1.Assessment of the results of an investigation on eliminating iodine deficiency disorders in Ya'an city of Sichuan province in 2009

Objective To evaluate the investigation outcome of eliminating iodine deficiency disorders(IDD)in Ya'an city of Sichuan province,and to master the current situation of IDD in the whole city.Methods In 2009,according toCounties'Assessment Guideline in Sichuan for Implementing the Measure to Eliminate Iodine Deficiency Disorders,assessments were conducted.At county level,composite score was given to collected data of the eight counties(districts)and monitoring data of household iodized salt were statistically analyzed.A township (town)was randomly selected by east,west,south,north and center locations,respectively.One primary school was randomly selected in each selected township(town),eighty children aged 8 to 10 in each selected school were checked of their thyroid by palpation(WS 276-2007)and urinary iodine level of20 children among them was measured by As3+_Ce4+ catalytic spectrophotometry(WS/T 107-2006).Thirty fifth-grade students and 10 housewives lived around school were asked to take a questionnaire survey.Results Average score of the 8 counties(districts)was 89.96,ranged from 85.42 to 92.90.The coverage rate of iodized salt of household was 100.00%(2328/2328),rate of qualified iodized salt was 97.98%(2281/2328).and consumption rate of qualified iodized salt reached 97.98%(2281/2328).The goiter rate of children aged 8-10 was 0.91%(33/3620),median urinary iodine was 235.85μg/L,and 1.50%(12/800)of child's urinary iodine was less than 50μg/L.The rate of passing a knowledge questionnaire survey of the fifth-grade students and women was 92.33%(1 131/1225)and 9413%(397/421),respectively.Conclusions Ya'an city in Sichuan province has reached the goal of eliminating iodine deficiency.disorders,but there is a coexist trend of IDD and iodine excess.They should pay attention to the daily consumption of iodized salt in the future;sustained elimination of IDD can be reached only by iodine supplementation scientifically.
2.Detailed disease management model in community management of hyperuricemic patients

Objective To evaluate the effect of detailed disease management model in community management of hyperuricemic patients.Methods One hundred and fifty patients with primary hyperuricemia were randomly divided into detailed management group and conventional group from April to June 2012,patients in two groups were managed by different models for 1 year.The weight,height,biochemical indexes including blood uric acid and serum lipids were measured and a questionnaire survey was conducted with the items of disease awareness score,smoking,drinking,diet,standard treatment,satisfaction of treatment etc.before and after management.Results One hundred and thirty six patients completed the study including 68 in detailed management group and 68 in conventional group.After one year management,the blood uric acid,triglyceride,low density lipoprotein (all P < 0.01) were statistically lower than baseline in detailed management group,while there were no statistical differences of these indexes in conventional group (P > 0.05).The disease awareness score increased from (65.4 ±5.4) to (82.7±4.2) (Z=-10.2,P< 0.01),high purine diet rate fell from 47.1% to 7.4% (x2 =33.6,P < 0.01),the standard treatment rates increased from 10.3% to 85.3% (x2 =76.6,P <0.01),treatment satisfaction rate increased from 51.5% to 88.2% (x2 =21.8,P <0.01) in detailed management group.In detailed management group,the blood uric acid (Z =-7.4,P < 0.01),total cholesterol (Z =-0.01,P < 0.01),triglyceride (Z =-2.4,P < 0.01) and low density lipoprotein (Z =-3.2,P =0.03) and high purine diet rate were lower (x2 =27.1,P < 0.01),the standard treatment rate (x2 =70.6,P < 0.01) and treatment satisfaction rate were higher (x2 =5.6,P =0.02) than those in conventional group after 1 year management.Conclusion Detailed disease management is an applicable and effective model for community management of hyperuricemic patients.

4.The Changes of Cerebral Oxygenation and its Relation to Severity of Brain Demage After Neonatal Hypoxia

Objective To investigate neonatal cerebral oxygenation after hypoxia.Methods Cerebral oxygenation of 39 babies with perinatal hypoxia and 42 neonates without hypoxia was ob- served under quiet state and stimulation of sound by Near Infrared Spectroscopy(NIRS) and com- paired with the findings of EEG and image studies.Results In normal neonates cerebral oxygena- tion was steady under quiet state,where as it increased after sound stimulation.However,newborns with hypoxia showed inhibited brain activities with little changes in cerebral oxygenation in response to sound stimuli.Episodes of deoxygenation were found in 13 cases during monitoring.Conclusion After perinatal hypoxic brain damage,the function of cerebral oxygenation can be still abnomal for a certain period,which is related to the degree of brain damage.
5.Effect of 3-n-butylphthalide pretreatment on expression of the HSP70 after brain ischemia/reperfusion.

OBJECTIVETo explore the effect of 3-n-butylphthalide pretreatment on the delayed neuronal death(DND) and the expreesion of heat shock protein70 (HSP70) in rat hippocampus after ischemia/ reperfusion.
METHODSAll rats were randomly divided into sham group (n = 36), total cerebral ischemia (TCI) group (n = 36), butylphthalide (NBP) group (n = 6), NBP + TCI group( n = 36), quercetin + NBP + TCI group (n = 6), dimethyl sulfoxide (DMSO) + NBP + TCI group (n = 6). The model of total cerebral ischemia/reperfusion was established by blocking vertebral arteries and carotid arteries. In sham group, TCI group and NBP group, the animals were further divided into instantly, 6 h, 12 h, 1 d, 3 d, 5 d groups according to the time interval after sham operation or TCI. Histological changes of the hippocampus were evaluated using thionin staining under light microscope by determining the delayed neuronal death (DND) and the expression of HSP70 was assayed using immunohistochemistry.
RESULTSNBP pretreatment could reduce delayed neuronal death in CA1 of hippocampus induced by TCI-reperfusion injury in rats, and up-regulated the expression of HSP70 in CA1 hippocampus of brain ischemic/reperfusion for 5 days. Quercetin blocked the acquirement of the brain ischemic tolerance induced by NBP preconditioning.
CONCLUSION3-n-butylphthalide (NBP) prevents the neurons from ischemia/reperfusion injury through upregulating the expression of HSP70.

Objective To investigate the effects of oestrogen-receptor(ER), progesterone-receptor(PR), vascular endothelial growth factor(VEGF) and kinase insert domaincontaining receptor(KDR) in uterine bleeding induced by intrauterine device(IUD). Methods Immunohistochemical method was used to detect the expressions of ER, PR, VEGF and KDR in endometrium from IUD bleeding group and normal control group, respectively. Microvessel density(MVD) was evaluated by using anti factor Ⅷ-related antigen (F8-RA) antibody to mark stromal microvessels. 7.Analysis on sensitivity of contrast-enhanced ultrasound and ultrasonic elastography in detecting different sizes of thyroid papillary carcinoma

Objective To explore the sensitivity of contrast-enhanced ultrasound (CEUS) and ultrasonic elastography(UE) in detecting different sizes of thyroid papillary carcinoma.Methods 64 cases of thyroid papillary carcinoma were divided into two groups according to the diameter size(group 1,≤1 cm;group 2,> 1 cm) and examined by the contrast-enhanced ultrasound and elastography,then the features were analyzed and the sensitivity for detecting of the two methods in different groups were compared.Results The sensitivity for detecting in CEUS was higher than in UE,which had significant difference between two methods.CEUS and UE were valuable for detecting and they had no difference in sensitivity for detecting in group 1.There was a significant difference in sensitivity for detecting between two methods in group 2.Conclusions Both CEUS and UE were valuable in diagnosis of thyroid papillary carcinoma and the sensitivity for detecting in CEUS is higher than in UE in group 2.
8.A Cognition Survey of Basic Doctors on Essential Drugs in Lu-oping County, Yunnan Province

Objective To investigate the cognition of basic doctors on essential drugs in Luo County. Methods The random sampling was used to investigate the basic doctors in Luoping County, and 200 copies questionnaire were distributed. Results The questionnaire response rate was 98%. The survey results indicated that the cognition on essential drugs among basic doctors was poor, but most basic doctors have positive attitudes and behaviors in essential drugs. Conclusion It is necessary to improve the awareness of essential drugs among basic doctors in Luo County.
9.Preliminary Analysis of Tannin-related Constituents in Pomegranate Peel by HPLC-ESI-MS

Objective:To identify the tannin-related constituents in the extracts of pomegranate peel by HPLC-ESI-MS. Methods:The separation was performed on a Kromasil C18 column (250 mm × 4. 6 mm,5 μm) with the mobile phase of water and acetonitrile. The flow rate was maintained at 1. 0 ml·min-1 and the detection wavelength was set at 256nm. The samples were analyzed in negative modes. Results:Five tannin-related constituents including gallic acid, punicalin,punicalagin,corilagin and ellagic acid were identified from the extracts of pomegranate peel. Conclusion:The new method is accurate and rapid. It can be used to identify the tannin-related constituents in pomegranate peel and provide new ideas for the research of active components in traditional Chinese medicine as well.
10.Clinical and genetic research in a Chinese family with Stickler syndrome type 1

Background Stickler syndrome is a genetic connective tissue disorder that affects the ocular,skeletal,orofacial and auditory systems.To determine the gene mutation loci can offer a basis for genetic diagnosis and management of Stickler syndrome.Objective The aim of this study was to research the clinical characteristics of a pedigree with Stickler syndrome and identify the disease-causing gene mutation.Methods This study was approved by Ethic Committee of Peking Union Medical College Hospital.The clinical study and pedigree analysis were performed in one family with Stickler syndrome type Ⅰ (STL Ⅰ).Nine family members were examined with informed consent.The entire coding regions of COL2A1 gene with flanking intronic regions were amplified by PCR and directly sequenced.The detected sequence change was confirmed to be mutationloci by examining whether they existed in normal control individuals.Mutant proteins were predicted with online software.Results There were 4 generations and 11 members in this family,and 2 members died,including 1 patient.Three patients were found in 9living families.Inheritance of this family complicd with an autosomal dominant inheritance mode.All affected individuals showed the consistent phenotypes with STL Ⅰ,including high myopia,membranous vitreous anomaly and surface central flat,short nose,palatoschisis,etc.Mutation screening of COL2A1 gene revealed that the first base of intron 12 was deleted(IVS12+1G del).Nucleotide sequence analysis showed that this mutation led to the functional abnormal of this gene by forming termination cordon in advance.This mutation occurred in all affected individuals,however,no mutation was observed in any unaffected member or 100 normal unrelated individuals.Conclusions This study identifies a novel splice-site mutation(IVS12+ 1G del)in COL2A1 gene in a Chinese STL Ⅰ pedigree.This is the first report on a mutation in a Chinese STL Ⅰ family.
