1.The clinical effect of pulmonary infection control window for sequence ventilation on treatment of severe pneumonia with respiratory failure

Objective To explore the clinical effect of pulmonary infection control window for sequence ven-tilation on treatment of severe pneumonia with respiratory failure,and to observe and analyze the prognosis.Methods 102 severe pneumonia patients complicated with respiratory failure were selected.They were divided into the observation group and the control group by the random number table method.The patients of the observation group were treated with pulmonary infection control window for sequence ventilation treatment,the control group were treated with conven-tional sequence ventilation treatment.The change of blood oxygen partial pressure(PaO2 ),carbon dioxide partial pres-sure(PaCO2 )and blood oxygen saturation (SaO2 )before and after ventilation,and the treatment time and symptoms improve time and the complications such as ventilator associated pneumonia(VAP)of the two groups were observed. Results PaO2 and SaO2 of the observation group after the ventilation were (75.60 ±2.89)mmHg and (0.78 ± 0.09),which were higher than control group after the ventilation (70.48 ±3.16)mmHg and (0.71 ±0.11),and data of two groups after ventilation were higher than those before the treatment,differences were statistically significant (t =11.30,10.92,13.26,14.08,all P <0.05).PaCO2 of the observation group after the ventilation was 49.21 ±3.22 mmHg, which was lower than that of control group after the ventilation of 54.35 ±3.82 mmHg,and data of two groups after treatment was lower than before treatment,the differences were statistically significant(t =12.44,11.85,14.16, 13.42,all P <0.05).The total mechanical ventilation time of the observation group,respiratory difficulty relief time, lung then disappear time and hospital stay time were (5.24 ±1.83)d,(38.13 ±2.91 )h,(6.72 ±0.40)d and (15.33 ±3.18)d,which were lower than those of control group,the differences were statistically significant (t =9.87,10.21,10.46,11.08,all P <0.05).The general adverse reaction and the death rate of the observation group was 9.80% (5 /51),which was lower than 19.61% of the control group(χ2 =9.85,P <0.05).Conclusion There was effect of pulmonary infection control window for sequence ventilation treatment of severe pneumonia with respirato-ry failure,and it could improve effectively lung function and the prognosis and reduce the incidence of complications.
2.A study on the relationship between acute pancreatitis and apoptosis of the acinar cells

ObjectiveTo investigate the relationship between the severity of acute pancreatitis and apoptosis of acinar cells. MethodsThe apoptotic ratio of acinar cells was measured in rat acute pancreatitis model by methods of in situ end labeling.ResultsThere was almost no apoptotic acinar cells in the model of acute edematous pancreatitis. With the increase in the severity of the pancreatitis apoptotic cells were more and more common until to the late stage of hemorrhagic and necrotic pancreatitis when the apoptotic ratio of acinar cells dwindled, meanwhile the necrotic acinar cell increased continuously.ConclusionsIn acute simple edematous pancreatitis apoptotic acinar cells is infrequently seen. In acute moderate hemorrhagic and necrotic pancreatitis the ratio is correlated positively with the severity of pancreatitis.In the late stage the ratio was correlated negatively with the severity of pancreatitis.
3.The mechanism by which taxol induces apoptosis of pancreatic acinar cells during pancreatitis in rats

Objective To investigate the mechanism by which taxol induces apoptosis of pancreatic acinar cells during experimental acute pancreatitis in rats. Methods Intrapancreatobiliary duct injection of sodium deoxycholate was carried out to establish acute pancreatitis model in Wistar rats. Taxol was injected intraperitonealy to induce pancreatic acinar cell apoptosis. The expression of apoptosis associated protein Bcl-2,Bax,Fas,FasL and p53 was detected by immunohistochemistry. Results The expression of Bax(2 506?942),Fas(279?150) and p53(180?56) increased,while Bcl-2(79?42) remained unchanged in pancreatic acinar cells during acute pancreatitis,and the expression of FasL could only be detected in infiltrative inflammatory cells. Conclusions During acute pancreatitis,the acinar cell apoptosis induced by taxol is associated with activity regulation of Bax,Fas/FasL system and p53.
4.Evaluation of diopter after cataract surgery in high myopia combined cataract

To observe the diopter after cataract surgery for vision gain in high myopia and its effect on visual outcomes.METHODS: Sixty ( 120 eyes ) high myopia combined with cataract cases received phacoemulsification and intraocular lens implantation in our hospital were retrospective studied. The patients were divided into three groups based on postoperative diopter: -1. 0 ~ -2. 0D (group A), -2. 25~-3. 0D (group B) and -3. 25 ~-4. 0D ( group C ) . The uncorrected distance visual acuity ( UCDVA) , best corrected distant visual acuity ( BCDVA) , uncorrected near visual acuity ( UCNVA ) and questionnaire of Activities of Daily Vision Scale ( ADVS ) were collected to assess the vision gain at 3mo after cataract surgery.RESULTS: At 3mo after surgery, UCDVA of group A was better than that of group B, and UCDVA of group B was better than that of group C. There were no differences in BCDVA among groups. There were significant differences between the three groups' UCNVA, it was best for the group C, followed by the group B, group A was the worst. For questionnaire, no difference was observed in patients' satisfactory for surgical results, but a significant difference was detected in postoperative glasses wearing frequency between groups.CONCLUSlON: For cataract surgery in high myopia combined cataract, postoperative diopter should be calculated based on patients' daily requirements. For patients had not used to wearing glasses and near distant working condition, it's appropriated to reserve a minor degree of myopia. Conversely, for patients require near distant working and received wearing moderate and low hyperopia mirror, higher degree of myopia can be reserved.

8.High glucose induces apoptosis of human peritoneal mesothelial cells by caspase-3 activation

Objective To investigate the mechanism of high-glucose-induced injury to human peritoneal mesothelial cells(HPMC). Methods (1)The cultured HPMCs were exposed to culture medium containing different concentrations of glucose(1. 5% , 2. 5% , 4. 25% )for 48 hours and 4. 25% mannitol and normal culture medium were as control. Then apoptosis was observed by flow cytometry and caspase-3 activity was measured by ApoAlert?CPP33/Caspase-3 Assay kits. (2) The cultured HPMCs were exposed to 4.25% glucose culture medium containing different concentrations of caspases inhibitor, Z-VAD. fmk (25, 50, 100 ?mol/L) for 48 hours and 4. 25% glucose culture medium containing DMSO was as control. Then apoptosis was observed by flow cytometry and caspase-3 activity was measured by ApoAlert?CPP33/ Caspase-3 Assay kits as well. Results (1) Glucose increased caspase-3 activity in a concentration-dependent manner. Compared to control, caspase-3 activity was significantly higher in 4. 25% glucose group and 2. 5% glucose group, but not significantly different in 1. 5% glucose group and 4. 25% mannitol group. (2) Apoptotic rate of HPMC was significantly lower in Z-VAD. fmk group than that in control. Z-VAD. fmk decreased the number of apoptotic cells in a concentration-dependent manner. Also, caspase-3 activity of HPMC was significantly lower in Z-VAD. fmk group than that in control. Conclutions (1) High-glucose can induce apoptosis and caspase-3 activation of HPMC in a dose-dependent manner. (2) Z-VAD. fmk inhibits high glucose-induced apoptosis of HPMC in a dose-dependent manner. (3)High glucose induces apoptosis of human peritoneal mesothelial cells by caspase-3 activation.
9.Clinical analysis of 35 cases of adult henoch schonlein purpura

Objective To examine the epidemiological and clinical features of the adults with Henoch Schonlein Purpura (HSP),and improve the diagnosis and therapeutic efficacy of HSP.Methods Thirty five adults with HSP from the hematological department of Tianjin Medical University General Hospital between 1993.9 to 2005.12 were enrolled in this study.Epidemiological,clinical and laboratory data,therapeutic methods and efficacy were investigated.Results Most patients with HSP in our department were male and adolescent.HSP occurred more commonly in the winter.The important predisposing factors were infection and having specific food.Purpuric lesions were the most common presenting signs.Multiple organ involvement was more common.Purpura was present in all case,lower extremities were more commonly involved.Joint involvement was observed in 15 cases(42.9%),abdominal involvement in 20 cases(57.1%),renal involvement in 20 cases(57.1%).Conclusion The adult patients with HSP had more tendenecy to have severe renal damage and multiple organ involvement.
