4.Focal Xanthogranulomatous Pyelonephritis: Reports of 2 Cases.

Xanthogranulomatous pyelonephritis is a chronic inflammatory condition which in most cases involves either an entire non-duplicated kidney or a moiety of a duplicated kidney. These cases present with quite characteristic radiologic findings including an enlarged non functioning kidney and renal stones. However, when the condition involves only a part of a non-duplicated kidney and especially where there is an absence of the specific findings described above, differentiation from cystic neoplasm may be difficult. We report the radiologic findings of twosuch cases of focal xanthogranulomatous pyelonephritis.

5.Two Cases of Xanthogranulomatous Pyelonephritis.

Xanthogranulomatous pyelonephritis is a rare form of chronic pyelonephritis, characterized by yellow masses macroscopically and foamy, lipid-laden macrophages microscopically. It may involve the kidney diffusely, or may be more focal as in the tumefactive form, which presents as a renal mass lesion and is virtually always unilateral. We have recently experienced two cases of xanthogranulomatous pyelonephritis in 28 and 34 year old Korean females.

6.A Case of Bilateral Xanthogranulomatous Pyelonephritis with Renal Failure.

Xanthogranlomatous pyelonephritis is a rare chronic inflammatory renal disease. Its clinical and radiological findings resemble other renal parenchymal diseases, such as a renal abscess or renal cell carcinoma. Only a histologic examination can confirm xanthogranulomatous pyelonephritis. It usually presents unilaterally and displays relatively normal renal function. Bilateral xanthogranulomatous pyelonephritis is extremely rare. The renal function of bilateral xanthogranulomatous pyelonephritis is usually normal at the time of diagnosis, but progresses to end stage renal failure as the disease progresses. We experienced a case of bilateral xanthogranulomatous pyelonephritis, and its initial manifestation was renal failure. We report this case with a review of the literatures.

Two conservatively treated cases of xanthogranulomatous pyelonephritis are reported here. The first case was a 31-year-old diabetic woman. She was referred to our department. Examinations for gross hematuria were made. US, CT and MRI revealed a renal tumor in the upper pole of the left kidney. The second case was an 86-year-old man with ileal conduit. He complained about pain in the right back. A mass of fist size was found. US, CT and MRI revealed right hydronephrosis connected to the subcutaneous tumor on the right back, and DIP revealed no-functioning right kidney. Because both cases showed some signs of inflammation in laboratory findings, we performed biopsy. Either of the two cases were pathologically diagnosed as xanthogranulomatous pyelonephritis. For this reason, Antibiotic therapy was continued. As a result, reduction of the tumors was seen. The prognosis was good.

Xanthogranulomatous pyelonephritis is an atypical form of severe chronic parenchymal infection and usually occurs in associated with urinary tract infection, obstruction and/or calculi. An accurate preoperative diagnosis is difficult because of its clinical and radiological similarities to various other renal lesions. We experienced two cases of xanthogranulomatous pyelonephritis, so we report with review of some literatures.

There is no single characteristic clinical or radiological feature to diagnose xanthogranulomatous pyelonephritis, so it makes accurate preoperative diagnosis difficult. In our case, the renal parenchyme of affected kidney was well preserved on excretory urogram. The abnormal finding was only a filling defect within the right upper pelvocalyceal system. Herein we report a case of xanthogranulomatous pyelonephritis in a 18 years old girl which was confused with renal pelvic tumor.

Fistula formation between the upper urinary tract and cutaneous tissue is an uncommon complication in urogenital disease. Xanthogranulomatous pyelonephritis(XGP) is a severe, chronic infection of the renal parenchyma that is often associated with calculi and obstruction. We present a rare case of XGP with a nephrocutaneous fistula.
