1.Observation of the effect of butylphthalide combined with nimodipine in the treatment of vascular cognitive impairment

Objective To investigate the effect of butylphthalide combined with nimodipine in the treatment of vascular cognitive impairment.Methods 160 patients with vascular cognitive impairment were divided into the control group and the observation group according to treatment methods,80 cases in each group.The control group was treated with nimodipine,and the observation group was given butylphthalide combined with nimodipine treatment.The therapeutic effect and cognitive function were compared.Results The obvious effective rate of the observation group was 58.75 % (47/80),which was significantly higher than that of control group 45 % (36/80) (P =0.03).After treatment,MoCA score of the observation group was (24.83 ± 9.48),which was significantly higher than that of the control group(21.64 ±9.52) (P =0.04).ADL score of the observation group was(58.31 ± 11.27),which was significantly higher than(53.28 ± 10.32) of the control group (P =0.04).After treatment,directional force,visual spatial and executive function,delayed memory and attention of the observation group were higher than those of the control group (P =0.03,0.02,0.04,0.02).Conclusion Butylphthalide combined with nimodipine in the treatment of patients with vascular cognitive impairment has good effect.
2.A meta-analysis on the association between smoking and tuberculosis

Objective To assess quantitatively the association between smoking and the risk of disease and death from pulmonary tuberculosis(TB) by a meta-analysis. Methods Both Chinese and English literatures published from Jan. 1999 to Jun. 2010 about the case-control study of the association between smoking and pulmonary TB were retrieved by searching the electronic resource databases. A meta-analysis was then performed on the comparison and synthesis of findings from included studies. Pooled odds ratio (OR) and 95% confidence interval (CI) were calculated using a software RevMan 4.2. Finally,possible publication bias was tested and a sensitivity analysis was conducted. Results The meta-analysis for total 22 high-quality case-control studies found a pooled OR of 2.56 (95% CI 1. 80 ~3.64) with a random-effects model. Sensitivity analysis showed that pooled OR was 2.56 (95% 1.76 ~3.71 ) by removing two papers of nested case-control study. The OR were 2.13 (95% CI1.76~2.58) and 4.60 (95% CI 1.81~11.69) for disease and death from TB,respectively. After confounding factors,including age and gender,had been controlled the OR was 2.53 (95% CI 1.27 ~5.03 ) whereas 1. 99(95% CI 1.54 ~ 2.56 ) without control of them. Conclusion Smoking can increase the risk of incidence and death of TB. TB control programs and treatment of patients might benefit from interventions aimed at reducing tobacco exposure.
4.Rodent models for experimental heat stroke research.

Because of the aggressive threaten of heat stroke and a lack of understanding of the mechanism of action, mammal animal models for experimental heat stroke were well developed. During the past 5 decades, anesthetized mouse, rat, rabbit, dog, baboon and monkey were used as animal model for experimental heat stroke. However, anesthetized mammals models have some limitations, such as neuroprotective effect of anesthetic agents, possible disturbance on injury and recovery of stroke animals by anesthetic agents, difficulty of discussing animal behavior before and after heat stroke, it was also difficult for the models to evaluate cognitive function of animal under hot environment. Considering humanitarian, only awaked and unrestrained mouse heat stroke model was accepted so far. Therefore, we also developed an awaked and unrestrained rat heat stroke model, and found it was helpful to evaluate drug effectiveness for animal behavior and cognitive function under hot environment.

5.Clinical value of N-terminal B-type natriuretic peptide precursor on auricular thrombosis in patients of coronary heart disease combined with atrial fibrillation

Objective To analyze the predictive value of N-terminal B-type natriuretic peptide precursor(NT-proBNP) on auricular thrombosis in patients of coronary heart disease (CHD) combined with atrial fibrillation (AF).Methods The clinical data of 124 patients of CHD combined with AF were analyzed retrospectively.All patients were divided into research group (60 cases,compliance with auricular thrombosis) and control group (64 cases,non-compliance with auricular thrombosis),according to the occurrence of auricular thrombosis.The values of NT-proBNP,fasting plasma glucose (FPG),postprandial plasma glucose(2 h PG),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),oxidized low density lipoprotein (ox-LDL),high sensitivity C reactive protein (hs-CRP) and left ventricular ejection fraction (LVEF),left ventricular end diastolic diameter (LVEDd) and left atrial diameter (LAD) were detected and compared between two groups.The relative factors to the occurrence of auricular thrombosis were confirmed by multivariate Logistic analysis.The best cutoff point of NT-proBNP was confirmed by the areas under the receiver operating characteristic curve (ROC).Results The values of NT-proBNP,ox-LDL,hs-CRP,LVEDd and LAD in research group were higher than those in control group [(4 312.6 ± 209.1) pmol/L vs.(3 421.6 ± 156.8) pmol/L,(4.0 ± 0.9) mmol/L vs.(3.4 ± 0.8) mmol/L,(7.4 ± 1.3)mg/L vs.(5.8 ± 1.0) mg/L,(74.3 ± 6.8) mm vs.(58.1 ± 5.5) mm,(39.6 ± 4.3) mm vs.(32.5 ± 3.8) mm],LVEF and HDL-C were lower than those in control group [(48.2 ± 3.1)% vs.(57.3 ± 3.8)%,(0.72 ± 0.16)mmol/L vs.(1.08 ±0.27) mmol/L],and there were significant differences (P <0.05).There were no significant differences in the values of FPG,2 h PG and TC between two groups (P > 0.05).Multivariate Logistic analysis showed that NT-proBNP and LAD were independent factors to the occurrence of auricular thrombosis (P =0.009,0.028).There was significant difference in the occurrence of auricular thrombosis between patients with NT-proBNP > 4 250 pmol/L and patients with NT-proBNP≤4 250 pmol/L (P =0.028).Conclusion NT-proBNP is an independent predictor for the occurrence of auricular thrombosis to patients of CHD combined with AF.
6.Application of three-dimensional contrast-enhanced MR angiography in the diagnosis and treatment of aortic coarctation

Objective To evaluate the clinical value of three-dimensional contrast-enhanced MR angiography (3D CE-MRA) in the diagnosis and treatment of aortic coarctation. Methods Twenty-one patients with aortic coarctation underwent 3D CE-MRA with a 3D FLASH sequence after injection of 0.2 mmol Gd-DTPA per kg b. w. The source images were subtracted and transferred to computer workstation subsequently post-procession. In six patients, cardioangiography was performed simultaneously. The diagnoses of all patients of them were proven by surgery. Results There were 6 cases (28.6%) of simple type of aortic coarctation and 15 (71.4%) accompanied other cardiovascular abnormalities. 3D CE-MRA could directly demonstrate the stenotic location, degree, extent and extracardiac vascular abnormalities, which accorded with surgical findings. The depiction of the entirety and collateral vessel formation was very helpful for surgery. It can be easier for pre- and post- operative evaluation. Conclusion 3D CE-MRA is a noninvasive and accurate technique in the diagnosis of aortic coarctation. It could replace the cardioangiography for extracardiac vascular abnormalities, and may be one of the first choice examinations.
7.Clinical Observations on Acupoint Selection at Distant Places and Local Tenderness Points for Treatment of Migraine Without Aura

Objective To investigate the efficacy of acupoint selection at distant places and local tenderness points in treating migraine without aura.Method Sixty patients were randomly allocated, using a random number table, to two groups, 30 cases each. The treatment group received acupuncture at distant points selected along meridians and local tenderness points and the control group, acupuncture at conventionally selected points. The therapeutic effects were evaluated using the visual analogue scale (VAS) and the short-form McGill Pain Questionnaire (SF-MPQ).Result After treatment, the VAS pain score was lower in the treatment group than in the control group; there was a statistically significant difference (P<0.05). The total efficacy rate was 93.3% in the treatment group and 73.3% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Both acupuncture at distant points selected along meridians and local tenderness points and acupuncture at conventionally selected points have a certain therapeutic effect on migraine, but the former produces a better effect than the later and can markedly relieve the pain and improve the quality of life in the patients. The outcome of this study provides a certain reference value for clinical treatment of this disease.
8.Evaluation of fulcrum bending radiograph for predicting curve flexibility and surgical correction result of adolescent idiopathic scoliosis

[Objective]A prospective comparative evaluation of fulcrum bending radiographs(fulcrum)to determine curve flexibility and predict surgical correction in adolescent idiopathic scoliosis(AIS).[Method]A total of 64 consecutive patients with AlS according to inclusion and exclusion criterias who underwent surgical treatment were studied.All curve types were single-curve and the PUMC classification were Ia/Ib/Ic.All patients were divided into 4 groups according to the magnitude of Cobbs angle in preoperative standing anteroposterior radiograph:moderate thoracic curve(20 cases,40?60?),moderate lumbar curve(21cases,35?60?).The Cobbs angles were measured and the flexibility ratio was determined on fulcrum radiograph.The amount of correction obtained by fulcrum radiograph was compared with the amount of surgical correction by evaluating the differences from surgery as absolute values.The correlation of Cobbs angles between postoperative standing anteroposterior radiograph and fulcrum radiograph were analyzed.[Result]For the moderate thoracic curves,curve flexibility provided by fulcrum radiograph has no significant difference(P=0.141)from surgical curve correction,there was no significant difference(P=0.094)between the mean Cobbs angle on fulcrum bending and that on postoperative standing anteroposterior radiograph.The postoperative Cobbs angle was positively correlated with the preoperative Cobbs angle on fulcrum radiograph and the correlation degree in the moderate lumbar curve was relatively weak.[Conclusion]Fulcrum radiograph can only be used to assess the flexibility and correction of moderate thoracic curves in AIS.Pedicle screw instrumentation provides even more correction.
9.RELATIONSHIP BETWEEN GALLBLADDER STONE AND LIPOROGLUCOSE METABOLISM XU

Objective To study the effects of glucose and lipid metabolism on gallstone formation. Methods Twenty five patients with gallstones and 25 normal volunteer controls were studied from January to April in 1998. The patients were well matched the control with sex and age (1∶1). In the study, Body Mass Index (BMI) and Waist-to-Hip circumference ratio (W/H) were measured. Blood glucose, glucosylated hemoglobin (HbA1C), insulin, C peptide and all parameters of lipids were detected at fasting state. The glucose,insulin, C peptide were detected again at 2-hour after taking 75g glucose orally. Results The result showed there was no difference on BMI and W/H between the patients and controls. HbA1C、mean fasting and 2-hour glucose concentration were not in significantly different between the two groups (P>0.05, P>0.2, P>0.1 respectively). There were 10 patients with abnormal glucose metabolism (7 with NIDDM, 3 with IGT), but only 4 controls were abnoumal (one with NIDDM, three with IGT). The difference was significant (P<0.05). Furthermore, the mean fasting and 2-hour insulin concentration of gallstone group was higher than that of the control (P<0.02, P<0.05). And the gallstone group had a higher fasting C peptide concentration than control (P<0.05). There was no statistical difference on the parameters of plasma lipid between the tow groups. Conclusion The study suggests that diabetes mellious and hyperinsulinemia acted as an important role on gallstone formation.
10.Protective Effect and its Mechanism of Galanin Receptor 2 Agonist Post-conditioning on Human Gastric Epithelial Cells Injury Induced by Hypoxia/Reoxygenation

Background:Gastric ischemia-reperfusion injury often leads to calcium overload,excessive free radical production, leukocyte infiltration and microcirculation disturbance.Post hypoxic treatment can effectively reduce the injury induced by hypoxia/reoxygenation (H/R).Galanin receptor 2 (GalR2)is distributed mainly in the digestive and nervous system, which can regulate many endocrine activity.However,the protective effect of GalR2 on human gastric epithelial cells injury induced by H/R is not clarified.Aims:To investigate the protective effect and its mechanism of GalR2 agonist post-conditioning on human gastric epithelial cells injury induced by H/R.Methods:H/R model was constructed on human gastric epithelial cells GES-1.Normal control group (N group),H/R group,M1145 (GalR2 agonist)treatment group (M group),SB203580 (p38MAPK inhibitor) +M1145 treatment group (S +Mgroup)and DMSO solvent control group (D group)were established.Survival rate of cells was measured by MTT assay.Apoptosis rate of cells was determined by flow cytometry,and cell apoptosis was examined by Hoechst staining.Level of lactate dehydrogenase (LDH)was measured by ELISA.Expressions of Bcl-2,Bax and p38MAPK were determined by real-time quantitative PCR.Results:Survival rate of cells was significantly lower in H/R group than that in N and M groups (P <0.05 ).Apoptosis rate of cells was significantly higher in H/R group than that in N,M and S +M groups (P <0.05 ),and apoptosis rate of cells was significantly lower in Mgroup than that in S +M group (P <0.05).Expression of LDH was significantly higher in H/R group than that in Mand S +Mgroups (P <0.05).Expression of Bcl-2 was significantly higher in N and M groups than that in H/R,S +Mand D groups (P <0.05);expression of Bax was significantly higher in H/R group than that in N,M and S +Mgroups (P <0.05);expression of p38MAPK was significantly lower in H/R and S +M groups than that in M group (P <0.05 ).Conclusions:GalR2 agonist M1145 plays an effective role in reducing the injury of GES-1 cells induced by H/R,the effect may be conducted through p38MAPK pathway.
