2.Risk factors of biliary complications after liver transplantation: a meta-analysis

Objective To determine the risk factors of biliary complications (BC) after liver transplantation (LT),and to provide a theoretical basis to reduce the rate of postoperative biliary complications.Method A meta-analysis was carried out using Revman 5.1.Results Eighteen studies were identified,including 524 patients in the case study group (BC group) and 3967 patients in the control group (Non-BC group).There were no significant differences in donor age,recipient age,primary disease,warm ischemia time,second warm ischemia time,anhepatic phase time and cytomegalovirus infection.The incidence of biliary complications after liver transplantation was significantly different in male than female patients (OR: 1.40; 95% CI: 1.09~ 1.79;P=0.008).Child C hepatic function increased the incidence of postoperative biliary complications (OR: 1.95; 95% CI: 1.02 ~3.76;P=0.04).Using a T-tube for biliary reconstruction significantly increased the incidence of postoperative biliary complications (OR: 2.00 ; 95 % CI: 1.30~ 3.08 ; P 0.002).The incidence of biliary complications after liver transplantation was significantly different in patients with rejection than those without (OR: 1.80;95% CI:1.11~2.93;P 0.02).Patients with hepatic artery complications were associated with a higher incidence of postoperative biliary complications (OR: 3.15;95% CI: 1.37~7.23 ;P=0.007).Patients in the BC group had a significantly longer cold ischemia time and operative time (P<0.01).Conclusions A male recipient,Child C hepatic function,T-tube drainage,rejection,hepatic artery complications,prolonged cold ischemia time and prolonged operative time were factors affecting the risk of biliary complications.Biliary complications after liver transplantation had no relationship with donor age,recipient age,primary disease,warm ischemia time,second warm ischemia time,anhepatic phase time,and the presence or absence of cytomegalovirus infection.
3. Interaction between components of Buyang Huanwu Decoction and targets associated with ischemic stroke based on molecular docking method

Objective: To investigate the interaction between the components of Buyang Huanwu Decoction (BYHWD) and the targets associated with ischemic stroke by using molecular docking technique, and explore the effective material basis and main targets of BYHWD. Methods: A total of 46 key protein targets associated with cerebral infarction of the human body were selected, 204 components of BYHWD were screened from the TCMSP database, and these components and targets were docked using molecular docking technology, thereby to get the Total Score. Cytoscape3.7.1 software was applied to establish network model of component-protein targets. At the same time, total quantum statistical moment similarity method was used to analyze the imprinting behaviors of these targets. Results: From molecular docking results, 22 components had good interactions with 40 or more protein targets, and 22 protein targets had good interactions with 80 or more components for the treatment of cerebral infarction. Conclusion: Molecular docking method can be used to find the active ingredients and main targets of BYHWD in treating ischemic stroke, providing certain reference for further study on the mechanism of BYHWD in treating ischemic stroke.
4. Integrated analysis of “eighteen incompatible medicaments” of Chinese materia medica based on supramolecular imprinting interaction

The “eighteen incompatible medicaments” is an important content of Chinese materia medica (CMM) compatibility contraindication, involving the key basic problem of safe and effective clinical use of CMM. Based on the rule of the automatic action of supramolecular “imprinting template” previously proposed, the current research basis of “eighteen incompatible medicaments” were integrated and analyzed. Firstly, the history of “eighteen incompatible medicaments” were summarized, then the theory of Chinese materia medica (CMM) compatibility was interpreted by supramolecular “imprinting template”: the compatibility of CMM are that two or more than two kinds of CMM effective components group of molecules (object) are combined by non-covalent bonds, and the new formed supramolecular system and the human body (subject) are interacted with each other according to “imprinting template”, and then the toxicity and efficacy were generated, while the compatibility law of CMM is displayed macroscopically. Based on this, three groups of “eighteen incompatible medicaments” were discussed from the perspective of supramolecular chemistry, and the supramolecular method integrating toxicity with efficacy was put forward, including “chemicalkinetics”, “network kinetics” and “spectrum toxicity and efficacy kinetics” methods, thereby providing the ideas and reference for the research on the mechanism of “eighteen incompatible medicaments” and providing reference for clinical application.
5.Relationship between Japanese encephalitis and S100B protein

Objective To investigate the role of S100B protein in the pathogenesis of patients with Japanese encephalitis (JE).Methods A total of 45 patients were enrolled in the Second Hospital of Hebei Medical University from August 2013 to October 2013,who were diagnosed as JE on the basis of clinical features and positive IgM antibodies against JE virus measured by enzyme-linked immunosorbent assay (ELISA) from the Center of Disease Control of Shijiazhuang.The JE patients were divided into initial phase group,acute phase group and convalescence group based on the course,mild JE group,moderate JE group and severe JE group based on the severity,MRI-no-lesion group and MRI lesions group based on the imaging findings of JE.Twelve cases with no evidence of infection in central nervous system in the meantime were chosen as control.The S100B protein was measured by ELISA.Results The content of S100B protein in cerebrospinal fluid was as follows:522.76 (393.35,620.37) pg/ml in mild JE group (acute phase group:609.77 (549.27,779.71) pg/ml,convalescence group:420.48 (344.36,453.19) pg/ml),792.09 (705.47,1 108.96) pg/ml in moderate JE group (acute phase group:770.19 (646.31,1 069.54) pg/ml,convalescence group:803.45 (602.90,1 396.84) pg/ml),and 1 021.94 (680.84,1 302.15) pg/ml in severe JE group (acute phase group:981.82 (680.84,1 826.28) pg/ml,convalescence group:989.00 (553.62,1 207.67) pg/ml).The S100B protein content was 561.52 (454.36,814.56) pg/ml,803.45 (602.90,1 104.01) pg/ml,762.22 (594.95,1 044.97) pg/ml,581.76 (442.51,1 069.10) pg/ml in MRI-no-lesion group,MRI lesions group,total acute phase group and total convalescence group,respectively.While in control group,the S100B protein content was 266.71 (205.72,390.05) pg/ml.The contents of S100B protein in moderate JE group,severe JE group,total acute phase group,total convalescence group,MRI-no-lesion group,MRI lesions group were higher than that in control group (H =4.864,5.497,5.075,3.918,2.971,4.981,P =0.000,0.000,0.000,0.000,0.009,0.000).The contents of S100B protein in mild JE group was lower than that in moderate JE group and severe JE group (H =-2.786,-3.514,P =0.032,0.003).Conclusions The level of S100B protein in cerebrospinal fluid is related with the severity,duration and imaging presentation of JE patients.The dynamic monitoring of S100B protein levels is of great significance for assessment of the patients' condition and curative effect.

In this study , 4116 traditional Chinese medicine ( TCM ) prescriptions of 994 outpatients of gastric
cancer from the Jiangsu Provincial Hospital of Traditional Chinese Medicine were analyzed with the use of Clinical Research Information Sharing System and the data mining method of scale-free networks . The core drugs and compatibility were visually displayed on diagrams which revealed the characteristics of gastric cancer treatment with TCM prescriptions in this hospital with high credibility . This method has certain meaning and value for the study of TCM prescriptions .

8.Intervention of compound danshen drop on serum cyclic adenosine monophosphate and cyclic guanosine monophosphate on transient cerebral ischemic attack

BACKGROUND: Blood platelet is active commonly appeared in both acute and recovery stages in ischemic cerebral apoplexy. Cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) are closely correlated with the blood platelet function.OBJECTIVE: To observe the therapeutic effects of compound danshen drop on clinical prevention and treatment of transient cerebral ischemic attack (TIA) and the influence on serum cAMP and cGMP.DESIGN: Randomized control experiment.SETTING: Department of Neurology of Binzhou Medical College affiliatedHospitalPARTICIPANTS: Totally 180 TIA patients participated in the experiment in volunteer in the clinic of Neurological Department of Affiliated Hospital of Binzhou Medical College from September 2000 to April 2001. Based on randomized number scale, 3 groups were divided according to dosages of compound danshen drop. In 10 drops/d group, there were 60 cases, of which 35 cases were male and 25 cases female, aged varied from 50 to 70 years, averagely (54.3±7.2) years old. In 20 drops/d group, there were 60 cases, of which, 32 cases were male and 28 cases female, aged varied from 49 to 62 years, averagely (55.7±5.1) years old. In 30 drops/d group, there were 60 cases, of which, 33 cases were male and 27 cases female, aged varied from 52 to 69 years, averagely (54.9±5.5) years old.METHODS: In 10 drops/d group, the drug was given once per day, 10drops each time. In 20 drops/d group, the drug was given twice per day,10 drops each time. In 30 drops/d group, the drug was given three times per day, 10 drops each time. Four weeks after medication, 3 mL blood was collected from each patient in the morning. With radioimmunoassay, the contents of cAMP and cGMP in serum were determined. Each case was tions, gem bleeding, epistaxis, subcutaneous bleeding, mouth numbness,headache, dizziness, etc.tack: In 10 drops/d group, there were 1 case in internal carotid arterial system, 2 cases in vertebral arterial system, 1 case of cerebral infarction and 2 cases of myocardial infarction. In 20 drops/d group, it was 2, 2 and 1 cases successively and in 30 drops/d group, it was 1, 2, 1 cases succesgroup: There were 1, 2 and 4 cases of harmful accident and side effects in three groups successively, of which, peripheral numbness of mouth 1 case in 10 drops/d group, 1 case of either peripheral numbness of mouth or headache in 20 drops/d group, gastrointestinal reaction 1 case, peripheral numbness of mouth 2 cases and dizziness 1 case in 30 drops/d group.There was no gem bleeding, epistaxis and subcutaneous bleeding, and no patient was withdrawn the treatment due to above-mentioned side effects.With comparison among groups, significant difference was not indicated The contents of cAMP were (21.22±3.94), (22.5±3.67), (23.1±7.7) ng/Land cGMP were (3.67±1.18), (4.74±2.12), (4.6±0.7) ng/L successively in three groups (P > 0.05).CONCLUSION: During second-grade prevention of TIA, compound danshen drop presents definite therapeutic effects, which is basically same in comparison of dosages. Simultaneously, no remarkable side effects appear.
9.Application of modular cementless femoral stems in complex revision hip arthroplasty.

OBJECTIVETo investigate and clarify the key surgical techniques and evaluate clinical effectiveness of modular cementless femoral stems(LINK MP modular stem) in complex revision hip arthroplasty.
METHODSFrom January 2002 to December 2013, 470 patients in Chinese PLA General Hospital received revision hip arthroplasties using MP stems. Among them, 246 patients were followed for an average of 5.7 years (ranged, 0.5 to 11 years). There were 148 males and 98 females, with an average age of 62.5 years old (ranged from 26 to 83 years old). The most common reason for revision was aseptic loosening of femoral stems (107 cases, 43.5%), followed by prosthetic joint infection (104 cases, 42.3%). Other reasons included unexplained hip or thigh pain, periprosthetic femoral fracture, prosthesis wear and dislocation. According to Paprosky classification of femoral deficiency, there were 171 cases of type IIIA (69.5%), 14 cases of type I, 39 cases of type II, and 22 cases of type IIIB. The average preoperative Harris hip score (HHS) was 37.4 ± 7.5. In addition, radiographic presentation, degree of patients' subjective satisfaction, and clinical effectiveness were assessed as well.
RESULTSThe average Harris score was im- proved to 87.8 ± 4.2 after operation,with significant statistical difference when compared to preoperative score (t = 92.13,P = 0.00). Analysis of patients' subjective satisfaction for leg length discrepancy, stability and overall degree of satisfaction showed that the number of patients with great satisfaction were 27, 60, 61 respectively. The most common complication was the periprosthetic fracture (intraoperatively 21 cases and postoperatively 8 cases). Other complications included infection (7 cases), dislocation (5 cases), numbness possibly caused by incomplete sciatic nerve injury (6 cases) and thigh swelling (3 cases). The X-ray taken at the final follow-up revealed that prosthesis subsidence occurred in 4 cases, 2 of which needed a second revision.
CONCLUSIONThe application of modular cementless femoral stems (LINK MP modular stem) in complex revision hip arthroplasty (especially in cases with the serious defect of proximal femoral bone) shows good results at both the short-term and long-term follow-up, especially in the revision for prosthetic infection with the high success rate. In order to avoid complications and improve clinical outcomes,the surgeons should carefully select proper surgery candidates,perform the procedure with care, and master the key surgical techniques.

10.Therapeutic observation on acupuncture-moxibustion at different intervals for persistent allergic rhinitis

Objective:To observe the therapeutic effect of acupuncture-moxibustion at different intervals on persistent allergic rhinitis. Methods: A total of 90 patients conforming to the inclusion criteria were randomized into three groups named A, B and C by randomized block method. Patients in all three groups received the same treatment of acupuncture and herbal cake-partitioned moxibustion at the same acupoints, while the treatment frequency was different. Patients in group A received the treatment once a week, group B twice a week and group C three times a week, and all the treatment lasted for 4 weeks. The total nasal symptom score (TNSS), total ocular symptom score (TOSS) and Sino-nasal outcome test-20 (SNOT-20) were evaluated before and after treatment. The self-rating score of symptoms was evaluated during treatment and 2 weeks after treatment. Results: The total effective rate was 80.0% in group A, 93.3% in group B, and 100.0% in group C. The total effective rate in group A was statistically different from that in group B and group C (both P<0.05), but there was no significant inter-group difference in total effective rate between group B and group C (P>0.05). After treatment, scores of TNSS, TOSS and SNOT-20 in all three groups dropped significantly, and statistically different from those before treatment (all P<0.05); between-group comparison showed group B and group C decreased more obvious than group A (all P<0.05). In self-rating score of symptoms, there were no inter-group statistically significant differences in the first 3-week treatment (all P>0.05); after 4-week treatment, the score in group A was higher than that in group B and group C, and showed statistical significant (both P<0.05); at 2 weeks after treatment, the score in group A was higher than that in group B and group C based on an everyday record, showing statistical significance (both P<0.05). At the 11th day after treatment, the score in group B was higher than that in group C (P<0.05). There were no significant differences between group B and group C at other time points (all P>0.05). Conclusion: All three protocols are effective for allergic rhinitis. With the increase of treatment frequency, the therapeutic efficacy with a treatment frequency of twice a week and three times a week is superior to that of once a week. Frequency of three times a week has a better long-term effect than once and twice a week, together with the least fluctuation of symptoms.
