Objective To evaluate the efficacy of seretide (fluticasone/salmeterol) in treatment of patients with cough variant asthma(CVA). Methods 86 CVA patients were treated with fluticasone/salmeterol propionate 8 weeks, after 8 weeks, 86 patients were randomly assigned to treatment group and control group, each group with 43 patients, the treatment group continued medicine for 4 weeks, the control group without any treatment.The two groups of patients FENO measurement values, cough symptom score before treatment and after treatment for 8 weeks, 12 weeks were compared. Results After treatment for 8 weeks, cough symptom scores and FeNO in the two groups values decreased significantly compared with those before treatment(P<0.05), 12 weeks after treatment, treatment group cough product, FeNO values, were lower than in control group(P<0.05).Conclusion Fluticasone salmeterol powder inhalation can effectively improve the symptoms of cough cough variant asthma.Symptom score and FENO value can be used as a monitoring index to evaluate the therapeutic effect of CVA, and the FENO value is more effective, which has the advantages of non-invasive, simple, safe, rapid and repeatable.

Objective To investigate the possible role and mechanism of the neurotoxic effect of dexamethasone on adult rats after having focal cerebral ischemia. Methods The rat model of focal cerebral ischemia was established by permanent middle cerebral artery occlusion (MCAO) One hour after ishemia,the experimental groups were treated with dexamethasone (5 mg/kg) while the control groups were treated with saline TUNEL staining and In-suit RT-PCR were used to show the changes of apoptosis and the expression of Fas mRNA at ipsilateral cerebral hemisphere. Results TUNEL positive cells were present in a time from 48 h to 72 h, localizing at peripheral ischemic area The expression of Fas mRNA at peripheral ischemic area in control groups began at 12 h, peaked at 24 h, and decreased to a lower level at 48 h and 72 h, and returned to the baseline at 120 h Treatment with dexamethasone after ischemia made apoptosis present at 24 h and the number of TUNEL positive cells at 48 h exceeded that in the control group at 48 h ( P

Objective To study the expression of TGF ? 1 protein and the changes of microglia of adult rats' brain treated with dexamethasone following permanent focal cerebral ischemia as to elucidating the neurotoxic effect of dexamethasone. Methods The adult rat's models of permanent focal cerebral ischemia were established by permanent middle cerebral artery occlusion.One hour after ischemia experimental groups were treated with dexamethasone (0.5 mg?kg -1?d -1) where as the control groups were treated with saline. The size of infarct was detected by Q570 image analysis system. Immunohistochemistry and picture analyses were performed to observe the expression of TGF ? 1 protein. The microglia was demonstrated by histochemical staining with isolectin-B4.? Result Dexamethasone treatment after ischemia increases the infarct volume significantly. The expression of TGF ? 1 and the changes of microglia were mainly located at the border zone of the infarct. The accumulation of the isolectin-positive microglia began at 12 h,peaked at 2d, decreased at 5 d. The densities of microglia in the groups treated with dexamethasone after ischemia decreased significantly at 12 h,24 h and 3 d as compared with the control groups, and disappeared at 5 d.The expression time-course of TGF ? 1 protein was two-phase. The first peak of expression was at 6 h; the second was from 24 h to 72h. The significant decrease in expression of TGF? 1 protein in rats treated with dexamethasone was observed at 6 h,24 h and 72h compared with that of control group. Conclusion The exceeding inhibition of the response of microglia and the two-phase expression of TGF ? 1 protein at the border zone of infarct may play a role in the neurotoxic effect of dexamethasone on cerebral ischemia.

BACKGROUND:Studies have shown that bone loss can lead to a series of diseases, such as osteoporotic fractures, thus seeking to increase bone mass has become a goal of the majority of researchers.
OBJECTIVE:To summarize the current studies of improving bone mass by using stem cel transplantation, hoping to the extensive application of stem cel transplantation in the clinical treatment of osteoporosis as early as possible.
METHODS:A computer-based search of PubMed and CNKI was performed by the first author to retrieve articles relevant to stem cel therapy for osteoporosis published from January 1997 to October 2014. The keywords were“to improve bone mass, regenerative medicine, bone marrow mesenchymal stem cel transplantation, stem cel therapy”in Chinese and English, respectively, which appeared in the title, abstract or keywords. Articles published recently or in authoritative journals were preferred, and final y 28 articles were included in result analysis.
RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cel s which are isolated and cultured easily can proliferate rapidly and have multi-lineage differentiation potential. Studies have shown that the osteogenic differentiation of bone marrow mesenchymal stem cel s can real y improve bone mass, and obtain more achievements in the treatment of orthopedic disorders. This new cel therapy can help to accelerate bone healing and reduce treatment time, offering a new therapeutic choice for orthopedic surgery, plastic surgery, oral and maxil ofacial surgery, and therefore, it has broad application prospects.

Objective: To determine total phenylethanoid glycoside and acteoside in Plantago Herba to provide reference for evaluating the quality of medicinal materials.Methods: With acteoside as the control sample, a UV visible spectrophotometric method was used to determine total phenylethanoid glycosides in Plantago Herba.An HPLC method was applied to determine acteoside in Plantago Herba , and the conditions were as follows: an ODS2 C 18 (150 mm× 4.6 mm ,5 μm) chromatographic column was used with acetonitrile-0.1% formic acid (13∶87) as the mobile phase at a flow rate of 1.0 ml·min-1 , the detection wavelength was 332nm, the column temperature was 30℃, and the sample volume was 10 μl.Results: The reference solution and the sample solution had the maximum absorption at 332 nm, and the linear relationship was good within the range of 0.003 1-0.155 0 mg·ml-1 (r=0.999 5).The content of total benzene alcohol glycosides in 3 batches of samples was 2.73% , 2.61% and 2.84% , respectively;acteoside over the range of 0.000 6-0.155 0 mg·ml-1 (r=0.999 1) showed a good linear relationship with peak area,the sample recovery was 98.5% and the RSD was 1.6% (n =6), and the acteoside content in 3 batches of samples respectively was 0.54% , 0.51% and 0.56%.Conclusion: The method is simple, accurate and reproducible, and can be used for the determination of total phenylethanoid glycosides and acteoside in Plantago Herba.

ObjectiveThe aim of this study was to evaluate and compare the surgical outcomes of endoscop ic and conventional open thyroidectomies in patients with thyroid disease.Methods116 patients with tyroid tumor were enrolled.56 patients underwent endoscopic thyroidectomy ( endoscopic group ),and 60 patients underwent conventional open thyroidectomy( conventional group).We analyzed the patients' clinic characteristics,surgical outcomes and complications between the two groups.ResultsThe blood loss was less in the endoscopic group than the open group[( 16.8 ± 9.6) ml vs ( 24.9 ± 14.2 ) ml,t =- 2.427,P ＜ 0.05].The degree of satisfaction for cosmetic outcome in endoscopy group( 96.4％ ) was higher than that in conventional group ( 16.7％ ) ( x2 =74.508,P ＜ 0.01 ).There was no significant difference in the operating time,volume of drainage and postoperative hospital stay between two groups,and there was no significant difference in the skin ecchymosis,redness and swelling and postoperative pain between two groups( all P ＞ 0.05).No severe postoperative complication was encountered,such as injuries of the re current or superior laryngeal nerve,parathyroid gland injury or massive hemorrhage.ConclusionEndoscopic thyroidectomy has less blood loss,mini-open and excellent cosmetic benefits compared with conventional open thyroidectomy.

Objective：This study was designed to investigate the apoptosis of HL 60 cells induced by the peptide from hemocyte of horseshoe crab (horseshoe crab hemocyte peptide). Methods： Cytotoxicity and cell viability were assayed by MTT method. Morphologic assessment of apoptosis was performed with fluorescence microscope; the apoptosis of the cells was confirmed by flow cytometry; cell cycle was analyzed by flow cytometry, changes of cell membrane were observed by scanning electron microscope. Results： HL 60 cells treated with horseshoe crab hemocyte peptide were showed apparently cytotoxicity with IC50 24 μ g/ml. The morphologic changes including reduction in volume, nuclear chromatin condensation, fluorescence strength were observed with fluorescence microscope. Treated with horseshoe crab hemocyte peptide 50- 100 μ g/ml for 6 h, apoptostic HL 60 cells were predominant. However the HL 60 cells were shown necrotic and apoptostic cells were reduced when treatment with horseshoe crab hemocyte peptide of 200 μ g/ml. SubG1 peak was detected by flow cytometry. Cell cycle analysis showed Phase G1 cells decreased and Phase G2 cells increased with horseshoe crab hemocyte peptide 50 μ g/ml treating for 0- 12 h. Ratio of apoptosis increase depended on concentrations of horseshoe crab hemocyte peptide in 25- 100 μ g/ml but it reduced in 200 μ g/ml, the HL 60 cells were shown necrotic mainly. Scanning electron microscope showed membrane of HL 60 cells was damaged by horseshoe crab hemocyte peptide treatment,cell membrane showed some holes. Conclusion： Peptide from hemocytes of horseshoe crab can induce apoptosis in HL 60 cell,it was early event,and may correlate with membrane damage. The HL 60 cell in Phase G1 seemed to be sensitive to the peptide.

OBJECTIVETo investigate the feasibility and effectiveness of modified Stoppa approach in treatment of bilateral pubic fractures of pelvic.

METHODSThe therapeutic effects of 16 patients with bilateral pubic fractures treated through the modified Stoppa approach from January 2010 to January 2014 were summarized and analyzed, involved 11 males and 5 females with an average age of 40.5 years old ranging from 17 to 59 years. According to Tile classification, there were 8 patients with type A, 6 with type B and 2 with type C. For 16 pelvic fractures, the modified Stoppa approach was used exclusively 11 cases, in combination with the iliac fossa approach in 4 cases, and in combination with the posterior approach in 1 case. The operation incision length, operation time , intra-operative blood loss and postoperative complications were observed. The fracture reduction and post-operative function were assessed by Matta criteria and Majeed system respectively.

RESULTSThe incision length of the modified Stoppa approach ranged from 8 to 10 cm (averaged in 9 cm). The operation time ranged from 75 to 135 minutes (averaged in 95 minutes). The intra-operative blood loss ranged from 400 to 900 ml (averaged in 600 ml). Sixteen patients were followed up from 7 to 18 months (averaged in 12.5 months). The fractures were all healed, the fracture healing time was 2.7 to 5 months (means 3.1 months). There were no infections, ectopic ossification, screw loosening, plate breakage and lateral ventral syndrome. According to Matta criteria for pubic fracture reduction, the result was excellent in 9 cases, good in 6, fair in 1. The Majeed function scores at 6 months after operation was 85.32±8.50; the result was excellent in 8 cases, good in 6 cases, fair in 2 cases.

CONCLUSIONThe modified Stoppa approach has characteristics of convenience and directness of incisions, clear operation field, easy reduction, few complications and fast recovery , it is an ideal choice in surgical treatment of bilateral pubic fractures.

OBJECTIVETo evaluate the risk factors and study the methods of prevention and treatment for the ruptures of aneurysms in keyhole minimally invasive approaches.

METHODSFrom 1999 to 2005, 115 cases of intracranial aneurysms were divided into 2 classes according to the risk factors of aneurysm rupture. Forty-three cases of lower risk underwent microsurgical procedures as keyhole approaches, including pterional approach in 20 cases, supraorbital approach in 18 cases, interhemispheric approach in 5 cases. Seventy-two cases, rest of microsurgical procedures, were performed as conventional craniotomy, including pterional approach in 31 cases, supraorbital approach in 11 cases, interhemispheric approach in 7 cases, pterional-supraorbital in 10 cases, pterional-interhemispheric in 6 cases, supraorbital-interhemispheric in 4 cases, pterional-supraorbital-interhemispheric in 3 cases.

RESULTSSix aneurysms leaked and 3 ruptured (rupture rate 7.0%) treated with keyhole approaches during operations. No one died by keyhole approaches. Eighteen aneurysms leaked and 9 ruptured (rupture rate 12.5%) treated with conventional approaches during operations. Two patients died by conventional approaches.

CONCLUSIONSKeyhole approaches as a time-saving, trauma-reducing procedure could improve the postoperative outcomes, but these approaches still exist probability of aneurysm rupture. It is possible that keyhole-bone flap becomes a limitation to deal with huge or ruptured aneurysms. And it is important to make a specially preventive strategy for aneurysm rupture.
