1.Study on Serum Levels of Pepsinogen and Gastrin-17 and their Relationships with Helicobacter pylori Infection in Uygur and Han Population with Gastric Cancer in Xinjiang Area

Background:Serum levels of pepsinogen( PG),gastrin-17( G-17)and Helicobacter pylori( Hp)infection are closely associated with gastric cancer. Studies on relationship of serum levels of PG and G-17 with Hp infection in Uygur and Han population with gastric cancer in Xinjiang area are rare. Aims:To investigate serum levels of PG and G-17 and their relationships with Hp infection in Uygur and Han population with gastric cancer in Xinjiang area. Methods:A total of 198 Uygur and 50 Han gastric cancer patients were enrolled,and healthy people with the same race were served as normal controls. Serum levels of PGⅠ,PGⅡ,G-17 were determined by ELISA,PGⅠ/ PGⅡ ratio(PGR)was calculated. Urea breath test and stool Hp antigen test were used to detect Hp infection. Results:Serum PGⅠ level and PGR in Uygur and Han patients with gastric cancer were significantly decreased than those in corresponding control groups(P < 0. 05),and serum levels of PGⅡ and G-17 were significantly increased(P < 0. 05). Serum PGⅠ level and PGR in Uygur gastric cancer patients were significantly decreased than those in Han gastric cancer patients(P < 0. 05),and serum G-17 level was significantly increased(P < 0. 05). Hp infection rate in Uygur and Han patients with gastric cancer were significantly increased than those in corresponding control groups( P < 0. 05). Serum level of PG Ⅰ and PGR were significantly decreased in Han gastric cancer patients with Hp-positive than in Hp-negative patients,and serum levels of PGⅡ and G-17 were significantly increased(P < 0. 05). Serum levels of PGⅠ,PGⅡ and G-17 were significantly increased in Uygur gastric cancer patients with Hp-positive than in Hp-negative patients,PGR was significantly decreased(P < 0. 05). Serum levels of PGⅠ,PGⅡ and PGR were significantly increased in Uygur gastric antral cancer patients than in gastric corpus cancer and proximal gastric cancer patients(P < 0. 05). Conclusions:Decreased serum PGⅠ level,PGR and increased serum level of G-17 can be used as biomarkers to screen gastric cancer in Uygur and Han population in Xinjiang area. Hp infection in Uygur and Han patients with gastric cancer is related to the changes of serum level of PGⅠ,PGR and G-17.
2.Clinical efficacy of different doses of esomeprazole based quadruple therapy on 248 cases of Helicobacter pylori caused gastritis in Uygur

Objective To compare the efficacy of different doses of esomeprazole based quadruple therapy on Helicobacter pylori(H.pylori) eradication in Uygur population, and to study the relationship between H.pylori eradication and dyspepsia symptoms in H.pylori caused gastritis.Methods From August 2015 to May 2016, Uygur patients diagnosed with H.pylori positive superficial gastritis in Xinjiang Uygur Autonomous Region Kashgar Prefecture were recruited.According to random number table, patients were divided into single-dose group, standard-dose group and high-dose group.In single-dose group patients received esomeprazole 40 mg, once per day;in standard-dose group patients received esomeprazole 20 mg, twice per day;and in high-dose group patients received esomeprazole 40 mg, twice per day;besides esomeprazole the treatment of three groups were all combination of amoxicillin, clarithromycin and bismuth, magnesium compound granules.The treatment course of all three groups was 14 days.The H.pylori eradication rate, clinical efficacy, symptom score and adverse drug reaction were compared.The H.pylori eradication rate was evaluated with intention to treat analysis (ITT) and per-protocol sets (PP).Chi square test, t test and one-way analysis of vaiance were used for statistical comparison.Results A total of 248 patients were enrolled,82 cases in single-dose group, 83 cases in standard-dose group and 83 cases in high-dose group.There were five, five and six cases lose to follow-up or dropped out respectively in single-dose, standard-dose and high-dose group.The results of ITT analysis showed that the eradication rates of H.pylori in single-dose group, standard-dose group and high-dose group were 62.2%(51/82),77.1%(64/83) and 89.2%(74/83),respectively;the differences between each two groups were statistically significant (x2=4.34,16.33 and 4.30, all P<0.05).The results of PP analysis showed that H.pylori eradication rates of single-dose group, standard-dose group and high dose group were 66.2%(51/77),82.1%(64/78) and 96.1%(74/77),respectively;the differences between each two groups were statistically significant (x2=5.06, 22.47 and 7.84, all P<0.05).After four weeks of eradication therapy, the total dyspepsia symptom scores of single-dose group, standard-dose group and high dose group were all significantly lower than those before treatment (52.3±27.1 vs 99.4±44.1, 38.2±25.4 vs 101.0±48.9 and 28.8±16.1 vs 99.9±46.6), and the differences were statistically significant(t=5.88, 8.79 and 9.11, all P<0.01).After treatment, the total dyspepsia symptom scores of standard-dose group and high-dose group were both lower than that of single-dose group, and the differences were statistically significant (t=2.78 and 2.91, both P<0.01).The score of upper abdominal pain, epigastric burning, abdominal distention and symptom total score of high-dose group were all lower than those of standard dose group (4.8±3.9 vs 8.1±3.5, 3.1±2.5 vs 5.8±4.1, 7.9±6.8 vs 10.5±9.6 and 28.8±16.1 vs 38.2±25.4), and the differences were statistically significant (t=1.52,1.08,1.23 and 1.96, all P<0.01).After four weeks of treatment, the total efficacy rates of single-dose, standard-dose and high-dose group were 45.5%(35/77), 74.4%(58/78) and 87.0%(67/77),respectively.There were statistically significant differences between each two groups of three groups (x2=13.49, 29.73 and 3.98, all P<0.05).The incidences of adverse drug reaction of single-dose, standard-dose and high-dose group were 6.5%(5/77), 7.7%(6/78) and 7.8%(6/77),respectively;there was no statistically significant difference among the three groups (all P>0.05).Conclusions High dose of esomeprazole based quadruple therapy can increase the eradication rate of H.pylori in Uygur population, and H.pylori eradication therapy can significantly improve the dyspepsia symptoms of H.pylori caused gastritis in Uygur.High dose of esomeprazole may improve the clinical efficacy of H.pylori caused gastritis with dyspeptic symptoms in Uygur with safety.
3.Bacterial Distribution and Antimicrobial Resistance of Nosocomial Infection in Ningxia:Monitoring by Ningxia Hui Autonomous Region Nosocomial Infection Surveillance System

OBJECTIVE To observe bacterial distribution and drug resistance in Ningxia.METHODS The patients with nosocomial infection mornitored by Ningxia Hui Autonomous Region Nosocomial Infection Surveillance System from Jan 2003 to Dec 2007 were analyzed and summarized.RESULTS Amomg 3276 isolates,1752 strains(53.48%) were Gram-negative bacilli,1471 strains(44.9%) were Gram-positive cocci and,53(1.62%) fungi.The most common pathogens were Escherichia coli(909),Staphylococcus aureus(509),S.epidermidis(260),Enterococcus faecalis(258),and Pseudomonas aeruginosa(206).Most of them were multidrug resistant.Most strains of Gram-negative bacilli were highly susceptible to imipenem,while most strains of Gram-positive cocci were highly susceptible to vancomycin.CONCLUSIONS Most pathogens of nosocomial infection are multidrug resistant,the resistance detection of bacteria has an important significance to clinical treatment and infection control.
4.Spatial dynamic distribution and stability of18F-FDG uptake locations within primary tumor during radiotherapy for esophageal squamous cell carcinoma

Background and purpose:Radiotherapy (RT) is one of the most important therapeutic tools for esophageal cancer. Because tumors are heterogeneous, including for18F-FDG uptake and, most likely, for radioresistance, selective boosting of high FDG uptake zones within the tumor has been suggested. Therefore, it is critical to know whether the location of these high FDG uptake patterns within the tumor remains stable during RT.Methods:Twenty-two patients with esophageal squamous cell carcinoma treated with concurrent chemo-radiation underwent repeated18F-FDG PET-CT scans before RT and after 20 fractions of RT. On all scans, the high and low FDG uptake regions were auto-delineated using several standard uptake value (SUV) thresholds, varying from 40% to 70% of SUVmax on the pretreatment scan [gross tumor volume (GTV)40%pre, GTV50%pre, GTV60%pre, GTV70%pre] and from 70% to 90% of SUVmax on the dur-treatment scan (GTV70%dur, GTV80%dur, GTV90%dur) and ifxed thresholds of 2.5 and 5 (GTV2.5pre, GTV5pre). The volumes and overlap fractions (OF) of these delineations were calculated to demonstrate the stability of the high FDG uptake regions during RT.Results:The high uptake regions within the tumor during RT largely corresponded (OF>70%) with the 50% SUVmax high FDG uptake area (GTV50%pre) of the pretreatment scan. The hotspot within the residual area (GTV90%dur) was completely within the GTV and pre-radiotherapy high uptake regions (OF=100%). Although the location of the high FDG uptake patterns within the tumor during RT remained stable, the delineated volumes varied markedly.Conclusion:The location of the high FDG uptake areas within the tumor remained stable during RT. This knowledge may enable selective boosting of high FDG uptake areas within the tumor.
5.Quality Evaluate of Betamethasone Tablet by Establishing a Mathematical Model

OBJECTIVE:To investigate the quality differences of Betamethasone tablet from different enterprises,and provide reference for the eventual establishment of the drug relative quality evaluation system. METHODS:According to the principle and steps of the ideal point method,a mathematical model was established. And the perfect point of the 3 inspection parameters (content,content uniformity and dissolution)of Betamethasone tablet was regarded as the upper limit theoretically,and the permis-sible point as the prescribed minimum in the model. After homogenizing the initial data or ones to be quantifiable were translated in-to evaluation parameters;and the evaluation parameters were calculated by ideal point method to receive comprehensive evaluation results. RESULTS:According to variance analysis,there was no significant difference in the comprehensive evaluation results of Betamethasone tablet from different enterprises(P=0.54>0.05),indicating that the overall quality of Betamehasone tablet was good. CONCLUSIONS:The mathematical model of quality evaluation with different drug has certain feasibility and operability.

Objective To study the relationship between apoptosis regulation and epidermal proliferation in psoriasis. Methods The expression of apoptosis molecule PDCD5 (programmed cell death 5) on psoriatic lesions was observed by direct immunofluorescence. The positive rates and the average of fluorescence intensity of PDCD5 were analyzed quantitatively with FACS and computer CELL Quest software. The expression of PDCD5 mRNA in psoriatic lesions was detected by RT-PCR. Results The expression of PDCD5 protein was obviously lower in psoriatic epidermal cells than that of the normal skin. 8.MRI tracking of transplanted neural stem cells labeled with superparamagnetic iron oxide in ischemic model

Objective To explore the migration of transplanted neural stem cells labeled with SPIO with 4.7T MR system and study cell differentiation by immunohistochemistry in ischemic rats. Methods Rat neural stem cells(NSCs)co-labeled with SPIO mediated by poly-L-lysine and bromode- oxyuridine(BrdU)were transplanted into the middle cerebral artery occlusion(MCAO)rats after 14 days when MCAO model was made successfully.4.7T MR scanner was used to monitor the migration of transplanted cells after 1.2,3,4,5 and 6 weeks post-transplantation.After MRI examination at 6th week,the rats were killed and Prussian blue staining and immunohistochemistric staining were per- formed to study migration and functional differentiation of NSCs.Results Three weeks after trans- plantation,linear hypointensity area derived from migration of labeled NSCs was observed in the corpus callosum adjacent to the injection site.Six weeks after transplantation,linear hypointensity area was moved toward the midline along the corpus callosurn.MRI findings were confirmed by Prussian blue staining and immunohistochemical straining.Co-labeled immunohistochemical methods demonstrated transplanted NSCs could differentiated into astrocytes and neurons.Conclusion MRI is very useful in demonstrating the migration path of labeled NSCs.Transplanted NSCs into ischemic rats can differen- tiated into astrocytes and neurons during the process of migration.
9.Application of copy number variation analysis based on next-generation sequencing in the genetic analysis of missed abortion chorionic villi

Objective To investigate the value of copy number variation analysis based on next-generation sequencing (NGS-CNVA) in the genetic analysis of missed abortion chorionic villi. Methods From August 2012 to May 2014, chorionic villi from 74 cases of missed abortion at 6-13 gestational weeks in Haidian Maternal and Child Health Hospital were collected and analyzed by karyotype analysis and NGS-CNVA. The results of the two methods were compared. Results (1) Karyotype analysis was carried out for the villi from the 74 missed abortion patients. Thirty cases were euploid, 26 cases were aneuploid, while 18 cases had structural abnormalities. The resolution of the karyotyping was 320 bands and the average report time was 22 days. (2) All of the 74 samples obtained NGS-CNVA results and the report time was 7-10 days. (3) The NGS-CNVA results of 56 cases were consistent with karyotype. Among them, 28 cases (28/56, 50%) had no copy number variants (CNV), and 19 cases (19/56, 34%)had CNV between 1 Mb and 10 Mb. 9 cases (9/56,16%) had CNV≥10 Mb found by NGS-CNVA, but not found by karyotyping. (4) According to the results of NGS-CNVA, karyotype were reviewed. The reviewed results found 7 cases with CNV<10 Mb and 3 cases with CNV≥10 Mb in 30 cases which got normal karyotype results at the first analysis. (5) Among the 18 cases of structural abnormalities, 6 cases were Robertsonian translocation. Sequencing technology could confirm the specific area of chromosome deletion/duplication in 8 cases, but could not locate them. Conclusions NGS-CNVA has lower failure rate, higher resolution, lower specimen requirement and shorter report time than karyotype analysis when used for the genetic analysis of missed chorionic villi . NGS-CNVA could be a useful genetic analysis method for the missed abortion villi.
10.Analysis of the Rate of Common Genetic Mutations of Deaf in Pregnant Women

Objective To carry out a molecular screening of Chinese common deafness gene mutations in Chinese pregnant women group,so as to expatiate on the content,provide molecular epidemiological data,reduce the birth rate and provide a theoretical basis to the deaf children. Methods The molecular detection was done to the pregnant women underwent normal antenatal care in our hospital,using gene chips to screen the four com?mon deaf genes(GJB2,GJB3,SLC26A4 and mitochondrial 12S rRNA)in China;then,the newborn infants carrying mutations were treated with the hearing screening,using the methods of Otoacoustic Emissions(OAE)and Brainstem Auditory Evoked Potentials(BAEP),and the husbands of mutation carrying pregnant women were adopted molecular testing of the deaf susceptibility genes in order to investigate the correlation of the rate of pregnant women carrying the mutant genes and newborn infants deafness. Results Totally 2 067 cases of pregnant women were accepted to do the molecular screening,there were 110 cases of deafness mutations detected(5.320%),in which GJB2 gene(67 cases),GJB3 gene(6 cases), SLC26A4gene(33 cases),mitochondrial 12SrRNAgene(4 cases)mutation detection rates were 3.240%,0.290%,1.600%and 0.190%,respec?tively;especially:GJB2gene 235 del C,GJB2gene 299 del AT double mutant 1 case;GJB2gene 299 del AT,GJB3gene 538 C>T double mutant 1 case;GJB2 gene 235 del C,SLC26A4 gene IVS7?2 A>G double mutant 1 case. About 108 cases children newborn accepted to do the hearing screening,in which 3 cases had problems with the left ear,3 cases with the right ear,and 4 cases with the double ears. Conclusion The use of ge?netic deafness gene chip to do the molecular diagnostics in pregnant women can be convenient,fast and efficient for prenatal diagnosis of deafness, which provides a theoretical basis and good method for reducing the birth rate of deaf children and should be popularized more widely.
