1.The clinical values of 64-slice computed tomography coronary angiography

Objective To evaluate the clinical values of(M-slice computed tomography coronary angiography(CTCA) in contrast to conventional coronary angiography(CCA).Methods Assessment of the major and minor branches of coronary arteries by 64-Slice CTCA,showed the information of 100 patients with 64-slice CTCA,comparing with CCA regarded as the golden standard recently implemented.Results Among the 100 patients,920 coronary arteries were evaluated with 370 lesions found.Of them ,348 lesions were consisted with CCA,22 lesions were falsely positive and 10 lesions were found by CCA not picked up by 64-slice CTCA.The sensitivity,specificity,positive predict values and negative predict values were 97.2% ,96%,94% and 98.1% respectively for the detection of coronary artery disease(CAD),Further statistical analysis using a 2-related X~2 test showed no obvious difference in the discovery of CAD between 64-slice CTCA and CCA.Comparing both imaging modalities,there was concordance in assessing lesion severity in 311 lesions,and accuracy approaches 86.8%.Conclusions 64-slice CTCA has excellent sensitivity and specificity in detecting CAD,and also can give fairly accurate assessment of stenosis severity in most patients as compared to CCA.As a alternative method it is a useful non-invasive for detecting coronary arteries disease in symptomatic patients

The method of acupuncture combined with rehabilitation training in the treatment of shoulder hand syndrome after stroke is reliablet, quick, with simple operation and less adverse reactions. Among them, characteristic acupuncture could focus on the targeted points with obvious effect than ordinary acupuncture, and the combination with rehabilitation training could greatly improve the curative effect. However, there still exist some problems in the current research, such as the difficulty of conducting blind clinical trials of acupuncture therapy; the lack of follow-up data in the evaluation of curative effect, the uncertainty of long-term curative effect; the small sample size of clinical research, etc., so multi-centered and large sample RCT research are still needed.

5.The human papilloma virus in endometrium cancer and its clinical significance

Objective To investigate human papilloma virus(HPV) infection in normal endometrium, atypi-cal hyperplasia of endometrium and endometrial carcinoma. Methods By the nucleic acid hybridization,we detected 28 pairs of endometrial carcinomas, 21 pairs of atypical hyperplasia of endometrium. Normal endometrium from 16 pa-tients with uterine myomas were as control. Results HPV16/18 DNA was detected in 25 endometrial carcinoma and 2 atypical hyperplasia of endometrium and 1 normal endometrium. Conclusions Endometrial carcinoma HPV16/18 DNA were significantly higher than those infected with atypical endometrial hyperplasia and normal endometrium. Note the occurrence of endometrial cancer,the development may be associated with HPV infection.
6.Observation of therapeutic effects in arterial interventional therapy in early osteosarcoma patients

Objective To explore the clinical therapeutic effects in arterial interventional therapy in early osteosarcoma patients.Methods Thirty-eight early osteosarcoma patients received arterial chemoembolization therapies and performed operations.94.7% patients received limb salvage surgeries.Levels of alkaline phosphatase were tested in preoperative and postoperative phases.Results Levels of postoperativealkaline phosphatase[average(191.7±107.0)U/L]were significantly decreased compared with that before interventional therapy[average(1129.1±572.3)U/J.The survival rate in follow-up at 1,3 and 5 years Was97%,79%,50% respectively.The rate of recurrence and metastasis WAg 24%.Conclusions Arterialchemoembolization therapies in patients with early osteosarcoma could improve clinical symptoms effectively,enhance survival rate,relieve recurrence and metastasis,remain affected extremity.The clinical thempeutic effects of iodinated oil were the best among the three embolism materials.
7.The expression and significance of serum microRNA-183 and TK1 in patients with colorectal cancer

Objective To detect the expression and significance of serum miRNA-183 and TK1 in patients with colorectal cancer, and the mechanism thereof. Methods Fifty-two serum samples of colorectal cancer patients and paired health serum samples were collected. The expression of miRNA-183 was detected by real-time quantitative PCR, and TK1 was detected by Western blot enhanced chemiluminescence assay. The correlation between miRNA-183 and TK1 and their relations with the clinicpathologic characteristics were analyzed. Results The serum miRNA-183 expression was significantly higher in colorectal cancer group than that in normal control group (P<0.01). The expression of serum miRNA-183 was significantly higher in stage Ⅲ-Ⅳ group than that in stage Ⅰ-Ⅱ group (P < 0.01). There was more significant increase in serum miRNA-183 in lymphatic metastasis group than that without lymphatic metastasis group (P < 0.01). Receiver operating characteristic (ROC) curve showed that of there was a diagnostic value for serum miRNA-183 in colorectal cancer, with an optimal value of 1.15. The diagnostic sensitivity and specificity were 78.8%and 67.3%, and the positive predictive value and negative predictive value were 78.9%and 70.2%. The serum TK1 expression was also higher in colorectal cancer group compared with that of normal control group (P<0.01). And the TK1 expression was also higher in stageⅢ-Ⅳgroup than that in stageⅠ-Ⅱgroup (P<0.01). Furthermore, miRNA-183 expression was positively correlated with TK1 expression (rs=0.692, P<0.01). Conclusion The serum expression levels of miR-183 and TK1 may act as tumor markers for early colorectal cancer diagnosis, and also can be used to predict the malignant degree and prognosis of colorectal cancer.
8.Six cases of deep cerebral veins thrombosis

Objective To analyse the clinical features,imaging characteristics diversity of deep cerebral veins thrombosis (DCVT).Methods From 2004 to 2013,6 patients diagnosed as DCVT were recorded and a retrospective review of the cases were undertaken for the purpose of this analysis.Results Among the 6 patients with DCVT,4 were male and 2 were female,aged from 28 to 69 years old.The disease duration of 4 cases ranged from 2 to 7 days,remnants were 20 days and 3 months respectively.The first symptoms of 4 cases were headache,1 was feeblemindedness,and the other was hemiplegia.The secondary symptoms were disturbance of consciousness,apathy,diplopia and non-infectious fever.Non-contrast computed tomography showed low signal in the bilateral thalamus in four patients,high signal in the transverse sinus and straight sinus in one patient and high signal in torcular in one patient.Abnormal signal was found in bilateral thalamus on magnetic resonance imaging in all patients and some of them had abnormal signal in the mesencephalon or basal ganglia.The patients were definitely diagnosed as DCVT by magnetic resonance venography (MRV) or digital subtraction angiography (DSA).Among them,2 patients were confirmed by brain biopsy.Four patients were followed up with good outcome and 2 were lost to follow-up.Conclusions The clinical manifestations of DCVT are not specific.For acute-onset DCVT patients,the first symptoms are always headache and vomiting,while the main symptoms are declined cognition and slow reaction for chronic-onset ones.Along with the progress,the main symptoms of DCVT are disturbance of consciousness,psychiatric symptoms and intracranial hypertension.Changes in the bilateral thalamus and basal ganglia are especially main characteristics which are easily misdiagnosed as brain tumor according to the images.DCVT can be definitely diagnosed by no signal of deep cerebral veins on MRV or DSA.

10.Significant impact factors of postoperative hemorrhage after pancreaticoduodenectomy

Objective To analyze the significant impact factors of postoperative hemorrhage after pancreaticoduodenectomy.Method The clinical data of 154 patients who underwent pancreatieoduodeneetomy from Jan.2009 to Jun.2012 were retrospectively analyzed.Results The incidence of postoperative hemorrhage was 10.4% (16/154).The mortality caused by this complication was 12.5% (2/16).Among these patients,intra-abdominal hemorrhage occurred in seven patients,and gastrointestinal hemorrhage occurred in nine patients.One patient suffered from both these complications.There were five and two patients presenting with early or delayed intra-abdominal hemorrhage,respectively.Early and delayed gastrointestinal hemorrhage occurred in three and six patients,respec tively.Statistical analysis showed that infection (P=0.002,P< 0.01) and pancreatic fistula (P=0.048,P<0.05) are significantly associated with postoperative hemorrhage.Conclusions Postoperative hemorrhage after pancreaticoduodenectomy is a severe complication.Adequate hemostasis and prevention of pancreatic fistula and infection are the key points in reducing postoperative hemorrhage.Proper treatments should be used according to the site,type of onset and severity of hemorrhage.
