6.The Mechanisms of Resistance to TNF in TNF-Sensitive Cancer Cells Transfected with TNF-alpha Gene Using Retroviral Vector.

BACKGROUND: Tumor necrosis factor(TNF) has been considered as an important candidate for cancer gene therapy based on it9 potent anti-tumor activity. However, since the efficiency of current techniques of gene transfer is not satisfactory, the majorities of current protocols is aiming the in vitro gene transfer to cancer cells and re-introducing genetically modified cancer cells to host In previous study, it was shown that TNF-sensitive cancer cells transfected with TNF-α CDNA would become highly resistant to TNF. Understanding the mechanisms of TNF-resistance in TNF-α gene transfected cancer cells would be an important step for improving the efficacy of cancer gene therapy as we]1 as for better understandings of tumor biology. This study was designed to evaluate the role of new protective protein synthesis in the acquired resistance to TNF of TNF-α gene transfected cancer cells. METHOD: We transfected TNF-α c-DNA to WEHI l64, a murine fibrosarcoma cell line, using retroviral vector (pLT12SN(TNF)) and confirm the expression of TNF with PCRf ELISA, MTT assay. Then we determined the TNF resistance of TNF gene transfected cells(WEHI 164-TNF) and the changes of TNF sensitivities after treatments with actinomycin D(transcription inhibitor) and cycloheximide(translation inhibitor). RESULTS: WEHI 164 which was sensitive to TNF became resistant to TNF after being trsnsfected with TNF-α gene and the resistance to TNF was partially reversed after treatment with actinomycin D, but not with cycloheximide. CONCLUSION: The acquired resistance to TNF after TNF-α gene transfection may be associated with synthesis of some protective proteins.

7.Visual Field in Normal Korean Subjects: Traquair's "Hill of Vision".

PURPOSE: To investigate Traquair's "hill of vision" of the normal Korean subjects in the photopic and mesopic background. METHODS: Automated perimetry was performed in 152 normal subjects using Humphrey 24-2 SITA standard (photopic background, n=100) and Octopus G1 full threshold (mesopic background, n=52) program. We calculated and compared the threshold sensitivity and sensitivity deviation of the visual field by the fashion of the superior/inferior hemi-field, temporal/nasal hemi-field, quadrant, and 5 degrees ring. RESULTS: In Humphrey, average mean deviation (MD) and pattern standard deviation (PSD) were -1.22+/-0.86 and 1.43+/-0.33 dB, respectively. In Octopus, average MD and PSD were -3.39+/-1.39 and 1.67+/-0.23 dB, respectively. In both Humphrey and Octopus, the average threshold sensitivity of the inferior hemi-field was slightly higher than that of the superior hemi-field. There was no significant difference between the nasal and temporal hemi-field. The sensitivity was decreased with increasing the eccentricity. In the deviation map of the Humphrey, there were no significant differences in the comparisons of the hemifield, quadrant, and 5 degrees ring. In Octopus, there were no significant differences in the comparisons of the hemifield and quadrant. However, the sensitivity deviation was peaked at the fixation point and decreased with increasing the eccentricity. CONCLUSIONS: These results indicate that the normal Korean subjects may have lower sensitivity than the normal age-matched controls of the automated perimetry, especially in the Octopus perimetry. In addition, this study suggests that the shape of the Traquair's "hill of vision" in the normal Korean subjects might be similar to that of the normal age-matched controls in Humphrey perimetry which was performed in photopic background, but it might be flatter in Octopus perimetry which was performed in mesopic background.

8.Comparison between Planar View and SPECT View on the Dipyridamole Thallium 201 Myocardial Perfusion Scintigraphy.

Exercise testing with Thallium imaging is widely used for the noninvasive evaluation of patients suspected of having coronary artery disease. However, many patients referred for stress testing connot exercise adequately for either physical or psychological reasons, and as a result may have nondiagnostic or suboptimal test results. Intravenous dipyridamole in conjunction with Thallium imaging is as effective alternative method without exercise. But, myocardial imaging using the standard scintillation camera technique(planner view) is hampered by superposition of proximal & distal cardial walls and by the segmental nature of myocardial ischemia. For this reason, Single Photon Emission Computed Tomography(SPECT) reslut in high specificity & sensitivity rates for the detection of coronar artery disease compared with conventional technique. So we performed dipyridamole Th-201 myocardial scintigraphy on 25 subjects who have suspicious angina or myocardial infarction instead of exercise Th-201 myocardial scintigraphy, and compared SPECT view with conventional plannar view. The results obtained are as follows : 1) T1-201 scintigraphic findings in 17 patients with suspicious angina were as follows ; redistribution defect was seen in 4 cases in plannar view and 13 cases in SPECT view. 2) T1-201 scintigraphic findings in 8 patients with myocardial infarction were as follows ; in planner view, perfusion defect was seen in all cases and 1 cases of them, redistribution defect was accompained, and in SPECT view, perfusion defect was seen in all cases and 6 cases of them, redistribution defect was accompained. 3) During dipyridamole infusion, the mean systolic & diatolic pressure decreased from 133+/-22.7/86+/-13.5 to 121+/-23.9/78+/-13.1mmHg and the heart rate increased from 68+/-12.4 to 84+/-12.4beats/min. 4) Adverse effects of dipyridamole were noted in 14(56%) of the subjects, but in 12 of these, the symptoms were mild in severity and subsided spontanously. To summarize, Dipyridamole-201 myocardial imaging is a useful and test for coronary artery disease, and the new tomographic technique, SPECT view, is more useful than the conventional plannar view.

9.Minimally Invasive Spine Surgery versus Open Posterior Instrumentation Surgery for Unstable Thoracolumbar Burst Fracture

Methods:
We enrolled 40 patients who underwent either MISS (M group, 20 patients) or open posterior instrumentation surgery (O group, 20 patients) for the treatment of traumatic unstable burst fractures. Clinical outcomes were evaluated based on postoperative back pain, operation time, blood loss, hospital stay duration, and perioperative complications. For radiologic evaluation, preoperative magnetic resonance imaging and plain radiography were performed before and after the surgery to evaluate the changes in the kyphotic angle and fracture union.
Results:
The change in the kyphotic angle was −8.2°±5.8° in the M group and −8.0°±7.8° in the O group. No significant difference was noted in terms of the change in the kyphotic angle (p=0.94, t-test) after 12 months of surgery. The Visual Analog Scale score was 1.5±0.7 points in the M group, while it was 5.2±1.4 points in the O group. In the M group, back pain has significantly decreased (p<0.01, t-test). The estimated blood loss was 195.5 mL in the M group and 1,077.5 mL in the O group; the operation time was significantly decreased in the O group from 290.7 to 120.7 minutes in the M group (p<0.05, t-test) (p=0.36, t-test). The average duration of hospital stay was 36.0 days in the M group and 41.9 days in the O group (p=0.36, t-test).
Conclusions
For the treatment of unstable burst fractures, MISS showed significant differences in terms of postoperative back pain, operation time, and blood loss as compared to open posterior instrumentation surgery.
