3.A Data Mining Research of Therapeutic Rules Based on Ancient Prescriptions of “Bentun Qi” Disease

This study was aimed to generalize the therapeutic rules in“Bentun qi” disease treatment through data mining, in order to show a general picture of“Bentun qi” disease by ancient physicians. Furthermore, it was aimed to reveal the nature of“Bentun qi” disease. Frequency statistics, factor analysis and association rules wereused to summarize the therapeutic rules of“Bentun qi” disease by data mining of therapeutic rules from ancient prescriptions. And then, the etiology and pathogenesis of“Bentun qi” disease were deduced. The results showed that besidesyang deficiency and cold accumulation, pathogenic fire derived from liver-qi stagnation, and drink dynamic due toyang deficiency,“Bentun qi” disease also had the etiology and pathogenesis ofqi stagnation and blood stasis due to depression of the liver-qi, blood stasis and cold stagnation due toyang deficiency, qi stagnation in the middle-jiao, and centralqi deficiency and so on. In addition, the herbs in these ancient prescriptions also related to five internal organs. It was concluded that based on theNan Jing(The Classic of Difficult Issues) andJin-Gui Yao-Lue(Essentials from the Golden Cabinet), the knowledge of“Bentun qi” disease began to show a diversity trend after theHanDynasty.
4.Analysis of risk factors of no reflow phenomenon on post-percutaneous coronary intervention in patients with acute myocardial infarction

Objective To explore the risk factors of no reflow phenomenon on post-percutaneous coronary intervention(PCI) in patients with acute myocardial infarction (AMI).Methods The clinical data of 268 patients with AMI who performed emergency PCI were collected and divided into normal group(normal flow) and no-reflow group (no flow).The risk factors of predicting the occurrence of no reflow phenomenon were screened by univariate and multivariate analysis.Results Thirty-two cases (11.9%) of 268 patients had flow occurrence and 236 cases(88.1%) were with no flow occurrence.There were significantly statistical differences between the two groups in terms of angina pectoris history before infarction,white blood cell count (WBC),history of oral of stains,administration of 600 mg of plavix before the operation,creatinine kinase,MB isoenzyme (CK-MB),C-reactive protein (CRP),Low density lipoprotein(LDL),time of coronary artery perfusion,numbers of balloons used during PCI and Killip ≥ grade 2 (all P < 0.05).The multivariate logistic regression model analysis displayed that Killip ≥ grade 2 (odds ratio (OR) =1.237,95% confidence interval (95% CI) 1.049 -2.264,P=0.021),history of oral of stains(OR=2.355,95%CI 1.547-3.438,P<0.001),WBC ≥ 12× 109/L (OR =4.139,95% CI 2.273-8.451,P<0.001),no administration of 600 mg of plavix before the procedure (OR =2.645,95%CI 1.628-5.246,P>0.005) and no angina pectoris history before infarction(OR=1.413,95%CI 1.150-2.426,P<0.001) were independent risk factors regarding of causing no reflow phenomenon.Conclusion The factors including Killip ≥ grade 2,history of oral of stains,WBC ≥ 12× 109/L,no administration of 600 mg of plavix before theoperation and no angina pectoris history before infarction were independent risk factors in terms of causing no reflow phenomenon.

6.Problems confronting hospitals under the new circumstances and ways out

Hospitals are currently confronted with many problems, including distribution according to the division of administrative areas and jurisdiction; construction with complete sets of facilities on the part of all hospitals,big or small; and worsening of the unbalanced allocation of health resources due to the to assumption by big hospitals of many tasks of bssic-level hospitals. Besides, the excessively speedy incresse of expenses for outpatient and inpatient services as a result of hospitals' introduction of high technologies has inhibited some people's demands for medical services and competition among hospitals will become particularly fierce. Thus, ways out for hospitals lie in clarifion of their respective functions and development according to regional planning; regrouping of health resources by the government through measures of shutting down,stopping down, sopping production, merging, changing products or moving to other sites; structural readjustment and implementation of the system of responsibility by hospital directors within hospitals;bringing into play the initiatives of middle-level cadres; establishment of a scientific mechanism for personnel employmennt and selection; giving priority to efficiency and consideration to fairness in distribution; improvement of service quality; and making a good job of rational charging.
7.Study on Tetrahydropulmatine Preparation with Fibraurea recisa Pierre

This study was aimed to extract and separate fibrauretine from Chinese herbal medicine Fibraurea recisa Pierre. and to prepare tetrahydropulmatine with fibrauretine. Extraction method of fibrauretine were optimized by the L9(34) orthogonal arrays as experimental designs. Then, elution chromatography method was studied with the macroporous adsorption resin chromatography technology. At last, tetrahydropulmatine was prepared by the reaction of sodium borohydride hydrogenating fibrauretine. The results showed that the content and yield of obtained fibrauretine were 36.40% and 3.17%, respectively. The reflux exacted methods of fibrauretine were with 10 times amount (v/w) of 50%ethanol solution for 2 hours and 3 times. Then, HPD-300 macroporous resin was used to absorb fibrauretine, and purify water. The 20% and 60% ethanol solution was used for elution. The content and yield of obtained fibrauretine were 65.47% and 82.44%, respectively. And the content and yield of obtained tetrahydropulmatine were 98.31% and 72.04%, respectively. It was concluded that the method can obtain fibrauretine with relative high content and yield. Using this material can obtain tetrahydrofibrauretine with relative high content and yield.
8.Clinical evaluation of different doses of brachical plexus anesthesia in the emergency suture of hand trauma in preschool children

Objective To evaluate the clinical effects of different doses of brachical plexus anesthesia in the emergency suture of hand trauma in preschool children .Methods Forty preschool children with hand trauma were choosen ,who were undergoing e‐mergency suture by general anesthesia combined with brachical plexus anesthesia .They were divided into group A(1 mg/kg ,n=20) and group B(2 mg/kg ,n=20) ,body movement during skin incision ,local injection with lidocaine ,propofol dose ,surgical time and wake up time were compared .Results There was no significant differences between the two groups in surgical time(P>0 .05) ,the cases of body movement during skin incision and local injection with lidocaine were 8 children ,which was higher than 2 children in group B(all P<0 .01) ,ropivacaine dose(14 .5 ± 5 .5)mg in group A were lower than group B (31 .6 ± 7 .2)mg(P<0 .01) ,propofol dose in group A(75 .6 ± 15 .2)mg were higher than group B(32 .7 ± 8 .6)mg(P<0 .01) ,wake up time in group A(11 .2 ± 3 .3)min were longer than group B(5 .9 ± 2 .3)min (P< 0 .01) .Conclusion The clinical effect of ropivacaine 2 mg/kg was superior than 1 mg/kg in brachical plexus anesthesia .
9.Effects of heat stress on the expression of skeleton protein and heat-shock protein in mice cortex neuron

Objective To observe the morphological changes of mice cortex neuron cultured in vitro under different temperature, and the expression of skeleton protein (?-tubulin) in the neuron, and to study the relationship between ?-tubulin and heat shock protein 70 (HSP70). Methods The cerebral cortex neuron of embryonic mice was cultured in vitro. The cultured neuron was put in different temperature 7 days later. To observe the morphological changes of the neuron using optical microscope and the changes of the expression of ?-tubulin and HSP70 under different temperature using laser scanning confocal images. Results Optical microscopy indicated that drifting cells increased, and neural network became sparse in 38℃; some cells necrosed in 39℃; most cells necrosed, cell broke to pieces, axons drifted or disappeared in 42℃. Results of laser scanning confocal images indicated that after hyperthermia the fluorescence intensity of ?-tubulin was lower than that of controls, and the fluorescence intensity declined as the temperature elevated. The fluorescence intensity of HSP70 showed a bell-shape distribution curve, i.e. the highest value emerged at 39℃, whereas the lower values appeared at 37℃ and 42℃. Conclusion Heat stress leaded to the morphological changes of neuron. The disordered of skeleton protein may be responsible for the changes and HSP70 may take part in the process.
