1.Research progress in targeted therapy of non small cell lung cancer

Objective To investigate the clinical effect of targeted drug therapy for non-small cell lung cancer.MethodsThe study group received gefitinib targeted drug therapy, the control group was given erlotinib treatment, recording two groups of patients with non-small cell lung cancer, survival time, follow-up treatment costs and adverse reaction incidence (drug).ResultsThe total efficiency of treatment (37.50%) and the control group (the total efficiency of treatment 35.42%) no obvious difference;no significant difference between the survival time of patients in group two non-small cell lung cancer, but the study group treatment costs significantly less than the control group (P<0.05);two groups of patients with non-small cell lung cancer were treated with erlotinib and gefitinib poisoning reaction contrast did not significant difference.ConclusionGefinitib targeted therapy of non-small cell lung cancer can be based on protecting the clinical curative effect and prognosis of the patients, reduce the economic pressure, more conducive to the positive reception and treatment.
2.Effect of olanzapine and quetiapine on swallowing function in patients with Alzheimer's disease

Objective To investigate the olanzapine and quetiapine quetiapine in the treatment of Alzheimer's disease patients of different clinical effects and influence on the swallowing function of patient.Methods According to the order of visits to the author departments were 73 cases of Alzheimer's patients were divided into group A(37 cases) and group B(36 cases).Patients in group A were treated with olanzapine in the treatment of, and group B with quetiapine in the treatment, the time of the two groups of patients are 12 weeks, to observe the differences of the two groups during the treatment of swallowing function indexes and clinical efficacy.Results After treatment, the MMSE scores of two groups of patients compared with pre-treatment, there was no statistical significance and BEHAVE-AD score before treatment significantly decreased ( P <0.05 ).Post-treatment, the patients in two groups of MMSE, BEHAVE-AD score difference does not have statistical significance.The difference of clinical efficacy between two groups post-treatment was not statistically significant.Watian drinking water test score of two groups Post-treatment, SaO2 decreased value compared with those pre-treatment significantly increased (P<0.05).After treatment of two groups of patients Watian drinking water test score difference does not have statistical significance, and post-treatment, patients in group B of SaO2 decreased value lower than that in group A ( P <0.05 ).Mild dysphagia, two cases of moderate swallowing dysfunction occurred in twenty-six patients in group A, group B with mild swallowing dysfunction occurred in twenty-one patients, A group of swallowing dysfunction more serious than group B serious ( P <0.05 ) .Conclusion Olanzapine and quetiapine quetiapine in the treatment of patients with Alzheimer's disease clinical effect quite, patient's swallowing function cause some adverse effects, effects of olanzapine is more serious.
3.Reasons for perioperative death in treatment of acute myocardial infarction by emergent percutaneous coronary intervention

ObjectiveTo analyze the dead reasons in treatment of acute myocardial infarction (AMI) by emergent percutaneous coronary intervention (EPCI) during perioperation. Methods The clinical characteristics of the dead patients,results of EPCI and reasons of death were retrospectively analyzed in 473 patients with AMI who received EPCI treatment during March 2004 to March 2011. ResultsAmong the 18 cases(3.8%)dead patients,there were 8 case with three-vessel lesions,5 cases with two-vessel lesions, 2 cases with single vessel lesion and 3 cases with left main lesion. Nine patients were accompanied with hypertensions,5 cases with diabetes mellitus, 1 case with old myocardial infarction, 2 cases with old cerebral infarction, 1 case with chronic obstructive pulmonary disease complicated by severe pneumonia, and 1 case with chronic renal insufficiency. Nine patients died of cardiogenic shocks , 4 cases (22. 2%) of heart ruptures, 2 cases (11.1%) of noreflow,1 case (5.6%) of massive hemorrhage of the upper alimentary tract, 1 case (5.6%) of respiratory failure and 1 cases (5.6%) of refractory ventricular fibrillation.ConclusionsPrimary reasons for perioperative death were cardiac shock and heart rupture in treatment of acute myocardial infarction by EPCI. Multi-vessel lesion, complications with hypertensions and diabetes mellitus may serve as succumbed factors of death.
4.Diagnosis,treatment,and prognostic analysis of thirty-one cases with primary bronchopulmonary carcinoid

Objective:We aimed to explore the clinical features, computed tomography (CT) findings, treatment, and prognosis of bronchopulmonary carcinoid. Methods:Clinical data of 31 patients with primary carcinoid tumor of the lung were retrospectively re-viewed. The prognostic factors were analyzed via Cox univariate and multivariate analyses. Results: Clinical symptoms included coughing or expectoration in 17 of the 31 cases, hemoptysis or blood-stained sputum in 7 cases, and chest pains or shortness of breath in 8 cases. Six cases were asymptomatic. The CT scans showed round or oval nodules with clear boundaries, and enhancement CT scans indicated mild, homogeneous enhancement. Immunohistochemistry results revealed the positive expression rates of synaptophy-sin (Syn), chromogranin A (CgA), and neuron-specific enolase (NSE) were 90.3%(28/31), 87.1%(27/31), and 90.3%(28/31), respec-tively. Therapy and prognosis results were as follows:28 of the total number of patients underwent surgery, among which 3 underwent postoperative adjuvant therapy, 2 received chemotherapy; and only 1 refused treatment. The 1-year overall survival rates were 100%(18/18) and 92.3%(12/13), whereas the 3-year survival rates were 94.4%(17/18) and 69.2%(9/13) in the typical and atypical carcinoid cases, respectively. Cox univariate analysis results revealed that lymphatic metastasis (P=0.02), tissue types (P=0.017), TNM stage (P=0.005), and therapies (P=0.01) were the prognostic factors. Cox multivariate analysis results showed that lymphatic metastasis (P=0.032) and tissue types (P=0.002) were the independent prognostic factors. Conclusion:Compared with other lung cancers, the bron-chopulmonary lung carcinoid has no special clinical manifestation in clinical and radiographic images. The diagnosis was mainly based on histopathology results. Surgery was the main and effective treatment, whereas chemotherapy and radiotherapy showed unsatisfactory results. The overall prognosis was satisfactory. However, the atypical carcinoid was inferior to the typical carcinoid in terms of progno-sis. Pathological typing and lymph node metastasis were significant prognostic factors.
5.Comparison of corneal Q values measured by Topcon KR-1W,iTrace and Topolyzer

Background The measurement of corneal Q value is essential for corneal refractive surgery and calculation of intraocular lens during cataract surgery.Topolyzer was often used for the measurement of Q value,and recently Topcon KR-1W and iTrace were applied in ophthalmology.However,whether the measured values are interchangeable is unclear.Objective This study was to assess the difference and consistency of corneal Q values measured by Topcon KR-1W,iTrace and Topolyzer.Methods Corneal Q values were measured on 100 right eyes of 100 healthy subjects under the approval of Ethic Committee of the Sixth Hospital Affiliated to Shanghai Jiaotong University and informed consent of each subject from November to December in 2014 with Topcon KR-1W,iTrace and Topolyzer.Three valid measurements were obtained for each device,and the average values from each device were calculated.The difference of the outcomes among the instruments was compared by repeated measures analysis of variance (ANOVA),and the consistency among the outcomes from different apparatus was analyzed by Bland-Altman plots.Results The mean corneal Q values were-0.184-±0.112,-0.117±0.167 and-0.269±0.117 from Topcon KR-1W,iTrace and Topolyzer,respectively,with a significant difference among them (P < 0.001).The measured Q value by Topcon KR-1W was 0.085±0.010 larger than that by Topolyzer,and the Q values by iTrace was 0.152± 0.014 larger than that by Topolyzer,while the Q values obtained by Topcon KR-1W was 0.067±0.016 smaller than that by iTrace (all at P<0.05).The 95% confidence interval of the values between Topcon KR-1W and iTrace,Topcon KR-1W and Topolyzer,iTrace and Topolyzer were-0.106 to-0.028,0.060 to 0.109 and 0.118 to 0.186,respectively.Bland-Altman plots showed that 6%,6% and 5% values were outside of 95% agreement of limit (LoA) between Topcon KR-1W and iTrace,iTrace and Topolyzer or KR-1W and Topolyzer,respectively,with the maximal differences of 0.28,0.43 and 0.38.Conclusions Corneal Q values measured by Topcon KR-1W and iTrace are larger than those measured by Topolyzer.Due to the poor agreements among the corneal Q values by the 3 kinds of devices,they are not interchangeable in clinical applications for the measurement of corneal Q value.
6.Studies on Chemical Constituents and Antitumor Activity of Viscum liquidambaricolum Hayata.

【Objective】The chemical constituents of Viscum liquidambaricolum Hayata.were studied to screen the constituents with antitumor activity.【Methods】Chromatography was used for the isolation and purification of chemical constituents of Viscum liquidambaricolum Hayata..And their structures were identified on the basis of spectroscopic evidences and physiochemical properties.Meanwhile,the in-vitro antitumor activities of two kinds of triterpenoids and one kind of sitosterol were investigated.【Results】Four compounds were isolated from Viscum liquidambaricolum Hayata.and their structures were identified as genkwanin(1),?-amyrin acetate(2),erythrodiol(3) and ?-sitosterol(4).Compounds 2 and 3 had an inhibitory effect on mice sarcoma 180(S180),Ehrlich ascites carcinoma(EAC) and ascties hepatoma(HeAP),and compound 4 had an inhibitory effect on mice S180 and EAC,their half-inhibitory concentrations being less than 400??g/mL.【Conclusion】Compounds 1~4 are isolated from Viscum liquidambaricolum Hayata.for the first time,and compound 1 is isolated from Loranthaceae for the first time.Triterpenoids and steroids from Viscum liquidambaricolum Hayata.exert certain antitumor activity.
7.Impact on the quality of surgical breast cancer patients at different stages of life

Objective To study different surgical methods for breast cancer patients under the influence of the quality of life at different stages. Methods The clinical data of finishing our hospital 160 cases of breast cancer patients , ac-cording to the patient's surgical approach , divided into breast-conserving resection of breast conserving surgery group , mastectomy breast reconstruction group, modified radical group, using the method of returning scale investigation, the statistical quality of life of patients at different stages. Results The patients in the perioperative period, physical, psy-chological and other breast-conserving surgery group and mastectomy breast reconstruction group than modified radical group ( P<0 . 05 ) , with significant difference , but there were no significant difference in the quality of life between the two groups (P>0.05); after 2 years and 5 years, breast-conserving surgery and mastectomy breast reconstruction group's quality of life was higher than that in the modified radical group (P<0.05), with significant difference between the two groups. The patient's physiological , psychological and other aspects of the situation was not statistically signif-icant (P>0.05);perioperative period, after 2 years, a period of 5 years, breast-conserving surgery group and mastecto-my breast reconstruction patients physical, psychological status, quality of life was not statistically significance (P>0.05). Conclusion Breast cancer treatment involves the patient's physical structure , resulting in a greater impact on quality of life and psychological state of the patient's will to a large extent, in the optional cases, give priority to breast-con-serving surgery or mastectomy breast reconstruction may be to some extent , improve the quality of life of patients.
8.Effect of epithelial-to-mesenchymal transition on the invasion and migration abilities of lung squamous cell carcinoma

Objective:To investigate the clinical significance of epithelial-to-mesenchymal (EMT) in lung squamous cell carcino-ma (LSCC) and to examine the effect of EMT on the invasive and migration abilities of LSCC. Methods:Immunohistochemical stain-ing was performed to determine the expression of E-cadherin, Vimentin, and TGF-β1 in 79 LSCC patients, and the clinical significance was explored. SK-MES-1 lung squamous carcinoma cells were cultured in conditioned medium containing various concentrations of transforming growth factor-β1 (TGF-β1) for 5 and 10 days. The expression levels of E-cadherin and Vimentin were detected via West-ern blot and reverse transcription-polymerase chain reaction (RT-PCR). With different concentrations and induction times, invasion and wound healing assays were performed to evaluate the invasion and migration abilities. Results:E-cadherin expression was significantly lower, whereas Vimentin expression was significantly higher in LSCC with lymph node metastasis than in that without noda metastasis (P<0.05). In the tissues of 79 LSCC patients, TGF-β1 expression was significantly related to lymph node metastasis (P<0.05). Western blot showed that Vimentin expression was higher, whereas E-cadherin expression was lower in TGF-β1 inducing medium with 10 ng/mL SK-MES-1 cells than in the other media. RT-PCR showed similar results. Scratch test and invasion assay both showed that treat-ment of cells with cytokines markedly enhanced the migration and invasion of the cells. Conclusion:Lymph node metastasis of LSCC correlates with EMT. SK-MES-1 cells undergo EMT via TGF-β1 induction, which enhances invasion and migration.
9.Establishment of an infected necrotizing pancreatitis model by retrograde pancreatic duct injection of sodium taurocholate and E. coli in rats.

A stable and reliable infected necrotizing pancreatitis (INP) model in rats was established in order to study the pathophysiological mechanism and pathological development rule of INP and explore the new therapeutic methods for the diseases. Forty-six SD rats were randomly divided into 5 groups. The animals in group A received the injection of 5% sodium taurocholate into the pancreatic duct and those in group B underwent that of E. coli into the pancreatic duct. The rats in groups C, D and E were subjected to the injection of 5% sodium taurocholate in combination with different concentrations of E. coli (10(3), 10(4), 10(5)/mL, respectively) into the pancreatic duct. The dose of injection was 0.1 mL/100 g and the velocity of injection was 0.2 mL/min in all the 5 groups. Eight h after the injection, the survival rate of animals was recorded and the surviving rats were killed to determine the serum content of amylase and perform pathological examination and germ cultivation of the pancreatic tissue. The results showed that acute necrotizing pancreatitis model was induced by injection of 5% sodium taurocholate into the pancreatic duct. The positive rate of germ cultivation in group A was 12.5%. The acute necrotizing pancreatitis model was not induced by injection of E. coli into the pancreatic duct and the positive rate of germ cultivation in group B was 0. The INP model was established in groups C to E. The positive rate of germ cultivation was 60%, 100% and 100% and 8-h survival rate 100%, 100% and 70% in groups C, D and E, respectively. It was concluded that a stable and reliable model of INP was established by injection of 5% sodium taurocholate in combination with 10(4)/mL E. coli into the pancreatic duct with a dose of 0.1 mL/100 g and a velocity of 0.2 mL/min. The pathogenesis of INP might be that the hemorrhage and necrosis of pancreatic tissue induced by sodium taurocholate results in weakness of pancreatic tissue in fighting against the germs. Meanwhile, the necrotic pancreatic tissue provides a good proliferative environment for the germs.

10.Immune responses induced by the suicidal DNA vaccines co-expressing the GP5 protein of PRRSV and the E2 protein of CSFV in mice.

Six recombinant plasmids co-expressing the wild-type GP5 gene or the codon-optimized GP5 gene (containing pan-DR epitope) of porcine reproductive and respiratory syndrome virus (PRRSV) and the E2 gene of classical swine fever virus (CSFV) or the E2 fused with the UL49 of pseudorabies virus (PrV) were constructed based on the suicidal DNA vaccine pSFV1CS-E2 described previously. Expression of GP5 and E2 was confirmed by indirect immunofluorescence assay. The immunogenicity of six plasmids was evaluated in BALB/c mouse model. For the six plasmids, low-level of E2 and GP5 protein specific antibodies could be detected in the sera of the immunized mice. Specific lymphoproliferative responses to the PRRSV or CSFV stimulation were induced in the splenocytes of the immunized mice as demonstrated by CFSE staining assay and WST-8 assay. Antigen specific IFN-gamma and L-4 secretion was detected in the splenocytes of some immunized mice by cytokine ELSIA. Fusion with the PrV UL49 in the suicidal vaccines induced significantly higher lymphoproliferative responses and cytokine secretion. Taken together, the suicidal DNA vaccines co-expressing GP5 and E2 could induce PRRSV and CSFV specific humoral and cell-mediated immune responses.
