1.Explore the Risk Factors of Metabolic Syndrome in Obese Children

Objective To explore the relationship between metabolic syndrome (MS) and obesity in obese children,and to explore the risk factors of MS.Methods The study included 56 obese children (26 boys and 30 girls) and 50 non-obese controls (25 boys and 25 girls).Anthropometry,fasting glucose,insulin concentrations,total cholesterol,high density lipoprotein (HDL),low density lipoprotein (LDL),triglycerides,systolic and diastolic blood pressure(BP) were measured in both of groups.Insulin resistance was determined by HOMA-IR.Finally subcutaneous and visceral fat thicknesses were measured by ultrasonographically.Results Obese children with metabolic syndrome had a high level of BMI,waist circumference,insulin levels,HOMA-IR,TG,blood pressure and visceral fat thickness than obese children without metabolic syndrome,but HDL-C was over than control group.The proportion of obese children with insulin ≥ 15 μU/ml,HOMA-IR ≥ 3.4 and TG ≥ 150mg/dl was significantly higher,while the proportion of HDL-C < 40mg/dl was significantly lower than the control group.In logistic regression analysis showed that HOMA-IR ≥ 3.4,TG ≥ 150mg/dl and HDL-C < 40mg/dl was strongly associated with the occurrence of MS and the risk was increased with the HOMA-IR score.The ORs of MS recurrence in obese children with HOMA-IR score between 3.4-4.9 was 3.8 compared with the reference group.That of HOMA-IR≥5 was 5.7,that of TG≥ 150mg/dl was 4.2,that of HDL-C <40mg/dl was 3.4 (P <0.05).Conclusion Children with HOMA-IR score between 3.4-4.9,HOMA-IR≥5,TG≥ 150mg/dl or HDL-C < 40mg/dL have an significantly increased risk for metabolic syndrome.So for these obese children,we should advance screening of high-risk patients and intervene as early as possible to prevent the occurrence of metabolic syndrome and its complications.

3.Application of arginine-rich enteral nutritional support therapy after gastrectomy

Objective To explore the clinical effects of arginine-rich enteral nutritional support therapy in patients undergoing gastrectomy.Methods The clinical data of 84 patients with gastric carcinoma who were admitted to the Wuhan Central Hospital from January 2013 to December 2014 were retrospectively analyzed.Mter gastrectomy,42 patients undergoing arginine-rich enteral nutritional support therapy were allocated to the argininerich enteral nutrition (AEN) group and 42 patients undergoing standardized enteral nutritional support therapy were allocated to the enteral nutrition (EN) group.The indexes of nutrition [BMI of patients were calculated and serum total protein (TP),albumin (Alb) and prealbumin (PAB) were detected] and immunology [T-lymphocyte subsets,total number of lymphocyte,white blood cell (WBC) and CD4 +/CD8 + were detected by flow cytometry],postoperative complications and indexes of prognosis were analyzed.The measurment data with normal distributionwere presented as (x) ± s.The comparision between groups were evaluated with the t test and repeated measures ANOVA.The count data were analyzed using the chi-square test.Results The value of TP from preoperation to postoperative day 7 was from (64 ± 16)g/L to (55 ± 13)g/L in the AEN group and from (65 ± 21)g/L to (52 ±11) g/L in the EN group,with no significant difference between the 2 groups (F =29.653,P > 0.05).The values of Alb and PAB from pre-operation to postoperative day 7 were from (33 ± 17) g/L to (32 ± 3) g/L and from (0.20 ± 0.01) g/L to (0.26 ± 0.06) g/L in the AEN group and from (32 ± 19) g/L to (27 ± 5) g/L and from (0.20 ±0.03)g/L to (0.22 ±0.03) g/L in the EN group,respectively,with significant differences in the Alb and PAB between the 2 groups (F =21.784,10.653,P < 0.05).The number of WBC,number of lymphocyte and percentage of CD8 + T-cell in the AEN group were (5.3 ± 0.7) × 109/L,(1.39 ± 0.06) × 109/L and 17.3 % ±3.5% before operation and (5.9 ±0.7) × 109/L,(1.33 ± 0.03) × 109/L and 18.4% ± 3.8% after operation.The number of WBC,number of lymphocyte and percentage of CD8 + T-cell in the EN group (5.1 ± 0.5) × 109/L,(1.40±0.04) × 109/L and 16.4%±2.8% before operation and (5.4±0.5) ×109/L,(1.23±0.04) ×109/L and 17.3% ± 3.1% after operation.There was no significant difference in the above indexes between the 2 groups (F =17.429,20.461,38.820,P > 0.05).The percentages of CD4 + T-cell and CD4 +/CD8 + before operation and at postoperative day 10 were 34.7% ± 5.4%,39.5% ± 3.9% and 1.80 ± 0.29,2.23 ± 0.32 in the AEN group,and 33.2%±4.6%,34.6%±2.4% and 1.73 ±0.26,1.82 ±0.42 in the EN group,respectively,with no significant difference in the above indexes between the 2 groups (F =14.398,7.473,P < 0.05).The incidences of postoperative complication in the AEN group and in the EN group were 9.5% (4/42) and 31.0% (13/42),showing a significant difference between the 2 groups (x2=4.459,P < 0.05).The duration of postoperative systemic inflammatory response syndrome (SIRS) were (1.3 ± 0.6)days in the AEN group and (2.4 ± 1.0) days in the EN group,with a significant difference between the 2 groups (t =6.360,P < 0.05).The time to anal exsufflation and time of bowel movements were (2.6 ± 0.3) days and (3.6 ± 0.5) days in the AEN group and (2.5 ± 0.3) days and (3.5 ± 0.5) days in the EN group,respectively,with no significant difference between the 2 groups (t =1.570,0.897,P > 0.05) . There was no perioperative death.The hospital expenses were (17 000 ±4 000) yuan in the AEN group and (22 000 ±5 000) yuan in the EN group,with a significant difference between the 2 groups (t =3.860,P < 0.05).Conclusion Arginine-rich enteral nutritional support therapy is superior to standardized enteral nutrition in effectively improving postoperative nutrition status,immune function and prognosis of patients undergoing gastrectomy.
4.Progress of imaging diagnosis in lacunar infarction

Recent studies suggest that lacunar infarction is a high risk ischemic cerebrovascular disease highly correlated with the time, which optimistic prognosis is not as good as previously thought.In addition, the role of macrovascular diseases has not been paid attention, it is not conducive to secondary prevention and early intervention for the etiology of lacunar infarction.This review focu-ses on application of imaging diagnosis of lacunar infarction, the etiology of non-lacunar infarct, and differential diagnosis of lacunar in-farction.
5.Comparison of the clinical effect of different treatment methods for the patients with great saphenous vein varicosity

Objective To compare the clinical effect of different treatment methods for the patients with great saphenous vein varicosity.Methods 140 patients with great saphenous varicose veins were selected,and according to random number table method they were divided into observation group and control group,70 cases in each group.The control group was given conventional stripped surgery,the observation group adopted high ligation joint int-racavitary laser treatment.The clinical effects of the two groups were compared.Results The operating time,intraop-erative blood loss,postoperative length of hospital stay,postoperative recurrence rate of the observation group were (51.32 ±4.97)min,(40.03 ±9.67)mL,(6.77 ±1.14)days,4.29%,respectively,which of the control group were (72.45 ±8.79)min,(108.68 ±17.75)mL,(11.69 ±2.68)days,20.00%,there were statistically significant differ-ences between the two groups(t =17.507,28.415,14.134,χ2 =8.101,all P <0.05).The incidence rate of postop-erative complications of the observation group was 4.29%,which of the control group was 14.29%,there was statisti-cally significant difference between the two groups(χ2 =4.155,P <0.05).The intervention pain score,the quality of life score of the observation group were (1.46 ±0.54)points,(90.04 ±9.77)points,which of the control group were (2.67 ±1.15)points,(78.61 ±6.82)points,there were statistically significant differences between the two groups (t =7.968,8.026,all P <0.05).Conclusion High ligation joint intracavitary laser in the treatment of great saphe-nous varicose veins is superior to the traditional operation method in operation time,intraoperative hemorrhage,length of hospital stay,and complications,and it can effectively relieve patients'pain and improve the quality of life.
6.Application of bioelectrical impedance technique in constructing physiological parameters-related models of sports human

Bioelectrical impedance technique is a non-invasive method used to acquire human biomedical data from electric property and its changes of tissues and organs. This paper introduced the principle and research methods of bioelectrical impedance technique, reviewed the application of bioelectrical impedance technique and analyzed the feasibility of bioelectrical impedance technique applied in sports science. Studies suggest that in measurement of body composition, blood and hemodynamics, the application of bioelectrical impedance technique can be used for reference in sports science; Electric impedance tomography enables non-invasive, real-time and dynamic monitoring of structures and functions of human organ and tissue under sports. The appreciable strategy for bioelectrical impedance technique application in sports science is to establish reliable and steady correlative models or regressive equations between bioelectrical impedance and physiological parameters of sports human for further study using statistical principles.
7. Advances in Study on Farnesoid X Receptor in Digestive System Tumors

8.The Analysis of Therapy in 3 012 Severe Acne Patients

Objective To analyze the current clinic application of severe acne treatment, and to provide evidences to improve its treatment. Methods Using sampling survey, a total of 3 012 severe acne patients who visited the dermato-logical department of 35 hospitals over the country for the first time were selected for this study. Each patient filled a ques-tionnaire about their acknowledgement, history of medical intervention and drug therapy of severe acne. Results Among all 3 012 patients with severe acne, 76.6%believed acne is a kind of skin disease, but only 35.2%of the patients went to see doctors at early stage of disease, while others choose interventions such as self-extrudation , topical medication or skin care products, herbal tea/folk recipes, beauty salons and application of coverture cosmetics. Among all severe acne patients, 2 388 cases (79.3%)had taken oral medications, which included 1 161(48.6%) patients who took anti-biotics. 394 cases (33.9%) took roxithromycine and 173 cases(14.9%) took other kinds of anti-biotics. 55.5%of all these patients who took oral medication less than 4 weeks in duration. 2 081 cases (69.1%) applied external drugs, in which includes 90 cases (4.3%) of using glucocorticoid, and 437 cases (21.0%) of using other kinds of external products. The adverse effects of topical treat-ments were commonly observed, such as erythema (512 cases, 24.6%), desquamation (683 cases, 32.8%), scab (73 cases, 3.5%) and hypersensitive (281 cases, 13.5%). Conclusion Severe acne is a disease need systematic treatment. but only mi-nority of the patients went to see doctors at early stage of disease. The current problems in treatment of severe acne include lack of target in choosing drugs, not long enough treatment course, and adverse effects of cutaneous administration.Early , safe and targeted medical attention with sufficient treatment course is encouraged.
