Objective:To observe the effects of soybean isoflavones(SI) on uteri and uterine Er?,ER? expression in perimenopausal rats. Method:Female SD rats in age of 7-month-old were used as control and 11-month-old SD rats were divided into 5 groups, model group, diethylstilbestrol group and 3 test groups exposed to three doses of SI(ig:30,12,4.6 mg/kg?d) respectively for 35 d. Serum concentrations of estradiol(E2) ,testosterone(T) ,follicle-stimulating hormone(FSH) and luteinizing hormone(LH) were determined. Ultrastructure of uterine luminal surface was observed by scanning electron microscopy and hematoxylin-eosin staining. Expression of ER? and ER? in the uteri were determined by immunohistochemistry. Results:Low dose SI significantly increased serum E2(P

There were some problems in the seven-year pediatric clinical Teaching of Learning on the job in the past.It is insufficient that the teaching mode and teaching form raised the clinical diathesis and competence of medical students.Aimed at these problems, countermeasures are proposed from the following aspects.First of all,let the students learn how to be on good behaviour.Using"students'simulation diagnose and treatment method" enables the students to assimilate the knowledge learned by themselves, use the evidence based medicine(EBM)to give directions to paediatric clinical practice,so as to make the clinical medicine change from the experience type to the science type.

We aimed to study the effect of allitridum (All) on the transient outward potassium current (Ito) of ventricular myocytes of spontaneously hypertensive rats (SHR). Totally 30 male SHRs were randomly divided into three groups: low-dose All group (7.5 mg·kg(-1)), high-dose All group (15.0 mg·kg(-1)) and normal saline group. The other 10 sex and age matched Wistar-kyoto rats (WKY) were also taken as control group (WKY group). All animals received i.p. administration for 8 weeks. The dual enzymatic method was used to separate single ventricular myocyte from animals. Patch-clamp technique was used to record Ito and analyze the effect of All on the current. It was shown that the left ventricular hypertrophy of SHR was reversed significantly by All. Furthermore, the density of Ito was recovered in both high and low dose All groups. The peak current densities of Ito were enhanced from 18.23±3.64 to 25.17±2.86 pA/pF (P<0.01) and 36.47±5.42 pA/pF (P<0.01) at +50 mV by All 7.5 mg·kg(-1) and 15.0 mg·kg(-1), respectively, which was not significantly different with WKY group. The effect was associated with positive shift of the steady-state, close-state inactivation, and shortened recovery from inactivation of Ito. It is concluded that All decreases the remodeling of Ito of ventricular hypertrophic myocytes of SHR.

Objective To investigate the effects of methylprednisolone on the release of cytokines in patients after successful cardiopulmonary resuscitation(CPR).Method Thirty patients after CPR with restoration of spontaneous circulation(ROSC)were randomly divided into two groups:group A(methylprednisolone group,n=14)and group B(control group,n=16)during the period from May 2005 through May 2007.The patients of group were treated with methylprednisolone 3 mg/kg by intravenously twice a day after ROSC.The levels of serum tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10 (IL-10)were measured by enzyme-linked immunosorbent assay(ELISA)before CPR,and 24,48,72 hours and 7 days after ROSC.The data were analyzed studentis t test and chi-gquare test.A P value less than 0.05 indicated significant difference. Results There was on significant difference in the mean time from cardiac arrest to return of spontaneous circulation, and the levels of serum cytokines between the two groups before CPR(P>0.05).In comparisorl with group B,the levels of serum TNF-α,IL-1β,IL-6,IL-8 decreased markedly at 24 and 48 hour after ROSC in group A(P<0.05-0.01),and the levels of serum IL-8 decreased markedly at 72 hours after ROSC in groupA(P<0.05).Thelevels of serum TNF-α,IL-1β,IL-6,IL-8 were not of significant differences between the two groups at 7 hys after ROSC(P>0.05).There was no significant difference in the levels of serum IL-10between the two groups at different time points after ROSC(P>0.05).Conclusions Methylprednisolone plays a role of preventive effects on patients with ROSC after PCR through decreasing the levels of serum TNF-α,IL-1β,IL-6,IL-8.

Objective To investigate the functional esophageal dismotility in patients with gastroesophageal reflux disease(GERD) by simultaneous high frequency intraluminal ultrasonography(HFIUS) and esophageal manometry, and evaluate the correlation between the peak pressure and the peak muscle thickness of the esophagus.Besides,to identify the potential efficacy and feasibility of the HFIUS for evaluating the functional gastrointestinal diseases(FGIDs).Methods Ten GERD patients and 5 healthy controls were enrolled into this study.High-frequency ultrasonic probe and esophageal manometry catheter were simultaneous administrated. The cross sectional image of esophageal motion was recorded by HFIUS and was analyzed by the contractility index (CI)of the longitudinal smooth muscle(LSM)and circular smooth muscle(CSM),the duration of the contraction (DC)and the maximum cross section area(CSA)during distension; In addition, esophageal peak pressure during wet-swaltow was measured by manometry. Results Tile contractility index of the LSM and CSM of esophagus at 5 cm,10 cm,1 5 cm and 20 em proximal to the LES was significantly lower in patients with GERD than that of controls(P<0.05).The duration of the contraction was longer in patients with GERD.But there was no siginificant difference of the maximum CSA during distension between GERD patients and normal controls(P>0.05).However,the simultaneous manometry only demonstrated nonspecific esophageal motility disorder on 6 of 10 GERD patients. And there was positive correlation between the peak pressure and the peak muscle thickness at all sites along the axis of the esophagus, revealed the pressure was increasing along with the thickening of the esophageal muscle layers(r=0.552-0.736).Conclusions Functional esophageal dismotility played an important role in the pathogenesis of GERD.Meanwhile, there was positive correlation between the esophageal pressure and the muscle layer thickness of the esophagus.HFIUS can detect the minor changes of the layer structure of the esophagus and is a potentially useful imaging modality for evaluating esophageal dismotility as well as FGIDs.

Objective To report the clinical and myopathological features in a patient with common variable immunodeficiency (CVID) with myositis. Methods A 33 years old man suffered from recurrent respiratory infection with fever over 10 years. The symptoms improved after anti-infection therapy. At the same time he presented with fatigue. Two years ago he developed general muscle weakness, hypertrophy and myotonia, especially in the hands, neck and thighs.Genetic test for myotonic dystrophy protein kinase (DMPK) and zinc finger protein 9 (ZNF9) was performed.Laboratory tests, electromyography, muscle ultrasound and muscle biopsy were performed. In addition to standard histological and enzyme histochemical stainings, immunohistochemical method was used with primary antibodies of mouse anti human monoclonal antibodies including CD8 for T-lymphocytes,CD20 for B-lymphocytes,CD68 for macrophages and MHC-Ⅰ for muscle membrane. Results Electromyography revealed myogenic changes and abound with myotonic potentials. There was muscle hypertrophy in muscle ultrasound.Lung biopsy showed chronic inflammatory changes.Serum hypoimmunoglobulin and anemia were found.Muscle biopsy showed muscle fiber necrosis and regeneration with lymphocyte and macrophage infiltration. There were no gene mutations in DMPK and ZNF9 gene. Conclusion Muscle hypertrophy and myotonia appeared in CVID with myositis.
