4.A Case of Congenital Deformity of the Eye, Combined with Cleft Lip and Palate.

We have experienced a case of congenital deformity of the eye, which was unilateral, combined with cleft lip and palate. Oblique facial cleft is the most uncomnon facial cleft. The cleft follows an oblique line extending from the upper lip, lateral to the philtrum and nostril, toward the eye. The coloboma of the lower lid and enlarged bifid of the cornea were formed by part of a deep groove dong the lateral aspect of the nose, together with the presence of the cleft lip and palate. In the present case, the malformation may have resulted from abnormalities in the germ cells themselves or from amniotic abnormal pressure, or from the formation of amniotic bands or adhesions.

Diagnosis of a thymic carcinoid was made on transthoracic fine needle aspiration in a 36-year old woman who had an anterior mediastinal mass on chest X-ray and CT scan. The aspiration smears showed numerous anastomosing ribbons and cords of small round tumor cells. The tumor cells had slightly eccentric nuclei and some granular cytoplasm. The small and uniform nuclei of the tumor cells had finely granular chromatin and thin nuclear membrane. The cytologic diagnosis of a carcinoid was confirmed on histopathologic, immunohistochemical, and electromicroscopic examination of surgical specimen.

7.A Case of Malignant Lymphoma of the Orbital Margin.

Secondary involvement of the eye and orbit occurs in about two to four percent of the patients with generalized lymphomatosis and/or other forms of lymphomatous tumors. Of the extranodal primary lesions which are more rare manifestation of the malignant lymphoma, only one percent occur in the eye and orbit. The malignant lymphoma of the orbit is consequently a relatively uncommon condition. The authors experienced a case of the malignantly mphoma of the orbital margin and treated by local radiation therapy with Co60, and the lesions regressed rapidly and completely disappeared. There has been no sign of recurrence at the orbital region, but we found that the malignant lymphomatous lesions involved in the pancreas and omentum by the surgical laparotomy. The patient died of poor course of the disease about 20 days after discharge.

9.Congenital Chylothorax.

Chyle is the digestive product absorbed from intestinal lymphatics which the component have high concentration of triglyceride and protein. The various pathological circumstances which give rise to intrathoracic effusion of chyle are well documented and so the etiology is clearly defined in most instances. However, the occurrence of chylothorax in early infancy, in the absence of other demonstrable diseases and without apparent birth trauma, suggests the existence of congenital malformation of the thoracic ducts. We lately experienced a 20 day old new born who was proved to have congenital chylothorax by clinical history and laboratory findings. So we report the clinical findings and laboratory findings with the review of the literatures on congenital chylothorax.

10.The Effect of Intraluminal Oxygen Insufflation on the Oxygenation of Gut Mucosa in Hemorrhaged Cats.

BACKGROUND: This study was purposed to study the therapeutic value of intraluminal oxygen insufflation on the oxygenation of gut mucosa in a feline model subjected to a hemorrhagic shock followed by reperfusion. METHODS: Eighteen cats were divided into three groups : For group 1, 6 cats were subjected to sham operation as a control group ; for group 2, 6 cats were subjected to a hemorrhagic shock for 2 hours prior to reperfusion as a hemorrhagic shock(HS) group ; for group 3, 6 cats were subjected to a simultaneous insufflation of intraluminal oxygen, continued during the shock and reperfusion periods as a oxygenated hemorrhagic shock(OHS) group. Mesenteric PvO2, mesenteric venous arterial(v-a) lactate difference, mesenteric P(v-a)CO2 and mesenteric pH(a-v) were measured for every 30 minutes during the shock and reperfusion. RESULTS: The lactate(v-a) was increased during the shock. For group OHS, the lactate(v-a) returned to the baseline value after reperfusion, but for group HS, it did not return. The values of pH(v-a) and P(v-a)CO2 were increased during the shock and returned to the baseline value for group OHS, but for group HS, the increase was greater than group OHS(p<0.05), but not returned to the baseline value after reperfusion. For both pH(v-a) and P(v-a)CO2, there were statistically significant differences(p<0.05) between group HS and group OHS during the shock and reperfusion. CONCLUSIONS: We conclude that the insufflation of intraluminal oxygen improves the oxygenation of gut mucosa in the feline model undergone a hemorrhagic shock followed by reperfusion.
