1.The use of standardized nutritional therapy in gastric cancer patients during perioperation

Objective:To investigate the effect of standardized nutritional therapy for gastric cancer during perioperation.Methods:Seventy-six patients with gastric cancer who underwent nutritional therapy were randomly divided into observation group (38 patients) and control group (38 patients),between May 2015 and May 2016.In the observation group,the nutritional therapy was made by medical workers who had received the training of Total Nutritional Therapy (TNT) and Goal Nutrition Therapy (GNT) for Cancer Patient.In the control group,the nutritional therapy was given by doctors according to their conventional experience.Nutritional conditions,gastrointestinal function recovery conditions,complications and hospital stays were compared between the two groups.Results:First,the weight,triceps skinfold thickness and arm muscle circumference of preoperative 1 day and postoperative 7 day in the observation patients were significantly higher than those in the control patients (P < 0.05).Second,the level of serum prealbumin of preoperative 1 day,postoperative 1 and 7 day in the observation patients was significantly higher than that in the control patients (P < 0.05).Third,the time of anus exhaust and defecate in observation group was significantly shorter than that in the control group (P < 0.01).Fourth,each group had one patient who suffered bleeding after operation,and 2 patients in control group had anastomotic leakage.Fifth,the time of hospital stays in the observation group was significantly shorter than that in the control group (P < 0.01).Conclusion:The standardized nutritional therapy for gastric cancer can improve the patient's nutrition status,promote the recovery of gastrointestinal function,and then decrease the time of hospital stays during perioperation.
2.Development of Android Platform Based Portable Trace Heavy Metal Ions Detector

Based on the Android platform, a portable electrochemical analyzer was designed for the detection of heavy metal ions. Its output voltage range was ±3 V with accuracy of 0. 1% and resolution of <1 mV. The current acquisition range was±10 mA with accuracy of 0. 1% and the minimum resolution of 10 pA. With the human-computer interaction advantage of Android smart devices, professional and fast detection mode which could meet the needs of professional and ordinary users respectively were developed to simplify the complex process of electrochemical detection and analysis. Some common heavy metal ions including copper, cadmium, lead and mercury were detected with this detector. The results of linearity, repeatability and accuracy were satisfactory.
3.Expression of vascular endothelial growth factor and microvessel density in gerbil model with inoculated hepatic alveolar echinococcsis

Objective To study the expression of vascular endothelial growth factor (VEGF) and microvessel density (MVD,CD34) in inoculated hepatic alveolar echinococcsis gerbil model and explore the clinical significance.Methods Sixty healthy gerbils were randomly divided into model group,control group and sham operation group.Each gerbil in both model and control groups had liver vaccination with 0.1 ml immature alveolar echinococcus protoscolex suspension or 0.1 ml phosphate buffered saline by open abdominal surgery,respectively,and the sham operation group only had abdominal surgical procedures.Gerbils were put to death on the 120th day post operation,extracted the eyeball and drawn the blood.The alveolar hydatid tissue,surrounding and distal liver tissue were removed from the model group,and normal liver tissue was collected from the other two groups.The VEGF and MVD-CD34 expression was detected by immunohistochemistry staining,Western Blot,and enzyme-linked immunosorbent assay (Elisa).Results Liver alveolar echinococcsis tissue was only observed in model group.The VEGF expression was observed as yellow or brown yellow granules located in the cytoplasma.The VEGF score and VEGF/β-actin ratio in the different parts were 5.13 ± 1.81 and 1.12 ± 0.05,4.20 ± 1.52 and 0.91 ± 0.02,2.46 ± 1.06 and 0.28 ±0.02,2.20 ±0.77 and 0.30 ±0.02,2.13 ± 1.72 and 0.25 ±0.01,respectively.The VEGF expression in different histological sites showed a gradient change,and bubble hydatid tissues had the highest VEGF expression (P < 0.05).CD34 was expressed in alveolar hydatid tissue and surrounding liver tissue in model group and the CD34 positive cells was stained brown,while CD34 was mainly located in the cell membrane of endothelial cells.The CD34 scores and the CD34/β-actin ratio in the different parts were 5.06 ± 2.46 and 0.66 ±0.01,2.43 ± 1.26 and 0.58 ±0.02,and the other tissues did not have CD34 expression.The alveolar hydatid tissue had higher CD34 expression than any other tissues (P < 0.05).The respective serum VEGF and CD34 level in each group was (106.17 ± 14.17)pg/ml,(38.13 ± 21.36)pg/ml,(42.88 ±17.25) pg/ml,and (1.47 ± 0.28) ng/ml;(0.82 ± 0.12) ng/ml,(0.72 ± 0.16) ng/ml,both are higher in model group than those in the other groups with statistical significance (P < 0.05).Conclusion Angiogenesis is detected in gerbil alveolar hydatid tissues,which might be involved in the infiltrating growth of alveococcus.
4.Analysis on influence factors of asymptomatic cholecystolithiasis prognosis

Objective To study the prognosis of patients with asymptomatic gallstones and explore prognostic factors.Methods The clinical data of 227 patients with asymptomatic gallstones in the First Affiliated Hospital of Shihezi University from December 2013 to March 2016 were collected.All patients were followed with questionnaire,finally 192 cases were had been enrolled,of which 57 cases with clinical symptoms were symptom group and 135 cases with no clinical symptons were asymptomatic group.The prognostic factors were analyzed by un-ivariate and multivariate logistic regression.Results Univariate analysis showed that there were statistically significant difference in the num-ber of stones,stone size,diabetes,high fat diet and body mass index between the two groups(P <0.05).The Logistic regression analysis showed that multiple stones,stone diameter ≥1 cm,diabetes were the major risk factors for prognosis in asymptomatic gallstones.Conclusion Multiple gallbladder stones,stone diameter ≥1 cm,diabetes,high fat diet and obesity may be the major prognostic factors for patients with a-symptomatic gallstones.
5.Complications after procedure for prolapse and hemorrhoids in treating severe mucocutaneous hemorrhoids

Objective To analyze the complications after procedure for prolapse and hemorrhoids (PPH) in treating severe mucocutaneous hemorrhoids.Methods The clinical data of 550 patients with severe mucocutaneous hemorrhoids who were admitted to the Hangzhou Hospital of Nanjing Medical University from January 2005 to December 2013 were retrospectively analyzed.All the patients were treated by PPH or PPH + external hemorrhoids resection.The postoperative complications were recorded and assessed.Patients were followed up via outpatient examination and telephone interview till June 2014.Results Operation was successfully carried out on all the 550 patients.The operation time and duration of hospital stay were (15 ± 10)minutes and (4.2 ± 1.8)days,respectively.A total of 445 patients were followed up for 6-60 months,with the median time of 28 months.The overall incidence of complications was 14.61% (65/445).Forty-five patients were complicated with anal pain,12 with hemorrhoid recurrence,7 with bleeding,5 with dysporia,4 with anastomotic stenosis and 2 with rectal and perianal infection.Of the 12 patients with hemorrhoids recurrence,5 was alleviated by medication and 7 received PPH for the second time.Of the 4 patients with anastomotic stricture,the condition of 2 patients were alleviated by anus dilatation,and the condition of 1 patient was alleviated by balloon dilatation under enteroscope,1 patient received circular incision of the stricture because of severe stricture and failed of conservative treatment.Of the 2 patients with rectal and perianal infection,1 was alleviated by intravenous and local medication,and the other patient who was complicated with anal fistula was alleviated by surgery.The condition of other patients was improved after symptomatic treatment.Conclusions PPH is effective and safe for the treatment of severe mucocutaneous hemorrhoids.The main complication is anal pain,and it could be cured or alleviated by symptomatic treatment.
6.Subgrouping military personnel on Paracel Islands using SCL-90 and cluster analysis

Objective To identify distinct subgroups of military personnel from Paracel Islands based on mental status for providing basis to intervention measures.Methods 174 enlisted military personnel were interviewed by symptom checklist-90 (SCL-90).The factor scores of SCL-90 were compared with army man norm and control group,then the cluster analysis was conducted.Results Interpersonal sensitivity,phobic and psychoticism of military personnel on Paracel Islands((1.65±0.56),(1.24±0.33),(1.44±0.46)) scored lower than those of army man norm (P<0.05).Interpersonal sensitivity of military personnel on Paracel Islands scored lower than those of control (P<0.05),depression and anxiety of them had no difference to control (P>0.05),while other factors of them scored higher than control.According to cluster analysis,174 military personnel were divided into three subgroups.The first subgroup with high scores for all nine SCL-90-R dimensions,the second cluster showed moderate scores and the third cluster had lowest scores.Statistically significant sociodemographic differences could be found between the cluster groups (P<0.05).Conclusion The mental health status of military personnel stationed in Paracel Islands is good on the whole,and can be divided into three clusters with different demographic and service characteristics.
7.Safe region for screw fixation in atlas lateral mass and its clinical application

Objective To investigate the safe region for screw fixation in atlas lateral mass by ra- diologic measurements to conduct the operative procedure.Methods The dimensions of the pedicles were determined in 30 patients with normal atlas by CT scan and three dimensional reconstruction images of the upper cervical spine.The space available for the screw(SAS)was defined as the perpendicular distance between two lines tangential to the spinal canal and the transverse foramen,respectively.SAS was evaluated at 0?(SAS1)and 10?(SAS2)insertion angles.The location of screws in 13 cases of opera- tion were observed by CT scan images measurements of the cervical spine.Results SASI at 0?inser- tion angle was(7.81?1.28)mm and SAS2 at 10?insertion angle was(9.69?1.23)mm,with statisti- cal difference(P<0.01).For SAS1,the distance from the midline of pedicle to the midline of posterior arch and the distance from the center of C2 inferior articular process to the midline of posterior arch showed insignificant statistical difference.However,the entry point at the posterior arch of the midline of SAS2 was located at 2.0 mm lateral to the midline of ASAI.Twenty-six pedicle screws in 13 cases were inserted correctly,without cortex breakage,or nerve and vessel injury.Conclusions There is a safe region for screw fixation in atlas lateral mass.Wider space available for the screw can be acquired when transverse angle is 10?.It is necessary to confirm the safe region by using axial CT of atlas before screw fixation.

OBJECTIVETo evaluate the effect of exteral use of papaverine cream on enhancement of skin graft.
METHODSFive mini-pigs were used for the study. 20 full-thickness wounds were created on the back of each animal. Along the midline of the abdomen, a 40 cm x 2 cm full-thickness skin graft was harvested and the wound sutured.The full-thickness graft was prepared and trimmed to 2 cm x 2 cm of 0.6 mm thickness split-skin grafts, which were transplanted to each wound with tie-over bolsters. The sutures were removed 2 weeks after the operation. Twelve pairs of 100%-survived skin grafts were selected and divided into group A (the left side) and group B (the right side), with each pair locating on the same and opposite position. From the day of suture removal, 2% papaverine cream was used to group A while plain cream was used to group B. The grafts were measured and observed for healing condition at the time of suture removal and the first, second, third, fourth, fifth, and sixth month. The surface area of the graft was measured and the shrinking ratio was calculated. After the animals were killed at the sixth month, samples of the skin grafts were taken and processed with 10% formalin, routine paraffin wax and Hematoxylin-eosin staining. The tissue structure was observed and the results were analyzed statistically.
RESULTSThe grafts in two groups did not have significant differences at the time of suture removal. Observations from the first to the sixth month showed that the two groups had significant differences in skin graft contracture and histological changes. HE stains demonstrated that the skin grafts in group A had less fibroblasts, more microvessels and orderly-arranged collagenous fibers.
CONCLUSIONSExternal use of papaverine cream can inhabit the contraction of skin grafts. It is a safe, effective, simple, and reliable method,which has the advantages of easy delivery,cheapness, less injury and infection,and no side effects.

10.Lower capsular contracture of miniprostheses as compared with the conventional silicone implant.

OBJECTIVETo determine if miniprostheses would form a capsule of significantly different biophysical, biochemical and histologic properties than the conventional silicone implant.
METHODSFour miniprostheses (experimental group) and one big silicone implants (control 1 group) were separately implanted beneath the panniculus carnosus muscle of 30 rabbits. After 3 months, measures related to contracture and capsular histology were performed on anesthetized animals.
RESULTSBaker ranking, capsular incision width and capsular thickness of the control groups were evidently higher than that of experimental groups (P < 0.01). Implant compression of the control groups was evidently lower than that of the experimental group. Histology revealed a thinner, more flexed capsule around the miniprostheses as compared with big silicone implants.
CONCLUSIONSThe miniprostheses form a looser and thinner capsule than the conventional silicone implant.
