1.THE OPTIMAL COMPUTER AIDED DESIGN FOR LAG SCREW INTERNAL FIXATION IN THE POSTERIOR COLUMN OF THE ACETABULUM

Objective To investigate a special optimized technique for computer aided measure,and provide an anatomical basis for the placement of lag screw in the posterior column of the acetabulum. Methods Eighty accurate hemi-pelvis models were reconstructed from 40 CT data sets.The optimization objective function,for improving the placement of lag screw,was built by adaptively sampling the weighted distance of screws to the acetabulum boundary according to surgical requirements,and the two end points of the screw were modified iteratively to reduce the objective value under constraints.The statistical measure data were provided according to new anatomical reference landmarks for clinical use. Results The optimal entry point for lag screw fixation in the posterior column was located(18.90?1.19) mm above the line connecting the anterior borderline of the sacroiliac joint with the notch between the anterior superior iliac spine and the anterior inferior iliac spine,and the connecting line was divided by the perpendicular project point with a proportion of 2∶3.The mean inclination of the lag screw was(85.99?2.04)? for angle with the line connecting the entry point with the notch between the anterior superior iliac spine and the anterior inferior iliac spine,and(37.54?1.55)? for angle with the line connecting the entry point with the perpendicular project point;the mean length of the lag screws in the posterior column was(133.07?3.22)mm.Conclusion The automatic optimized technique for the computer aided measure is very efficient and has many advantages over the conventional manual dissection methods,and is convenient to design new anatomical reference landmark systems for clinical use.
2.Synovial mesenchymal stem cells for knee cartilage injury

BACKGROUND:Knee cartilage injury is difficult to heal, which is an urgent clinical problem to solve. OBJECTIVE:To summarize the advantages of synovial mesenchymal stem cel s in the treatment of knee cartilage injury. METHODS:A computer-based search of PubMed was performed for articles related to synovial mesenchymal stem cel s for knee cartilage injury using the keywords of“knee joint, cartilage, synovial membrane, tissues, injuries and repair”. A total of 48 articles were retrieved initial y, and 35 articles were included in result analysis. RESULTS AND CONCLUSION:Synovial mesenchymal stem cel s for treatment of knee cartilage injury can achieve more ideal outcomes. Mesenchymal stem cel s inherent in the damaged tissue are the optimal seed cel s for tissue repair.
3.Study on the Principal Component of Out of Pocket Expenditure Ratio

Objective:To compare and analyze the changes and the influencing factors of out of pocket(OOP) expenditure before and after the new medical reform.Methods:2000-2015 time series data was used to compare the changes of OOP in different years.The cross-section data was applied to compare the OOP of China and other countries.The principle component analysis method was used to research the contribution rates of different factors for the OOP ratio changes.Result:According to the vertical comparison,it found that the OPP ratio showed steady decreasing trend.The horizontal comparison found that there were great gap for OOP ratios of China and other countries.The per capita GDP,under five mortality and proportion over 65 years were significant factors for OOP.Conclusion:The OOP ratio still needed to be devreased,which needed to promote the economic growth,implement medical insurance system,improve the public health input and decrease the health economic burden for residents.
4.Acetabular medial wall displacement osteotomy in total hip arthroplasty for treatment of adult hip hysplasia An 8-case analysis

The present study retrospectively analyzed 8 patients with hip dysplasia (8 hips) comprising 2 males and 6 females, aged 46 years old (range, 35-65 years old) who received treatment at the Department of Orthopedics, Second Affiliated Hospital of Nanchang University between September 2004 and November 2006. Preoperative radiographs and CT examination showed that all patients presented with sharp angle of affected hip > 45°, poor containment of femoral head, narrow joint space, and non-successive Shenton's line. Following Crowe classification, there were 5 cases (5 hips) of grade Ⅱ,and 3 cases (3 hips) of grade Ⅲ. Preoperative Harris hip score was 50 points (range, 44-62 points). Total hip arthroplasty was employed in each patient. Prosthesis was fixed by Cementless or cemented modes. Cementless fixation was applied in 6 hips and cemented fixation in 2 hips. Cementless prosthesis was used on the affected side in each patient. Replacement time, bleeding volume during surgery, hospital days post-surgery, Harris scores, and peri-operative complications were recorded.Simultaneously, post-surgery healing was observed. All 8 cases were followed up for an average period of 6 months. All patients exhibited well recovered joint function. No infection, sciatic nerve injury, dislocation of hip joint, fracture of femoral shaft, thrombosis of deep veins, or prosthesis loosening were found in any follow-up case. Six months after surgery, Harris hip score was 88 points (range, 86-94 points). Radiographs taken at 3 months post-surgery displayed the bone union of surgical region. All these findings suggest that acetabular medial wall displacement osteotomy in total hip arthroplasty produces obvious early- and middle-stage effects in treatment of hip dysplasia.

Objective To study the effect of recombinant rat augmenter of liver regeneration (rrALR) on proliferation and apoptosis of renal tubular cells in vitro. Methods The cultured human renal tubular cell line (HK2 cells) was divided into several groups with various concentration of gentamicin(GM) or/and rrALR. The proliferation of HK2 cell was evaluated by 3H-TdR incorporation. The apoptosis of HK2 cells was assessed by AO/EB staining and flow cytometer using Annexin V-FITC and propidium iodide (PI) staining. Objective To design and identify small interfering RNA (siRNA) targeting tumor necrosis factor‐α (TNF‐α) ex‐pression ,siRNAs were electroporated into synovial cells of Human to screen those which can effectively suppress TNF‐α expres‐sion .Methods Three TNF‐α specific double stranded siRNA were designed targeting different regions of TNF‐α mRNA(compared with negative control group) .RT‐PCR and Elisa were applied to detect TNF‐α ,mRNA expression and the secretion of TNF‐α in cell supernatant ,respectively .Results Two of the three customized TNF‐α siRNA could inhibit the expression of TNF‐α mRNA in synovial cells of Human (P< 0 .01) .At the same time ,the secretion of TNF‐α decreased in cell supernatant ,the difference was sig‐nificant statistically compared with the control group(P< 0 .01) .Conclusion TNF‐α siRNA can be successfully designed and syn‐thesized ,which can specifically and effectively suppress TNF‐α mRNA expression .
7.EFFECTS OF NON-NUTRITIVE SUCKING ON THE DEVELOPMENT OF NEONATES

Objective To explore effects of non-nutritive sucking ( NNS) on various aspects of neonates.Methods 58 cases refusing oral feeding were selected from June 2012 to June 2013 in our hospital with no significant differences in weight, gastrointestinal function and sleep (p>0.05).The specimen was randomly divided into the experimental group and the control group.The experimental group was given with NNS.Weight gain, quiet time to fall asleep, sleep time and gastro-intestinal disorders were observed and recorded in both two groups.One month later, the data recorded from both two groups were processed and analyzed statistically.Results The weight gain in the experimental group was significantly higher than that in the control group (p>0.05).The quiet time to fall asleep in the experimental group was shorter than that in the con-trol group (p>0.05).The sleep time in the experimental group was longer than that in the control group (p>0.05).The incidence of gastrointestinal disorders in the experimental group was significantly lower than that in the control group ( p >0.05).Conclusion Non-nutritive sucking in neonates can accelerate weight gain, improve sleep quality and reduce the in-cidence of gastrointestinal disorders to promote the growth and development of newborns to some extent .

9.Effect of CGRP on level of lung endogenous NO in rabbits with acute lung injury

AIM: To examine whether calcitonin gene-related peptide (CGRP) enhances nitric oxide (NO) level in pulmonary circulation blood and observe the influence of CGRP on mean pulmonary artery pressure (mPAP) in rabbits with acute lung injury (ALI) caused by oleic acid. METHODS: The level of NO was assessed by measuring the presence of nitrite in cervical artery blood by the Griess reaction, mPAP was measured with right ventricular catheter. RESULTS: The level of nitrite in cervical artery blood was significantly increased and the mPAP was markedly reduced after administration of CGRP intravenousely.CONCLUSION: CGRP enhanced the NO level of pulmonary circulation blood and reduces the mPAP significantly in rabbits with ALI.
