, the clinical efficacy, bacterial clearance rate and adverse reaction conditions were compared after the treatment.Results Compared with before treatment, serum levels of CRP, IL-1, IL-6 and TNF-αdecreased(P<0.05), levels of serum PCT, ET, NO and Lac decreased(P<0.05), compared with the control group, serum levels of CRP, IL-1, IL-6 and TNF-αin the study group were lower(P<0.05), levels of serum PCT, ET, NO and Lac were lower(P<0.05). There was no statistical difference in clinical efficacy rate between study group and control group (91.89% vs.81.08%), bacterial clearance rate of control group ( 70.27%) was lower than that the study group ( 89.19%) , and the difference was statistically significant ( P <0.05 ) . Conclusion Linezolid in the treatment of MRSA was effective with high safety, and it could reduce serum inflammation index.
4.Investigation and analysis of the sources of pressure for nurses during the prevalence of SARS

Objective To provide basis for the formulation of policies and measures that will reduce the work pressure on nurses and to provide them with a better working environment. Methods An investigation was made via questionnaires into 942 nurses working in designated SARS hospitals and a comprehensive analysis was conducted by means of multivariant stepwise regression. Results The major factors affecting the pressure on nurses include: lack of support from and communications with families, arduousness of tasks to an unbearable point, extreme strictness in management, and unsoundness of treatment and living conditions. Conclusion Good communications with families are of great significance to nurses working in isolated areas. It is imperative to rationally allocate the human resources of nurses, show more concern for them, encourage them to participate in management, and strengthen scientific prevention so as to create a better working environment.
5.Study of changes in Chinese herbal medicine distribution channel.

Distribution channel of Chinese herbal medicines has been changing. From Han to Ming Dynasty, Chinese herbal medicine were mainly trafficked to urban by dealers or farmers; From the Ming Dynasty to the foundation of new China, distribution channels are primarily intermediated with township "bazaar" and national distribution center with fixed place and regularly trading hours. In the planned economy period, the state-owned herbal medicine company was the sole medium with monopoly nature. From the mid1980s to the end of last century, planned economy and market economy have been co-existing. Stepping into 21st century, producing area highlighted in the distribution channels. Presence or absence and rise or fall of different types of distribution market went throughout the changing process of distribution channels, which became an important clue. Changes were motivated by economical consideration of channel subject, which originated from commodity characteristic and social environment changes.

6.Clinical effect of Salmeterol xinafoate and fluticasone propionate powder for inhalation in patients with chronicobstructive pulmonary disease

Objective To observe the clinical effect of Salmeterol xinafoate and fluticasone propionate powder for inhalation in patients with chronic obstructive pulmonary disease.Methods 70 patients diagnosed with chronic obstructive pulmonary disease from March 2014 to March 2015, were randomly divided into two groups ( n =35 ) .Control group were given basic treatment, observation group was given Salmeterol xinafoate and fluticasone propionate powder for inhalation on the basis of control group , patients were followed up and changes of related indicators wererecorded. Results After treatment one month, serum airway remodeling index b-FGF, TIMP-1 values were (93.86 ±17.36 μg/L, 38.06 ±4.28ng/mL) respectively, more than control group(135.03 ±16.06μg/L, 53.95 ±4.15ng/mL)(P<0.05).After treatment one month, inflammatory markers IL-8, TNF-αwere(7.26 ±1.57 pg/mL, 4.29 ±1.02 ng/L)respectively, were more than the control group (14.27 ±1.71 pg/mL, 8.90 ±1.21 ng/L) (P<0.05).After treatment, the observation group, the total effective rate was 91.43%, higher than 77.14%(P <0.05).Conclusion Salmeterol xinafoate and fluticasone propionate powder for inhalation has good effect in treatment of chronic obstructive pulmonary disease , better than the use of basic treatment alone.
7.Multi-slice Spiral CT Manifestations of the Arrhythmogenic Right Ventricular Dysplasia

Purpose To investigate the diagnostic value of the arrhythmogenic right ventricular dysplasia (ARVD) using multi-slice spiral CT (MSCT). Materials and Methods Thirty-four patients who were suspected as ARVD received right ventricular radiography, including 16 ARVD patients and 15 non-ARVD patients regarded as control group. The structural and shaped change of heart on reconstructed images of long axis, short axis and four cavity surface and analyze were observed, and MSCT features of right ventricular radiography characteristics were analyzed for ARVD patients. Results Sixteen cases of ARVD were correctly diagnosed by MSCT, and 14 cases had fatty infiltration including 11 cases of apex of heart, 8 cases of inferior wall, 5 cases of anterior wall, 5 cases of anterior wall of funnel area, 3 cases of diagram, 4 cases of papillary muscle, 6 cases of muscular trabecula and moderator band and 1 case of whole right ventricular free wall. Five cases showed scallop sign, 16 cases excessive trabecular change, 11 cases thinned changes, and 16 cases enlarged changes of right ventricular wall. Conclusion MSCT features of right ventricular for ARVD have relative diagnostic characteristics, and the scallop sign and excessive trabecular change of right ventricular wall are its specific imaging characteristics.

9.Expectant treatment of fresh cervical spinal cord injury in children with non-fracture dislocation

BACKGROUND: Cervical spinal cord injury without radiographic abnormality (SCIWORA) is a special type of spinal cord injury. Because of the particularity of children in the period of growth and development on the aspects of anatomy and biomechanics, their injured mechanism and therapeutic method differ from those of adults.OBJECTIVE: To explore clinical feature and therapy of first-episode cervical SCIWORA in children.DESIGN: Retrospective analysis and self pre-and post-control observation.SETTING: Department of Spine Surgery, Second Affiliated Hospital,Harbin Medical University.PARTICIPANTS: Totally 14 patients with cervical SCIWORA, who were treated at the Departmentof Spine Surgery, Second Affiliated Hospital,Harbin Medical University between June 1997 and June 2003. Inclusive criteria: ①patients who had dysfunction of sensation, exercise and sphincter to different degree in clinic, ②patients who had no fracture dislocation after cervical vertebra X-ray plain film and MRI examination. The MRI examination showed the sign of spinal cord injury. T1WI spinal cord became thick. T2WI distributed at strip-shape high-signal region along spinal prosenchyma and spread upward and downward. There were 3 patients with complete spinal cord injury and 11 patients with incomplete spinal cord injury, among the patients with incomplete injury, there were 6 patients with central cord syndrome (CCS), 3 with Brown-Sequard syndrome and 2withfrontal spinal cord injury syndrome.METHODS: Retrospective analysis was performed in 14 children patients with cervical SCIWORA. Thirteen patients were not treated with operation and one patient was treated with cervical posterior atlantoaxial fusion. MAIN OUTCOME MEASURES: ASIA grading before and after treatment in patients.RESULTS: There were 1 dead patient and 13 cases with 3-year follow up averagely. Three patients were with insignificant amelioration, and others had recovery of limb function to different degree. According to ASIA grading standard, before treatment there were 3 cases in A grade, 5 cases in B grade, 5 cases in C grade and 1 case in D grade, and after treatment there were 1 case recovering to B grade, 2 cases to C grade, 5 cases to D grade and 2 cases to E grade.CONCLUSION: Restoration of neurofunction of children with cervical SCIWORA has closely correlation with degree of primary injury of spinal cord. Most of the patients were not treated with operation, and those with obviously instable cervical vertebra can be treated with operation.
10.Influence of psychological intervention on aged patients with femoral neck fractures when cured by artificial hip replacement

Objective To know the effect of psychological nursing for aged patients with femoral neck fractures when cured by artificila hip replacement. Methods Divided 158 aged patients with femoral neck fractures into the intervention group (84 cases) and control group (68 cases) randomly. Routine nursing cares were used in the control group, while the psychological nursing intervention were used in the intervention group in addition. Compared the degree of postoperative pains and preoperative anxiety between the two groups, and then evaluated the function of limbs. Results The degree of postoperative pains and preoperative anxiety in the interventioni group was better than that of in control group respectively, the condition of limb function was better in the intervention group than in the control group. Conclusions Psychological intervention can promote rehabilitation for aged patients with femoral neck fractures when cured by artificila hip replacement, which can guarantee the satisfactory prognosis.
