1.Effect of pelvic exercise on walking ability in hemiplegia

ObjectiveTo observe the effect of pelvic exercise to improve walking ability in hemiplegia. Methods60 stroke patients were devided into 2 groups, pelvic exercise group( 30 cases) and control group(30 cases). The walking ability in all patients was evaluated to compare the effect between two groups.ResultsThe excellent and good rate was 93.3% in pelvic exercise group. As compared with control group, there is a significant improvement in walking ability in pelvic excise group. ConclusionsPelvic exercise can significantly improve walking ability in stroke patients.
2.Differential diagnosis of cystic lesions in different cervical regions on multiple-slice spiral CT

Objective To evaluate the value of MSCT in differentiate cystic lesions in different cervical region. Methods Forty cases with cyst lesions in the neck which was confirmed by pathology were collected in our study. The following diseases were excluded: cystic lesions from thyroid gland and thymus or bronchus, cystic changes of solid lesions ( necrotic lymph node) and cystic or solid lesions in the parotid gland. All patients underwent MSCT scan on GE Lightspeed 16. The parameters were following: 120 kV,260 mA, thickness 5 mm, pitch 1. 375. The delay time for contrast enhanced CT was 25 seconds for arterial phase and 60 seconds for vein phase. Results Lesions were analyzed in 4 different cervical regions including parotid gland, midline, lateral cervix and posterior cervix. Among the lesions in parotid area,2 were first brachial cyst and the other 2 were epidermoid cyst. First brachial cyst occurred intraparotid gland and epidermoid cyst occurred intraparotid or extraparotid gland. Among the lesions in midline area, 17 cases were thyroglossal cysts,81.0% were in the level hyoid bone, 12 cases of thyroglossal cysts protruded at the preepiglottic space or oral base. Another kind of lesions in midline included 3 cases of epiglottic cysts and 1 epidermoid cyst in the lower level of the neck which involved infrasternal level. The 12 cases of second brachial cysts occurred in the lateral neck. Ten cases were in the left side. Type Ⅱ was the most common type and account for 5 cases among all of second brachial cysts. Type Ⅰ and type Ⅱ were running up,accounting 3 cases respectively. Type Ⅳ account for 1 case. Two cases with lymphangioma and 1 case with third brachial cyst located at the posterior neck. Conclusion According to the location and typical imaging appearance, MSCT can do the differentiation of different cystic lesions in the different area of neck.
3.Endothelial nitric oxide synthase gene polymorphism and acute myocardial infarction.

Objective To investigate the association of the Glu298-Asp(894G→T)mutation at exon 7 of the endothelial nitric oxide synthase gene and acute myocardial infarction.Methods By using the designed primers based on flanking sequences of the Glu298-Asp mutation at exon7 of eNOS, 894G→T fragments were amplified by nested PCR from genomic DNA of healthy control and AMI subjects, digested by restriction enzyme after amplification and detected by agarose gel electrophoresis for 894G→T genotyping. To count the genotype and allele frequencies, the significance of difference in the genotype and allele between two groups were assessed by statistical analysis.Results Three genotypes containing GG, GT, TT were identified on the 894 site of the eNOS gene exon 7 in both AMI and controls. In AMI group, there were 25 patients having 9 homozygotes, the other 16 having heterozygotes. In controls, there were 13 cases with G894T mutation,all having heterozygotes. Since the definition of sepsis has been first proposed in 1991, the main content of the definition has been developed by infection + systemic inflammatory response syndrome as infection + organ dysfunction,and organ dysfunction is indicated by an increase in sequential organ failure assessment(SOFA) score.These are components of Sepsis-3 which was published in 2016.In Sepsis-3,the SOFA score in the identification and diagnosis of sepsis plays an important role,but Sepsis-3 was mainly used in adults,how to adjust it to children,some questions and details were proposed in this article.
6.Antagonistic Effects of Extracts from Cultural Mycelium of Cordyceps sinensis on Arrhythmia

AIM The purpose is to study the anti-arrhythmic effects of extracts from cultured Cordyceps sinensis. METHODS Aconitine and barium chloride were used to induce experimental arrhythmia models in rats. RESULTS The results showed that PE was more potent than other ingredients. Pretreatment with PE by oral administration significantly antagonized arrhythmia induced by aconitine. PE significantly prolonged the onset of arrhythmia, decreased the duration and diminished the degree of severity. And in barium chloride-induced arrhythmia model, PE had some anti-arrhythmia effect,which could diminish the degree of severity effectively to some extent. CONCLUSION PE is an active component to antagonize arrhythmia induced by aconitine and barium chloride.
7.The interventitive effect of Ziyin-Huatan Decoction by regulating exosomes on mice with gastric cancer

Objective:To observe the interventative effect of Ziyin-Huatan Decoction by regulating exosomes on subcutaneous tumor of mice with gastric cancer. Methods:MGC-803 cells were randomly divided into exosome control group, low-dose group and high-dose group. The low-dose group and high-dose group were intervened with Ziyin-Huatan Decoction of 25 μg/ml and 100 μg/ml respectively. After 48 hours, the exosomes secreted by MGC-803 cells in each group were extracted. Twenty BALB/c male mice were randomly divided into exosome control group, low-dose Ziyin-Huatan Decoction group, high-dose Ziyin-Huatan Decoction group and blank control group, with 5 mice in each group. Except for the blank control group, the mice in the other groups were injected with exosomes extracted from the cells of the corresponding group through the orbit, 10 μg/time for each mouse, once every other day, a total of 15 times; the blank control group was injected with the same amount of PBS. Then SGC-7901 cells were inoculated into mice to establish a tumor model. The tumorigenic rate and body weight of mice were observed. The levels of CD31, VEGF and bFGF in tumor tissues were detected by immunohistochemistry. Results:Compared with the blank control group, the tumor weight [(170.00 ± 10.00) mg vs. (343.33 ± 20.82) mg] and the expression of CD31 (37.43 ± 0.55 vs. 63.30 ± 0.85), VEGF (11.37 ± 1.19 vs. 70.30 ± 0.72) and bFGF (43.77 ± 1.53 vs. 84.97 ± 1.86) in the high-dose Ziyin-Huatan Decoction group significantly decreased ( P<0.05). Compared with exosome control group, the expressions of CD31, VEGF and bFGF in low and high dose Ziyin-Huatan Decoction groups were decreased ( P<0.05). Conclusion:Ziyin-Huatan Decoction can significantly inhibit the growth of subcutaneous tumor of gastric cancer in mice by regulating exudation, which may be related to the inhibition of tumor angiogenesis.
9.A thin-slice radioanatomic study of jugular foramen

Objective To observe and analyze the CT and MR imaging of the structures in the region of the jugular foramen (JF) on the base of thin-slice anatomic study. Methods Having been scanned by multislice CT and 1.5T MR scanner, two formalin-preserved adult cadavers were dissected into 1.0 mm thickness contiguous sections in transverse plane. Twenty cases without skull base and nasopharyngeal history received routine and post-contrast CT examinations with spiral and HQ mode. Twenty healthy volunteers received MR scanning, including SE T 1WI, FSE T 2WI, and 3D RF-FAST (3D Radio-Frequency Fourier Acquired Steady-State) sequences. Results JF region was divided into three levels, which included inner aperture, the jugular cavity, and the outer aperture. At the entrance of JF, there were glossopharyngeal canal and vagal canal, which wrapped the Ⅸ nerve and Ⅹ and Ⅺ nerves, respectively. CT images could display these canals in 20 cases (100%). Furthermore, the Ⅸ, Ⅹ, and Ⅺ nerves could be identified on different MR sequences. 17 cases (85%) were displayed on 3D RF-FAST, 14 cases (70%) on SE T 1WI, and 10 cases (50%) on FSE T 2WI, respectively. From the anterior to the posterior compartment within the JF cavity, the structures ranged as follows: the Ⅸ nerve, the inferior petrosal sinus, the Ⅹ and Ⅺ nerves, and the jugular bulb. These neuro-vessel structures could not be distinguished on CT, SE T 1WI, and FSE T 2WI images, except for 3D RF-FAST (16 cases, 80%). The outer aperture of JF was adjacent to the hypoglossal canal. The shape of JF outer aperture could be evaluated on CT images. On the oblique plane, which was parallel to the hypoglossal canal, the posterior cranial nerves could be detected to enter/exit the skull through the JF and hypoglossal canal separately. Conclusion The complement of CT and MR imaging would be helpful to distinguish the structures in the region of JF.
10.Clinical and endoscopic features of pediatric eosinophilic gastroenteritis in 49 cases

Objective To explore the clinical features and endoscopic manifestations of eosinophilic gastroente-ritis(EG)in children.Methods A retrospective analysis was conducted of the clinical manifestation,laboratory examination,endoscopy(upper and/or colonoscopy)performance,diagnosis and treatment of 49 cases of patients who were diagnosed as EG in Children's Hospital of Nanjing Medical University from July 2013 to July 2015.Results The common clinical manifestations of EG in children were hematochezia(23 cases),diarrhea(20 cases),vomiting(18 cases)and abdominal pain(15 cases).The ages of children admitted to hospital for the first time ranged from 1 month and 7 days old to 13 years and 7 months old,and the mean age was 59.4 months old,in which most patients were younger than 1 year old,accounting for 38.77%(19 cases)of all.Peripheral blood eosinophilia was present in 20 cases(40.82%)of the patients,and only 6/18 cases(33.33%)of the patients had elevated serum IgE.Upon endoscopic analysis,the lesions involved esophagus(4 cases),stomach(14 cases),duodenum(20 cases),small intestine(15 cases),colon(28 cases)and rectum(6 cases),and the most common manifestation under gastroscopy was mucosal hyperemia edema(27 cases)and erosion(9 cases),while the most common manifestation under colonoscopy was mucosal hyperemia edema(25 cases)and nodular hyperplasia(24 cases).All patients improved with food restriction,in which 8 cases were treated with glucocorticoid while 9 cases with oral Singulair and 9 cases with oral Loratadine.All children with symptoms were alleviated somewhat,but 5 cases of them relapsed after drug withdrawal.Conclusions The clinical manifestations of EG in children varied and were mainly hematochezia,vomiting,diarrhea and abdominal pain.Some patients had the elevated peripheral blood eosinophilia and serum IgE.The most common manifestations under gastroscopy were mucosal hyperemia edema and erosion while the most common manifestations under colonoscopy were mucosal hyperemia edema and nodular hyperplasia.
