Objective To study the role of extra-anatomic arterial bypass grafting in the management of aorta-iliac occlusive disease. Methods Forty-one cases of aorta-iliac occlusive disease managed by extra-anatomic arterial bypass grafting were retrospectively analyzed. Clinical features included claudicafion 11, resting pain 12 and ulcer/gangrene 18 patients. Axillofemoral bypass was performed in 15 cases with aortic and bilateral iliac artery occlusion, femorofemoral bypass in 18 cases with unilateral iliac occlusion and femoro-cnntralateral popliteal bypass in 8 cases with iliac and ipsilateral femoral artery occlusion. Results Postoperatively, clinical features were reheved in 40 cases (97.6%). Ankle brachial index increased significandy from 0.41 ± 0.15 to 0.89 ± 0.21 perioperatively (P < 0.05 ).Thirty-five cases were followed up for 1 year to 5 years. The overall graft patency rates in this series were 93%, 86% and 80% in 1, 3 and 5 years, respectively. Conclusion Extra-anatomic arterial bypass has merits as less invasive and rapid postoperative recovery, and this is one of the effective methods for the treatment of aorta-iliac occlusive disease.

Chinese Journal of Ultrasonography 2013;(4):296-299
Objective To understand demonstration methods and ultrasonographic features of left and right atrial appendages in normal fetuses. Methods 200 consecutive normal fetuses during gestation age 19-28 weeks from November 2011 through April 2012 entered the study.Ultrasonographic features and the demonstrated rates of atrial appendages at different incidence angle of ultrasonography and different gestational weeks were recorded and calculated, respectively. Results Atrial appendages of normal fetuses can be demonstrated on atrial appendages plane, which was little lower the view of parasternal great artery short axis. The display rate of left, right and left-right atrial appendages was 92%,68%,65.5%,respectively.When ultrasound beam entranced into chest from the right side, the display rate of left-right atrial appendages was highest(94.3%).The best time to observe atrial appendages was 22 to 24 week of pregnancy. The majority of left atrial appendages were fingerlike hook shapes, with narrower bases and longer bodys, and the endocardial surfaces were relatively smooth.Sometimes with incisures on external edges.Whereas the majority of right atrial appendages were obtuse-angled triangle shapes, with broad bases and shallower bodys. On endocardial surface the pectinate muscles were often seen, which looked like serrated echoes, sometimes prominent taenia sagittalis were noted in right atrial appendages. Conclusions Atrial appendages of normal fetuses can be demonstrated using two-dimensional ultrasonography on atrial appendages plane. The demonstrated rates of atrial appendages were different according to different incidence angle of ultrasonography and gestational weeks. There were some differences in ultrasonographic features between left and right atrial appendages, which is very helpful in determinating atrial situs.

Objective To investigate the safe region for screw fixation in atlas lateral mass by ra- diologic measurements to conduct the operative procedure. Methods The dimensions of the pedicles were determined in 30 patients with normal atlas by CT scan and three dimensional reconstruction images of the upper cervical spine. The space available for the screw(SAS)was defined as the perpendicular distance between two lines tangential to the spinal canal and the transverse foramen, respectively.SAS was evaluated at 0?(SAS1)and 10?(SAS2)insertion angles. The location of screws in 13 cases of opera- tion were observed by CT scan images measurements of the cervical spine. Results SASI at 0?inser- tion angle was(7.81?1.28)mm and SAS2 at 10?insertion angle was(9.69?1.23)mm,with statisti- cal difference(P<0.01).For SAS1,the distance from the midline of pedicle to the midline of posterior arch and the distance from the center of C2 inferior articular process to the midline of posterior arch showed insignificant statistical difference. However, the entry point at the posterior arch of the midline of SAS2 was located at 2.0 mm lateral to the midline of ASAI.Twenty-six pedicle screws in 13 cases were inserted correctly, without cortex breakage,or nerve and vessel injury. Conclusions There is a safe region for screw fixation in atlas lateral mass.Wider space available for the screw can be acquired when transverse angle is 10?.It is necessary to confirm the safe region by using axial CT of atlas before screw fixation.
