1.Treatment of chronic diabetic foot ulcer with the combination of an inorganic activity induced skin wound dressing and a nano-silver dressing

Objective To evaluate the effect of Dermlin,an inorganic activity induced skin wound dressing,in combination with a nano-silver dressing,on chronic diabetic foot ulcer.Methods A total of 93 elderly diabetic patients with chronic foot ulcer were recruited and randomly assigned into the treatment group and the control group.In additional to routine systemic and topical treatment for both groups,the treatment group was given a combination of Dermlin and nano silver dressngs,while for the control group,iodophor gause was used to cover the ulcer.The two groups were compared on ulcer grading,wound healing time and positive rate of bacterial culture.Results The wound healing time in the treatment group was significantly shorter than that in the control group [(41.6±2.7) d vs.(61.9±2.9) d,t=-35.09,P=0.000].Positive rates of bacterial culture for the treatment group at admission,14 d after admission and 28 d after admission were 88.46%,23.07% and 15.38%,respectively,in contrast to 88.89%,59.26% and 40.74,respectively,for the control group at the same time points (x2 =0.01,6.88,7.34,P=0.065,0.008,0.006).The rate of effectiveness for the control group was 63.83% (30/47),clearly lower than the 82.61% for the treatment group (38/46) (x2=4.17,P=0.035).The patients' foot ulcers were graded using the Wagner system.In the treatment group,27 patients with grade 2 ulcers showed an 88.89%effectiveness rate (24/27),considerably higher than the rate of 67.86% in the control group (18/28)(x2=4.60,P=0.032).For grade 3 ulcers,the effectiveness rate was 73.86% (14/19) for the treatment group and 63.15% for the control group (12/19),with no statistical difference between the two groups (x2=0.49,P=0.364).Conclusions The combination of Dermlin and Nanosilver can promote healing for grade 2 chronic diabetic foot ulcers,but has no obvious effect on grade 3 ulcers.

4.Effects of SB203580 on biological functions of human extravillous trophoblast cells under oxidative stress

Objective · To investigate the effects of a p38 mitogen-activated protein kinase (p38 MAPK) inhibitor SB203580 on biological function changes of human extravillous trophoblast cells induced by hypoxia/re-oxygenation (H/R). Methods · In-vitro cultured early pregnancy villus explants and human extravillous trophoblast cell line HTR8/SVneo were assigned to 4 groups according to different interventions, i.e. control group, SB203580 group (p38 MAPK inhibition), H/R group (simulation of preeclampsia by the oxidative stress model), and SB203580+H/R group. The effects of SB203580 on biological functions of human extravillous trophoblast cells under oxidative stress in early pregnancy villus explants were observed. Protein expression and phosphorylation of p38 MAPK in HTR8/SVneo cells were measured with Western blotting. Cell migration and invasion were observed with Transwell migration and Matrigel invasion assay, respectively. Gelatin zymography was used to measure the activity of matrix metalloproteinase (MMP) -2/9 in supernatant. ELISA was used to detect the levels of soluble fms-like tyrosine kinase-1 (sFlt-1) and soluble endoglin (sEng). Results · SB203580 could promote the exogenous migration of human extravillous trophoblast cells in early pregnancy villus explants under oxidative stress. H/R could decrease the migration and invasion of HTR8/SVneo cells , and increase the phosphorylation level of p38 MAPK in HTR8/SVneo cells and the secretion of sFlt-1 and sEng. SB203580 could increase the activity of MMP2/9 in supernatant and cell migration and invasion, decrease the phosphorylation level of p38 MAPK in HTR8/SVneo cells under oxidative stress and the secretion of sFlt-1 and sEng. Conclusion · SB203580 can protect biological functions of human extravillous trophobalst cells under oxidative stress.
5.Evaluation of left atrial function in patients with atrial fibrillation after having catheter ablation by real-time three-dimensional echocardiography and speckle tracking imaging

Objective To evaluate left atrial (LA) function by real-time three-dimensional echocardiography (RT-3D) and speckle tracking imaging (STI) after circumferential ablation of pulmonay vein ostia for atrial fibrillation guided by carto mapping system. Methods Thirty patients who had ablation procedure for paroxysmal atrial fibillation were accepted echocardiography to estimate the LA function at pre-operation,3 days and 3 months of post-operation. The LA maximal, minimal and pre-systolic volume (LAVmax, LAVmin and LAVp), LA passive ejection fraction (LAPEF), LA active ejection fraction (LAAEF) were measured by RT-3DE respectively. The strain rate curve was acquired in each LA segment (anterior, posterior, lateral, inferior and septal) by STI. Systolic strain rate (SRs), early diastolic strain (SRe) and late diastolic strain rate(SRa) of each left atrial segments were measured. Results Comparing with preoperative,3 days post-ablation SRs,SRe and SRa decreased significantly in each wall of LA. After 3 months,SRa and SRe in anterior, inferior and septal were significantly improved, however, SRs showed no significant difference. LAVmax and LAVpre were significantly decreased. LAPEF and LAAEF were significantly improved. Conclusions The patients with circumferential pulmonary vein ablation were in a deterioration after three days. The pump function and conduit functions of left atrial were increased after 3 months.
6.Study on the best titer of rAAV2 delivered to myocardium by ultrasound mediated microbubbles

Objective To study the best titer of recombinant adeno-associated virus serotype 2 (rAAV2) delivered to myocardium by targeted ultrasound microbubbles. Methods Twenty one adult SD rats were divided into seven groups. SonoVue attached with different titer [1.5 × 109~ 3.0 × 1011 vg/ml (virus genome/ml)] of rAAV2-EGFP was infused into the tail vein of rats, following ultrasound mediated microbubbles destruction,as experiment groups. Normal saline was infused into the tail vein of rats as the control group (without rAAV2). Rats were killed after 14 days and hearts were harvested. GFP protein expression which showed rAAV2 transfer was observed under fluorescence microscope in frozen section and integrate optical density(IOD) was measured by Image Pro Plus software. Results When the titer of rAAV2 was 1.5 × 1011 vg/ml infused into the tail vein of rats there was much more GFP expression in myocardium than lower titers (P <0.01). Conclusions The best titer of rAAV2 delivered to myocardium by targeted ultrasound microbubbles is 1.5 × 1011 vg/ml in the tail vein infusion of rats.
7.Correlation among segmental dyssynchrony, regional ejection fraction and global ejection fraction of left ventricle in patients with dilated cardiomyopathy by real-time three-dimensional echocardiography

Objective To evaluate the relationship among segmental dyssynchrony indexes, reginal ejection frction(EF) and global EF of left ventricle(LV) in the patients with dilated cardiomyopathy. Methods Standard deviation(SD) and maximal difference (Dif) in the time to minimal systolic volume in user-selected LV segments (Tmsv SeI-SD, Tmsv SeI-DIF) of LV global EF (LV-EF), reginal ejection fraction (REF) were acquired by real-time three-dimensional echocardiography in 27 controls and 24 DCM dyssynehrony patients. Data were grouped separately for 3 levels of the basal, mid and apical segments. Results The values of Tmsv SeI-SD and Tmsv SeI-DIF were linearly negatively correlated with REF in mid segment of DCM (r =-0.6921,-0.6384, P<0.01). It existed exclusive linear regression between Tmsv Sel-SD,Tmsv SeI-DIF and LV-EF only in mid segment's dyssynchrony indexes among three segments(β= -0.7836, -0.7801, P<0.01). Each REF in three segments was linear regression relationship with LV-EF(β= 0.2234,0.7506,0.1880, P <0.05) especially in mid segment. Conclusions The Tmsv SeI-SD, Tmsv SeI-DIF and REF of mid segment are important factors to influence left ventricular function in the patients with dilated cardiomyopathy.
8.Design of bridge type bone plate series and their clinical application

Objective To investigate the clinical effects of bridge type bone plate series designed by ourselves. Methods A sort of bridge type bone plate series were designed according to mechanics and structure of bridges. They were used in the internal fixation of 12 shinbone fractures and 41 fractures of middle and lower parts of thighbone. The treatment results were followed up and statistics was done to evaluate fracture healing at 8, 12, 16, 20, and 24 weeks respectively. Results Of the 53 fractures, 48 achieved clinical union and 27 solid union within 12 weeks, 51 reached clinical union and 47 solid union within 16 weeks, 53 reached clinical union and 52 solid union within 20 weeks, and all reached solid union within 24 weeks. Conclusion The self designed bridge type bone plate series are a nice internal fixation device for clinical treatment of fractures.
9.Clinical significance of CD39 CD73 subsets and both of them positive subgroups of regulatory T cells in the microenvironment of colorectal cancer

Objective The objectives of this study were to investigate the clinical significance of CD39,CD73,double positive subgroups for CD39 and CD73,and other lymphocytes with clinicopathological parameters in the microenvironment of colorectal cancer.Methods Tumor infiltrating lymphocyte(TIL)was collected from 24 patients with colorectal cancer after radical resection.The expression of CD39+,CD73+ or CD39+ with CD73+ in T cells were measured by flow cytometry.The association between these subgroups and clinicopathologic parameters was analyzed.Results The CD73+ and CD39+ with CD73+ subgroups were associated with lymph node metastasis and poor degree of differentiation,and this mechanism was closely related to tumor-associated inflammation.Conclusion CD39+ with CD73+ colorectal tumor infiltration Treg has a more unique biological activity than other Treg group.This study provides a new idea and theoretical basis for predicting the prognosis of colorectal cancer.
10.Value of ultrasonography in evaluating the femoral artery thrombosis in rabbits

Objective To investigate the value of ultrasonography in evaluating the femoral artery thrombosis in rabbit. Methods Thrombosis was induced by external application with 10% ferric chloride and transient femoral artery-occlusion with bulldog clamp in twenty New-Zealand rabbits. The thrombosis was monitored by both Doppler flowmeter and ultrasonography. The histopathological result was used as the golden standard. The consistency of the above two results with the golden standard was analyzed. Results Thirty-two femoral arteries thrombosis were produced successfully. There was high consistency between the results of ultrasonic evaluation and histopathological examination (Kappa = 0. 720, P < 0. 001 ), and moderate consistency between the results of Doppler flowmeter evaluation and histopathological examination (Kappa = 0. 546, P <0. 001 ). Within the thirty-two femoral arteries thrombosis, there were nineteen arteries with occlusive thrombus examined by histopathological means. Both the results of Doppler flowmeter and ultrasonography were highly consistent with the results of histopathological examination (Kappa = 0. 800 and 0.798 respectively, P < 0. 001 ). Conclusions There is high consistency between the results of ultrasonic evaluation and histopathological examination for the common femoral artery thrombosis in rabbit.In the non-occlusive thrombus diagnosing,ultrasonography is better than Doppler flowmeter.
