2.Clinical Observation on Psychological Rehabilitation in Patients with Cerebral Apoplexy

Objective To discuss the value of applying psychological therapy for patients with cerebral apoplexy in recovery.Methods In recent years,65 patients with cerebral apoplexy admitted in our hospital were randomly selected and divided into observation group and control group.Patients in the control group only received the conventional treatment and those in the observation group received psychological treatment on the basis of the traditional therapy.Psychological characteristics,activity of daily living and the improvement of nerve function were compared between two groups before and after treatment.Results After treatment,mental improvement,FMA,MBI in the observation group were significantly better than those of control group (P<0.05).Conclusion To improve the prognosis of patients with cerebral apoplexy effect,physical and mental development should be equally emphasized.Psychological intervention has become a key for stroke rehabilitation treatment,which significantly improves the treatment compliance,rehabilitation and the quality of life in patients.
3.Influence of Hypnosis Therapy on Rehabilitation of Patients with Spinal Cord Injury

Objective To observe the effects of hypnosis therapy on rehabilitation of patients with spinal cord injury.Methods The data were randomly selected from 66 cases of patients with spinal cord injury treated in our hospital from November 2011-November 2012.According to the treatment,the patients were divided into research group and control group.Control patients were given routine therapy,and patients in the research group were given hypnosis therapy,we observed and compared the recovery of two groups of patients after treatment.Results After treatment,the spinal cord function score (SCIM) of patients in the research group was better than control group,the difference was statistically significant (P < 0.05) After treatment,the depression and anxiety scale (HAMD,HAMA) of patients in the research group was better than control group,the difference had statistical significance (P < 0.05) The quality of life after treatment of research group patients (WHOQOL BREF) was better than control group,the difference had statistical significance (P < 0.05).Conclusion Hypnosis therapy helps to improve depression and anxiety in patients with spinal cord injury,improve patients with spinal cord function independently,should be widely used in clinical treatment and promotion.
4.Comprehensive Rehabilitation Treatment for Dysphagia in Patients with Acute Stroke

Objective To analyze the therapeutic effect of acute stroke dysphagia using rehabilitation therapy.Methods A total of 94 cases diagnosed with dysphagia after acute stroke were randomly selected from January 2012 to May 2013 in our hospital.The patients were equally divided into control group and research group.The control group received the conventional support treatment and the research group received comprehensive rehabilitation treatment.The results of swallowing dysfunction recovery of two groups were compared.Results After the treatment for 3 weeks,swallowing disorder degree score and swallowing function ratings were improved in both groups and the research group's scores were significantly higher than those of the control group (P <0.05) Conclusion Application of comprehensive rehabilitation treatment for dysphagia in patients with acute stroke has significant curative effect and is worth promoting.
5.Influencing factors of ankylosing spondylitis patients’life quality and medical coping styles

Objective To investigate the influencing factors of ankylosing spondylitis(AS)patients’life quality and medical coping styles.Method Two hundred patients visiting at the outpatient department of our hospital took part in the assessment on AS life quality and medical coping styles to look into the living quality of AS patients and its medical coping methods and influencing factors.Results The total score on life quality was(66.82±5.57),among whose dimensions the psychological function was scored the lowest by(10.65±4.05). Pearson correlation analysis showed that the scores on confrontation of coping styles were positively related with the dimension of social function in life quality(r=0.18,r=0.13,P<0.05),the scores on the avoidance of coping style were positively related to the dimension of physiological function in the life quality(r=0.12,r=0.14,P<0.05),the scores on yielding in coping styles was negatively correlated to physiological function of the life quality(r=-0.20,P<0.05).Regression analysis results showed that the AS patients’life quality was impacted by the course of the disease,coping styles and modes for paying medical expenses(P<0.05).Conclusions The AS patients’life quality is low and it is related to their coping style,the course of the disease and modes for paying medical expenses.Medical personnel should implement targeted interventions to guide patients into taking correct medical coping styles according to the specific condition of patients,in order to improve their quality of life.
6.A molecular epidemiological survey of 114 newly diagnosed HIV-1-infected persons from Shanghai

Objective To clarify the genetic diversity of HIV-1 strains currently circulating in Shanghai and establish the molecular epidemiological database of HIV-1 infection.Methods The samples from 114 newly diagnosed HIV-l-infected individuals between June 2004 and June 2005 were investigated.HIV-1 pol gene(protease 1-99 aa and reverse transcriptase 1-252 aa) from plasma sam- ples were amplified by RT-PCR,sequenced and phylogenetieally analyzed.Protease inhibitors(PRIs) and reverse transcriptase inhibitors (RTIs) resistance-associated mutations in protease (PR) and re- verse transcriptase(RT) regions were analyzed.Results (1)Epidemiological survey showed the exist ence of different modes of transmission of HIV-1 including sexual contacts 51(44.74%),intravenous drug users 43 (37.72%),contaminated blood/blood products transfusion 3(2.63%) and unknown in- fection 17(14.91%);(2)Phylogenetic analysis revealed that 9 of the analyzed sequences were identi- fied as subtype B(7.89%),15 as subtype B'(13.16%),1 as subtype C(0.88%),1 as subtype G (0.88%),38 as CRF01 AE(33.33%),46 as CRF07 BC(40.35%) and4 asCRF08 BC(3.51%); (3)Analysis of drug-resistance associated mutation showed that 21 of 114 (18.42%) persons infected with drug-resistant HIV-1,among which major mutations in PR and RT regions accounted for 2.63 (3/114) and 17.54% (20/114),respectively.The frequencies of major mutation in PR region were M46I(66.67%),M46L(33.33%) and in RT region were M41L(7.69%),A62V (7.69%),T69S (7.69%),V75I/L(15.38%),K103R(25.00%),V118I(23.08%),V179D/E/T(33.33%),G190R (8.33%),L210K/M/X(38.46%),227L/I(16.67%),M230R(8.33%),P236R(8.33%).Conclusions The results revealed the current presence of multiple HIV-1 subtypes and recombinants strains infec- ting residents and migrants living in Shanghai.The broad diversity of HIV-1 has been introduced into Shanghai mainly through drug injection and heterosexual contacts.This study also revealed that HIV-1 strains infecting these newly diagnosed treatment-naive persons have acquired major mutations in both PR gene and RT gene of HIV-1.

8.Titanium elastic nail versus plate-screw fixation for the treatment of upper segment fractures of femoral shaft in children.

UNLABELLEDABSTRA CT OBJECTIVE To compare the complications and clinical outcome of titanium elastic nail (TEN) versus plate-screw fixation (PF) for the treatment of upper segment fractures of femoral shaft in children.
METHODSFrom May 2006 and August 2012,32 consecutive children with upper segment fractures of femoral shaft were studied prospectively. They were randomly divided into TEN group and PF group. Ninteen patients were treated with titanic elastic nail (TEN) fixation including 11 males and 8 females with an average age of (6.9?2.2) years old ranging from 3 to 11,11 cases of them were type A, 6 cases were type B,2 cases were type C according to AO classification. The other 13 patients were treated with plate-screw fixation (PF) including 9 males and 4 females with an average age of (7.5±2.1) years old ranging from 5 to 12 years, and 3 cases of them were type A,6 cases were type B,2 cases were type C. Operative time, blood loss,incision length,the time of hospitalization,fracture healing time, postoperative complications and function recovery between two groups were compared and evaluated.
RESULTSThe average follow-up period of the patients was 25.3 months (ranging from 15 to 48 months) in TEN group, and 36.2 months (ranging from 13 to 36 months) in the PF group. The operation time, time of hospitalization, the time of implants removed after the initial surgery, fracture healing time in TEN group were respectively (56.7±11.2) min, (6.6±3.9) d, (8.1±2.3) months, (12.6+3.8) weeks; and in PF group were respectively (51.5±8.3) min, (7.8±4.8) d, (7.8±1.6) months, (11.8±2.8) weeks, there was no significant difference between two groups (P>0.05). However, the length of incisions was (4.3±1.7) cm and the intraoperative blood loss was (12.7+3.2) ml in TEN group,which were significantly less than that in PF group respectively (89.2±21.1) ml and (11.6?2.3) cm (P<0.05). There was no statistically difference in postoperative in complication between two groups, but the patients in TEN group had a higher incidence of soft tissue irritation and misalignment. Outcome scores according to Sanders had no significant difference between two groups (P>0.05). In TEN group,the result was excellent in 13 cases, good in 3,fair in 2, and poor in 1, while in PF group excellent in 10, good in 2, fair in 1.
CONCLUSIONThere is no significant difference in therapeutic effects between TEN and PF for children with upper segment fractures of femoral shaft. The internal fixation should be selected according to the associated fracture type, weight, the expected value of the parents, scars, and so on.
