1.The angiographical basis of gastroduodenal artery reconstruction in pancreas transplantation

Objective To study the variation of the pancreatic head arteries and its value in pan- creas transplantation.Methods The DSA data of 300 cases were studied retrospectively:192 cases of celiac trunk angiography,102 cases of superior mesenteric artery angiography,and 6 cases of the com- bined.The distribution and variation of the arteries on the pancreatic head were observed.Results In the celiac angiography cases,typical gastroduodenal artery,superior pancreaticoduodenal artery and its branches were visualized in 73 cases(38%).The posterior superior panereaticoduodenal artery was not anastomosed with the posterior inferior pancraticoduodenal artery in 3 cases.The posterior pancre- aticoduodenal arcade was discontinued in 1 case.The anterior superior pancreaticoduodenal artery, posterior superior pancreaticodoodenal artery,and dorsal pancreatic artery were only distributed across the upper head of the pancreas separately but were not anastomosed each other in 1 case.The variation rate of pancreaticoduodenal arterial arcades was 6.8%(5/73).In 102 cases of the superior mesenteric artery angiography,the inferior pancreaticoduodenal artery was visualized in 42 cases(41%)while its branches were not visualized.In 6 cases of the combined angiography,superior panereaticoduodenal artery was visualized in all of the cases,of which anterior and posterior arcades were visualized in 4 cases(66.7%).The gastroduodenal artery reconstruction was performed in 3 cases of clinical pancre- as transplantation,all of the receivers maintained a normal blood glucose level after transplantation and no surgical complications were found.Conclusions The superior pancreaticoduodenal artery might of- fer more blood supply than inferior pancreaticoduodenal artery in the pancreatic head.The arterial re- construction of the transplanted pancreas should include the gastroduodenal artery because of the pos- sibility of arterial variation on the pancreatic head.

3.Histocompatibility of a polypropylene matrix for hair follicle transplantation in rabbits.

OBJECTIVETo observe the histocompatibility of a polypropylene matrix implanted subcutaneously for potential hair follicle transplantation in rabbits.
METHODSThe polypropylene matrix for harboring the hair follicles was prepared and implanted subcutaneously at the neck of 5 New Zealand white rabbits by means of hair follicle unit transplantation. At 1 week after the transplantation and then on a monthly basis in the following 6 months, full-thickness skin tissues were sampled at the site of grafting to evaluate the histocompatibility of the matrix material using HE staining and scanning electron microscopy.
RESULTSAt 1 week after implantation of the matrix material, a small number of inflammatory cells and lymphocytic infiltration were observed around the graft, with mild hyperemia in the proliferative capillaries and mild inflammatory responses. In the following 6 months, the inflammatory cells and lymphocytes around the graft decreased obviously or even disappeared, and such graft rejection responses as tissue lysis and necrosis were not observed. A large quantity of collagen fibers were found to encapsulate the polypropylene material.
CONCLUSIONPolypropylene matrix graft has good histocompatibility with the rabbit subcutaneous tissue without producing obvious graft rejection responses, suggesting its feasibility for further experiments of hair follicle transplantation.

7.Experimental research of hair follicle reconstruction with the aid of embryonic mice dermal cells.

OBJECTIVETo investigate the effect of embryonic dermal signal on the hair-inductive capacity of neonatal mice dermal cells which have been amplified in vitro.
METHODSEmbryonic mice dermal cells of embryonic day 14 were added to a chamber on the back of nude mice with neonatal mice dermal cells which had been amplified in vitro for 3 days and freshly isolated neonatal mice epidermal cells. The hair regeneration was compared between the groups with or without embryonic mice dermal cells. Meanwhile, chambers with following cells respectively were constructed as controls: embryonic mice dermal cells + neonatal mice epidermal cells; freshly isolated neonatal mice dermal cells + neonatal mice epidermal cells; amplified neonatal mice dermal cells only; embryonic mice dermal cells only; freshly isolated neonatal mice dermal cells only; neonatal mice epidermal cells only.
RESULTSThe number of regenerated hairs with the aid of embryonic mice dermal cells (207 +/- 15. 948) was significantly higher than that (67 +/- 8.963) in the group without embryonic mice dermal cells (n = 3, t = 7.653, P = 0.002).
CONCLUSIONEmbryonic dermal signal can enhance the hair-inductive capacity of neonatal mice dermal cells which have been amplified in vitro.

Objective To assesse the quality of life of patients after cardiac pacemaker implantation using the Chinese version of SF-36.Methods Ninety-eight patients with permanent cardiac pacemaker implantation were investigated before and after the operation in terms of quality of life by using the Chinese version SF-36.Results Successful surgery was performed on all the 98 patients.The previous symptoms of the patients were improved to vari- ous extend after the operation.The quality of life of the patients was significantly improved after operation as demon- strated by the significant difference of the scores in 9 domains of SF-36 when compared with those before the operation (P

To investigate the enhancement of immunological activity of CpG ODN by chitosan gene carrier in mice, the effect of lymphocyte proliferation was detected in mice by using MTT, the levels of IgG and cytokines (IL-2 and IL-12) in serum were measured by ELISA and peripheral blood T lymphocyte subsets CD4(+), CD8(+) were analyzed by flow cytometry. Our results showed that spleen lymphocytes isolated from the CS-CpG ODN group of mice showed the strongest proliferation (SI=1.551), and the levels of IgG, IL-2 and IL-12 in serum were higher than those of other groups. Compared with the immunization with CpG ODN, the immunization with CS-CpG ODN gene carrier was more efficient in up-regulating the percentage of CD4(+)T cells and the ratio of CD4(+)/CD8(+) of mice. It was concluded that CS gene carrier of CpG ODN was much more effective in improving immunity of CpG ODN in mice.
10.Evaluation of long term effects of arthroscopic knee debridement and reconstructing for treating osteoarthritis in patients with Kaschin-Beck disease

Objective To observe the long term effects of arthroscopic knee debridement and reconstructing operation for treating osteoarthritis in patients with Kaschin-Beck disease. Methods Thirty-one cases of patients with Kaschin-Beck disease were followed for 6 years after operation of articular clearing by arthroscope. Index of pain, symptoms of self-evaluation, range of motion, walking distance, standing test by affected leg when bending at 30° or 60° were recorded and compared with the preoperative results. Results Twenty-four cases were followed up for 6 years. Six years after operation the pain index(3.38 ± 2.87) was dramatically decreased compared to that before operation (6.88 ± 1.45, t = 5.30, P < 0.05). Patients symptoms markedly improved by subjective self-evaluation was 70.83% (17/24), the effective rate was 100% (24/24). The number of cases that could stand up when leg bending at 30° or 60° were 21,18 cases, respectively, compared with that of preoperative of 14, 11 cases, respectively, the difference was statistically significant(x2 = 5.17,4.27, all P < 0.05). Six years after operation the walking distance(3 cases < 1 km, 11 cases 1 - 5 km and 10 cases > 5 km) were greatly improved compared to the results before operation (12 cases < 1 km, 9 cases 1 - 5 km and 3 cases > 5 km, U = 2.88, P <0.05). Six years after operation the knee activity[(132.25 ± 14.52)°] remained at the same level, compared with that of preoperative [(131 .58 ± 14.68) °], the difference was not statistically significant (t = 0.16, P > 0.05) .Conclusions The method of arthroscopic joint debridement to cure Kaschin-Beck disease knee osteoarthritis can significantly reduce pain, improve function and walking distance, with more stable long-term satisfactory outcome.
