PurposeTo explore the segmentation accuracy of different filtering and segmentation methods in brain tumor MRI, and to identify the best algorithm for brain glioma.Materials and Methods Using the nonlocal average ifltering, median ifltering, the anisotropic ifltering and improved mean shift algorithm segmentation, the watershed segmentation algorithm, fuzzy c-means segmentation algorithm to realize image segmentation in MATLAB program, 39 glioma images from different patients were analyzed. Pathology manual segmentation was used as gold standard to evaluate different segmentation precision.Results The signal-to-noise ratio was 7.9243, 6.2160 and 6.5426 for different iflter methods, respectively. The segmentation methods accuracy was 92.31%, 88.03% and 84.93%, respectively.Conclusion The nonlocal average ifltering effect is more accurate than median ifltering and the anisotropic ifltering. The improved mean shift algorithm segmentation is more accurate than watershed segmentation algorithm and fuzzy c-means segmentation algorithm with precision of 92.31%.

Objective To evaluate the efficacy and safety of direct stenting technique in emergent percutaneous coronary intervention(PCI)for pa?tients with acute myocardial infarction(AMI). Methods Totally 460 consecutive patients with acute myocardial infarction who underwent emergent PCI during August 2010 to July 2013 were involved,among whom 346 patients were assigned to the control group(traditional balloon dilation and stenting),114 patients were assigned to the experiment group(direct stenting). The safety and efficacy of direct stenting technique was observed. Results In the control group,7 cases had no reflow during operation,2 cases had reflow after the pre?expansion and 5 cases after stent implanta?tion,no similar cases in the experiment group(P=0.125 9). The follow?ups showed the control group had 2 cases of major adverse cardiac events including 1 case of acute stent thrombosis who required urgent revascularization,and 1 case of cardiac death at four days after operation;the experi?ment group had 1 case of stent thrombosis,there was no statistically significant difference(P=0.730 7). Conclusion Direct stenting technique may be performed selectively for certain coronary diseases when performing emergent PCI for AMI.

OBJECTIVE:To investigate the variance of antimicrobial resistance of Klebsiella.spp isolated in our hospital during the recent 5 years.METHODS:Disk diffusion test was used to study the anti microbial resistance.The results were evaluated based on the criteria recommended by American Clinical and Laboratory Standards Institute.The data analysis was performed using WHONET-5software.RESULTS:Atotal of 840 strains of Klebsiella.spp were collected fromour hospital during2003～2007,of which,59.3% were fromrespiratory secretion speci men,12.0%fromblood speci men,and10.1% from secretions.The detection rate of ESBLs-producing Klebsiella.spp was from51.0% to65.1%.No I mipenem-and Meropenem-resistant Klebsiella.spp strain was detected.Susceptibility test showed that Klebsiella.spp-producing ESBLs was multiple resistant,and the anti microbial resistance of Klebsiella.spp has increased year on year.CONCLUSION:Monitoring of the drug resistance of Klebsiella.spp should be strengthened.The variation of the anti microbial resistance of Klebsiella.spp should be noted so as to direct rational drug use in the clinic and prevent spreading of drug resistant strains.

OBJECTIVE: To investigate the change of the resistance of common pathogenic bacteria isolated in the First Clinical Hospital of Jilin University during the past 5 years to quinolones.METHODS: Disk diffusion test was used to study the susceptibility test,with the results evaluated based on US Clinical and Laboratory Standards Institute(CLSI).WHONET-5 software was used to analyze the drug resistance data.RESULTS: During 2003～2007,the detection rates of extended spectrum beta-lactamases(ESBLs)-producing Escherichia coli and Klebsiella.spp were 43.6%～60.1% and 51.0%～65.1%,respectively;the detection rates of methicillin-resistant Staphylococcus aureus(MRSA) and methicillin-resistant coagulase negative Staphylococcus(MRSCN) were 50.8%～83.3% and 80.0%～81.5%,respectively;the resistance of common pathogenic bacteria to quinolones showed an increasing tendency,much as in MRSA,MRSCN,feces enterococci,Enterococcus faecalis,and Escherichia coli.CONCLUSION: Monitoring of the resistance of common pathogenic bacteria to quinolones should be strengthened.The change of the antimicrobial resistance should be investigated in order to direct rational drug use in the clinic and prevent the propagation of drug-resistant strains.

Objective To investigate the efficacy of intracoronary tirofiban during percutaneous coronary intervention(PCI)for patients with acute myocardial infarction(AMI).Methods Seventy-six consecutive AMI patients,treated with primary coronary angioplasty in 12 hours were enrolled.They were randomly divided into two groups:tirofiban group(39 cases)and control group(37 cases).Tirofiban group was treated with intracoronary timfiban during PCI and after the operation for 48 hours.Both of them were given heparin in PCI and aspirin,clopidogrel before PCI.At the end of PCI procedure,angiographic features such as TIMI flow grade and TIMI myocardial perfusion grade(TMPG)were analyzed.The difference of two groups in complication and major adverse cardiac events(MACE) was investigated.Results The myocardial reperfusion of tirofiban group was better than that of control group[TIMI grade 3 flow 94.9%(37/39) vs 78.4%(29/37),P<0.05;TMPG 3 grade 89.7%(35/39)vs 67.6%(25/37),P<0.05].There was no significant difference between two groups in bleeding complication.The occurrence of MACE in tirofiban group was less than that in control group[7.7%(3/39)vs 18.9%(7/37),P<0.05].Conclusion Intracoronary tirofiban dunng primary PCI in patients with AMI can improve coronary flow and myocardial perfusion,and has no more bleeding and less MACE occurrence.

Objective To establish an HPLC method to determine the content of diosgenin in Paridis Rhizoma; To examine contents of diosgenin in different varieties of Paridis Rhizoma medicinal plants and from different habitats. Methods Shimadzu Inertsil ODS-C18 guard column (150 mm×4.6 mm, 5μm) was used in HPLC system; elution conditions were the mobile phase using 92% acetonitrile and 8% water (isocraticelution) at a flow rate of 1.0 mL/min with 203 nm detection wavelength. The temperature of the column oven was maintained at 30℃.Results Different varieties of Paridis Rhizoma medicinal plants and Paridis Rhizoma medicinal plants from different habitats contained diosgenin. TheParis polyphylla var.pseudothibetica in Yubei district had the highest content (6.813 7 mg/g);Paris polyphylla var.stenophylla in Chengkou County had the middle content (5.758 4 mg/g);Paris yunnanensisFranch in Shizhu county showed the lowest content (1.952 2 mg/g). The results showed that the obvious difference of chemical quality between various cultivated varieties of Paridis Rhizoma medicinal plants, which might be related with origin, variety and the cultivation technology, and so on.Conclusion The diosgenin contents ofParis polyphylla var. pseudothibetica andParis polyphylla var.stenophylla were higher than that of varieties recorded in Chinese Pharmacopeia.Paris polyphylla var.pseudothibetica andParis polyphylla var.stenophylla had important medical value.

Objective:To explore the expression of Ezrin and CD44v6 in gastric carcinoma and their correlations to clinicopathologic features. Methods:The expression of Ezrin and CD44v6 were detected by immunohistochemistry in gastric carcinoma(n=53) and normal gastric mucosa(n=20).Results:①The positive expression rates of Ezrin in gastric carcinoma and normal gastric mucosa were 56.6% and 30.0%.While the positive expression rates of CD44v6 in gastric carcinoma was 58.5% and no expression in normal gastric mucosa.②The expression of Ezrin was correlated to the differentiation(P0.05).③The expression of Ezrin was positively correlated with that of CD44v6 in gastric carcinoma(rs=0.740,P

Objective To assess the examinational value of tissue equalization (TE)technique in DR thoracolumbar vertebral body photography. Methods 48 cases were reformed using the conventional DR method in the thoracolumbar vertebral body photography,compared with the TE technique in DR thoracolumbar vertebral body photography. Results The details in different thickness area of body could be revealed clearly in the same image produced by TE technique. The visualization of image details could be obviously improved by using the TE technique. Conclusion It is important to apply the TE technique in DR thoracolumbar vertebral body photography.

OBJECTIVETo explore the effect of 3-n-butylphthalide pretreatment on the delayed neuronal death(DND) and the expreesion of heat shock protein70 (HSP70) in rat hippocampus after ischemia/ reperfusion.

METHODSAll rats were randomly divided into sham group (n = 36), total cerebral ischemia (TCI) group (n = 36), butylphthalide (NBP) group (n = 6), NBP + TCI group( n = 36), quercetin + NBP + TCI group (n = 6), dimethyl sulfoxide (DMSO) + NBP + TCI group (n = 6). The model of total cerebral ischemia/reperfusion was established by blocking vertebral arteries and carotid arteries. In sham group, TCI group and NBP group, the animals were further divided into instantly, 6 h, 12 h, 1 d, 3 d, 5 d groups according to the time interval after sham operation or TCI. Histological changes of the hippocampus were evaluated using thionin staining under light microscope by determining the delayed neuronal death (DND) and the expression of HSP70 was assayed using immunohistochemistry.

RESULTSNBP pretreatment could reduce delayed neuronal death in CA1 of hippocampus induced by TCI-reperfusion injury in rats, and up-regulated the expression of HSP70 in CA1 hippocampus of brain ischemic/reperfusion for 5 days. Quercetin blocked the acquirement of the brain ischemic tolerance induced by NBP preconditioning.

CONCLUSION3-n-butylphthalide (NBP) prevents the neurons from ischemia/reperfusion injury through upregulating the expression of HSP70.

BACKGROUND:Preliminary studies of our research group have confirmed that the proliferation of preosteoclasts and the differentiation and function of osteoclasts could be inhibited when they were cultured in lower oxygen tension even hypoxia (2%O 2 ), but the gene expression of osteoclasts cultured in vitro have not been reported.
OBJECTIVE:To examine the effect of oxygen tension on specific gene expression of osteoclasts in vitro and explore the mechanism of osteoclast differentiation influenced by oxygen tension.
METHODS:The preosteoclasts were induced with 10μg/L macrophage colony stimulating facto and 10μg/L soluble receptor activator of nuclear factor-κB ligand into mature osteoclasts. Then the osteoclasts were cultured in normoxia, tissue oxygen and hypoxia (20%, 7%, 2%O 2 ) respectively. cells were then stained for tartarate-resistant acid phosphatase to assess osteoclastic formation. cells were col ected at 1, 2, 3, 4, 5, 6, 7 days after culture respectively. The soluble receptor activator of nuclear factor-κB ligand, tumor necrosis factor receptor-associated factor 6, tartarate-resistant acid phosphatase, and cathepsin K mRNA expression levels were determined using real-time quantitative PCR.
RESULTS AND CONCLUSION:The number of osteoclasts positive for tartarate-resistant acid phosphatase in the hypoxia was significantly lower than that in the tissue oxygen and normoxia (P<0.05). Under different oxygen tension, the mRNA expression levels of soluble receptor activator of nuclear factor-κB ligand in osteoclasts maintained unchanged. The mRNA expression levels of tumor necrosis factor receptor-associated factor 6 reached the peak at 5 days after culture in tissue oxygen and normoxia (P<0.05). The mRNA expression time of tartarate-resistant acid phosphatase and Cathepsin K were delayed accompanied by decreased oxygen tension, but the maximum were maintained in tissue oxygen. Compared with normoxia and hypoxia, osteoclasts cultured in tissue oxygen are more prone to differentiate and maintain the activity and functions.
