Objective To investigate the feasibility and clinical effect of hip joint arthrodesis with the application of Kirschner wire and reconstruction plate in the treatment of advanced tuberculosis in total hip joint.Methods A total of 30 patients with advanced tuberculosis of the hip in our hospital from March 2014 to April 2016 were divided as Kirschner wire group and reconstruction plate according to different surgical methods,with 15 cases in each group.The operation time,bleeding volume during operation (intraoperative and postoperative),fusion time,postoperative complications of two groups were observed and compared.Results Patients of two groups reached bony fusion.Compared with reconstruction plate group,Kirschner wire group had less blood loss,shorter operative time,longer fusion time,the differences were statistically significant (P ＜ 0.05).Conclusion Both Kirschner wire fixation and reconstruction plate arthrodesis are effective for patients with advanced tuberculosis of total hip joint,but the clinical efficacy of Kirschner wire fixation in hip arthrodesis was significantly better than the reconstruction plate.

Objective To explore the clinical application and the relative factors that effect on the diagnostic accuracy in CT-guided percutaneous puncture biopsy(PPB) of pulmonary masses.Methods PPB in 72 cases with pulmonary masses were performed.The results ofsurgery,pathology, image,and the relative factors that effect on the diagnostic accuracy of PPB were analyzed.Results The punctureaccuracy was 100%,the diagnostic accuracy was 89.6%(61/72),the rate of complication was 11.1%(8/72) in CT-guided percutaneous puncture biopsy of pulmonary masses.The determining factors that effect on the diagnostic accuracy and the rate of complication were age of patients,present of emphysema,size and internal content of pulmonary masses in CT-guided percutaneous puncture biopsy.Conclusion CT-guided percutaneous puncture biopsy was a safe,accurate and simple mean of diagnosis and differential diagnosis for pulmonary masses.To improve the accuracy and reduce complication ,the related factors of patients ,lesion,and the technique should be attended.

To clarify the origin and development of the traditional Chinese medicine "Duhuo" and "Qianghuo" with medicinal literatures. Medical literatures of past dynasties were analysed and combined with the modern material. The "Duhuo" in Herbal writing Shen Nong Ben Cao Jing include traditional Chinese medicine "Duhuo" and "Qianghuo", "Qianghuo" was separated from "Duhuo" due to the distinguish of clinical application. The origin of "Qianghuo" is Notopterygium incisum and N. forbesii, However, The origin of "Duhuo" is very complex, Angelica pubescens f. biserrata as authentic "Duhuo" was used from Song Dynasty. "Qianghuo" was originated from "Duhuo".

Objective To study CT and MR imaging features of pulmonary carcinoma of mediastinal type.Methods CT and MRI data (including anatomic site,size,shape,margin of the tumors and the relationship between tumors and mediastinum) of 15 patients with pulmonary carcinoma of mediastinal type confirmed by surgery and pathology were analyzed retrospectively.All patients underwent plain CT scans,and 11 of them also underwent contrast-enhanced CT scans,and 3 of them underwent plain MR scans and Gd-DTPA enhanced MR scans.Results On CT and MRI,the single mass was located subpleural of mediastinum with wide base.There were 5 cases of small cell lung carcinoma,8 squamous carcinoma,1 adenocarcinoma and 1 adenosquamous carcinoma.The majority of tumors manifested as ellipse.The tumors were lobulated in 12 and spicular margins in nine.8 tumors located in superior mediastinum,5 in medium mediastinum,2 in infra mediastinum and 12 in the borderline between the anterior mediastinum and medinum mediastinum.The tumors and enlarged lymph nodes of hilar and mediastinum connected closely in 7 cases.The bronchus were stenosed or obstructed by the tumors in 9 cases,and metastatic bone destruction of thoracic cage in 4 cases.Conclusion The pulmonary carcinoma of mediastinal type is easy misdiagnosed,CT and MRI findings in combination with clinical data can improve its diagnostic rate.

Objective To explore the clinical characteristics of vestibular migraine in patients from the clinic and ward of the neurological department in comprehensive hospitals.Methods A total of 226 patients diagnosed as vestibular migraine were enrolled in the study.Clinical data were collected and analyzed,including the medical history,clinical symptoms and signs,as well as the result of diagnostic examinations.Results The mean age of the patients at the visit was 51.7 years old,with the male to female ratio of 1:1.48.The occurrence of vertigo and migraine varied in order,with 53.1％ (120/226) patients presented migraine several years before vertigo.The duration time of vertigo ranged from seconds to days,with 1.8％ (4/226) patients presented no headache during the whole course.Several punctate long T2 or high FLARE (fluid attented inversion recovery) signals scattered at the centrum ovale.Conclusions Vestibular migraine has complex mechanism and presents multiple clinical manifestations with certain regularities of the onset.Differential diagnosis should be made from the similar diseases.

Objective To evaluate the accuracy and clinical efficacy of low field phase-contrast magnetic resonance angiography(PC-MRA),CTA and 3D-DSA in the diagnosis of cerebrovascular disease.Methods 70 patients with clinically suspected cerebrovascular diseases were examined with PC-MRA,CTA and 3D-DSA.The sensitivity in detecting the cerebrovascular disease with these three methods were compared.Results Fifty-one patients were detected with cerebrovascular diseases in seventy patients by three imaging methods,including 36 aneurysms in 33 cases and 18 arteriovenous amlformations.33,34 and 35 aneurysms,17,16 and 18 arteriovenous malformations were detected by PC-MRA , CTA and 3D-DSA respectively , the sensitivity were 91.67% and 94.44%,94.44% and 88.89%,97.22% and 100% respectively.There were no significant difference in the sensitivity of detecting aneurysm and arteriovenous malformation between these three imaging methods(P＞0.05).Conclusion PC-MRA,CTA and 3D-DSA are no of difference in diagnosis of cerebrovascular malformation.

Objective To investigate changes of serum levels of interleukin(IL)-4,-6,-8 and tumor necrosis factor-?(TNF-?),and probe their clinical significance in children with Henoch-Schonlein purpura(HSP)/ HenochSchonlein purpura nephritis(HSPN).Methods Serum levels of IL-4,-6,-8 and TNF-? of 45 children with HSP/HSPN and 43 healthy children were examined by enzyme linked immunosorbent assay(ELISA),changes and correlation between the aforementioned cytokines in children with HSP /HSPN were analyzed.Results 1.Serum levels of IL-4,-6,-8 and TNF-? of children with HSP were higher than those of control group(P0.05);The serum level of TNF-? was positively correlated to the serum levels of IL-6,-8 of patients with HSP(r=0.670 P

To clarify the origin and application development of the traditional Chinese medicine " Zi-hua Qianhu" and " Qianhu", the medicinal literatures of past dynasties and modern researches were analysed. The plant Angelica decursivum was used as a substitute for traditional Chinese medicine "Angelica sinensis Radix" for a long historical period, it is used incorrectly for traditional Chinese medicine "Qianhu" due to origin research in modern times. The plant origin of "Qianhu" is Peucedanum praeruptorum. There are significant differences in clinical applications and chemical composition of the two drugs. The same efficacy description of "Zi-huaQianhu" and "Qianhu" could not stop "Zi-huaQianhu" used as "Qianhu" in practical application. Therefore, we need to further research for the plant A. decursivum, delimit its medicinal attribution.

Objective To investigate the clinical characteristics and evaluate surgical experiences in Hashimoto's disease patients suffering from coexistent thyroid nodules. Method Clinical data in 299 cases of Hashimoto's disease coexistent with thyroid nodules treated surgically from Jun 1985 to Dec 2004 were analyzed. Results The coexistent rate of Hashimoto's disease (HD) with thyroid carcinoma (TC) . thyroid adenoma.nodular goiter.hypothyroidism and hyperthyroidism was 17. 4% , 34. 4% , 5. 7% , 3. 7% and 2. 3% respectively. The coexistent rate of HD with TC was 9. 2% before 1995, and 19. 7% after 1996. There were 35 cases of papillary carcinoma, 11 cases of follicular carcinoma, 4 cases of mixed type and one case of lymphoma. There were 17 cases of occult thyroid carcinoma, accounting for 32.7% of the malignancy. Operative procedures included biopsy only and thyroidectomy according to frozen pathology. Conclusions There is a high incidence of coexistent thyroid neoplasm in patients with Hashimoto's disease, the incidence of thyroid carcinoma especially occult carcinoma is rising recently.

Objective To explore the acquired immunity against Schistosoma japonicum infections in the offspring of mother-mice infected with Trichinella spiralis.Methods The offspring of mother-mice infected with Trichinella spiralis and the mice after passively transferring the offspring's sera were sacrificed at day 42 post infection with S. japonicum cercariae. The percentage of reduction in worm burdens was calculated to determine the protective effects. Results Comparing with the control group, the reduction rates of the worm were 24.31% in the offspring, 18.65% and 20.77% in mice with passively transferring the offspring's sera by intravenous injection and muscular injection respectively. Conclusion The offspring of mother-mice infected with Trichinella spiralis and the mice with passively transferring the offspring's sera could acquire protective immunity against Schistosoma japonicum infections.
