1.Evaluation of urine analysis by flow cytometry and strip test in diagnosing urinary tract infection

Objective To evaluate the clinical application of automated urine formed elements analyzer and/or urine dipstick analyzer for examination of urinary formed elements in screening urinary tract infection (UTI). Methods 148 fresh midstream clear-catch urine samples from the UTI patients and 284 fresh midstream clear-catch urine samples from non-UTI subjects were selected. Bacteria culture was performed for bacterial colony counting and identification. Bacteria counts ( BACT), yeast-like fungus and WBC were performed by UF-looOi automated urine formed elements analyzer. Leukocyte esterase test (LEU) and nitrite test (NIT) were performed by URISYS 2400 urine dipstick analyzer. We evaluated data obtained from urine dipstick analyzer, UF-1000i and combination of UF-1000i with urine dipstick analyzer and the results was compared with those obtained from quantitative bacterial culture. Then we evaluated the sensibility, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy. Results Among the 148 patients with UTI, the positive rate of the quantitative bacterial culture was 73.6% (109/148), the positive rate of LEU and NIT detected by dipstick test 26. 4% (39/148).There was significantly statistical difference between bacterial culture and strip test(χ2 = 55.68 ,P < 0. 05 ). The positive rate of urine flow cytometry by UF-1000i with either positive of BACT and WBC was 91.2%(135/148), which was higher than the positive rate of the quantitative bacterial culture. There was significant difference between two methods (χ2 = 14. 70, P < 0. 05 ). The positive rate of anyone positive among BACT, WBC, LEU and NIT was 94. 6% (140/148) when detected with combination of dipstick test and UF-1000i, which was higher than the positive rate of the quantitative bacterial culture. And there was significant difference between two methods (χ2 = 20. 45, P < 0. 05 ). The sensitivity of dipstick test was low (26. 4% ,39/148 ), and specificity was high ( 99. 3%, 282/284 ) . The sensitivity, specificity, positive predictive value, negative predictive value of BACT detected by UF-1000i in diagnosing urinary tract infection were 92. 6% ( 137/148 ), 39. 8% ( 113/284 ). 44. 5% ( 137/308 ) and 91.1% ( 113/124 ), respectively. If the dipstick test was combined with UF-1000i, the sensitivity, negative predictive value, specificity, positive predictive value and accuracy were 98.0% ( 145/148 ), 97.1% ( 100/103 ). 35.2% (100/284) ,44. 1% (145/329) and 56. 7% (245/432), respectively. Conclusions The combination of urine dipstick test and automated urine formed elements analyzer UF-1000i plays an important role in early diagnosis of UTI. And it has significant value in diagnosis of UTI, especially for the patients with negative bacterial cultures of urine sample.

Objective:To investigate the effects of mucosal strengthening treatment on preventing recurrence of closedvarices. Methods: In 264 patients with esophageal varices obliteration by banding ligation and sclerotherapy, of 136 patientswere gone on to treat with strengthening mucosa, and 128 patients without strengthening treatment. BACKGROUND: How to promote the regeneration after peripheral nerve injury, especially after large defects, is a difficulty to be solved.OBJECTIVE: To review the mechanism of Wallerian degeneration and the development of nerve conduits.METHODS: PubMed database was searched for the literatures addressing the modular mechanism of Wallerian degeneration and nerve repair using the English keywords peripheral nerve regeneration, Wallerian degeneration, nerve guidance conduits. A total of 74 eligible literatures were included based on the exclusion criteria.RESULTS AND CONCLUSION: Rat SARM1 and fruit fly dSARM have been found to be highly implicated in Wallerian degeneration, indicating that the changes of nicotinamide adenine nucleotide/nicotinamide adenine dinucleotide may be related to the activated SARM1. Whether delaying Wallerian degeneration is good or bad is still in dispute. The future study should focus on the early inhibition of Wallerian degeneration and promotion of neuroregeneration following peripheral nerve injury.

To estimate the tolerance limit and the tolerance interval of disease incubation,under the theory that the observations(samples)were subject to Poisson distribution,the tolerance limits and tolerance interval of disease incubation were calculated based on beta-distribution with integer parameter.Expressions on the relation were obtained,including the statistics on tolerance limits of both minimum and maximum orders while the tolerance was the difference between minimum and maximum order statistics and the parameters of Poisson distribution on the sample size.Using the incomplete observations as an example,reasonable unit of sample size was considered and chosen.

Aim To observe the antifibrotic effect of Radis Astragali and to explore the mechanism. Methods The fibrosis of animal model was induced with CCl4,and the model animals were treated with Radis Astragali in treatment group and saline in control group respectively. Results The serum hyaluronic acid (HA), fibrosis score and ICAM_1_positive hepatocytes all the decreased in the treatment group as compared to those in the control group.Conclusion Radis Astragali has satisfactory effect on experimental fibrosis. The mechanism may be correlated with its affection on ICAM_1 in liver tissue.

8.Clinical application of percutaneous endoscopic gastrostomy in gastrointestinal nutrition among critically ill pa-tients

Objective To explore the clinical application of percutaneous endoscopic gastrostomy in gastroin- testinal nutrition among critically ill patients.Methods Twenty-two patients were undertaken percutaneous endo- scopic gastrostomy under the lead of gastroscope.The puncturatio site was located in anterior wall of stomach.Results All the procedures were performed successfully for one time.Intraoperational blood pressure was very steady.At the same time,intraoperational SpO_2 of the patients all exceeced 97%.The bleeding amount and operation time were respectively (3.8?1.9) ml and (15.5?2.3) min.Severe complications such as gastrostoma,gastrocolic fistula, pneumoperitoneum,refluxing or aspiration of gastric juice,inhalant pneumonia didn't occur after the operation.Con- clusion Percutaneous endoscopic gastrostomy was safe and feasible among critically ill patients who needed gastroin- testinal nutrition because of its slight injury,little bleeding and shorter operational time.
9.Development of a multiple ligator and its endoscopic therapy for patients with esophageal varices.

The ligators we have developed is a kind of economical and effective six-ring ligator. Endoscopic variceal ligation (EVL) was performed to treat bleeding from esophageal varices in patients with liver cirrhosis using self-made ligator and foreign multiple ligator. There are similar effects with both self-made ligator and foreign mutiple ligator in the control of variceal bleeding, variceal obliteration and rebleeding (93.8%, 87.5%, 0 in the group with self-made ligator, 94.5%, 87.1%, 2.4% in the group with foreign multiple ligator, P>0.05). In terms of the quality index, successful operation rate, hemastatic rate, variceal obliteration rate, rebleeding rate, complications and variceal recurrence rate, the self-made ligator is as good as the foreign multiple ligator, but much cheaper.
