Intraocular lens ( IOL) implantation is the major method to replace the cataract lens. How to improve the biocompatibility of IOL has been the focus of current research. Major reactions after the IOL implantation include endophthalmitis, corneal endothelial edema, iritis, uveitis, and posterior capsule opacification, etc. At cellular level, macrophages, monocytes, fibroblasts and lens epithelial cells can be detected on the surface of IOL. Their adhesion, proliferation migration, and transformation may be induced by the operation related cytokines released into the aqueous humor. Detailed analysis of cytokines profile after IOL implantation may be beneficial to explore the mechanism of posterior capsule opacification.

Objective:To study the relationship of retaining time of tympanic ventilation tube and aural complications. Method:Three-hundred-five patients(659 ears)with otitis media with effusion(OME)received tympanostomy tube insertion. The tube were removed 6-36 months after tube insertion. Then aural complications were recorded in different tube retaining time, followed with a statistic analysis. Result: Fifty-five tubes of 29 patients were removed at 1-6 months after tube insertion, with spontaneous extrusion 3.4%, blocked tube 10. 3%, intrusion into the middle ear O, granulation 'tissue O, cholesteatoma O, otorrhea 6.9%, perforation O. One hundred and ninty tubes of 96 patients were removed at 6-12 months after tube insertion,with spontaneous extrusion 7. 3%,blocked tube 15.6%, intrusion into the middle ear 1%, granulation tissue O, cholesteatoma O, otorrhea 5.2%,perforation O. Three huandred and eight tubes of 156 patients were removed at 12-24 months after tube insertion, with spontaneous extrusion 9%,blocked tube 12.8% ,intrusion into the middle ear 1.3%,granulation tissue 1.9% ,cholesteatoma 0.6%,otorrhea 2.5%,perforation was O. One hundred and sixty one tubes of 83 patients were removed at 24-36 months after tube insertion, with spontaneous extrusion 36.1%, blocked tube 53%, intrusion into the middle ear 6%, granulation tissue 3. 6%, cholesteatoma 2.4%, otorrhea 2.4%, perforation 2.4%. Conclusion:The occurrence of complication didn't increase with time going by when the ventilation tube retained less than two years. However, when the ventilation tube retained more than two years, the occurrence of spontaneous extrusion and blocked tube increased obviously.

Objective To evaluate effects of pretreatment with glutamine on lung injury induced by intestinal ischemia/reperfusion(II-R) in rats. Methods Glutamine or saline were injected through tail vein before the model of intestinal ischemia/reperfusion in rats were established.The gene expression of intercellular adhesion molecule-1(ICAM-1) and heat shock protein-70(HSP-70) were tested with RT-PCR methods.The levels of heat shock protein-70 in the lung were measured with Western Blotting.Myeloperoxidase and malondialdehyde and pathological changes were also measured. Results The gene expression of heat shock protein-70 was enhanced by pretreatment with glutamine,and the level of HSP-70 was parallelly increased.Nevertheless,MPO,MDA and the gene expression of ICAM-1 were inhibited. Conclusion Pretreatment with glutamine can lessen the lung injury induced by intestinal ischemia/reperfusion in rats,the induction of HSP-70 gene may be one of the potential mechanisms.

AIM: To explorethe therapeutic efficacy of treating cataract phacoemulsification remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.
METHODS:Twenty-seven cases ( 27 eyes ) of glaucoma combined cataract surgery(3mo) in the early failure were treated with that the phacoemulsification auxiliary hook into anterior chamber under the original sclera flap, open from the inside have adhesion of the sclera flap. Postoperative follow-up 12mo, the intraocular pressure and best corrected visual acuity were observed.
RESULTS: The postoperative follow- up was 12mo. There were functional filtering bleb in 14(52%) eyes; and the IOP was below 21mmHg in 17 eyes(63%) without any anti- glaucoma drug, and in 7 eyes ( 26%) with 1 - 2 categories of lower intraocular pressure( IOP) drugs, and 3 eyes(11%), failure and the success rate was 89% after 12mo. Average intraocular pressure after 1wk, 1mo, 3mo, 6mo and 12mo were(7. 1±4. 3) mmHg,(10. 5±5. 1) mmHg,(15. 1±4. 8 ) mmHg, ( 16. 8±5. 2 ) mmHg, ( 17. 3±5.1 ) mmHg, significantly lower than the IOP preoperatively(30. 2±6. 8) mmHg (P<0. 01). Average visual acuity ( all of the following were best corrected vision) after 1wk, 1mo, 3mo, 6mo and 12mo was(0. 32±0. 52),(0. 52±0. 42),(0. 55±0. 39),(0. 53±0. 47),(0. 54±0. 42 ). Postoperative 1wk visual acuity deeper than preoperative( 0. 46 ± 0. 44 ) ( P < 0. 05 ), more than all improved compared with preoperative ( P < 0. 05 ). Postoperative anterior chamber bleeding in 5(19%) eyes, all absorbed itself in 3 to 5 days; ciliochoroidal detachment in 7 ( 26%) eyes, except one case need operate, others were conservative treatment to heal. Postoperative continuous low intraocular pressure concurrent macular edema was 1 eyes(4%), 4mo healed after treatment.
CONCLUSION: It is a safe and effective method thattreating phacoemulsification cataract remove combined with trabeculectomy early postoperative failure with open sclera flap recanalization by phacoemulsification auxiliary hook interred the anterior chamber.

Objective To evaluate the clinical application of partial splenic embolization in the treatment o hypersplenism caused by schistosomiasis. Methods Twenty four patients with hypersplenism caused by schistosomiasis were treated with peripheral partly splenic embolization with gelf.oam. The counts of white blood cell(WBC) and platelet (PLT) were compared between pre-and post-splenic embolization. Objective:To compare the clinical effects of restrictive blood transfusion combined with hyperbaric oxygen preconditioning (HBOPC) and restrictive blood transfusion in the treatment of hip,knee arthroplasty (THA,THA).Methods:40 patients in the period of epidural anesthesia,femoral nerve hysteresis hip and knee arthroplasty were selected and randomly divided into two groups:restrictive transfusion group (maintain 80 g/L≤ Hb ＜100 g/L,n=20) and restrictive blood transfusion combined with HBOPC (HBOPC+maintain 80 g/L =Hb ＜100 g/L,n=20).The red blood cell transfusion,red blood cell transfusion rate,perioperative Hb,blood oxygen saturation (SO2),the incidence of hypotension during operation,hospitalization time and postoperative cerebral infarction,acute pulmonary embolism,pneumonia,myocardial infarction,wound infection rate and 90 days mortality rate were compared between two groups.Results:Compared with the restrictive transfusion group,the postoperative Hb,blood oxygen saturation (SO2) of restrictive blood transfusion combined with HBOPC group were significantly increased(P ＜ 0.05);the red blood cell transfusion,red blood cell transfusion rate,incidence of pneumonia,wound infection rate were significantly decreased (P＜0.05).Conclusion:Restrictive blood transfusion combined with hyperbaric oxygen preconditioning could improve the anoxic state of the hip,knee arthroplasty patients,which could effectively reduce red blood cell transfusion,reduce postoperative complications,has good clinical curative effect.
