1.Effects of health and safety management for elderly patients receiving gastroenteroscopy under sedation

Objective:To investigate the clinical effects of health and safety management in elderly patients receiving gastroenteroscopy under sedation.Methods:From February 2018 to November 2019, 126 elderly patients admitted to the cadre medicine and gerontology department of our hospital to undergo gastroenteroscopy under sedation were included as research subjects.According to the digital random number method, they were divided into the control group and the test group(n=63, each group). The control group was given routine management, while the test group was given routine management plus health and safety management.Clinical effects and safety were analyzed and compared between the two groups.Results:There was no significant difference in scores of the Hamilton anxiety scale(HAMA)and the Hamilton depression scale(HAMD)between the two groups before intervention.After intervention, the scores of HAMA and HAMD were lower in the test group than in the control group[(39.2±3.1)scores vs.(48.6±2.1)scores, (40.1±4.1)scores vs.(52.6±5.5)scores, t=19.926 and 14.463, both P=0.000]. The incidence of postoperative complications was lower in the test group than in the control group(7.94% or 5/63 vs.19.05% or 12/65, χ2=14.209, P=0.002). The differences between maximum and minimum values of heart rate(HR), diastolic blood pressure(DBP), respiratory rate(RR)and systolic blood pressure(SBP)were lower in the test group than in the control group(all P<0.05). The length of the procedure was shorter and the anesthetic dose was lower in the test group than in the control group(9.11±2.56 min vs.13.56±3.05 min, 15.35±2.66 ml vs.21.67±6.15 ml, t=8.870 and 7.486, both P=0.000). Patient satisfaction was higher in the test group than in the control group( χ2=16.383, P=0.014). Conclusions:Health and safety management can improve emotional health, stabilize vital signs and enhance satisfaction in elderly patients undergoing gastroenteroscopy under sedation.
2.Treatment and correlation factors of aqueous misdirection during surgery on glaucoma patients

Objective To investigate the emergency treatment and correlation factors of aqueous misdirection during glaucoma and cataract surgery in glaucoma patients.Design Retrospective case series.Participants 126 cases(132 eyes)of angle-closure glaucoma patients with aqueous misdirection during surgery,100 cases(100 eyes)of angle-closure glaucoma patients without aqueous misdirection during surgery and 100 cases(100 eyes)of senile cataract patients.Methods We performed pars plana vitreous water-bag aspiration or anterior vitrectomy on glaucoma patients with aqueous misdirection during surgery.All the factors in observation group and control groups were compared and analyzed by logistic regression.Main Outcome Measures Preoperative highest intraocular pressure(HIOP),intraocular pressure after maximum dose(IOP),anterior chamber depth(ACD),lens thickness(LENS),axial length (AXL),mean keratometry(K),intraocular lens diopter within-0.5D corrected by SRK-T formula(IOLD).Results Among all the 132 angle-closure glaucoma eyes whose posterior chamber pressures were increased during surgery,we performed pars plana vitreous water-bag aspiration in 123 eyes(93.18%),and anterior vitrectomy in 9 eyes(6.8%).Postoperative complications included shallow anterior chamber in 20 eyes(7 eyes with malignant glaucoma,9 eyes with choroidal detachment,4 eyes with delayed local suprachorodal hemorrhage),and vitreous local hemorrhage in 1 eye.Means of all the observation indexes on three groups showed significant difference(all P
3.Initial stress distribution of the maxillary anterior teeth, periodontal ligament and alveolar bone by different intruding loadings A three-dimensional finite element analysis

BACKGROUND: It is very important to explore an optimal loading force that can both allow effective intrusion and cause no damage to the teeth.OBJECTIVE: To investigate the optimal loading conditions, including loading force values and directions, in the intruding mechanics of maxillary anterior teeth.METHODS: A three-dimensional finite element model (3D FEM) of the maxillary anterior teeth was constructed to include the periodontal ligament (PDL), alveolar bone, and appliance (brackets and wire). The initial stress distribution and force of the maxillary anterior teeth were analyzed when the 3D FEM was loaded with different intruding force values and directions. RESULTS AND CONCLUSION: The optimal value of the intruding force for 6 anterior teeth fell within the range from 0.5 N to 1.0 N. The maximum stress was detected at the distal cervical level of the lateral incisor, and a relatively even distribution of initial stress was observed at 20° palatal deviation to the Y axis. The results suggest that using mini-screw implants at the labial region between the canine and lateral incisor and a combined 0.5-1.0 N intruding force with a distal force could be an optimal intrusion force system by which patients with normal occlusion could obtain a pure intrusion of anterior teeth.
5.Role of tyrosine kinase receptor RON in tumor growth and metastasis:research progress

Recepteur d′origine Nantais(RON),a tyrosine kinase receptor ,is a growth factor receptor belonging to the proto-oncogene met family and has been proved to display abnormal expres?sion in many types of tumors. The RON receptor is activated by binding to the ligand macrophage stim?ulating protein,overexpression of the receptor,variants of the proto-oncogene and by point mutations of the kinase region. The downstream transduction of RON by mitogen-activated protein kinases and phosphoinositide3-kinase signaling pathways can help regulate the proliferation,apoptosis,migration and invasion of tumor cells. A better understanding of the mechanisms and related signaling pathways of RON activation in tumor progress and development will provide more information for the RON-based target therapy.
6.Progress of clinical application of ETO in rTHR.

How to remove the well fixed cement or cementless prosthesis and get a completely distal cement removal in the rTHR are critical to the outcome of revision. Because of higher rate of union, excellent intraoperative exposure, and adjustment of abductor tension, ETO has been widely applied to rTHR and complicated primary THR by foreign scholars. Furthermore, this technology has wide indications, very few contraindications, high cure rates,and low complications rate. ETO turns out to be a safe and effective revision technology. In the article, the indication, contraindication, complications and advantages of this technique were reviewed.

7.Experimental study on prevention adhesion after orbital blowout fracture repaired by man-made plates of amniotic membrane

Objective To explore the effect of preventing adhesion after the orbital blowout fracture on orbit tissue by amniotic membrane. Design Experimental study. Participants Twenty-six New Zealand rabbits. Methods Establishing rabbit orbital blowout fracture model, the right orbit of twenty rabbits was repaired by hydroxyapatite (HA) parceled of amniotic membrane, and left orbit was only repaired by HA, the orbits of the other six rabbits weren't repaired. Then we got the tissue around bone after one week and one month, and compared the difference of three groups. Main Outcome Measures Inflammatory response of tissue around bone was ana- lyzed by HE, picric acid Sirius Scarlet dying, TGF-?immunohistoehemical observation. Results One week after operation, HE and im- munohistochemistry showed that the difference was not significant in inflammatory response between experimental group and control group (P=0.351, P=0.413), and difference is significant between blank group and experimental group (P=0.012, P=0.041). One month after operation, HE and immunohistoehemistry showed that the difference was significant in inflammatory response between experimental group and control group(P=0.037, P=0.048), and there is no significant difference between experimental group and blank group(P=0.285, P=0.472). Conclusion It has an important role of anti-inflammatory and anti-adhesion in the chronic stage of inflammatory after orbital blowout fracture repaired by man-made plates of amniotie membrane.
8.Ginkgo preparations of Chinese medicine and treatment of diabetes: mechanisms and clinical applications.

Ginkgo is one of the most successful cases of botanical drugs developed by modern science and technology during the past fifty years all over the world. At present ginkgo has been applied to the prevention and treatment of cardiovascular disease widely, and has good clinical efficacy. Type 2 diabetes has been proved to be the risk equivalents of cardiovascular disease, therefore it has an important scientific significance for looking for more effective drugs of prevention and control of diabetes. To seek more efficient and safe drug from the plant medicine which has the function of regulate blood sugar and improve insulin resistance becomes a hotspot at home and abroad. Basic and clinical studies have shown the ginkgo preparations of Chinese medicine have certain regulation effect on blood sugar and insulin resistance. In this paper, we review the mechanisms and clinical applications of ginkgo preparations on diabetes and its applications during the past 10 years.

9.The construction of human prostate specific homeobox gene NKX3.1 eukaryotic expression vector and preliminary study on the biological function of NKX3.1

Objective To investigate the basic biological functions of NKX3.1 with construction of a series of eukaryotic expression vector of NKX3.1 to study its effects on cell cycle and apoptosis.Methods The full-length reading frames (ORF) of NKX3.1 was amplified from normal human prostate tissue with polymerase chain reaction (PCR),the ORF was cloned into T vector,and then NKX3.1 was cloned into eukaryotic expression vector pcDNA3.1 (+) and pcDNA3.1 (-) with this T vector as template.NKX3.1 in pEGFP C1 was transfected into PC3 cell line,and C1 pEFP was taken as control to observe the localization of NKX3.1 in cells.At the same time,NKX3.1 in pcDNA3.1 and pcDNA3.1 were transfected into PC3 cells and the cells were collected after 24 ~48 hours to detect cell cycle and apoptosis by flow cytometry.Results The eukaryotic expression vector of NKX3.1 was constructed successfully.Fluorescent protein was ocated in the nucleus after transfection of NKX3.1 in pEGFP C1,but pEGP C1 was both expressed in nucleus and cytoplasm.There was significant effect on the early apoptosis (P<0.01),but no effect on the cell cycle (P > 0.05) after transfection of NKX3.1 compared to PC3 cell-transfected with empty vector of pcDNA3.1.Conclusions The result data suggest that NKX3.1 be nuclear protein and have effect on the early apoptosis of PC3 cell line based on the construction of eukaryotic expression vector.
10.Hepatic VX2 tumor after portal vein occlusion in rabbits:evaluation with DSA

Objective To study the value of DSA for hepatic vascular anatomy,and to evaluate the efficacy of portal vein occlusion in rabbits with hepatic VX2 tumor.Methods Twenty New Zealand white rabbits were randomly divided into two groups with 10 in each group,including test group A and positive control group B of ham operation.For the test group A,portal branch ligation(PBL)was performed for the left external branch after 3 weeks of the tumor implantation to the left external lobe.Two weeks later,the DSA of hepatic artery and portal vein were performed in all of the rabbits.Results The total displaying effectiveness of the branches of hepatic artery by DSA was better than that by vascular perfusion.There was hypovascular blood supply to hepatic artery implantation of the tumor in the test group A,comparing with that of the group B.Conclusion DSA can clearly display spacial details of the hepatic vascular anatomy in rabbits,and play an important role in post-procedual evaluation of the portal vein occlusion in rabbits.
