1.Study of risk factors associated with hypospadias

Objective To study the risk factors which lead to increase in the incidence rate of hypospadias. Methods A case-control design was used.The mothers of 210 cases and 420 controls were interviewed by telephone or face-to-face,and the questionnaires were answered.Thirty probable risk factors were investigated.Statistical analysis was performed using univariate and multivariate unconditional Logistic regression analyses for evaluation of the data. Results All boys with hypospadias in the case group were of Han nationality.Only 5 cases had positive family history.Odds ratio (OR) of the factors investigated were:pregnant age (OR,38.63),threatened abortion (OR,4.71),progestin (OR,3.64),estrogen (OR,5.1),pesticide (OR,2.39),organic solvents (OR,2.12),season of pregnancy (OR,1.86) and detergent (OR, 1.87).But OR of frequency of abortion,weeks of gestation and birth weight were less than 1.The results of other 17 investigated factors were negative. Conclusions Threatened abortion,nonfull-term fetus,low birth weight and some environmental endocrine-disturbing chemicals ( pesticide,detergent,organic solvents,progestin and estrogen) may be risk factors associated with hypospadias.These environmental endocrine-disturbing chemicals may increase the incidence rate of hypospadias.
2.Study on correlation of Cytomegalovirus infection, endothelin and tumor necrosis factor with atherosclerosis in type 2 diabetes mellitus

Objective To evaluate the association between cytomegalovirus (HCMV) infection and atherosclerosis (AS) and explore the possible role of endothelin (ET) and serum tumor necrosis factor(TNF ?) in the pathogenesis of AS in diabetic patients and the relationship between HCMV infection and concentrations of ET and TNF ?. Methods Blood samples were collected from 21 patients with AS of type 2 DM, 47 patients with non AS of type 2 DM and 20 controls. Special antibodies to HCMV (IgM,IgG) were measured by enzyme linked immunosorbent assay (ELISA) . The concentrations of serum TNF ? and plasma ET were determined by RIA. Results (1) Type 2 diabetic patients, especially those with established AS, had a higher prevalence of active infection of HCMV. (2) The levels of plasma ET and serum TNF ? in type 2 diabetic patients were higher than those in normal people. In the pathogenesis of AS in diabetic patients, the ET played more important role than the TNF ?.(3) There was a significantly positive correlation between anti IgM to HCMV and the level of plasma ET in type 2 DM patients. It suggested that the injury of endothelium cell was related to the active infection of HCMV. Conclusion The study suggests that HCMV infection may be important in the pathogenesis of AS in type 2 diabetic patients and ET possibly plays an important role on the pathological course of AS.
3.Comparison of enhancement features of hepatic tumors on SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT

Objective To compare enhancement features of hepatic tumors on SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT.Methods SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT were performed on 49 patients with 54 focal liver lesions,including 29 primary liver carcinomas,4 metastatic liver tumors,11 hepatic carvernous hemangiomas,7 focal nodular hyperplasias,2 focal fatty sparing,1 cirrhotic.Results Contrast-enhanced ultrasonography and helical CT showed consistent results in demonstrating hemodynamics of hepatic tumors.In the differentiating of malignant tumors and benign tumors,the sensitivity,specificity and accuracy of contrast-enhanced ultrasonography were(90.9)%(30/33),(90.5)%(19/21) and(90.7)%((49/54)),respectively.Those of contrast-enhanced CT were(90.9)%(30/33),(95.2)%(20/21) and(92.6)%((50/54)),respectively.Conclusions Both of the two imaging methods are valuable for differential diagnosis of liver tumors.
4.A dosimetric comparison of volumetric modulated arc therapy with fixed-fields intensity modulated radiotherapy for ⅠE and ⅡE nasal NK/T-cell lymphoma

Objective To investigate the dosimetric characteristics and their clinical applications of volumetric modulated Arc therapy (RapidArc) with fixed-fields intensity modulated radiotherapy for early stage nasal NK/T-cell lymphoma.Methods Ten patients with stage Ⅰ E and Ⅱ E nasal NK/T-cell lymphoma were enrolled in the study.Five field coplanar plan (5F),nine field coplanar plan (9F),five field non-coplanar plan (5F-N) and RapidArc plans were designed for each patient,in which 5F plan was set as the control group.Conformity index (CI) and homogeneity index (HI) as well as the maximum dose of organs at risks were compared.Results The target CI of 5F,9F,5F-N and RapidArc plan was 0.419±0.159,0.478 ±0.181,0.465 ±0.121 and 0.518 ±0.111,respectively.Compared with 5F (0.136±0.038),the target HI of 9F and RapidArc plan was 0.111 ±0.027 and 0.112 ±0.031 (t =3.11,3.04,P < 0.05).9F plan significantly increased the Dmax of lens in the contralateral side(t =2.82,P < 0.05) and in ipsilateral side (t =3.25,P < 0.05),while 5F-N plan decreased the Dmax of optical nerves by up to 9%.RapidArc plan effectively reduced the radiation to organs at risk in lens (t =3.25,P <0.05),eyes (t =3.25,P <0.05),optical nerve (t =2.57,P <0.05) and optical chaism(t =7.62,P <0.05).The delivery efficiency of four plans ranked as RapidArc > 5F > 5F-N > 9F.Conclusions RapidArc produced statistically significant improvement in the dose distributions of targets,and also reduced the Dmax of organs at risk,which would be the better choice of radiotherapy for nasal NK/T-cell lymphoma.

6.Study on Reference Value of Fetal Azygos Vein and Its Relationship with Gestational Age

Purpose To establish normative reference ranges of fetal azygos vein between gestation 20.0 and 40.0 weeks and analyze the possible correlation between azygos vein diameter and gestational age.Materials and Methods Retrospectively analyzed the data which collected from pregnant women with singleton pregnancies,1375 fetuses with normal outcome were included.The gestation was from 20.0 weeks to 40.0 weeks.The research objects were divided into 5 groups according to their gestational age and were examined standardized.The fetal normal reference value of azygos vein and its correlation with gestational ages were analyzed.Results The normal reference value of the fetal azygos vein diameter was established.We found that a significant increase linear between the diameter of fetal azygos vein and the gestation age.There was no obvious correlation with sex.The ratio of azygos vein and descending aorta inner diameter was about 0.36.There was on obvious changes in different gestational ages.Conclusions Fetal echocardiography technology can accurately assess fetal cardiac structure,the establishment normal reference value of azygos vein diameter could provide guidance for fetal echocardiography examination.
7.Establishment of normal reference intervals of plasma and urine neutrophil gelatinase associated lipocalin in children

Objective To establish normal reference intervals of plasma and urine neutrophil gelatinase-associated lipocalin (NGAL) in children′s hospital.Methods A total of 183 fresh EDTA anticoagulant samples and 125 fresh urine in healthy children were collected from May 2014 to October 2014.According to the CLSI C28-A2 ,the unilateral upper limit 95% was established the normal reference value in different age group.Results There was significant difference in four groups (P<0.05).The normal reference intervals of plasma NGAL in healthy children:0 to <7 months;<291.28 μg/L;7 months to <5 years old;<150.87 μg/L;5 years old to <9 years old:<127.93 μg/L;9 years old to ≤16 years old:<161.74 μg/L;the normal reference intervals of healthy children urine NGAL:0 to <7 months:<257.31 μg/L;7 months to <5 years old:<201.55 μg/L;5years old to <9 years old:<197.69 μg/L;9 years old to ≤16 years old:<151.46 μg/L.Plasma and urine NGAL results in neonatal group were higher than the other three groups.Conclusion The normal reference intervals of plasma and urine NGAL in children′s hospital is established.this could provide clinical evidence for the diagnosis and treatment of acute renal injury in pediatric patients.
8.Effects of antidepressant agents on the neuropathic pain induced by oxaliplatin

Background and purpose:Oxaliplatin is a new cytotoxic platinum compound widely used in antineoplastic treatments.Peripheral neuropathy characterized by allodynia remains the most common way to limit the usage of oxaliplatin.Oxaliplatin-associated neuropathic pain is often resistant to standard analgesics.The effects of antidepressant agents such as desipramine and fluoxetine on chemotherapy-induced neuropathic pain were investigated so as to provide experimental evidence for clinical treatment.Methods:A single injection of oxaliplatin(30 mg/kg)intraperitoneal was injected into a test subject,a mouse that had chronic neuropathic pain.Using the von Frey filament as a touch stimulator,the mechanical withdrawal threshold(MWT)was measured when observing allodynia.The MWT was measured before and 1 h after the administration of desipramin and fluoxetine.Results:Desipramine and fluoxetine both have the potential to increase the MWT in mice with oxaliplatin-induced neuropathic pain.Pretreatment with antagonists such as an opioid receptor like naloxone could deepen their effects.Furthermore,when desipramine is combined with an opioid analgesic as buprenorphine,it causes an augmentation in the MWT.Conclusion:Antidepressants desipramine and fluoxetine antagonize oxaliplatin-induced neuropathic pain by inhibiting allodynia.Furthermore,the tricyclic antidepressing agent desipramine could enhance the effects of buprenorphine in subjects with oxaliplatin-induced pain,suggesting a synergistic effect for opioid analgesic.
9.Clinicopathological and prognositic analysis of asymptomatic IgA nephropathy in children

Objective To study the clinicopathological characteristics and prognosis of asymptomatie IgA nephropathy (IgAN) children with pmteinuria and/or microscope haematuria .Methods Clinical and pathological characteristics of 54 children with IgA nephropathy confirmed by renal biopsy were analyzed . These children with IgAN were divided into two groups according to their clinical characteristics at the first onset: asymptomatic IgAN group (AsIgAN) and symptomatic IgAN group (SIgAN) . Histologic changes were classified by Lee SM and Katafuchi semiquantitative scoring system . Results Eighteen children were in AslgAN group and 36 children were in SIgAN group . The degree of proteinuria in SIgAN group [(2 .3±2 .2) g/d] was higher than that in AslgAN group [(0 .4±0 .3) g/d] at the time of biopsy (P<0 .05) . Although asymptomatic IgAN children were mainly of Lee's type Ⅰ ~Ⅱ, 2 children (11%) were of Lee's type Ⅳ~Ⅴ and 5 cases (27%) presented interstitial injury . Symptomatic IgAN children were mainly of Lee's type Ⅱ ~Ⅲ, there was no significant difference between two groups (P>0 .05) . Urine micrealbumin was increased in 87% children presented with microscope haematuria . After an average of (26 .9±8 .8)months follow-up, only one case of Lee's Ⅴ progressed into renal failure and the others maintained normal renal function . Conclusions Although children with asymptomatic IgAN have minor clinical symptoms, severe renal pathological lesion and poor prognosis also occur . Urine screening helps to detect renal diseases and renal biopsy should be performed in the patients with increased urine microalbumin .
