Objective To investigate the clinical features and prognosis of replantation of fingertips distal to the middle nail.Methods Between August 2001 and August 2005,16 fingertips amputated in the Ishikawa zoneⅠwere replanted.We anastomosed only the digital arteries,and adopted postoperative bloodletting.Results Fourteen fingertips survived,and two failed.The survival rate was 87.5%.Of the 14 successfully replanted fin- gertips,12 were followed up from 3 to 12 months.The length and appearance of the fingertips recovered and good joint motion were maintained.Their two-point discriminations ranged from 3 to 6 mm.Conclusion Fingertip replantation is an optimal treatment for fingertip amputation distal to the middle nail to regain satisfactory appearance and function of the digits with a high survival rate,but severe crush injury to the amputated part and high cost should be taken into consideration.

Objective To study the practicality and repeatability of B-mode ultrasonography on diagnosis and score of carotid atherosclerosis. Methods Ninety-nine B-mode ultrasonography pictures of carotid atherosclerosis were obtained from field investigation. According to the established standard(normal scored as 0, other scored as 1 - 7 by the severity), the 99 pictures were diagnosed and scored once together by three trained inexperienced members of the research team. Ten days later, these pictures were diagnosed and scored respectively, by the three members.The diagnostic results were analyzed with statistical methods to test the feasibility and repeatability of this set of diagnostic and scoring system. Results The joint diagnostic results by the three group members were regard as standard with scores 0 in 27 cases, 1 in 27 cases, 2 in 10 cases, 3 in 4 cases, 4 in 13 cases, 5 in 8 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member A was 0 in 29 cases, 1 in 24 cases, 2 in 11 cases, 3 in 2 cases, 4 in 12 cases, 5 in 11 cases, 6 in 6 cases, and 7 in 4 cases. Independent diagnostic results by member B was 0 in 29 cases, 1 in 22 cases, 2 in 14 cases, 3 in 4 cases, 4 in 13 cases, 5 in 5 cases, 6 in 8 cases, and 7 in 4 cases. Independent diagnostic results by member C was 0 in 28 cases, 1 in 25 cases, 2 in 8 cases, 3 in 5 cases, 4 in 14 cases, 5 in 10 cases, 6 in 4 cases, and 7 in 5 cases. Comparison of the 4 diagnostic scores, the difference was not statistically significant(F = 0.019, P ＞ 0.05). The correlation coefficient of diagnostic results of each member were 0.969,0.935,0.928, respectively (all P ＜0.01 ). Diagnostic results of each member were consistent with the standard and the compliance rate were 88.9% (88/99) ,90.9%(90/99) and 86.9%(86/99), respectively. Conclusions B-mode ultrasonography is a non-injure method for diagnosis of carotid atherosclerosis in epidemiology investigation. The method is easy to grasp and has a good repeatability.

Objective To observe blood pressure variability(BPV) and analyze the correlation between BPV and carotid artery-intima media thickness (CA-IMT) in elderly hypertension. Methods Totally 156 patients with primary hypertension were divided into 3 groups:patients aged 45-59years (48 cases) 60-79 years (52 eases),and 80-94 years (56 cases).24 h ambulatory blood pressure monitoring (24 h ABPM) was used to measure 24 h mean systolic blood pressure (24 hSBP),24 h SBP standard deviation (24 h SBPSD),and to calculate 24 h SBP coefficient of variation (24 hSBP CV).Meanwhile,SBP of daytime (DSBP) and nighttime (NSBP),DSBPSD and NSBPSD were measured,and DSBPCV and NSBPCV were calculated.CA-IMT was examined by ultrasound.Spearman rank correlation coefficient was employed to assess the relationship between above indicators and CA-IMT,age. Results No differences were found in 24 h SBP,DSBP and NSBP among the groups.24 hSBPSD( 14.3 ± 3.1),24 h SBPCV ( 11.5 ± 2.1 ),DSBPSD ( 13.8 ± 3.8),DSBPCV ( 10.7 ± 2.6),NSBPSD (13.4±3.3)of patients aged 80-90 years were higher than those of patients aged 60-79 years (12.2±2.5,10.1±2.0,11.6±2.6,9.5±2.3,11.9±2.8)and patients aged 40-59 years (11.7±2.8,9.4±1.9,10.9±3.1,8.4±1.8,11.6±3.0) (P＜0.05).NSBPCVof patients aged 80-90years was increased as compared with patients aged 40-59 years (10.9±2.7 vs.9.6±2.4,P＜0.05).24 h SBPSD,24 h SBPCV were associated with CA-IMT and age(r=0.26,0.39;0.28,0.42,all P＜0.01). Conclusions SBPSD and SBPCV of 24 h ABPM are higher in very older patient with hypertension and could be the useful markers for assessment of BPV.

Objective To observe the change of blood pressure variability (BPV)in elderly hypertension patients,and to analysis the correlation between BPV and stages of renal function damage.Methods 127 elderly primary hypertensive patients with chronic kidney disease(CKD) were divided into three groups:stage 2 CKD group (aged 60-90 years,n=50),stage 3 CKD group (aged 62-93 years,n=40) and stage 4 CKD group (aged 64-94 years,n=37),according to National Kidney Foundation's Kidney Disease Outcomes Quality Initiative (K/DOQI) guide.24 h ambulatory blood pressure monitoring (24 hABPM) was used to measure 24 h mean systolic blood pressure (24 hSBP),24 hSBP standard deviation (24 hSBPSD),and to calculate 24 hSBP coefficient of variation (24 hSBPCV).Differences in above indicators among groups were analyzed by ANOVA.The relationship between above indicators and estimated glomerular filtration rate (eGFR),urinary albumin excretion rates(UAER) were assessed by Spearman rank correlation coefficient.Logistic regression analysis was employed to assess the relationship between BPV and the degrees of impaired renal function.Results 24 h SBPSD and 24h SBPCV were lower in stage 2 CKD group than in stage 3 CKD and stage 4 CKD groups [(11.91±2.56) vs.(13.98±2.89),(14.25±3.16);(9.79±2.01)vs.(11.28± 1.70),(11.53±2.65) ;all P＜0.05].24 h SBPSD,24 h DBPCV and 24 h diastolic blood pressure (24 hdBP) CV were negatively correlated with eGFR (all P＜0.01) and were positively correlated with UAER (all P＜0.01).Logistic regression analysis showed that 24 h DBPSD and 24h SBPCV were related to impaired renal function (both P＜0.05).Conclusions Blood pressure variability is increasing along with impairing renal function in elderly hypertensive patients.Blood pressure variability is associated with renal function damage.

Objective To investigate the trend of the national Kaschin-Beck disease condition, and provide the scientific basis for the government to make preventing strategy. Methods The data were collected from national surveillance on Kaschin-Beck disease(KBD) condition from 2000 to 2007 by restrospective method. The national X-ray detective rates and KBD condition in the eastern and western areas of China were analyzed. Results Monitoring data were collected from 189 monitoring points in 14 provinces of China, and 21 287 X-ray films were photographed of right hands of children from 2000 to 2007. X-ray detective rate was decreased significantly of national KBD from 2000 to 2007. The condition in the west of China is slightly more serious than the eastern areas, but the average of X-ray detective rate was also decreased from 21.75% to 7.30%. National condition is basically controlled. X-ray detective rate had been controlled below 5%, exept for Qinghai, Tibet and Inner Mongolia and so on. Conclusions Ninty percent of the wards have reached the controlled level according to the X diagnosis standard. In terms of the severity of the disease, most of the wards are occupied by patients with slight condition, 10% ward with moderate condition, less than 1% ward with serious condition.

AIM: To determine multifocal visual evoked potential (mVEP) in the prediction of postoperative visual acuity in cataract. METHODS: We examined 30 normal eyes as control and 60 eyes of 52 cataract patients, senile cataract in 27 cases 30 eyes, cataract with glaucoma in 25 cases 30 eyes by mVEP examination. All patients underwent phacoemulsifi-cation (Phaco) and intraocular lens (IOL) implantation. After surgery,best corrected visual acuity (BCVA) was examined at 1 week, 1 month, and 3 months respectively.RESULTS: The mean amplitude and latency in mVEP responses between normal control group were 183±11nV, 95±8ms, and in senile cataract group were 177±10nV, 96± 8ms respectively, there were no significant difference between two groups (P>0.05). The mean amplitude and latency of cataract with glaucoma 138±7nV, 99±6ms were significantly different comparing to both control and senile cataract group (P<0.05). After surgery, the am-plitude and latency were 276±11nV and 93±8ms respec-tively, which did not change significantly comparing to the normal eyes (P<0.05), their visual function got no obvious damage and visual recovery were better (BCVA≥0.8). While those with central amplitude 221±6nV and latency 105±7ms that were significantly deviated from control group (P<0.05), and their visual function were seriously damaged and visual recovery were much poorer (BCVA<0.3).CONCLUSION: mVEP waveform might enable us to evaluate objective visual function detection before cataract surgery. A subject with visual function damage, their mVEP responses to stimulation were severely changed when it compared to controls.
