1.An animal model of lung transplantation:damage, protection and immune response

BACKGROUND:Establishing an animal model of lung transplantation has operational significance to the development of clinical lung transplantation.
OBJECTIVE:To analyze and summarize animal selection, model establishment method, points to note and mechanism of lung ischemia-reperfusion injury and pulmonary immune rejection in the establishment of animal models as preclinical research of lung transplantation.
METHODS:A computer-based retrieval of CNKI and PubMed database from January 1982 to September 2013 was performed for literature related to lung transplantation. The key words were“lung transplantation, animal models, reperfusion injury”in English and Chinese, respectively. After eliminating duplicate and obsolete literature,50 articles were included for further analysis.
RESULTS AND CONCLUSION:Currently, animal experiments about al ogeneic lung transplantation are
common, which are of significance for clinical lung transplantation. Single lung transplantation is mainly seen in animal models. The commonly used lung transplantation models include single lung orthotopic murine model, rabbit orthotopic lung transplantation, canine and porcine orthotopic left lung transplantation, canine bilateral sequential lung pulmonary transplantation, autologous pig lung transplant model, rabbit lung ischemia-reperfusion model. Because the anatomical and physiological features are similar to humans, pigs are becoming the preferred choice for large animal experiments. Animal experiments and clinical studies have shown that lung ischemia-reperfusion injury in lung transplantation represents a biphasic pattern:early occurrence of lung ischemia-reperfusion injury (24 hours after infusion) is related to lung donors, and the late occurrence mainly depends on receptors. Its pathophysiology runs through the whole process of donor lung resection, preservation and reperfusion as wel as postoperative management. Immune rejection is a complex immune response induced by identifying the donor cellsurface histocompatibility antigen, and activated T lymphocytes play a crucial role in the immune response in organ transplantation.
2.Studies on structure, physicochemical properties and anticancer activity of glycyrrhizic acid aniline-platinum complex compounds

Object To investigate the structure, physicochemical properties and anticancer activity of three kind of glycyrrhizic acid aniline platinum complex compounds. Methods Using the FT IR methods, elementary analysis and X ray diffraction, the physicochemical properties of the tested drugs were determined; by means of MTT test the inhibitory effects of the drugs on human tumor cell growth were examined, with hematopoietic cell K 562 , melanoma cell LiBr and liver cancer cell 7721 as the model of anticancer activity. Results Experimental values of elemental analysis of three complex compounds tally with the theoretically calculative values. The infrared spectroscopy showed that the functional group location agreed with the structure. The X ray diffraction exhibited that this type of the compounds was monoclinic system. Conclusion The experimental results show that the anticancer activity of the test complex compounds is related to their concentration and ligands. Magnetic field treatment has significant influence on anticancer activity of this type of compounds.
3.Cloning and Screening of Mutant Human Hemoglobin Gene(?_(99) ,?_(82) Lys→Cys)

Objective:To mutate the codons which code the lysine at loci ? 99 ,? 82 in human hemoglobin gene to the codons which code the cystein.Methods:The objective gene to be mutated was cloned into pAlter Ex2 plasmis and then was mutated with the site directed mutagenesis mediated by oligonucleotide.Results:The sequencing of clones obtained by preliminary screening showed that the anticipated results were reached without any unexpected mutation.Conclusion:The results lay a foundation for the further development of research on expression and purification of recombinated mutant human hemoglobin gene.
4.Serial Expression of Human ?,? globin Genes in Escherichia coli

Objective To make an attempt to express serialy the human ?,? globin in Escherichia Coli in order to carry out the research on blood substitute based on the genetic recombination of hemoglobin.Methods The routine molecular biology technology was used.Results The serial genetic fragment of ?,? globins containing the hemoglobin was amplified by PCR.The genetic sequencing showed that there was no unexpected mutation.And then,the serial genes were cloned into the pBV220 expression carrier.Undergoing thermal inducement,the expression product could reach about 20% of total bacterial protein.The expression product assumed the form of inclusion body,and it was vertified by Western Blotting.Conclusion This much can lay the solid foundations for further launching the research on expression and pruification of recombinant human hemoglobin genes.

6.A prospective randomized study of TOT and TVT procedure for female intrinsic sphincter deficiency(ISD)

Objective To compare the outcomes of TOT and TVT procedure treating female intrinsic sphincter deficiency (ISD).Methods From May 2010 to September 2015,42 stress urinary incontinence (SUI) patients whose abdominal leak point pressure was less than 60 cmH2O were enrolled in this study.Thirty-five patients were followed up.The mean age was (56.8 ±10.5) years,with a range of 30-80 years.The mean history was(10.5 ± 9.1)years,with a range of 4 months to 30 years,with 26 (74.3%) of them being postmenopausal,5 (14.3%) having a history of pelvic surgery,and no pelvic organ prolapsed or hormone replacement.All of them were randomly divided into 2 groups to undergo either TVT operation (13 cases) or TOT operation (22 cases).The baseline characteristics of the two groups including age,length of history,urodynamic parameters and scale scores showed no significant difference.The scales including urinary incontinence severity score (UISS),detrusor instability score (DIS),Quality of Life Scale Evaluation (I-QOL),lower urinary tract symptoms affect score (UDI-6) were used.The outcomes between TVT group and TOT group were compared.Result After procedure,patients in TVT group got a lower UISS score than TOT group(17.2 ± 2.2 vs.17.7 ± 3.1),and their severity of urinary incontinence improved significantly (P < 0.05).Patients from TVT group got a lower DIS score than TOT group (12.6 ± 4.2 vs.14.2 ± 3.5),and their detrusor instability symptoms improved more significantly (P < 0.05).Patients from TVT group got a higher I-QOL score than TOT group(17.5 ± 14.5 vs.16.1 ± 13.0),and their quality of life improved more significantly (P < 0.05).Patients from TVT group got a lower UDI-6 score than TOT group (10.1 ± 3.0 vs.11.2 ± 3.4),and their lower urinary tract symptoms improved more significantly (P < 0.05).Conclusion Urinary incontinence of female ISD patients were improved greater by TVT than TOT procedure.
7.Comparison of placement of peripherally inserted central catheters using vascular ultrasound guidance system and traditional method in 938 tumor patients

Objective To compare the placement of peripherally inserted central catheters(PICC)by using vascular ultrasound guidance system and traditional method.Methods Totally 938 patients undergoing PICC were divided into the traditional method group and the vascular ultrasound system and microintroducer techniques group according to the puncture time.The differences in the one-attempt success rate and the overall success rate of the placement and the incidences of complications were compared between the two groups.Results The one-attempt success rate was 93.01% in the traditional method group and 98.76% in the vascular ultrasound system and microintroducer techniques group(P =0.005).The successful rate of PICC placement was 100%.The incidence of complication was 11.29% in the traditional method group and 2.47% in the vascular ultrasound system and microintroducer technique group(P =0.000).Conclusion The PICC placement using vascular ultrasound guidance system and microintroducer techniques can increase one-attempt success rate and decrease complications.
8.Clinical experience of 2086 cases of kidney preservation

Objective To summarize the experience of 20 years in using hypertonic citrate adenine (HC A) for clinical kidney preservation. Methods A total of 2 086 cases of kidneys for transplantation from August 1980 to December 2000 were reported.The donors' age ranged from 18 to 49 years.The cadaveric kidneys were harvested with en bloc method,and were perfused via renal arteries with 250 ml HC A at 2~4 ℃ and 100 cm H 2O, and then stored in HC A at 2~4 ℃ throughout preservation.Serum creatinine was measured at 1 week and 1 month after transplantation.Graft survival rates at 1 year and 3,5,10 years and mean graft survival periods were followed up.According to the cold ischemic time,the cases were divided into 3 groups:Group 1, n =573,with cold ischemic time less than 12 h;Group 2, n =779,with 12 to 24 h;and Group 3, n =734,with more than 24 h. Results The mean cold ischemic period of 2 086 cases was (23.68?4.51)h.The mean level of serum creatinine at 1 week and 1 month after transplantation of all cases was (174.5?125.3)?mol/L and (152.5?107.6)?mol/L,without significant differences among the 3 groups.Abnormal renal function at 1 month after transplantation in 66 cases ( 3.16 %) was associated with kidney preservation.There were no differences in graft survival rates at 1 year and 3,5,10 years after transplantation,with graft survival rate at 5 and 10 years in Group 2 reaching 63% and 52%,respectively.The mean graft survival period in Group 3 was significantly less than that in Group 1 and Group 2 ( P
9.Histocompatibility of polyaiticglycolic acid/RNAⅢ inhibiting peptide sustained release microspheres

AIM: RNAⅢ inhibiting peptide (RIP) has been previously proved to possess good histocompatibility and safety for preventing and curing staphylococcal infection, and this study evaluated histocompatibility of polyaiticglycolic acid/RIP (PLGA/RIP) sustained release microsphere. METHODS: The experiment was performed in the Orthopedic Institute, Pharmacologic Research Institute and Animal Experimental Center of General Hospital of Chinese PLA from October 2005 to October 2007.①Preparation of PLGA/RIP: The solid-phase synthesis (Fmoc) method was used to synthesize RIP from C end to N end, then the synthesized peptide was purified by the reverse phase high performance liquid chromatography, and composition was collected by means of ultraviolet absorption peak. The purified RIP was obtained after freezing and drying. Liquid-phase multiple emulsion method was used to synthesize PLGA/RIP microsphere of 50-70 ?m diameter.②Acute general toxicity test was studied in PLGA/RIP. Effect of PLGA/RIP on the cell proliferation was detected with cytotoxicity test by MTT method. Intramuscular implanting test was used to observe the irritation reaction of muscles by implantation materials. Sensitivity test was used to observe the sensitization of PLGA/RIP. Changes of animal's body temperature were determined with pyrogen test. RESULTS: ①Acute general toxicity test: Neither toxicosis reaction nor animal death was found after animals were injected with 100% and 50% eluents of PLGA/RIP peritoneally. Animal's body weight was not changed significantly.②Cytotoxiciity test by MTT method: The average proliferation rate of cell in two kinds of eluents exceeded 85% and cytotoxicity was graded in 1 rank, indicating no cytotoxicity.③Intramuscular implanting test: At 4 weeks after RIP and PLGA/RIP were implanted into the animals, there was not obvious synathresis, denaturation or necrosis in tissues. No inflammatory cell infiltration occurred around the materials. There had been the fibrous capsules around the materials.④Sensitivity test: Average primary irritation index of three groups were 0.38, 0.33 and 0.31 respectively. There was no significant difference among three groups.⑤Pyrogen test: Fervescence of each animal in the experiment was under 0.5 ℃, confirming that the materials had no pyrogenic characteristics. This was in coincidence with evaluation criterion of pyrogen test. CONCLUSION: PLGA/RIP has good histocompatibility and safety, without general toxic reaction, cytotoxicity, immunological rejection, hypersensitive response or pyrogenic characteristics.
10.Acupuncture at Zhibian (BL 54) through Shuidao (ST 28) for polycystic ovary syndrome.

OBJECTIVETo compare the efficacy differences between acupuncture at Zhibian (BL 54) through Shuidao (ST 28) and regular acupuncture for polycystic ovary syndrome.
METHODSOne hundred and one patients, by using random digital table, were randomly divided into an observation group (49 cases) and a control group (52 cases). Patients in the observation group were treated with acupuncture at Zhibian (BL 54) through Shuidao (ST 28) as main acupoints, and accompanied acupoints were selected according to syndrome differentiation. Elongated needles (175 mm in length) were inserted at Zhibian (BL 54) with an angle of 200 with sagittal plane. The insertion depth was 100-115 mm, reaching Shuidao (ST 28) and making acupuncture sensation arrive at diseased location. Patients in the control group were treated at the same acupoints with regular acupuncture technique. The menstruation condition was ignored for patients in both groups. The treatment was given once a day, 15 times as one session, and 5 consecutive sessions were given. The basic temperature and level of sex hormone were observed before and after treatment in the two groups, and clinical efficacy was compared.
RESULTSThe cured rate was 65.3% (32/49) and the effective rate was 91.8% (45/49) in the observation group after treatment, which was significantly superior to 48.1% (25/52) and 63.5% (33/52) in the control group (both P<0.01). The number of patients with typical biphasic fever and patients with atypical biphasic fever in the observation group was higher, than that in the control group, but that of unidirectional temperature was less than that in the control group (P<0.05). After treatment, the follicle-stimulating hormone (FSH), luteinizing hormone (LH), LH/FSH, testos terone (T) were significantly reduced in the observation group (P<0.01, P<0.05); the LH and T were also apparently reduced in the control group (P<.01, P<0.05). The reduction of FSH, LH, LH/FSH and T in the observation group after treatment was more significant than that in the control group (P<0.05, P<0.01).
CONCLUSIONAcuouncture at Zhibian (BL 54) through Shuidao (ST 28) could significantly improve the basic temperature of patients with polycystic ovary syndrome, and regulate hormone level of reproductive endocrine, which is superior to regular acupuncture.
