1.Analysis and Detection of IgM to Mycoplasma pneumoniae in 225 Children

OBJECTIVE To estimate the value of Mycoplasma pneumoniae(Mp)-IgM,C-reactive protein(CRP) and WBC on diagnosis of children respiratory infection.METHODS Serum Mp-IgM antibody,CRP and WBC were tested in 225 children with respiratory infection by passive particle agglutination,ITM immunoturbidimetry and analyzer of blood cell.RESULTS Forty-six cases of 225 children were identified positive in Mp-IgM.The positive rate of Mp-IgM was 20.4%.Compared the CRP and WBC among cases positive in Mp-IgM,bacterial infected and virus infected,there was significant difference bewteen cases positive in Mp-IgM and bacterial infected,no significant difference bewteen cases positive in Mp-IgM and virus infected.CONCLUSIONS The joint detection of serum Mp-IgM antibody,CRP and WBC can be a valuable clue to diagnosis of children respiratory infection.
2.Postoperatively repair methods of head and facial skin malignancies in the advanced ages.

OBJECTIVE:
To investigate postoperatively repairing methods and their clinical effects of the olders over 80 years old with head and facial skin malignancies.
METHOD:
Eighteen cases of skin cancers in the head and face, whose malignancies were resected with Mohs microscopic surgery, according to the local or systemic condition of patients after surgery we choose different repairing methods: free skin flap grafting in 2 cases, local skin flap transferring 12 cases; including rotation skin flap grafting 6 cases, sliding flap 4 cases, nasolabial flap 2 cases; transposition flap 4 cases.
RESULT:
Operations of the 18 cases went well without special complications. All of the flaps were alive with partial flap necrosis in 1 case, which was cured 1 month later by dressing changes. During the follow-up period ranged from 6 months to 2 years,no tumor recurred,the functional recovery and appearance were satisfactory.
CONCLUSION
Choosing operating methods in head and facial skin malignancies should consider patients' age, disease state and general condition. Reparing methods of Head and facial skin malignancies in the advanced ages should be selected according to their specific circumstances, especially the simple, convenient and rapid with less trauma way.

3.Study of changes in Chinese herbal medicine distribution channel.

Distribution channel of Chinese herbal medicines has been changing. From Han to Ming Dynasty, Chinese herbal medicine were mainly trafficked to urban by dealers or farmers; From the Ming Dynasty to the foundation of new China, distribution channels are primarily intermediated with township "bazaar" and national distribution center with fixed place and regularly trading hours. In the planned economy period, the state-owned herbal medicine company was the sole medium with monopoly nature. From the mid1980s to the end of last century, planned economy and market economy have been co-existing. Stepping into 21st century, producing area highlighted in the distribution channels. Presence or absence and rise or fall of different types of distribution market went throughout the changing process of distribution channels, which became an important clue. Changes were motivated by economical consideration of channel subject, which originated from commodity characteristic and social environment changes.

4.Retrograde neurocutaneous flap to repair dorsal skin defect of fingers

[Objective]To research the method to repair dorsal skin defect of fingers by using retrograde neurocutaneous flap.[Method]Adopting nutrient vessels of 4 nerve branches in hands of radial-ulnar nerve as pedicle,the flaps were designed and applied to repair dorsal skin defect of fingers.[Result]All 10 flaps in 5 cases survived and wound area were covered.[Conclusion]Retrograde neurocutaneous flaps have sufficient blood supply,with no injury to nominous vesels,and with long distance to repair.Therefore,it is an advantageous method to repair dorsal skin defect,futhermore,two flaps could be used to repair skin defect of two fingers at the same time.
5.Posterior fusion applying internal fixation and bone inducing material to treat atlanto-axial unstableness

[Objective]To investigate effects of surgical treaments applying different fixation apparatus and bone inducing material on atlanto-axial unstableness.[Method]Twenty-four cases of Afianto-axial unstablmaess were treated by posterior internal fixatiun and bono grafting,19 cases were operatad with Apofix instrument system and 5 cases with AOSYS instrument system,bone grafting was used with artificial bone inducing material.The fusion rate,physical curve of cervical vertibrae and side-effect of implantation were observed and function were evaluated.[Result]The follow-up period was from 6 months to 36 months.The average was 18 months.Bony implanting side-effect was not found.Pre-operative symptoms of nervours systems disappeared in 18 cases,diminished in 5 cases,improved in 1 case after operation.Function evaluation:excellent 20 casas,good 2 cases,better 2 casas,excellent rate 92.4%.[Conclusion]Authors consider that odontoid fracture with atlantoaxial unstabilization can be treated by surgical operation of posterion fusion appling Apofix,AOSYS systems with osteoinduction active materials to obtain instant stabilization and permanent arthrodesis,which is an effective method to re-establish the stableness of upper cervicals.
6.Analysis on operative effect of posterior instrumentation and fusion for the surgical treatment of congenital kyphoscoliosis

[Objective]To evaluate the surgical effect of posterior correction and instrumentation on the treatment of congenital kyphoscolosis deformity. [Methods]From july 2002 to august 2007,there were 15 congenital kyphoscoliotic patients,which included 8 males and 7 females with an average age of 11.6 years.The average Cobb’s angle of scoliosis was 78?(35?~156?),the average Cobb’s angle of kyphosis was 25?(15?~38?).For the 9 kyphoscolsis patients with hemivertebra,hemivertebra resection and intervertebral fusion and fixation with pedicle screw instrument system were used,Luque’s systems was used in 2 cases,for the 3 cases accompanied with Chiari malformation and/or syringomyelia and 1 case with neurofibroma,posterior correction and long segmental pedicle screw fixation were performed and the compensative curve was included in the instrumentation level.[Results]The operation time was 3.5~7.5 h (average,5.5 h ),the blood loss was 300~2 100 ml (mean,850 ml);the average follow-up was 43 months (10~82 months),and the radiographs taken from standing posteroanterior position and lateral side showed that the mean Cobb’s angle of scoliosis was corrected from preoperative 78? to postoperative 28?(11?~38?),with a correction rate of 64%.Postoperatively ,the angle of kyphosis was improved from the preoperative 25? to 16?( 7?~33?),with a correction rate of 36%.Solid fusion was achieved in all 15 cases of patients. But significant blood loss occurred in the operative procedure of neurofibroma. And long term follow-up showed 3 cases of rod fracture,1 case of crankshaft phenomenon.[Conclusion]The congenital kyphoscolosis deformity is a kind of complex syndrome concerning skeleton-muscle-nerve system,aim to different cause,strategy of treatment should be individualized.The posterior hemivertebral resection and correction is a safe and effective method for the prepubertal children with the congenital hemivertebrae deformity.
7.Clinical Progress of Acupuncture and Rehabilitation Contribute to Post-stroke Dysphagia

Dysphagia is a common problem in cerebrovascular diseases such as intracerebral hemorrhage , cere-bral infarction and so on . When food or liquid passes through the larynx , it may cause intensively bucking . Nowadays there are no specific drugs , but the swallowing function will progress after acupuncture and rehabili-tation therapy . We searched and analyzed articles on acupuncture and rehabilitation therapy in the treatment of post-stroke dysphagia in recent three years . It showed that both of them were effective for the swallowing function of patients. The combination of both therapies receives better effects. However, there are still argu-ments in the clinical using opportunity of acupuncture and rehabilitation therapy for the treatment . There are so many methods in the treatment of this disease by acupuncture. Hence, there are no efficient contrasts or standard therapies. In the future, the clinical practice should pay more attention to the therapy of high quality and optimization in order to promote its clinical application .
8.Exploration of Operation Pattern at the First Time of Spontaneous Perforation of Congenital Choledochal Cyst in Children

ObjectiveTo explore the diagnostic methods and surgery pattern at the first time of spontaneous perforation of congenital choledochal cyst.MethodsEleven cases(4 male,7 female) with spontaneous perforation of congenital choledochal cyst were 6 months to 5 years old,and their average course of disease were 4 days.Gustily abdominal distension,abdominal pain,crying and fever were present in all cases.Jaundice(7 cases) and emesis(5 cases) appeared.All cases were detected with physical sign of peritonitis by physical examination.Choledochal cysts were confirmed by CT or B ultrasound in 8 cases.All cases accepted abdominal paracentesis and biliary ascites was drawn.Three different operative procedures were performed:choledochocyst excision & Roux-Y choledocho-jejunostomy(2 cases),choledochotomy with T-tube drainage(3 cases),and cholecystostomy(6 cases).Nine children receiving external drainage operation accepted a second operation to rebuild biliary tract(choledochocyst excision & Roux-Y choledochoje-junostomy) after 3 to 6 months.ResultsAll cases had got satisfactory therapeutic efficacy without any grave complication such as fistula of anastomotic stoma,infection of biliary tract or obstruction of biliary tract.During operation,perforations were located in the juncture of choledochus and cystic duct in 5 children and were not found in the other 6 children.In the second operation,the cases receiving cholecystostomy had less peritoneal adhesion,anatomic structure changes,haemorrhage[(30-50) mL vs(100-200) mL] and operation time[(2.5-3.0) h vs(3.5-5.0) h] than those receiving choledochotomy with T-tube drainage,and did not appear inadequate drainage for cystic duct obstruction.ConclusionsFor children with more organ inflammatory edema and adherence and in a bad overall condition,the first-time operation of cholecystostomy is more reasonable.

The infection of hepatitis B virus(HBV) is a serious global public health problem around the world,with high incidence and fatality rate,especially in China.The study of diagnosis, treatment and prevention of chronic hepatitis B(CHB) had been heavily concerned. The rapid development of molecular biology technology and the wide application of biotherapy techniques have provided the important theoretical foundation for HBV detection, diagnosis and treatment.This article summarized the study progress of clinical and laboratory examination of CHB.
10.Open reduction and internal fixation with bone grafting to treat calcaneal displaced intra articular fractures

Objective To investigate the classification and treatment of displaced intra articular fractures of the calcaneus. Methods Twelve displaced intra articular fractures of the calcaneus in 8 patients were included in this study. Classification of calcaneus fractures was done according to Sanders' computed tomographic classification system. There were 2 cases of type Ⅱfractures, 4 type Ⅲfractures and 6 type Ⅳfractures. The open reduction and internal fixation (ORIF) and bone grafting were performed using a standard extended lateral approach, and the fractures were fixed with small fragment AO bridge plates with autogenous bone grafting. Average follow up was 28.6 months (range 24 to 33 months). The Creighton Nebraska Health Foundation Assessment Score System for fractures of the calcaneus was used for evaluation. Results The average score was 89.7 for type Ⅱ, 86.5 for type Ⅲ, and 73.5 for type Ⅳfractures. There was statistically significant superiority in treatment by ORIF compared with nonoperative methods (P0.05); whereas in type Ⅳfractures, the differences were significant between with and without bone grafting (P
