1.Investigation on medical statistics knowledge command and demand of medical postgraduates with different types

Academic and scientific research are important indices which refer to the medical postgraduates' training quality.But research methods and statistical knowledge are important influencing factor for them. Understanding the demands of statistical knowledge for medical postgraduates can provide reference for adjusting the setting of teaching contents and satisfy the needs of the students.The results have showed that two kinds of medical postgraduates' knowledge of statistical knowledge is weak,while postgraduates with different types have different requirements for statistical knowledge.Therefore more attention should be paid to the needs of the learners of postgraduates with different types to provide more pertinent and effective teaching support service.
2.Combination of arterial infusion chemotherapy and embolization for the treatment of giant carcinoma of kidney in 32 cases

Objective To explore the efficacy of arterial infusion chemotherapy combined with embolization for the treatment of giant carcinoma of the kidney.Methods Arterial infusion chemotherapy combined with embolization was performed through the renal artery in 32 cases of giant carcinoma of the kidney from April 1990 to April 2003.The chemotherapeutic agents were epirubicin(30~40 mg),mitomycin(20~30 mg),and 5-FU(500 mg).The embolization was conducted with anhydrous alcohol,sodium morrhuate,and lipiodol plus gelatin sponge.Results The arterial infusion and embolization was successfully administered in all the 32 cases.Surgical resection was accomplished in 8 cases 1~3 months after the embolization,and little or no blood supply to the tumor was seen during the surgery.Pathological findings revealed marked necrosis of tumor cells with inflammatory cell infiltration around and fibrous proliferation as well as capillary embolization.The total response rate(CR+PR) was 87.5%(28/32) and the tumor decreased in size in 90.6% of the cases(29/32).The survival rates at 6 months,1-,2-,and 3-year were 78.1%(25/32),68.8%(22/32),46.9%(15/32),and 31.2%(10/32),respectively.The mean survival time was 41.5 months,with the longest time of 84 months.Conclusions Combination of infusion chemotherapy and embolization through the renal artery for the treatment of giant carcinoma of the kidney offers good clinical effects.
3.Synthesis of 3-mercaptopropionic acid-stabilized CdTe quantum dots and its application for the detection of Cu2+

The water-soluble 3-mercaptopropionic acid (MPA)-stabilized CdTe (MPA-CdTe) quantum dots (QDs) were synthesized by aqueous suspension.The study showed that the fluorescence quenching process of Cu2+ to MPA-CdTe QDs,whose largest emission peak was 599 nm,could be described well by the theory of fluorescence quenching in competitive absorption systems and its modification of Stern-Volmer equations.By fittings,the results showed a good polynomial relationship between the fluorescence intensity F0/F and the concentration of Cu2+,when the concentration was in the range of 2.28 × 10-6-18.24 × 10-6 mol/L and 4.8 × 10-7-12 × 10-7 mol/L,and two polynomial equations were respectively elucidated based on dynamic and static quenching in competitive-absorption systems:F0/F =7.999-2.470c +0.339 c2,F0/F =3.154-0.160 c +0.049 c2 and degree of fitting are 0.991 and 0.993,respectively.The detection limit was 1.326 × 10-7.
4.Treatment with 1Hz repetitive transcranial magnetive stimulation in auditory hallucination of schizophrenia: A double blind, randomized controlled trial

Objective: To examine the therapeutic effects of dual dorsolateral prefrontal repetitive transcranial magnetic stimulation (rTMS) on auditory hallucinations experienced by schizophrenic patients. Methods: A double-blind randomized controlled study was conducted. Totally 25 schizophrenic patients met DSM-Ⅳ criteria for schizophrenia and had anditory hallucination with stable antipsychotic drugs treatment were recruited. Subjects were grouped into active treatment subgroup and sham subgroup randomly. Measurements including the Positive and Negative Symptom Scale (PANSS) for clinical symptom and the Treatment Emergent Symptom Scale (TESS) for side-effects were evaluated Results: Totally 25 subjects completed the trial. The treatment efficiency of active treatment group was higher than that of sham group (75% vs. 31%) .No overt side effects of rTMS were observed. Conclusion: It is effective for rTMS to treat schizophrenia with auditory hallucination at 1Hz. rTMS may be a new selection of schizophrenia treatment.
5.Clinical significance of regularly monitoring bone marrow minimal residual disease with flow cytometry in patients with acute myeloid leukemia

Objective To evaluate the clinical significance of bone marrow minimal residual disease (MRD) monitoring by multi-parameter flow cytometry (FCM) regularly after the first complete remission (CR1) in patients with acute myeloid leukemia (AML).Methods A total of 63 paitents with AML who had got CR1 after chemotherapy were regularly monitored for MRD in bone marrow by FCM,and MRD ≥ 10-4 was positive.According to the latest standards of National Comprehensive Cancer Network (NCCN) for disease risks,they were categorized into three groups:better risk group (20 cases),intermediate risk group (27 cases) and poor risk group (16 cases).The probability of continuous complete remission (CCR) was calculated by KaplanMeier formula,and the statistical difference between MRD positivc and MRD negative CCR probabilities was evaluated by log-rank test.Results The positive rates of MRD were 20%(4/20),30%(8/27) and 10/16 in better risk group,intermediate risk group and poor risk group respectively.The difference between better risk group and intermediate risk group had no statistical significance (P=0.454),and the difference between poor risk group and intermediate risk group had statistical significance (P =0.035).Twenty-two cases showed positive MRD,and 41 cases showed negative MRD.The probability of CCR at 24 and 36 months in MRD positive patients were 18% (4/22),18% (4/22),in MRD negative patients were 83% (34/41),80% (33/41),and there were significant differences (P < 0.01).Conclusions The dynamic detection of MRD by FCM can be used to evaluate the therapeutic effect and prognosis of AML.MRD monitoring has important clinical significance and can help to adjust the intensity of chemotherapy,carry out individualized treatment,predict prognosis,and choose appropriate therapy.
6.Application of PRVC in laparoscopic surgery

Objective To monitor and compare the breathing mechanics on PC,VC and PRVC during pneumoperitoneum,and to discuss the significance of the clinic use of PRVC.Method Ninety laparoscopic cholecystectomy patients were equally divided into 3 groups (PC,VC,PRVC).Levels of PES,PAWM,PAP,PaCO2,ETCO2,TV MAP and HR were detected before pneumoperitoneum,and at 5,10,15 and 20 minutes postpneumoperitoneum.Results Pneumoperitoneum made three respiratory patterns with different levels of PAWM,PAP,and PES.PES post-pneumoperitoneum in the VC model was obviously higher than that in the PC and PRVC group.At 10 min post-pneumoperitoneum,levels of PaCO2 and ETCO2 increased obviously in the PC and VC group(P < 0.05).Levels of PaCO2 and ETCO2 were increased in the PC group,but TV level post-pneumoperitoneum was significantly lower than that in the other two groups (P < 0.05).Level of PaCO2 and ETCO2 were increased in the PC and VC group post-pneumoperitoneum,along with increases of MAP and HR (P < 0.05).Levels of MAP and HR in the PRVC group post-pneumoperitoneum were significantly lower than those in the PC and VC group (P < 0.05).Conclusion PRVC mode can effectively reduce the increases of pneumoperitoneum-induced PAWM,PAP,PES,without the unusual increase of PaCO2 and ETCO2 during surgeries,contributing to the stability of vital signs of perioperative patients.
7.Construction of plant expression vectors with PMI gene as selection marker and their utilization in transformation of Salvia miltiorrhiza f. alba.

OBJECTIVETo construct plant expression pCAMBIA1301-PMI by substituting PMI for hygromycin resistance gene in pCAMBIA1301 and obtain transgenic Salvia miltiorrhiza f. alba using PMI-mannose selection system.
METHODThe 6-phosphomannose isomerase gene (PMI) of Escherichia coli was amplified by PCR. Sequence analysis showed that it shared 100% amino acids identities with the sequences of PMI genes isolates reported in the NCBI. Based on pCAMBIA1305, the plant expression pCAMBIA1305-PMI was constructed successfully by substituting PMI for hygromycin resistance gene in pCAMBIA1305. pCAMBIA1305-PMI was transformed into Agrobacterium tumefaciens LBA4404, and then the leaves of S. miltiorrhiza f. alba were inoculated in LBA4404 with pCAMBIA1305-PMI.
RESULTPlant expression pCAMBIA1301-PMI was successfully constructed and the leaves of S. miltiorrhiza f. alba inoculated in LBA4404 with pCAMBIA1305-PMI were selected on medium supplemented with a combination of 20 g x L(-1) mannose and 10 g x L(-1) sucrose as a carbon source. The transformation efficiency rate was 23.7%.
CONCLUSIONGenetic transformation was confirmed by PCR, indicating that a new method for obtaining transgenic S. miltiorrhiza f. alba plants was developed using PMI-mannose selection system.

8.Effects of ultraviolet B-induced autophagy on apoptosis in human skin fibroblasts: a preliminary study

[Objective] To observe the effects of autophagy induced by different doses of ultraviolet B (UVB) irradiation on the apeptosis in human skin fibroblasts.[Methods] Skin fibroblasts were isolated from the circumcision specimen of a 23-year-old healthy male,and subjected to a primary culture.After 3 to 10 passages,the cells were collected and applied in the following experiment.Methyl thiazolyl tetrazolium (MTT) assay was performed to evaluate the proliferation of some fibroblasts treated with different concentrations (0,0.5,2.0,5.0and 10.0 mmol/L) of 3-methyladenine (3-MA).To qualitatively and quantitatively detect the autophagy in fibroblasts treated with different concentrations of 3-MA and in fibroblasts treated with 3-MA of 0.5 mmol/Lfollowing UVB irradiation,monodansylcadaverine (MDC) staining was carried out,and immunofluorescence was used to detect the expression of microtubule-associated protein 1 light chain (LC3).Some fibroblasts were classified into 8 groups to remain untreated,be irradiated with UVB of 30,50 and 100 mJ/cm2 alone,treated with 3-MA of 0.5 mmol/L alone,or treated with 0.5 mmol/L 3-MA following irradiation with UVB of 30,50 and 100 mJ/cm2,respectively,then,cell apoptosis was qualitatively detected by Hoechst and propidium iodide (PI)staining,and quantitatively detected by flow cytometry with annexin V and PI.[Results] The percentage of autophagic cells was (63.037 ± 5.876) % in fibroblasts treated with starvation condition,significantly decreased to (34.425 ± 5.183) % in fibroblasts treated with 3-MA of 0.5 mmol/L.The expression of LC3 showed a gradually increasing trend from untreated fibroblasts,to fibroblasts irradiated with UVB of 30,50 and 100 mJ/cm2,while the increase was attenuated by the 4-hour treatment with 3-MA immediately after the irradiation.Compared with the other concentrations,the 3-MA of 0.5 mmol/L showed the least influence on the viability of fibroblasts.The addition of 3-MA of 0.5 mmol/L increased the percentage of cells both positive for Hoechst and PI staining in fibroblasts irradiated with UVB of 50 mJ/cm2,but decreased that in fibroblasts irradiated with UVB of 100 mJ/cm2.Similarly,the percentage of middle and late apoptotic cells was significantly higher in fibroblasts irradiated with UVB of 50 mJ/cm2 followed by treatment with 3-MA of 0.5 mmol/L than in those irradiated with UVB of 50 mJ/cm2alone ((10.933 ± 0.839) % vs.(7.267 ± 0.473) %,t =5.20,P< 0.05),but lower in fibroblasts irradiated with UVB of 100 mJ/cm2 followed by treatment with 3-MA of 0.5 mmol/L than in those irradiated with UVB of 100 mJ/cm2alone ( (7.100 ± 0.781 ) % vs.( 1 0.133 ± 0.681 ) %,t =6.29,P < 0.05 ).[Conclusion]s The irradiation with UVB of 50 mJ/cm2 may protect fibroblasts by inducing autophagy and suppressing apoptosis,while the high level of autophagy induced by UVB of 100 mJ/cm2 may lead to autophagic cell death in fibroblasts.
9.Assessment of apparent diffusion coefficient in clinicopathologic and prognostic features of rectal cancer

Objective To assess the value of apparent diffusion coefficient (ADC)in clinic ,pathology and prognosis of rectal cancer. Methods The MRI and DWI findings of 109 patients with pathological proved rectal adenocarcinoma were retrospectively analyzed. DWI with b=0 s/mm2 and b= 1 000 s/mm2 were acquired.Mean tumor ADCs were measured and compared between subgroups stratified by histologic differentiation grades,T-stage,N-stage,mesorectal fascia status and presence of lymphangiovascular or peri-neral invasion.Results Mean tumor ADCs were significantly different when comparing groups stratified by histologic differentiation grades,T-stage,mesorectal fascia status and presence of lymphangiovascular invasion.Tukey’s post hoc test showed that the differences of mean ADCs between good-moderate differentiated group and moderate differentiated group(P =0.996),moderate-poor differentiated group and poor differentiated group(P =0.957)were not significant.The differences among other groups of differentia-tion grades differed significantly(P <0.05).In the T-stage groups,the mean ADCs of T1 stage tumor was significantly higher than that of T3 stage tumor(P <0.05).There were no significant differences among other T-stage groups(P >0.05).There were no sig-nificant differences among N0,N1 and N2 in N-stage groups(P >0.05).Conclusion ADC values can reflect pathologic and prognos-tic features of rectal cancer.
10.Ginsenoside Rg1-induced alterations in gene expression in TNF-alpha stimulated endothelial cells.

BACKGROUNDIn China the ginseng root began to be used in medicine over 2000 years ago. Ginsenosides are the most important component isolated from ginseng. The authors investigated the effect of ginsenoside Rg1 on the spectrum of gene expression in the endothelial cells stimulated by TNF-alpha and further explored the potential molecular mechanism of endothelial protection by ginsenoside Rg1.
METHODSNitric oxide (NO) production in the cultured human umbilical vein endothelial cells (HUVECs) was measured by using an NO assay kit. A home-made oligonucleotide microarray containing approximately 400 cardiovascular disease-related genes was constructed. The alteration of the spectrum of gene expression induced by ginsenoside Rg1 in HUVECs which were activated by TNF-alpha were detected by oligonucleotide microarray analysis.
RESULTSNO production in HUVECs was decreased significantly after TNF-alpha treatment, while pretreatment with ginsenoside Rg1 enhanced NO production in TNF-alphastimulated HUVECs. Ginsenoside Rg1 affected the expression levels of genes involved in vascular constriction, cell adherence, coagulation, cell growth and signal transduction in TNF-alphastimulated HUVECs.
CONCLUSIONSGinsenoside Rg1 could enhance NO production and the expression of eNOS mRNA in TNF-alpha stimulated HUVECs. Ginsenoside Rg1 regulated sets of genes in endothelial cells and protected endothelial cells from TNF-alpha activation. Microarray analysis provided us with valuable insights into the atheroprotective mechanism by gingsenoside Rg1.
