1.Analysis on early predictive value of combination detection of serum calcium and CRP in severity of acute pancreatitis

Objective To investigate the predictive value of C-reactive protein(CRP)and serum calcium in the patients with acute pancreatitis(AP).Methods 85 cases of AP in our hospital from January to December 2013 were selected as the study subjects,in-cluding 40 cases of mild acute pancreatitis(MAP)and 45 cases of severe acute pancreatitis(SAP).40 cases of normal volunteers were selected as the control group.The levels of serum calcium and CRP were detected in the three groups by the automatic bio-chemical analyzer and turbidimetric immunoassay.Results The levels of serum calcium in the MAP and SAP groups was on the low side and significantly lower than that in the control group,but which in the MAP group were higher than that in the SAP group,the differences were statistically significant(P <0.05).The level of serum CRP in the MAP group and SAP group was high-er than that in the control group,and which in the SAP group was higher than that in the MAP group,the difference was statistical-ly significant(P <0.05).The level of serum calcium on the third after admission in the MAP and SAP groups was significantlyin-creased compared with the day of admission(P <0.05),but the levels of CRP in the MAP and SAP groups were significantly de-creased,the difference was statistically significant(P <0.05).The CRP detection was more in clinic,but the experiment indicating that simultaneous detection of serum CRP and calcium could significantly increase the accuracy,sensitivity and specificity of clinical judgement,the difference had statistical significance(P <0.05).Conclusion Serum calcium and CRP can be used as the predictive indexes for the condition progression and prognosis of acute pancreatitis and their joint examination can improve the sensitivity,spe-cificity and accuracy of the detection.
2.Correlation analysis between platelet parameters and efficacy of platelet transfusion and prognosis

Objective To evaluate the clinical significance of detecting platelet parameters in platelet transfusion patients by Investigating the relationship between platelet parameters and efficacy of platelet transfusion and prognosis.Methods A total of 50 patients who underwent platelet transfusion in our hospital in 2014 to 2016 were reviewed.Patients divided into two groups (with normal platelet counts group or subnormal platelet counts group) according to outcomes of blood biochemistry and routine after five days of platelets transfusion.Platelet parameters (platelet count,platelet distribution width,mean platelet volume,large platelet ratio) and CCI were evaluated by statistical analysis.Results The numbers of patients with CCI>4.5 in normal platelet counts group is 21 (77.78%),that in subnormal platelet counts group is 11 (47.83%).Platelet parameters before transfusion showed no significant difference between two groups (P> 0.05).After transfusion,in normal platelet counts group Plt,PDW,MPV and P-LCR were significantly higher than another group (P<0.01).MPV and P-LCR were increased in normal platelet counts group,and decreased in normal platelet counts group after transfusion.Condusion Detection of platelet parameters,especially MPV and P-LCR,is important for evaluating the efficacy and prognosis of platelet transfusion.
3.Effect of perioperative blood transfusion on outcome of patients in orthopedic surgery

Objective To analyze the effect of perioperative blood transfusion in orthopedic surgery.Methods A total of 201 patients who underwent an orthopedic surgery in our hospital in October 2015 to September 2016 were reviewed.Hemoglobin concentrations,hematocrit,postoperative hospital stay,infection and other related data were analyzed.Results 90 patients were transfused with blood and in 111 patients were not during orthopedic surgery (massive blood transfusion group including 17 cases;non massive blood transfusion group including 94 cases).Hemoglobin concentrations of transfusion group (103.09±16.37)g/L was lower than the non transfusion group (115.79± 13.68)g/L before liying hospital,postoperative hospitalization time of transfusion group (12.012±8.36) was longer than that of non transfusion group (16.14 ± 10.47)days.Postoperative infection rate in non transfusion group was 1.11%,and in transfusion group was 15.32%,in massive transfusion group is 52.94%.Conclusion Perioperative blood transfusion is not conducive to the recovery of hemoglobin level patients in orthopedic surgery,prolong hospital stay and increase the rate of infection after surgery.Perioperative blood transfusion makes it difficult to support the outcome of the disease
4.Research on the value of prenatal ultrasound combined with MRI in the diagnosis of fetal pulmonary sequestration

Objective:To investigate the value of prenatal ultrasound combined with MRI in the diagnosis and differential diagnosis of fetal pulmonary sequestration.Methods: The prenatal examination data of 9 cases with pulmonary sequestration confirmed by autopsy and postnatal follow-up was analyzed retrospectively. All fetuses were examined by MR examination performed within 24 hours after ultrasound examination. The results of prenatal ultrasound and MRI were analyzed, and the results were compared with the follow up results.Results: The diagnostic accuracy of ultrasound was 77.78%(7/9), 2 cases were misdiagnosed(one case was misdiagnosed as abdominal mass and one case as pulmonary cystadenoma). The diagnostic accuracy of MRI was 100%.Conclusion: Prenatal ultrasound is the primary method for pulmonary sequestration. As the supplement, MR can display directly the location, shape, size of the lesion. There is high value in diagnosis and differential diagnosis of fetal pulmonary sequestration. The combination of ultrasound and MRI can further improve the diagnostic accuracy of the disease.

Objective To evaluate early changes of femoral artery elasticity in type 2 diabetes mellitus (T2DM) combined with microalbuminuria (MA) patients using ultrasound radio-frequency technology,and to study the clinical significance.Methods 51 T2DM without MA patients were enrolled in Group A,and 53 T2DM with MA patients were enrolled in Group B.In order to observe the early elastic changes in the subjects,two-dimensional ultrasound was used to eliminate patients whose intima-media thickness (IMT) was >1 mm.The femoral arteries were imaged by two-dimensional ultrasonography,and then the IMT,femoral artery inner diameter (D),distensibility coefficient (DC),compliance coefficient (CC),elasticity coefficient (α and β),and pulse wave velocity (PWV) were real-time tested using quality intima-media thickness (QIMT) and quality arterial stiffness (QAS) analyzing systems.Besides,the differences and correlation analyses of these parameters between the two groups were conducted.In addition,some other parameters of sujects in both group A and B were also determined and analysed,including total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C),creatinine (CREA),glycosylated hemoglobin (HbA1c),fasting blood glucose (FPG),2h postprandial plasma glucose (2hPG) and body mass index (BMI).Results There were no significant differences in the elastic parameters of femoral arteries between the male and female subjects (P>0.05),and between left and right sides (P>0.05) within a same group.Compared with group A,group B had lower CC and DC (P<0.05) and higher α,β and PWV (P<0.05).Pearsons correlation analysis indicateal that,CC was negatively correlated with α,β and PWV (P<0.05),and DC was also negatively correlated with α,β and PWV (P<0.05).There was no significant difference in the inner diameters of femoral arteries between the two groups (P>0.05).The levels of TG,HbA1c,FPG and 2hPG in group B were significantly higher than those in group A (P<0.05).There were no significant differences in TC,HDL-C,LDL-C,CREA and BMI between the two groups (P>0.05).Conclusions The decrease of arterial elastic function is earlier than morphological changes,that is,it may have occurred before MA in T2DM patients.The application of ultrasound radio-frequency analyzing technology has certain significance in the clinical early diagnosis and treatment.

Objective:To establish a glioma cell line U251 stably expressing programmed cell death 4(PDCD4)gene,and to observe the influence of exogenous PDCD4 gene on the proliferation and cell cycle of U251 cells.Methods:Recombinant eukaryotic expression vector pEGFP-PDCD4 was transfected into human glioma cell line U251 by Lipofectamine 2000,and the U251 cells stably expressing PDCD4 were established by G418 selection.Reverse transcription polymerase chain reaction(RT-PCR)and Western blotting were employed to detect the expression of PDCD4 mRNA and protein.Furthermore,cell proliferation and colony forming ability were determined by cell counting and colony formation assay;the cell cycle was detected by FACS.Results:High expression of PDCD4 mRNA and protein was observed in U251 cells transfected with pEGFP-PDCD4,whereas no PDCD4 mRNA and protein expression was detected in the non-transfected and vector-transfected cells.Further,cells transfected with pEGFP-PDCD4 grew more slowly and had lower colony formation rate than cells of the other two control groups(P

Objective To know the nursing students' willingness engaging in geriatric nursing and analyze affecting factors so as to provide references for nurturing professionals for caring for the aged.Methods 231 nursing students were surveyed.The questionnaire included general information,life experience with the elderly,willingness engaging in geriatric nursing,attitude towards the elderly and cognition of aging.The investigation results underwent analysis.Results Single factor analysis showed that whether ever living with the elderly (x2=6.027),time living with the elderly and relationship with the elderly (x2=4.928) influeced the nursing students' willingness engaging in geriatric nursing.Multiple correspondence analysis indicated that nursing students living with elderly more than one year and having good relationship with the elderly had positive willingness.Conclusions Geriatric nursing curriculum should be optimized and more opportunities should be created for nursing students to be in touch with the elderly.At the same time,we must pay attention to occupation guide and moral education about how to respect and care for the elderly.
9.Experience of diagnosis and treatment of 19 cases with acute severe viral myocarditis in children

Objective To explore the clinical features and treatment of children with acute severe viral myocarditis.Methods The clinical data of presentation,diagnosis,therapy and prognosis of children who were admitted in our hospital from Jan 2005 to Jan 2012 with acute severe viral myocarditis(severe myocarditis group) were analyzed retrospectively.Twenty-three cases of normal healthy children in the same period were selected as control group.The levels of serum cardiac troponin(CTn)-Ⅰ and N-terminal pro-brain natriuretic peptide(NT-proBNP) were detected by ELISA method,the changes of left ventricular ejection fraction and left ventricular fraction shortening were understood by color doppler echocardiography.Results The level of CTn-Ⅰin severe myocarditis group was significantly higher than that of control group,the difference was statistically significant [(18.67 ± 12.31) ng/ml vs (0.02 ±0.01) ng/ml,P <0.05].Compared with the acute phase,the level of CTn-Ⅰshowed a trend of gradual decline in 7 d [(0.55 ±0.24) ng/ml],basic close to normal in 14 d [(0.06 ±0.03) ng/ml] (P <0.05).The level of NT-proBNP increased significantly in severe myocarditis group compared with control group [(3 067.26 ± 902.79) pg/ml vs (80.04 ± 17.79) pg/ml,P <0.05].Compared with acute phase,the levels of NT-proBNP were closed to normal in 7 d [(648.63 ±342.37) pg/ml] and 14 d [(213.58 ± 129.51) pg/ml] (P < 0.05).The left ventricular ejection fraction [(52.63 ± 6.98) % vs (71.39 ± 2.41) %] and left ventricular fraction shortening [(32.1 ± 2.97) % vs (40.04 ± 2.31) %] in severe myocarditis group were significantly lower than those in control group (P < 0.05).Conclusion Acute severe viral myocarditis of children was characterized by rapid onset,severe illness and high mortality.Early use of adrenal cortical hormone and gamma globulin under the comprehensive treatment and application temporary pacemaker can help patients to recover from the disease.
