? AIM: To explore the curative effect of dacryocystorhinostomy under endoscopy with mitomycin for the treatment of chronic dacryocystitis.?METHODS: Totally 73 cases ( 78 eyes ) with chronic dacryocystitis were treated with dacryocystorhinostomy under endoscopy with mitomycin and followed up for 6-12mo.?RESULTS: In the 73 patients, 66 cases with 70 eyes (90%) were cured, 2 cases with 3 eyes (4%) improved, 5 cases with 5 eyes ( 6%) not changed. In the recurrent 5 eyes, 2 eyes were treated under endoscopy to remove granulation, enlarge the opening, then anesthetic tube was placed after cotton sheet with 0. 4g/L mitomycin was put on the incision for 5min. The rest 3 eyes were treated in superior hospital with laser, and all were successful. There was no severe complication observed.?CONCLUSION:Dacryocystorhinostomy under endoscopy with mitomycin for chronic dacryocystitis is effective.

Objective Using Intelligence Scale of Mini Mental State Estimated (MMSE) as the gold standard to determine the relevance of International HIV-associated Dementia Scale (IHDS)in minority ethnic areas in Guangxi populations with different cultural values.Corresponding boundary value related to the authenticity and reliability on IHDS were also evaluated.Methods 200 patients with HIV infection were randomly selected from the minority ethnic groups in Guangxi.For each infected person,MMSE and IHDS blind scale were tested at the same period.Using the results from MMSE scale test as the gold standard,ROC curve and IHDS scale in Guangxi minority populations with different education levels which related to the diagnosis of dementia-HIV values were determined.The value of a specific sector under the IHDS sensitivity,specificity,and internal consistency coefficients was also evaluated.Results When considering the infected person did not differ on their educational level,the IHDS scale diagnostic cutoff appeared as 8.25,while 1HDS sensitivity as 0.925,specificity as 0.731 and Kappa as 0.477 (P＜0.001).When considering the extent of cultural differences did influence the prevalence of infection,the different education groups showed different IHDS diagnostic cutoff values.People with high school,secondary school or higher education levels,the IHDS diagnosis appeared to be 8.25,when sensitivity was 0.917,specificity was 0.895 and Kappa was 0.722 (P＜0.001).People with only primary education level,the IHDS appeared to be 7.25.When sensitivity was 0.875,specificity was 0.661 and Kappa was 0.372 (P＜0.001).Conclusion The IHDS diagnostic sector in Guangxi minority groups was lower than the internationally recommended level of diagnostic cutoff value (IHDS≤ 10 points).When using IHDS to perform the HIV related dementia screening program,in the minority areas of Guangxi,culture context,the degree and difference of HIV infection should be considered,especially in using IHDS diagnostic cutoff values.

OBJECTIVETo study measurement methods of acromioclavicular and coracoclavicular ligament injuries,its therapeutic effects and complications during internal fixation operation for the treatment of fresh acromioclavicular joint dislocations of Tossy type III.

METHODSFrom July 2003 to May 2012,127 patients with acromioclavicular joint dislocations of Tossy type III were treated with wire fixation from coracoid process to clavicle or hook-plate fixation. The patients were divided into group A (63 cases) and group B (64 cases) according to whether acromioclavicular ligament and coracoclavicular ligament were repaired or not. In group A (ligaments repaired), there were 39 males and 24 females with an average age of (33.25 +/- 8.46) years old (ranged from 17 to 59 years). And in group B (no ligaments repaired), there were 41 males and 23 females with an average age of (34.10 +/- 7.19) years (ranged from 19 to 57 years). The operation times, intraoperative blood loss, postoperative infections, internal fixation failure, recurrence and other complications, together with therapeutic effects were compared between two groups.

RESULTSThe outcome was analyzed according to Karlsson standard. In group A, 54 patients got an excellent result and 9 good according to Karlsson standard;the average operative time was (55.90 +/- 26.56) min; the average intraoperative bleeding amount was (99.80 +/- 50.30) ml; 1 patient had wire broken without re-dislocation at 16 weeks after operation, 3 patients got wound fat liquefaction and recovered after treatment, 1 patient had pain after shoulder joint motion and pain disappeared after implants were taken out. In group B, 52 patients got an excellent result and 12 good according to Karlsson standard; the average operative time was (49.50 +/- 23.14) min; the average intraoperative bleeding amount was (87.30 +/- 46.41) ml; 2 patients got wound fat liquefaction, and 2 patients had pain after shoulder joint motion. All the patients were followed up, and the duration ranged from 9 to 16 months. All internal steel-wire or hook plate were taken out during 4 to 9 months without acromioclavicular joint re dislocation. There were no significant difference in the average operative time, the average intraoperative blood less, complication recurrence rates of fixation failure, wound fat liquefaction, postoperative infection, acromioclavicular joint re-dislocation, and therapeutic effects between two groups.

CONCLUSIONBoth wire and clavicular hook plate fixation, performed for fresh acromioclavicular joint dislocation with Tossy type III, are simple, effective, less invasive method with less blood loss. In addition, the treatment without ligaments repaired could not increase incidence of complications.

In the early diagnosis of knee osteoarthritis, the simple and practical methods such as ultrasonography is becoming a trend, and the specific biomarkers of early knee osteoarthritis have become the hot research. This overview article outlined the methods of early diagnosis from the ultrashort echo time MRI, ultrasonography and biomarkers.

BmK AngM1 is a long-chain scorpion toxin purified from the venom of Buthus martensii Karsch. It has been reported to exhibit evident analgesic effect and low toxicity, and has the potential to be a novel analgesic drug. The BmKAngM1 gene was transformed into Pichiapastoris GS115. Mut+ and Mut(s) recombinant strains were screened by phenotype and Mut+ recombinant strains were used to detect BmK AngMl gene copy number in the real-time PCR. Expression of BmK AngM1 in the Mut+ recombinant strain was compared with that of the Mut(s) recombinant strain with the same single copy of BmK AngM1 gene under the same condition. The results indicated that the transcription level of BmK AngM1 gene in the Mut(s) recombinant strain was 2.7 fold of that in the Mut recombinant strain in the real-time PCR, and the expression of BmK AngM 1 in the Mut(s) recombinant strain was 1.5 fold of that in the Mut+ recombinant strain. Therefore, Mut(s) recombinant strain showed better ability to express BmK AngM1 than Mut+ recombinant strain.

Objective:To explore the possibility of using peritoneal alternatively activated M2 macrophages to prevent rejection after islet allotransplantation in a murine model.Methods:Peritoneal monocytes from C57BL/6 mice were induced and modulated to M2 and M0 macrophages in vitro,then the phenotype of macrophage was assessed by flow cytometry.C57BL/6 mice were induced diabetic by streptozotocin (STZ) injection and transplanted with islets isolated from BALB/c mice under the left kidney capsule.The recipients were randomly divided to 3 groups (n=8).A total of 2.5× 106 M2 macrophages were injected intravenously at 0,3,7 d after transplantation in islet+M2 group;2.5×106 M0 macrophages were injected intravenously at 0,3,7 d after transplantation in islet+M0 group;the mice in islet+PBS group were injected with PBS.Blood glucose was monitored after transplantation.On day 10 after transplantation,2 recipients in each group were randomly selected and sacrificed,and the left kidneys were resected for pathological examination.Results:Achievement of euglycemia was significantly prolonged after islet transplantation in the islet+M2 group than that in the other two groups (P＜0.01).The median survival time of islet allografts in the islet+PBS group,the islet+M0 group,and the islet+M2 group were 6.5 (4-10),7.5 (4-10),and 24(＞ 15) d,respectively.Pathological examination also showed that the grafts in islet+M2 group remained an intact structure with positive insulin stain and no apparent lymphocytes infiltration,while the graft was rejected in other 2 groups with negative insulin stain and massive lymphocytes infiltration.Conclusion:Peritoneal alternatively activated M2 macrophages can prevent rejection after islet allotransplantation,prolong the survival time of islet allografts and enhance the tolerance of the recipient to blood glucose in mice.

The aim of this work was to identify and analyze the chemical constituents of Terminalia chebular Retz by establishing high performance liquid chromatography-linear ion trap/electrostatic field orbitrap combined high resolution mass spectrometry (HPLC-LTQ-Orbitrap MS) technique. With the application of C18 reverse phase column (4.6 mm×250 mm, 5 μm), gradient elution was performed with methanol and water (0.2％ acetic acid) as mobile phase.In negative ionization mode, the data of LTQ-Orbitrap was collected. Accurate molecular mass of molecular ion peaks and fragment ions provided by the high-resolution mass spectrometry were compared with the literature and the reference substance to determine the possible structure of the compound. The results indicated that 62 compounds from the extract of Terminalia chebular Retz. were identified, including acids and hydrolysable tannins (60), triterpenoids (2) . There were14 compounds were detected from this specie for the first time. It is proved as an effective method to provide reference for Chemical composition mass spectrometry and the quality control in further phytochemical studies of Terminalia chebular Retz.

The spectrophotometric method was established for the determination of total tannin content in gardenia medicinal materials and tannins, and HPLC method was applied to simultaneously determine gallic acid, punical glucoside A, methyl gallate, punny glucoside B, corilagin, pentagalloyl glucose and the content of ellagic acid. Taking gallic acid as a reference substance, a phosphomolybdate tungstic acid colorimetric method (Pharmacopoeia method) was used to determine the total tannin content. With the chromatographic column Agilent ZORBAX SB-Aq-C18 (250 × 4.6 mm, 5 microns), mobile phase for phosphoric acid (0.1％), methanol, water flow rate of 1 ml/min, column temperature 30℃, detection wavelength of 270 nm, gradient elution, 7 kinds of component contents in myrobalan medicinal materials were measured at the same time. The result showed that the average content of total tannins of Radix Scutellariae was 30.31％, RSD was 0.55％; the average recovery rate was 99.70％, and the RSD was 21.97％ (n = 6) . In the scorpion medicinal materials, gallic acid, punical glucoside A, punny glucoside B, methyl gallate, punny glucoside B, creatin, pentagalloglucose and ellagic acid have a good linear relationship in their respective ranges. The recovery rate of the sample is between100.10％ and 102.77％. The establishment of the determination method of total tannin, gallic acid, punical glucoside, methyl gallate, corridain, pentagalloglucose and ellagic acid in the scorpion medicinal materials is simple and accurate, which has strong specificity and can be used for quality control of Terminalia chebular.

This paper was aimed to establish the HPLC fingerprint of Terminalia chebular Retz The analysis was performed on Agilent ZORBAX SB-Aq C18 column (4.6×250 mm, 5 μm) . The mobile phase was 0.1％ phosphate watermethanol solution with gradient elution. The flow rate was 1 mL·min-1. The temperature was maintained at 35℃. The detection wavelength was 270 nm. The injection volume was 20 μL. The chromatographic data of 10 samples was analyzed by similarity of chromatographic data, SPSS software and SIMCA software. The results showed that there were17 common peaks in the diagram. The similarity of 10 samples was between 0.984 and 0.999, which showed that there was no significant difference among these chemical components. The samples were classified into four types according to cluster analysis, which shared the similar results as the principal components analysis (PCA) and partial least sq uares analysis (PLS-DA) . VIP analysis result showed that peak 5 (methyl gallate), 11 and 1 (gallic acid), 10, 12 may be the main chromatographic peaks to identify the samples. It was concluded that chemometrics analysis could be used to analyze, find and distinguish the marker ingredients of Terminalia chebular Retz. This method was accurate and stable, which can provide a reference for the quality control and evaluation of Terminalia chebular Retz.
