Objective: To evaluate color velocity imaging-quantitative method (CVI-Q) in estimating cerebral hemodynamic change in patients with cerebral infarction. Methods: The carotids of 60 normal people and 40 cerebral infarction patients were detected by CVI-Q. We observed endangium thickness and atheromatous plaques,and measured the diameter (d), peak velocity(Vmax), resistance index(RI) and blood flow volume(Q) of the common carotid arteries. Results: In cerebral infarction group there were 75% cases with endangium thickening to different degrees, 45% cases with atheromatous plaques and 71% plaques in carotid enlargement section or bifurcation. The data measured in 2 groups were compared: (1)The d value in cerebral infarction cases increased than that in normal（P<0.05 or P<0.01）; (2)The Vmax reduced in cerebral infarction cases(P<0.05); (3)The RI increased in cerebral infarction cases (P<0.05 or P< 0.01); (4) The Q value reduced in cerebral infarction cases (P<0.01). Conclusion: CVI-Q can be used for detecting cerebral hemodynamic changes and provide quantitative indexes for clinicians to estimate ischemia degree and treatment in cerebral infarction patients.

Abstract?AlM: To observe the anterior chamber angle shape of patients with diabetic retinopathy ( DR) at different stages using OCT, and to explore a more sensitive and accurate glaucoma preventing measure.?METHODS: A total of 613 cases of DR patients were divided into proliferation phase group ( 353 cases ) and background phase group ( 260 cases ) , another 100 cases of patients with only diabetes were selected as control group. OCT was used to detect the anterior chamber angle related parameters, including angle opening distance ( AOD) , trabecular-iris space area ( TlSA) , angle recess area ( ARA ) , anterior chamber depth ( ACD ) and anterior chamber angle ( ACA) .?RESULTS: There were significant difference among AOD500, AOD750, TlSA500, TlSA750, ARA500 and ARA750 of control group, proliferation phase group and background phase group ( P< 0. 01 ). The proliferation phase group was the lowest, AOD500, AOD750, TlSA500, TlSA750, ARA500 and ARA750 were 0. 60 ± 0. 22mm, 0. 78 ± 0. 39mm, 0. 22 ± 0. 12mm2 , 0. 45 ± 0. 19mm2 , 0. 29 ± 0. 18mm and 0. 46 ± 0. 15mm, respectively. There were significant difference among ACA500, ACA750, ACD500 and ACD750 of control group, proliferation phase group and background phase group ( P< 0. 01 ). The proliferation phase group was the lowest, ACA500, ACA750, ACD500 and ACD750 were 30. 29o ± 8. 19o, 21. 20o ± 7. 40o, 2. 32 ± 0. 23μm and 2. 52±0. 16μm, respectively. ln addition, ROC results indicated that all these indices exert significant difference on evaluating the stage of DR. ?CONCLUSlON: Anterior chamber angle shape detected using OCT can reflect the change of retinopathy. OCT is an accurate and simple technique for detecting DR staging.

The principles for the design of performance appraisal program and the specific appraisal indicators in Information Department of Chinese PLA Second Artillery Forces were thus described in this paper in an attempt to provide reference for the information management in other hospitals.

?AIM: To investigate NO levels in tears and anterior chamber inflammation after phacoemulsification and intraocular lens implantation with trabeculectomy through different operative incisions.?METHODS: Totally 49 patients ( 50 eyes ) with primary acute angle - closure glaucoma and cataract were randomly divided to single-incision group and double-incision group. Both were treated by phacoemulsification and intraocular lens implantation and trabeculectomy after routine IOP-lowering drugs treatment. Preoperative and postoperative NO levels in tears were compared, and the aqueous flare and cells were examined using a laser flare-cell meter ( LFCM) .?RESULTS:Postoperative tear NO was 9. 86±0. 78μmol/L in single-incision group , it was 9. 13 ± 0. 67μmol/L in double-incision group, the differences was statistically significant(t=3. 57,P<0. 05). Postoperative aqueous flare values was 62. 42±18. 16 pc/ms in single-incision group;it was 52. 20 ± 17. 57 pc/ms in double-incision group, the differences was statistically significant(t=2. 02, P<0. 05).?CONCLUSION: The early inflammatory injury of double-incision was significantly lower than that of single -incision. It has the advantages of safety. But the surgical skill should be improved to less the injuries caused by operations.

BACKGROUND: Nerve growth factor (NGF) is characterized by poor stability both in vitro and in vivo, and liable to lose its bioactivity.OBJECTIVE: To study the stability of liposome-encapsulated NGF injection preserved under various conditions.DESIGN: A controlled study of liposome-encapsulated NGF.SETTING: Department of Surgery of the First Affiliated Hospital of Nanjing Medical University.MATERIALS: This study was carried out at the Central Laboratory of the First Affiliated Hospital of Nanjing Medical University between July 2002and March 2004. NGF from rat submaxillary gland was purified, encapsulated by liposome, and prepared into lyophilized dosage form before preserved under different conditions (at 4 ℃, room temperature, 40 ℃ or 40 ℃ with saturated humidity, respectively).METHODS: Chicken embryo dorsal root ganglion cultured in serum-free medium was used to evaluation the bioactivity of NGF in vitro. The dorsal root ganglion from 8-day-old chicken embryo was inoculated in a polylysine-coated 24-well culture plate and cultured in Dulbecco modified Ea-gle medium (DMEM) containing different testing samples. Only DMEM was used for culture in the negative control group, while DMEM containing NGF at different concentrations used in the positive control group. The ganglion was cultured at 37 ℃ with 50 mL/L CO2 and saturated humidity for 24 hours, and the growth of the nerve fibers was observed under an inverted microscope. The bioactivity of NGF was also evaluated in simulated condition in vivo by adding lyophilized liposome-encapsulated NGF and positive control NGF specimen into 0.5 mL rat serum, which, along with the blank control serum, was added into 2.5 mL DMEM at 0.5, 1, 2, 3, 4,6 hours and thoroughly mixed. The bioactivity of NGF was assessed accord-ing to the length and density of the dorsal root ganglion and graded the prominences (recorded as "++" or "+++ "), and very long and dense growth of the prominences (++++).MAIN OUTCOME MEASURES: In vitro bioactivity of NGF preserved for 10, 30, 60, and 90 days by testing the growth of cultured chick embryo dorsal root ganglion in serum-free DMEM and in test of rats serum containing NGF added at 0, 0.5, 1, 2, 3, 4, and 6 hours into DMEM.RESULTS: Lyophilized liposome-encapsulated NGF exhibited stable bioactivity (+++) after preservation at 4℃ and room temperature for 10-90days; at 40 ℃ for 10-60 days, the it retained its the bioactivity (+++),which, however, slightly decreased by 90 days (++); its bioactivity was preserved (+++) at 40 ℃ with saturated humidity for 10 days (+++), slightly decreased at 30-60 days (++) and noticeably lowered (+) at 90 days. When preserved for 0, 0.5, 1, 2, and 3 hours in rat seum, the NGF preparation retained stable bioactivity (++++ or +++), which slightly decreased at 4 hours and 6 hours (++).CONCLUSION: Liposome-encapsulated NGF has stable bioactivity but its preservation at relatively high temperature with high humidity is difficult.Lyophilized liposome-encapsulated NGF exhibits better bioactivity than NGF-liposome suspension after preservation under various conditions.

Objective To investigate the value of 3. 0T MR diffusion-weighted imaging (DWI) in diagnosing the renal pelvic cancer and the association of apparent diffusion coefficient (ADC) value with the histological grade and clinical stage of cancer. Methods The clinical data of 33 patients with renal pelvic carcinoma were retrospectively analyzed. Routine MRI and DWI with b-values of 0 and 800 s/mm2 were performed, and the examination results were compared with the corresponding pathological results. We then calculated the sensitivity, specificity, accuracy, positive and negative predictive values of DWI. The DWI signal characteristics of lesions were recorded and analyzed, and three regions of interest (ROIs), large and small ROIs andminimum ADC (minADC) value, were selected for each lesion. The differences of ADC values between different genders, locations, cancer tissue and normal renal parenchyma, among three ROIs, among different pathological grades, among different clinical stages were compared and analyzed. Immunohistochemical staining was used to detect the expression of Ki-67 in renal pelvic cancer tissues, and the relationship between ADC value and Ki-67 expression was also analyzed. Results The results of DWI with 800 s/mm2 b-value showed that the sensitivity, specificity, accurate rate, positive and negative predictive value for renal pelvic carcinoma were 90. 9%, 91. 4%, 91. 2%, 90. 9% and 91. 4%, respectively. The ADC values were significantly different between normal renal parenchyma and renal pelvic carcinoma tissues (P<0. 05), and they were notsignificantly different between different genders, locations or among small ROI, big ROI and the minADC values. The ADC value of low grade (G1-G2) tumors was significantly higher than that of high grade (G3) ones (P<0. 01). The ADC value of cancer cells confined to the renal pelvis (Ta-T2) was higher than that with local invasion (T3)(P<0. 05). Ki-67 expression was found associated with the pathological grades and tumor stages of pelvic carcinoma, and there was a negative correlation between ADC value and Ki-67 expression (r= —0. 88, P<0. 01). Conclusion Our findings suggest that the size of ROI may be of little value in diagnosis of urothelial carcinoma, and DWI may contribute to the preoperative assessment of histological grade and clinical stages of renal pelvic carcinoma.

Objective To explore the effects of proton pump inhibitors (PPIs) therapy on esophageal acid exposure of patients with gastroesophageal reflux disease(GERD),and the correlation of anxiety and depression with recurrence of acid-related symptoms after discontinuation of PPIs.Methods From February 2010 to June 2011,28 patients with GERD diagnosed by ambulatory 24 h esophageal pH monitoring admitted to Beijing Jishuitan Hospital were treated with esomeprazole 20 mg 2 times/d for 8 weeks (male 16,female 12).Symptoms after drug discontinuation were monitored.Ambulatory 24 h esophageal pH monitoring was performed on patients,whose symptom recurred within 8 weeks after treatment.BMI,Self-rating Anxiety Scale(SAS),and Self-rating Depression Scale (SDS) were detected.Results Among the 28 patients with GERD,15 (53.6％) recurred symptoms after withdraw of PPIs.Compared with the asymptomatic group after withdraw of PPIs,the pretreatment duration of pH 4 (supine),24 h total acid reflux time,number of time periods with acid reflux ＞5 minutes,the maximal acid reflux time and 24 h total number of acid reflux in the symptomatic recurrence group were statistically significantly increased ( 11.7％vs 4.5％,138.8 minutes vs 62.1 minutes,6.0 vs 2.0,27.0 minutes vs 12.4 minutes,74.0 times vs43.0times,respectively,all P values ＜ 0.05 ).There were no significant differences in BMI,SAS and SDS between the two groups.Conclusions The basic level of esophageal acid exposure of patients with GERD before PPIs therapy may influence the esophageal acid exposure after PPIs therapy and then may affect the recurrence of symptoms.Although anxiety and depression is common in patients with GERD,it is not found that the recurrence of acid-related symptoms after the discontinuation of PPIs therapy is related to the anxiety and depression.
