1.Association between white blood cell count on admission and clinical outcomes of elderly patients undergoing percutaneous coronary intervention for acute myocardial infarction

Objective To evaluate whether the prognosis of 6 months major adverse cardiac events(MACE)after acute myocardial infarction,and the reperfusion state of myocardial tissue are related to the white blood cell(WBC)count in elderly patients.Methods A total of 101 consecutive patients aged≥60 years(mean age 70?6 years)who had a primary percutaneous coronary intervention as their first-line therapy for ST-elevation myocardial infarction(STEMI)were enrolled, and both their WBC count on admission and ST-monitoring were available.A prospectively defined cut-off value of WBC count was 10,000/?l,a cut-off 50% measured after 60 min was used as ST -segment resolution(STR).Results The high WBC count group(n=48)showed the significantly higher(52.1% vs 22.6%,P<0.01)incidence of 6 months major adverse cardiac events(MACE)than did the,low WBC count group(n=53).The low WBC count group had 45 patients with STR≥50%, high WBC count group had 23 patients with STR≥50%,STR after PCI was significantly different between the 2 groups(P<0.01).Conclusions The WBC count is a predictor of the adverse outcome in elderly patients with STEMI.Furthermore,elevation in WBC count is associated with poor myocardial microcirculatory reperfusion.
2.QA and QC of 16 Slices Helical CT Angiography in Lower Limb

Objective To investigate the quality assurance(QA) and quality control(QC)of 16 slices helical CT angiography in the lower limb. Methods 42 cases clinically suspected as the arterial disease of lower limb undergoing MSCTA were analyzed retrospectively. All reconstructed images were reformed by means of MIP, VR, MPR and CTVE. Results 22 patients were diagnosed correctly by using MSCTA. Compared to DSA and/or surgical results. Sensitivity, specificity and accurate rate were all 100%. Conclusion It is important to strengthen the QA and QC of the 16 slices helical CT angiography in the lower limb.

4.Clinical value of the plasma 1,3-β-D-glucan detection in the diagnosis of penicilliposis marneffei

Objective To evaluate the clinical value of the quantitative detection of plasma 1,3-β-D-glucan in penicilliposis marneffei. Methods The 1,3-β-D-glucan concentrations were detected by using a G test. Results In 60 New Zealand rabbits infected by Penicillium marneffei,the levels of 1,3-β-D-glucan were positively correlated with infectious severity. The sensitivity, specificity, positive predictive rate and negative predictive rate were 88.3%、100%、 100% and 58.8% respectively,which were significantly higher than those of the control group(P <0.01 ).Conclusion The quantitative method for the detection of 1,3-β-D-glucan in plasma was simple,sensitive and reliable, and could be used as the assisting diagnosis of penicilliposis marneffei at early stage.

OBJECTIVETo explore the scavenging action of tenuigenin (TEN) on intracerebral amyloid β protein (Aβ) aggregation and the abnormal phosphorylated tau protein and its mechanism in Alzheimer's disease (AD) rats' brain.
METHODSAβ1-40 was injected into the right CA1 region hippocampus to establish the AD model. Successfully modeled rats were divided into the model group, the low, middle, high TEN group. Rats were administered with TEN (18.5, 37.0, 74.0 mg/kg) by gastrogavage. Besides, a sham-operation group was set up. Expression levels of Aβ1-40 and Tau p-Ser262 were detected by immunohistochemistry. Expression levels of ubiquitin (Ub) and Ub-protein ligase E3 were measured by Western blotting.The content of 26S proteasome was detected by ELISA.
RESULTSImmunohistochemical results showed that the number of Aβ and Tau p-Ser262 positively reacted neurons significantly increased in model group, when compared with the sham-operation group (P < 0.01). Results of Western blot showed expression levels of ubiquitinated protein were up-regulated and those of Ub-protein ligase E3 were down-regulated in the model group (P < 0.01). ELISA results showed that the content of 26S proteasome significantly decreased in AD rats' brain (P < 0.01). Compared with the model group, expression levels of Aβ1-40, Tau p-Ser262, and Ub significantly decreased; expression levels of Ub-protein ligase E3 apparently increased; the content of 26S proteasome significantly increased in each TEN treatment group (P < 0.05, P < 0.01). Best effect was shown in 37.0 mg/kg and 74.0 mg/kg TEN groups.
CONCLUSIONSUb proteasome pathway (UPP) participated in the occurrence of AD. TEN could obviously reduce intracere- bral Aβ1-40 accumulation and abnormal tau phosphorylation.

7.Clinical application of spiral CT virtual endoscopy to stomach

Objective To explore the imaging method and the value of clinical application of spiral CT virtual endoscopy(CTVE)to stomach.Methods After 36 patients took the aerosis agent or 5% contrast medium,virtual endoscopic images were obtained by using GE-Navigator smooth image model and multiplanar reconstruction.Results Two types of contrasts complemented each other.Normal gastric structure,stenosis and obstruction were clearly visualized in 36 patients.Conclusion CTVE can provide a noninvasive diagnostic method,supplemented to fiberoptic endoscopy,but its sensitivity and specificity should be improved.
8.Diagnostic value of multi-slice spiral CT for lesions in middle and inner ears

Objective To assess the clinical value of multi-slice CT(MSCT)in showing the ear lesions.Methods MSCT high resolution scan images of 52 ears of 11 normal adult and 15 patients with hearing loss were reconstructed,by bone plus with 0.625 mm collimation,9.6 cm small field of vision(FOV)and 0.3 mm reconstruction interval before volume rendering(VR)and multiplanar reformation(MPR)were performed.Results VR,MIP and MPR images of 40 normal ears clearly showed the complicated anatomic structures.VR,MIP and MPR images of twelve abnormal ears displayed 3 ears of malformations of crus longum incudis,1 ear of destructed semicircular canals,2 ears of malleus dysplasia,2 ears of fusion of incudostapedial joint,2 ears of bony atresia of external auditory complicated with auditory ossicles dysplasia,and 2 ears of crus stapedis dysplasia.MPR is easier to manipulate than VR that has three dimensional effect butis difficult to handle.Conclusion HRCT images,combined with post-processing technique of VR,MIP and MPR,can clearly show the anatomic structures and lesions of middle and inner ears.
9.Inhibitory effects of Co-SZ eye drops on apoptosis of lens epithelial cells and their signal transduction mechanisms

AIM: To investigate the inhibitory effect of Co-SZ eye drops on apoptosis of lens epithelial cells(LEC) induced by H_2O_2 and to study the cellular and molecular mechanisms.METHODS:(1) All lenses of Sprague-Dawley rats were incubated with H_2O_2 and Co-SZ eye drops.The apoptosis rates of LEC were determined by TUNEL method.The changes of LEC ultrastructure and the formation of apoptotic body were observed by electron microscopy.(2) Bovine LEC were incubated with H_2O_2 and Co-SZ eye drops.The inhibitory of LEC apoptosis was detected by MTT after incubation.The changes of fractional DNA content in LEC were detected by flow cytometry(FCM).[Ca~(2+)]i,cAMP and cGMP of LEC were determined by spectrofluoremeter and radioimmunoassay,respectively.RESULTS: The LEC apoptosis rates in Co-SZ eye drops group were decreased significantly compared with H_2O_2 group by TUNEL.The ultrastructure changes in LEC of Co-SZ eye drops group were lighter than that in H_2O_2 group.The LEC apoptosis rates of Co-SZ eye drops group were dose-dependently decreased significantly compared with H_2O_2 groups via MTT assay.LEC apoptosis induced by H_2O_2 was inhibited by Co-SZ eye drops,and showing dose-dependent.The DNA contents in LEC of Co-SZ group were increased.The [Ca~(2+)]i and cAMP in Co-SZ group were decreased obviously.The cGMP was increased.CONCLUSION: The LEC apoptosis induced by H_2O_2 was inhibited by Co-SZ eye drops.The mechanism of apoptosis inhibition by Co-SZ eye drops maybe contribute to the increase in DNA content.The signal transduction mechanisms are related to the decrease in [Ca~(2+)]i and cAMP and the increase in cGMP.
10.Effects of early developed sepsis on postoperative immune status in patients undergoing liver transplantation

Objective To investigate immune status changes in liver transplant patients suffering from early developed sepsis.Methods In this study 19 patients undergoing liver transplantation for severe hepatitis from Oct 2008 to Jul 2009 were enrolled.Immune status was compared between patients of severe hepatitis and 20 healthy volunteers.According to whether early sepsis developed or not,patients were divided into sepsis group (HSS) and non-sepsis group (HSNS).T lymphocyte subgroups of the peripheral blood were compared between post-transplant and pre-transplant in these two groups on different stages.Results Comparing to volunteers,T% and IFN-γ/IL-4 of severe hepatitis patients significantly decreased,CD4 + CD25 + Foxp3 + Treg( % ),Foxp3 mRNA and IL-10 significantly increased.Early sepsis developed in 9 patients.Compared with pre-transplant levels,T% in both groups significantly decreased on the first day post transplant.T% in HSNS group increased to the level of pretransplant while T% of HSS group remained at the low level.Treg% ( t =3.265,P =0.004 ) and Foxp3 mRNA ( t =2.750,P =0.013 ) of HSNS group on day 14 decreased significantly lower than that before transplantation.Those two parameters of HSS group even increased slightly.IFN-γ/IL-4 in HSNS group increased significantly on day 3 (t =2.261,P =0.036),while there was no change in HSS group.The concentration of IL-10 in both groups significantly decreased,and the level in HSNS group remained at a low level,while that in HSS group increased on day 14.Conclusions Patients with severe hepatitis have weakened immune status.The imbalance of immune status recovers gradually since 7-14 days after transplantation in patients uncomplicated with sepsis.However,the immune status of receipients complicated with sepsis fails to improve.
